1 INTRODUCTION The enclsed infrmatin addresses expectatins f Subcntractrs, Tier Subcntractrs and their invitees (herein called subcntractrs) visiting r perfrming wrk n Pwer prjects. Each subcntractr wrking n Pwer prjects is bligated t cmply with all Federal, State and Lcal safety requirements, Site Specific Safety Prgrams, and any Owner Safety Requirements (herein after called safety requirements). These safety requirements cnstitute the minimum level f perfrmance expected frm each emplyer and its emplyees r their subcntractrs, r agents. In additin, subcntractrs are respnsible fr adherence t site specific safety requirements defined by the prject safety requirements. All subcntractrs shall adhere t these requirements fr the perfrmance f their wrk n Pwer prjects designed t prmte the prject s safe cmpletin. SUBCONTRACTOR COMPLIANCE In accrdance with the OSHA requirements, each subcntractr shall prtect the emplyment and places f emplyment f each f its emplyees engaged in cnstructin wrk by cmplying with the apprpriate standards prescribed in the applicable standards. Subcntractrs shall hld each f their agents, vendrs, tier subcntractrs and suppliers respnsible fr cmpliance with these safety requirements. Subcntractrs shall include these safety requirements in cntracts with all tier subcntractrs, vendrs and suppliers. Entry nt prject, prperty, r the jb site cnstitutes acknwledgement by the subcntractr, subcntractr emplyees r invitee f their bligatin t adhere t these safety requirements. Each subcntractr shall establish and maintain an effective safety and health prgram that addresses the requirements herein. The subcntractr shall be slely respnsible fr implementing the safety prgram and shall have sle respnsibility fr mnitring the wrk f its emplyees, subcntractrs, agents, vendrs and suppliers t ensure cmpliance. The subcntractr is respnsible fr prcuring and cmpliance with the current versin f Pwer s Subcntractr Site Safety Requirements and at Fr prjects designated t be cvered by General Cntractr s CCIP as identified in the Prject Specific Agreement s Exhibit C, and in additin t any prject specific requirements defined herein, Subcntractr shall als be held t the requirements defined in the Appendix A CCIP Safety Prgram Requirements f this dcument. VIOLATION OF SAFETY REQUIREMENTS If a subcntractr r invitee is fund nn cmpliant t any f the safety requirements, the subcntractr and emplyee(s) may be subject t the fllwing (ne r mre): At a minimum, the resulting actin may result in a written warning; Individual(s) may be remved frm the prject fr a specified duratin; Individual(s) may be remved frm prject and/r future Pwer prjects; Re training fr individual(s), crew and /r freman; A $1,000 fine per ccurrence; Additinal full time supervisin and/r safety representative t the prject at the subcntractr s expense; Subcntractr Site Safety Requirements and 1 f 20

2 A meeting will be cnducted with the subcntractr s supervisr and management, and the Pwer Prject Team. The meeting shuld cnclude in a dcumented agreement utlining the subcntractr s intended crrective actins and timeframe fr implementatin; Remval f unsafe cnditin by using ther wrk frces, the cst f which will be reimbursed thrugh back charges r prvisins f the cntract; Pwer may exercise its ptin t terminate all r part f the cntract with the subcntractr fr inadequate safety perfrmance, r failure t fulfill any f the safety requirements f the cntract; Any resulting damages (including damage fr delay) will be paid fr in accrdance with the subcntract. All csts and expenses paid r incurred by a subcntractr in the implementatin and administratin f the safety requirements shall be paid by said subcntractr. When vilatins f the safety requirements are bserved, the respnsible subcntractr shall be infrmed rally fr immediate crrectin. It is the sle respnsibility f the subcntractr t devise and implement the crrectin. If Pwer deems it is necessary t stp wrk being perfrmed due t the nature f a vilatin, wrk will be halted until the subcntractr crrects the vilatins. Any csts incurred by the stppage f wrk due t the vilatin will be the sle respnsibility f the vilating subcntractr. DESIGNATION OF COMPETENT PERSON Each subcntractr shall designate a cmpetent persn as defined by OSHA t implement the safety requirements. A cmpetent persn frm each subcntractr must be n site whenever they have emplyees wrking n site, and the name f that persn shall be submitted n the Pwer Daily Reprt. Each subcntractr is required t maintain this psitin, and a cmpetent persn(s) will remain n site until the cmpletin f their wrk. The subcntractr shall nt relinquish r defer respnsibility fr prject safety t its wn r subcntractr emplyees at any time under any circumstances. Where the nature r size f the cntract warrants, Pwer may require the subcntractr t emply a full time, nsite qualified Safety Representative. The fllwing trade cntractrs are required t prvide a cmpetent persn wh has cmpleted, at a minimum, the OSHA 30 hur Outreach Training Prgram fr Cnstructin within the previus six years at the prject full time when said cntractr has three r mre emplyees wrking n site fr the duratin f mre than 5 sequential wrking days. The subcntractr must prvide prf f this training t Pwer prir t cmmencing wrk n site. All structure related steel activities (erectin, decking, jist, etc.) Miscellaneus metals (stairs, framing, supprts, etc.) All structure related cncrete activities (frame, skin, cast in place, pre cast, masnry, etc.) Mechanical trades, Mechanical pipe and HVAC Electrical Plumbing Sprinkler fitter Subcntractr Site Safety Requirements and 2 f 20

Based n the risk assciated with the subcntractr s wrk, Pwer may require additinal training fr the cmpetent persn r additinal cmpetent persns fr the specific prject. The subcntractr must prvide prf f this training t Pwer prir t cmmencing wrk n site. Subcntractrs may fulfill the cmpetent persn requirement fr tier subcntractrs granted the subcntractr cmpetent persn remains at the prject full time while the tier sub emplyees are wrking. Otherwise, the tier subcntractr is respnsible fr prviding the cmpetent persn based n the afrementined criteria. Each subcntractr is slely respnsible fr ensuring emplyees have the training required as part f this plicy and any ther federal, state r lcal requirement. Any questins r variance requests must be directed t Pwer s Safety Directr fr cnsideratin. SUBCONTRACTOR SAFETY REPRESENTATION The subcntractr s designated Crprate Safety Representative is expected t make at a minimum mnthly jbsite visits t audit implementatin f the subcntractr s safety and health plan and the safety requirements. This representative will als be required t attend regular n site safety meetings, as determined by Pwer. Additinal n site safety and health staffing will be defined in the individual trade scpes. The subcntractr will prvide a cpy f all dcumented inspectins, bservatins, r reprts t Pwer. OSHA AND STATE AGENCY INSPECTIONS If after an inspectin, a subcntractr receives any citatin(s), a cpy f all citatins shall be immediately prvided t Pwer. It is the respnsibility f the subcntractr t ntify Pwer and OSHA f any reprtable injury under the Recrdkeeping regulatin (29 CFR 1904) which includes all wrk related fatalities, inpatient hspitalizatins, amputatins r lsses f an eye. 4 TRAINING AND MEETING REQUIREMENTS Precnstructin Meeting Subcntractrs shall ensure that their prject management and ther key persnnel, including their Site Supervisr and Safety Representative at a minimum, attend a pre cnstructin meeting with the Pwer prject management staff where planning fr safe executin f the prject will be addressed. Site Safety Orientatin Prir t starting wrk n the Prject, all emplyees are required t attend a site specific safety rientatin prvided by Pwer. The rientatin will cver general safety and health rules, regulatins and site specific plicies and hazards. The rientatin serves as the verbal warning fr safety requirements fr all individuals n the prject. The subcntractr shall be respnsible fr scheduling rientatin f their emplyees and visitrs with Pwer. If an individual is fund n site withut having received the training, that persn will be remved frm the prject fr the remainder f the day, and the crew freman may be disciplined. Freman s Site Requirements Review The freman identified by the subcntractr will cmplete a review f the safety requirements and expectatins with a member f the Pwer prject team. If the subcntractr has additinal freman r changes freman n the prject, these individuals must als cmplete this review. Freman s Meeting Freman s meetings will typically be cnducted n site. The date and time f these meetings will be determined by the prject team. Attendance is expected frm each subcntractr unless excused by Pwer. Safety Review Fllwing an incident r safety issue, Pwer may request a meeting be held with the parties invlved t discuss the incident r issue in greater detail. Requested subcntractrs shall attend and participate in the investigatin, discussin, and develp an actin plan. Safety Specific Meetings As required by and at Pwer s discretin, site wide safety meetings r stand dwns will be cnducted and subcntractr emplyee attendance and participatin will be required fr said meetings. Pwer expects subcntractrs t cperate with and participate in additinal training exercises cnducted at the prject (i.e. OSHA Training Institute visits). INJURY CARE AND MEDICAL FACILITY Each subcntractr is respnsible t establish a medical facility fr use by emplyees wh sustain a wrk related injury. The facility t be used shall be cmmunicated t the subcntractr s emplyees. The subcntractr is respnsible fr all csts brne ut f medical treatment, substance abuse testing and any ther assciated csts (Pwer is nt t be charged fr any treatment csts including substance abuse testing). Each subcntractr is t prvide an apprpriate sized First Aid kit that is maintained and adequately stcked. The lcatin f the kit shall be cmmunicated with the subcntractr s emplyees. 5 Subcntractrs will assure that when required r apprpriate, trained first aid persnnel are available, certified, and equipped fr their respnsibilities. DOCUMENTATION AND REPORTING Site Specific Safety and Health Plan Each subcntractr shall establish and submit fr review a written Site Specific Safety and Health Plan that includes details cmmensurate with the wrk t be perfrmed. The subcntractr s Site Specific Safety and Health Plan shall clearly describe the subcntractr s methds fr meeting its bligatins t prvide a safe and healthful wrk envirnment, as well as t prtect ther trades, vendrs, visitrs and members f the general public frm the expsures generated by the subcntractr s wrk. The subcntractr s Cmpany Safety Manual will nt be accepted as a substitutin fr a Site Specific Safety and Health Plan. The fllwing will be submitted prir t the subcntractr s mbilizatin t the prject: A written Prject Site Specific Safety & Health Plan (paper r electrnic cpy as required by the Pwer prject team); Identify safety rles and respnsibilities fr subcntractr emplyees; Subcntractr s disciplinary actin prgram; Prcess fr managing tier subcntractrs; Hazard Cmmunicatin Prgram, including paper r electrnic cpy (as required by the Pwer prject team) f current SDS and table f cntents. A prject specific SDS file shall be maintained n site fr emplyee review; Specific jb hazard identificatin and wrker training (i.e. qualified rigger training); Jb Hazard Analysis plan; Emergency prcedures including a designated clinic with address, phne number and map. Cmpetent persn qualificatins and training recrds; As a cnditin f their cntract, all Subcntractrs shall submit t Pwer r designee a Site Specific Safety Plan within fifteen (15) days after receipt f ntice t prceed and prir t start f cnstructin activities. The subcntractr shall be slely respnsible fr implementing the Site Specific Safety and Health Plan as well as ther safety requirements. Jb Hazard Analysis (JHA) Detailed JHA s addressing hazards assciated with the Subcntractr s scpe f wrk are required as part f the subcntractr site specific safety and health plan submittal. The Subcntractr shall als prepare additinal JHA s upn request and mdify as the wrk prcess and/r assciated risks change. These prcedures will be reviewed with all affected emplyees prir t starting the wrk r after mdificatins t the JHA, by the subcntractr. Daily Reprt and Daily Huddle Prcess Each subcntractr, including tier subcntractrs, must submit t Pwer, n a daily basis, a Daily Reprt. Subcntractr must cmplete all f the infrmatin required n the Daily Reprt frm. Each subcntractr is slely respnsible fr prviding a device (i.e. tablet, laptp r smartphne) and internet access fr their emplyees t cmplete the Daily Huddle and Daily Reprt thrugh Pwer s Safety E frms prcess. Access t the system is prvided at n cst t the subcntractr and their tier Subcntractr Site Safety Requirements and 5 f 20

6 subcntractrs. Each subcntractr will be respnsible fr cnducting a daily huddle with each crew t identify the wrk activities fr the day, the assciated hazards f the wrk and the cntrl measures t be implemented. Tlbx Talks Each subcntractr shall cnduct weekly tlbx safety meetings relevant t the wrk being perfrmed by their emplyees. A cpy f the tlbx talk r a descriptin f the tpic discussed alng with all attendees names shall be submitted t Pwer. Safety Inspectins In accrdance with OSHA, each subcntractr shall perfrm frequent and regular safety inspectins f their wrk area(s) by a cmpetent persn. A cpy f the reprt r dcumented inspectin shall be submitted t Pwer within 24 hurs f the cmmencement f the inspectin. Subcntractr supervisrs shall take immediate actin t crrect vilatins, unsafe practices and unsafe cnditins. The subcntractr will be slely respnsible t review and mnitr the wrk area r lcatin f all their emplyees n a regular basis during the perfrmance f wrk. Incident Reprting Subcntractrs are respnsible t immediately ntify Pwer f all incidents including persnal injuries and illnesses, near hits (defined as an ccurrence that has the attributes f an incident, yet has n apparent damage t persn r prperty), prject prperty lsses r damages, and incidents invlving the public r their prperty. Each subcntractr is required t investigate all incidents incurred by their emplyees, r incidents that are the result f their peratins. Each subcntractr shall prvide t Pwer a dcumented Incident Investigatin Reprt within 24 hurs f the ccurrence. Pwer may cnduct an independent investigatin at their wn discretin r when they deem it necessary as a supplement t that required f the subcntractr. Subcntractrs and their emplyees are expected t fully cperate with the investigatin prcess including cmpletin f witness statements, phtgraphs, cmpletin f Pwer required dcuments and any ther elements f the incident investigatin prcess. Upn request, subcntractrs invlved in the incident shall participate in Incident Review Meetings. Each subcntractr is slely respnsible fr prviding access and the ability, t their prject supervisin, t uplad and manage electrnic dcuments as required by Pwer Cnstructin. SUBSTANCE ABUSE POLICY The illegal use r abuse f drugs and r alchl cnstitutes a threat t the safety and health f emplyees and the general public. The Substance Abuse Plicy requires emplyees t reprt t wrk fit fr duty, and t perfrm their wrk, free f detectable levels f drugs, alchl r ther substances, which may affect their ability t wrk safely. Each subcntractr shall establish and maintain an effective substance abuse prgram that at minimum is equivalent t Pwer s Substance Abuse Prgram (cpy f this prgram is available upn request). Drug and alchl testing is required f subcntractr emplyees in the fllwing situatins: Subcntractr Site Safety Requirements and 6 f 20

7 If there is reasnable suspicin that the individual is under the influence f drugs r alchl (immediate testing required); If the individual has sustained a wrk related injury requiring utside medical attentin (immediate testing required); If the individual has caused r cntributed t anther emplyee being injured in a wrk related incident (immediate testing required); If the individual has caused r cntributed t a wrk related incident resulting in, r which has the ptential t result in, prperty damage (immediate testing required); If the individual was invlved in a near hit, defined as an ccurrence that has the attributes f an incident, yet has n apparent damage t persn r prperty (immediate testing may be required). Subcntractr emplyees wh fail t prvide prf f a required drug and alchl test, refuse the required test r vilate the subcntractr s substance abuse plicy will nt be permitted n Pwer sites. All csts assciated with any substance abuse testing are the respnsibility f the subcntractr. SPECIFIC REQUIREMENTS GENERAL SAFETY REQUIREMENTS Subcntractrs must reprt t the Pwer Prject Team any safety cncerns, bserved cnditins r vilatins f jb safety, regardless f whether they are within the bserver's pwer r respnsibility t crrect. Subcntractrs shall assure that supervisry emplyees have a wrking knwledge f applicable safety requirements as they pertain t their areas, and encurage all emplyees t imprve their accident preventin awareness. Smking is prhibited at any time in any prject field ffice r Pwer ffice. In additin, smking will nt be permitted anywhere within the building in the fllwing situatins: In a build ut r additin t an ccupied building (tie in t existing structure); When the windw installatin has begun; When any temprary r permanent enclsure is installed (partial r cmplete) n the building; When site specific requirements prhibit smking n the prject r the campus; In any ther areas identified as Nn smking n the prject; Any areas cvered by the Illinis Smke Free Law. The use f persnal radi headsets (I pds/mp3 players, etc.) while n site is nt allwed. Radis are nt permitted in wrk areas adjacent t ccupied spaces r when the prject des nt allw their use. Pwer s prject supervisr will determine radi use in stand alne buildings. HOUSEKEEPING Each subcntractr shall be respnsible fr daily and cntinual clean up during and upn cmpletin f wrk activities and shall leave the wrk areas brm swept. In additin, the fllwing items are required as applicable: Subcntractr Site Safety Requirements and 7 f 20

8 Subcntractr is respnsible t cmply with the City f Chicag Husekeeping Ordinance, when applicable. Subcntractr is respnsible fr dust eliminatin/minimizatin during cnstructin and the clean up prcess. Subcntractrs are respnsible fr assuring that trash and debris remain ut f the wrk areas. Subcntractrs shall mnitr their wrk areas daily r mre frequently if needed t assure that all debris is remved frm the wrk area t minimize hazards. Each subcntractr is respnsible t prvide resurces t mve their trash and debris t an area designated by Pwer. If debris is nt remved n a timely basis, r after apprpriate warning, Pwer will prvide resurces t remve the debris and the respnsible subcntractr will be respnsible fr csts incurred. Grinding, sawing, sanding and cutting peratins shuld utilize dustless tl systems where airbrne particulates becme present due t their wrk and ther dust cntrl methds are nt feasible. Effective husekeeping shuld nt be an assigned task, it is cnsidered t be a part f each emplyee's respnsibility. Keeping the wrk site clean nt nly prduces a safer jb site, but a better place t cme t wrk each day. It is each emplyee's respnsibility t keep their wrk area in rder, cleaning up during and after wrk is cmpleted. PERSONAL PROTECTIVE EQUIPMENT Each subcntractr is slely respnsible t supply their emplyees with the required Persnal Prtective Equipment. Eye and Face Prtectin All emplyees shall wear safety glasses 100% f the time while n the cnstructin site. Minimum eye prtectin shall include apprved safety glasses with side shields which meet the standards specified in ANSI Z (this shall als include prescriptin eyewear). Additinal eye and face prtectin in cmbinatin shall be wrn when: Welding, burning r cutting with trches; Using abrasive wheels, prtable grinders; Drilling, cutting r chipping at r abve shulder level; Chipping cncrete, stne r metal; Wrking with any materials subject t scaling, flaking r chipping; Drilling r wrking under dusty cnditins; Using explsive actuated fastening r nailing tls; Wrking with cmpressed air r ther tls; Using masnry saw; Carrying material that culd splash; Additinal PPE as determined by the prject specific requirements, Daily Huddle and/r Jb Hazard Analysis. Head Prtectin All emplyees, vendrs, delivery persnnel and ther visitrs t the prject shall wear hard hats that meet ANSI Z , 100% f the time while n the cnstructin site (including while wearing face shields, welding helmets, etc.). Hard hats shuld be wrn facing frward, with the Subcntractr Site Safety Requirements and 8 f 20

9 bill in frnt (unless manufacturer permits the hardhat t be wrn with bill facing back and situatin dictates the hard hat t be wrn with the bill facing back). Ft Prtectin All persnnel n the cnstructin site shall wear hard sled wrk shes. N ne is permitted t wear sneakers, tennis shes r athletic shes f any type, sandals, high heels r pen ted shes n the cnstructin site. Additinal ft prtectin may be required depending n wrk activity as dictated by subcntractr s JHA. Respiratry Prtectin If respiratry prtectin is required t prtect emplyees frm expsures t airbrne cntaminants, subcntractrs must adhere t the fllwing: Cmpliance with the applicable standards (respiratry, specific cntaminants, etc.); Annual medical clearance fr emplyee; Annual fit testing fr emplyee; Training fr emplyee n respiratr; Written respiratry prtectin prgram; Validatin f respiratr selectin based n sampling r histrical data. Hearing Prtectin Hearing prtectin is required in accrdance with OSHA standards. Clthing Subcntractr emplyees must meet the fllwing as it relates t clthing n Pwer prjects: Lng pants and shirts that are free f damage, inapprpriate wrding r symbls and cver the shulders and mid sectin; High visibility material when expsed t vehicles and equipment traffic; Flaggers must wear the apprpriate reflective vests when flagging traffic; Hand prtectin as required by jb hazard analysis Prtective clthing as required based n the expsure. Any ther additinal PPE in accrdance with wrk activities r jb hazard analysis. FALL PROTECTION AND ACCESS REQUIREMENTS As part f the planning prcess, subcntractrs are required t identify the means necessary t safely access the wrk areas included in their scpe f wrk. These methds, including fall prtectin measures, shuld be identified in site specific safety and health plans, Jb Hazard Analysis, Daily Huddles and ther dcumentatin. Overhead Prtectin Subcntractr is respnsible fr identifying verhead prtectin and falling bject preventin and prviding prtective measures fr prtecting the wrkers n the jb as well as the public. Overhead prtectin must be in place prir t the start f any verhead wrk that expses any emplyee r members f the general public t falling bjects such as tls, equipment, material, debris (including ht wrk), etc. n the prject r in the vicinity. Certain prtective measures shall include: Subcntractr Site Safety Requirements and 9 f 20

10 String material at least 10 away frm the building perimeter and 6 frm interir penings Secure tls and materials t prevent them frm falling during wrk and while nt n site Use restricted access znes, te bards, screens, nets, canpies r catch platfrms t catch r deflect falling bjects Ladders and Stairways The building frame subcntractr shuld prvide access t the building thrugh the fllwing as sn as reasnably pssible: Minimize the use f ladders fr temprary access; Install the permanent access systems (i.e. stairs) within 2 flrs f the wrking flr; Install temprary stair twer r similar methds when permanent systems cannt be installed r cmpleted; Prvide inspectin, maintenance and repair f all access systems. All ladders and stairways shall be inspected, cnstructed and used in accrdance with 29 CFR 1926 Subpart X, applicable ANSI requirements and lcal cdes. In additin, the fllwing items are required as applicable: Prtable extensin, straight and typical step ladders cnstructed f cnductive materials (metal) are nt permitted (drywall step ladders and trench bx ladders may be used if n electrical hazard is present); Jb made ladders must meet ANSI standards; Each subcntractr shall prvide ladder training fr all emplyees using ladders; Stairs and platfrms must be used t prvide access t ffice, equipment and material strage trailers; Stairways must be kept free f flammable materials, stred materials r debris. Scafflds All scafflds shall be erected, used, and dismantled in accrdance with 29 CFR 1926 Subpart L and lcal cdes (i.e. City f Chicag Scaffld Ordinance). In additin, the fllwing items are required as applicable: Dcumentatin f training is required fr the use f any scaffld as determined by the City f Chicag and/r OSHA. This dcumentatin must be submitted t Pwer prject management upn request. The designated cmpetent persn must cmplete the scaffld inspectin prir t allwing wrkers n the scaffld each shift, and submit any dcumentatin t Pwer at the end f each shift. Scafflds must be erected in accrdance with manufacturer s specificatins and requirements. Hallway / Narrw Frame (Baker scafflds) with a wrking deck abve 6, shall adhere t the fllwing: Fall prtectin required (i.e. guardrails); Outriggers installed; Wheels lcked t prevent mvement when ccupied; Prper access. The wrking level f a scaffld shall be fully decked r additinal fall prtectin is required. Subcntractr Site Safety Requirements and 10 f 20

11 Subcntractrs using scafflds shall adequately guard, barricade r prtect areas lcated belw the scaffld. Aerial Lifts All aerial lifts (scissr r bm) shall be used in accrdance with OSHA standards, ANSI requirements and manufacturer recmmendatins. In additin, the fllwing items are required as applicable: Any emplyee wh is perating r using an aerial lift (scissr r bm) shall be prperly trained in the peratin, use, and emergency prcedures invlved with the lift. Dcumentatin shall be made available t Pwer upn request. Emplyees shall be crrectly fall prtected in bm lifts (i.e. fall restraint). Emplyees wrking in aerial lifts must remain n the platfrm f the lift in accrdance with OSHA requirements and interpretatins. Fall Prtectin and Preventin All individuals expsed t fall hazards greater than 6 abve a lwer level shall be prtected by means f fall preventin r fall prtectin devices. Where fall hazards cannt be eliminated, 100% cntinuus fall prtectin fr fall hazards greater that six (6 ) feet shall be implemented. Subcntractrs are slely respnsible fr determining the methds used fr fall prtectin, installatin f methds, inspectin and maintenance f methds; and training emplyees n the fall prtectin methds. In additin, the fllwing items and scenaris are included as part f this requirement: Emplyees wrking abve a guardrail must utilize fall prtectin; Emplyees utilizing a ladder in accrdance with applicable requirements are nt required t maintain additinal fall prtectin; Subcntractrs invlved in rfing peratins must determine the apprpriate fall prtectin methd fr their peratin based n feasibility; Each Subcntractr shall be respnsible fr utlining specific means and methds fr meeting fall prtectin requirements in their Jb Hazard Analysis. The JHA shall detail in writing when fall prtectin is required, what equipment and systems will be utilized, the training users f the system have received, and hw this prtectin is t be installed and used. Prvisins fr prmpt rescue shall als be prvided. All ther elements f the 29 CFR 1926 Subpart M are applicable t situatins requiring fall prtectin. In additin, the fllwing items are required as applicable: When cable is used as the guardrail system, the system will be 3/8 IWRC galvanized cables (tprail and midrail), with a minimum f tw wire rpe clamps at each cnnectin pint. If mesh is used n this guardrail, a third cable (minimum 1/4 IWRC galvanized cable) will be installed at the tp f slab. The subcntractr will wrk with Pwer t determine the required type f guardrail system t remain n the prject upn subcntractr s cmpletin. Tarps and ther prtective systems are nt t be secured t the tprail r midrail cables. If these devices are attached t the bttm cable, the subcntractr is slely respnsible fr determining if the cable is adequate fr supprting the lads impsed by the subcntractr s devices. Guardrail cables and guard rail systems are nt t be used as attachment pints fr fall arrest r restraint unless the subcntractr accepts the respnsibility fr the design, installatin, inspectin and all ther applicable requirements. If a subcntractr needs t remve a guardrail, the subcntractr is required t btain a Guardrail Remval Permit frm Pwer. The subcntractr is slely respnsible fr implementing the temprary measures used t prtect their wn emplyees and thers Subcntractr Site Safety Requirements and 11 f 20

12 wrking in the surrunding areas. The subcntractr is respnsible fr strict adherence t the cnditins f the permit. Subcntractr wrk that necessitates the use f either "Cntrlled Access Znes" r a "Fall Prtectin Plan" are required t submit thse plans t Pwer prir t beginning wrk (fr recrdkeeping purpses). The subcntractr is slely respnsible fr determining feasibility f fall prtectin systems and methds. When using a mnitring system, the mnitr will wear a reflective vest, s the mnitr can be readily identified. Trip and impalement hazards created by subcntractr s installatin must be cntrlled and eliminated by the subcntractr creating the expsure. The fllwing are examples f trip and impalement hazards that must be addressed: Electrical cnduits pre and pst cncrete placement (eliminated when pssible); Plumbing cnduits that create a trip and impalement hazard prtected pre and pst cncrete placement (eliminated when pssible); Other mechanical cnduits and sleeves that create a trip and impalement hazard prtected preand pst cncrete placement (eliminated when pssible); All hles greater than 2 diameter prtected with cvers mechanically secured. ELECTRICAL SAFETY Each subcntractr is respnsible fr inspecting their hand and pwer tls and electric crds prir t each use. Damaged equipment shall be remved frm service. Temprary Electric and Lighting: Installatin f temprary electric and lighting must cmply with 29 CFR 1926 Subpart K. The electrical subcntractr must prvide a Temprary Pwer and Lighting plan as required fr each prject. This plan shuld include, at minimum, the fllwing items: Temprary pwer design and supply in accrdance with OSHA and ther applicable requirements (utlets within 50 f wrk areas); Temprary lighting design and supply in accrdance with OSHA and ther applicable requirements (including lighting cntrl fr ff hurs, lighting efficiency, etc.); Inspectin f temprary systems; Maintenance f temprary systems; Methds fr prtectin, securing and labeling f temprary systems; Emergency cntacts fr managing temprary systems. The electrical subcntractr is slely respnsible fr the design, installatin, inspectin, maintenance and repair f the temprary electric and lighting system. Any items fund t be nn cmpliant r nn functining must be repaired immediately. All elements f the temprary electrical system, including GFCI breakers and utlets, wiring, panels, and ther equipment, must be tested prir t installatin and peridically by the electrical subcntractr. The electrical cntractr will maintain the recrd f the tests and prvide t Pwer n a bi weekly (every ther week) basis. Temprary electric must be prtected by a grund fault circuit interrupter (GFCI) at the surce f pwer and cver plate fr bth temprary and permanent utlets. The electrical subcntractr respnsible fr temprary electric panels must pst at each panel the name and cntact infrmatin fr the individual identified t address temprary pwer issues. 13 Temprary lighting must be prvided at minimum in accrdance with OSHA and/r additinal cntract requirements. Any additinal lighting (task lighting) required will be prvided by each subcntractr. Open cnductrs must be prtected in cnduit, bxes r equivalent when within 8 feet f a walking surface. Wire fr lighting within stairwells must be prtected by cnduit r wiring design. Unprtected pen cnductrs are nt permitted. The electrical subcntractr must supply methds fr electrical crd management t avid creating a trip hazard r in areas where the crd will be damaged. Suspended crds must be secured with a nn cnductive material. All live circuit panels must have an OSHA cmpliant panel cver installed. Energized panels are nt t be left unattended r unprtected in accrdance with NFPA 70E. Panels that are turned ff must be prperly lcked and tagged. Whenever wrk is t be dne n a piece f equipment, including building equipment and r subcntractrs equipment, OSHA s Cntrl f Hazardus Energy standard must be fllwed. Each cntractr whse wrk required wrking n energized equipment shall submit a plan in accrdance with NFPA 70 E r a Lck ut/tag ut prgram t Pwer prir t perfrming wrk. Wrk n energized equipment shuld be dne under a lck ut/tag ut cntrl whenever pssible. Wrk perfrmed in prximity t verhead utilities Wrk perfrmed within prximity t verhead pwerlines must be cmpleted in accrdance with Pwer s Utility Lcate and Overhead Pwerline Plicy and as well as 29 CFR and Prir t the start f, and during the curse f any wrk in prximity t verhead utilities, the subcntractr shall make a thrugh survey f the entire wrk site t determine the type and lcatin f all utilities n the wrk site. The subcntractr must verify this infrmatin with Pwer by ntifying the Prject supervisr and shall crdinate cnstructin wrk in the vicinity f these utilities with the apprpriate utility wner. The subcntractr shall make emplyees aware f any precautins and prcedures t be fllwed while wrking in the prximity f any utility. Apprpriate clearance distances shall be maintained thrughut the curse f the wrk. If the subcntractr cannt maintain apprpriate clearances they must cntact the invlved utility and discuss alternative methds fr addressing the utility and assciated hazards. The subcntractr will be required t investigate any and all cntingencies where cntacting a utility culd adversely affect any peratin r render inperative any prtective apparatus in the surrunding area and submit a plan fr prtectin r reruting f critical systems. This plan shall be turned int Pwer prir t perfrming wrk. FIRE PROTECTION AND HOT WORK PROCEDURES All welding and burning wrk shall be dne in accrdance with OSHA standards and industry best practices. Prir t cnducting any activity that generates sparks, heat, flame r ther ht wrk, a Ht Wrk Permit must be btained frm Pwer. The subcntractr is slely respnsible fr strict adherence t the requirements utlined in the permit and fr prviding fire extinguishers fr all types f Ht Wrk. 14 The Fire Watch Criteria will dictate the type f Fire Watch required fr the Ht Wrk peratins: i) Facility requirements, active fire prtectin systems near the wrk, adjacent shafts/rfs/penetratins and/r nearby flammables/cmbustibles will require that an independent Fire Watch be present during and fr a minimum f 60 minutes after cmpletin f the wrk prvided by the subcntractr. ii) Ht Wrk perfrmed in a cavity r enclsure that is nt cmpletely visible will require that the area is mnitred fr 60 minutes after cmpletin f the wrk by the subcntractr. iii) Any ther types f Ht Wrk may nt require a Fire Watch (including flring, unless required by a site specific plicy). This will be crdinated with a Pwer representative nce the permit is pened. The Ht Wrk Permit is valid fr a single shift, pened prir t the start f Ht Wrk and clsed upn cmpletin f the Fire Watch. At minimum, a charged and inspected 10 pund dry chemical ABC fire extinguisher must be within 20 feet f any ht wrk peratin. Each subcntractr is t prvide fire extinguishers apprpriate fr the wrk they are perfrming. The area surrunding the ht wrk peratin (minimum 35 radius) must be cleaned f flammables and cmbustibles prir t the start f the ht wrk peratin. Items that cannt be remved frm the area must be cvered with flame retardant materials. The mvement, strage, and use f cylinders shall be dne in accrdance with OSHA standards. Cmpressed gases and flammable liquids will nt be stred within enclsed structures (i.e. buildings under cnstructin, strage trailers; tl sheds, in stairways r building exits/entrances). All persnnel using gas welding r burning equipment will be fully trained in the use and maintenance f the equipment. All cntainers must be FM apprved r UL listed. The cntainer must have a self clsing lid and a wire mesh flame arrester. If the can is damaged, it is t be remved frm site. In accrdance with OSHA s Hazard Cmmunicatin Standard, cntainers will be clearly marked shwing the cntents, hazard level and any special use r handling requirements. Fire extinguishers which are prvided by Pwer are available fr general use. They are generally lcated at entrances, stair wells, and n each flr. If a fire extinguisher is used, return it immediately t the prject trailer t replace it with a charged extinguisher. CRANES AND RIGGING Cranes All crane and histing peratins, including rigging, must be cmpleted in accrdance with 29 CFR 1926 Subpart CC and Subpart H. All lift service cranes shall have anti tw blck devices and lad mment indicatrs (LMI) installed and functining prperly at all times during peratins. Pick and carry peratins with rubber tire mbile cranes is nt permitted unless specifically permitted by the crane manufacturer and a Jb Hazard Analysis is created and fllwed by the subcntractr. All histing classified as a Critical Lift requires additinal planning and must be reviewed by the Pwer prject team. A critical lift is defined as a lift that exceeds 75% f the rated capacity f the crane, multiple crane picks and/r high risk lifts. A minimum lift plan must include: Subcntractr Site Safety Requirements and 14 f 20

15 Cmpleted Pwer Critical Lift frm r equivalent; Lad analysis; Lifting prcedures; Rigging analysis. A third party inspectin is required fr all cranes n an annual basis. Pwer s Mbile Crane Checklist must be cmpleted by a cmpetent persn n a daily basis and prir t perating a mbile crane at the prject. The subcntractr in charge f the crane shall ensure that the capacity, grund cnditins, and all ther cnditins assciated with the crane are acceptable. If the cnditins are nt acceptable, the subcntractr shall ntify Pwer in writing their prpsal t implement any crrectins r mdificatins necessary. Pwer s Twer Crane Checklist must be cmpleted by the subcntractr prir t erecting the twer crane; thrughut the duratin f twer crane use at the prject; jumping the twer crane; and prir t dismantling the twer crane. The subcntractr respnsible fr prviding the twer crane must prvide the fllwing: Cmpliance with FAA requirements including all determinatins, ntices and required elements; Cncrete barriers arund the twer crane base as required by the City f Chicag; A physical enclsure designed t prhibit access t the twer crane by unauthrized individuals; Cmmunicatin systems as required by the authrity having jurisdictin; A third party inspectin f the crane upn cmpletin f the installatin and annually thereafter; Cpy f said third party inspectins; Ntice f re trque cmpletin and any ther required maintenance; Cpy f settlement readings fr the twer crane and base; Cpy f any ther infrmatin required by the authrity having jurisdictin. The fllwing are additinal items regarding crane use: The swing radius f the crane must be barricaded r therwise guarded; Only ne persn is t signal the crane peratr at a time (hand signals, radi, hard line, etc.); Lads shall be cntrlled with a nn cnductive tag line, unless the use f the tagline wuld pse a greater hazard; Cell phne use is prhibited while perating a crane (cell phne r 2 way phne is nt permitted fr crane signaling); Subcntractr is respnsible fr submitting prf f training t Pwer fr qualified rigger(s), signal persn(s) and crane peratr(s). Cranes perating within the limits f the city f Chicag must cmply with the Rules Regarding Crane Operatins and peratrs be licensed in accrdance with municipal rdinances set frth by the city. Rigging and material handling Each subcntractr is respnsible fr cmplying with rigging requirements set frth by OSHA, ASME and rigging manufacturers. Rigging equipment must nt be used beynd its rated capacity. Rigging shall be inspected prir t use and as necessary thrughut the curse f the day by the subcntractr s Qualified Rigger. If any rigging is fund t be damaged, it shall be remved frm service immediately. Subcntractr Site Safety Requirements and 15 f 20

16 Stred materials are t be stacked neat and rderly. Materials must be stacked in a manner t prevent tipping, falling, shifting r rlling and maintain clear paths f travel. Material shuld be stred in a manner that allws fr ease f mvement (i.e. n carts r pallets) and minimizes the amunt f frce and persnnel handling the materials. EQUIPMENT REQUIREMENTS All self prpelled cnstructin equipment shall be maintained, equipped, inspected and perated in accrdance with all OSHA and manufacturers requirements (subcntractr is slely respnsible fr implementing these prvisins). Seatbelts must be wrn in equipment when required by OSHA, the manufacturer r when the equipment is equipped with a rll ver prtective structure (ROPS). Cell phne use is prhibited while perating a piece f equipment. Persnnel shall nt be transprted r ride n equipment r vehicles that are nt equipped with seats fr passengers. Riding in the back f pickup trucks r n equipment withut the use f a seatbelt is prhibited. Bi directinal earthmving equipment and mtr vehicles with an bstructed view t the rear shall be equipped with a functining warning hrn and an autmatic back up alarm. Equipment wrking in the street must als be equipped with a functining beacn light and slw mving vehicle symbl. Subcntractrs using equipment n the prject with the ptential fr leaks r spills, with the ptential t create an envirnmental impact, must maintain measures n site t cntrl and dispse f said materials in the event f a release. Frklifts Only trained and authrized persnnel will be allwed t psitin r perate any type f pwered industrial truck (frklift). These individuals shall perfrm pre peratinal inspectins prir t each shift. Dcumentatin shall be made available t Pwer upn request. Frklifts shall be perated in accrdance with the manufacturer s specificatins and requirements. STEEL ERECTION All steel erectin activities shall be in cmpliance with 29 CFR 1926 Subpart R (with the exceptin f fall prtectin). A written site specific erectin plan (t include fall prtectin) shall be submitted t Pwer prir t the start f wrk. When special r unusual hazards will be encuntered (i.e. wrk ver existing structures, near utilities r water), the subcntractr will clearly address these issues in the site specific erectin safety plan. In additin, the subcntractr shuld utline prvisins fr the fllwing: Plan fr accessing wrk including delivery trailers; Plan fr securing items verhead; Plan fr cntrlling access int the erectin area; Fall prtectin plan including rescue prcedures. Pwer s Authrizatin t Prceed with Steel Erectin frm must be cmpleted with the steel erectr prir t the cmmencement f steel erectin. Multiple lifts f structural members must be dne in accrdance with Subpart R and crane manufacturer s requirements. The maximum number f allwed pieces per lift is five. 17 If wrk is perfrmed within r adjacent t ccupied structures, the subcntractr will be required t make prvisins fr fire prtectin, and the safe remval f all welding fumes frm the building. The methds shall be submitted t Pwer prir t the start f the wrk and will be the subcntractr s respnsibility t implement and manage. EXCAVATION WORK All excavatin wrk will cnfrm t Pwer s Utility Lcate and Overhead Pwerline Plicy and and the requirements f 29 CFR 1926 Subpart P (cpy f plicy is available upn request). N excavatin shall cmmence n site, regardless f size, depth, r equipment used unless the peratin has been authrized by Pwer thrugh an Excavatin Permit prcedure t ensure the dig area has been evaluated and marked fr existing utilities, and that adequate cave in prtectin is prvided as required. Each subcntractr is respnsible fr ne call ntificatins, btaining their wn dig authrizatin number and required t prvide and crdinate secndary independent lcates. All investigatin and assciated excavating will be dne in accrdance with Pwer s Utility Lcate and Overhead Pwerline Plicy and. Each subcntractr is respnsible fr barricading and/r cvering any excavatin at the end fshift, as warranted by the size and expsure f said excavatin. Each subcntractr engaged in excavatin wrk must have a persn designated as the "Cmpetent Persn" (as defined by OSHA) t ensure cmpliance with the OSHA standard fr excavatins. The name f that persn and their qualificatins will be prvided t Pwer befre wrk begins. Atmspheric testing must be perfrmed in excavatins 4 r deeper that are near hazardus areas r pse a pssibility fr an envirnmental threat (e.g. landfills, near gas pipes, areas with cntaminated sil, etc.). The Cmpetent Persn makes the determinatin if testing is required. Excavatins deemed hazardus thrugh testing and/r sil analysis will require emergency planning (i.e. rescue plan, emergency equipment). CONCRETE AND MASONRY All cncrete, masnry r ther dust/silica generating prcesses must be dne with wet methds r ther engineering and hazard cntrls. Grinding silica will be cnducted as t nt expse surrunding emplyees and existing facilities t elevated levels f silica and respirable dust. All prgram, training and hazard recgnitin and cntrl methds required by OSHA r ther entities must be implemented by the subcntractr creating the dust related expsure. Cncrete subcntractrs (r masnry subcntractr when applicable) are respnsible fr ntifying Pwer prject team f any changes r mdificatins t anchr blts and any issues related t cncrete strength r perfrmance. The fllwing items must be addressed fr cncrete peratins as part f the subcntractr s site specific safety and health plan: Falling bject and verhead prtectin including cntrl f perimeter shres and shres arund interir penings; Falling bject, verhead and fall prtectin methds fr cncrete cre frm peratins; Falling bject, verhead and fall prtectin methds fr cncrete decking, stripping and ther leading edge peratins; Pst tensin stressing peratin hazard cntrl plan including access, restricted access znes, ht wrk, falling bject and verhead prtectin, etc.; Subcntractr Site Safety Requirements and 17 f 20

18 Rebar and impalement prtectin plan; Cncrete remedial wrk hazards (i.e. silica); Hygiene and burn preventin prcedures; Strage and cntrl f materials; Deck and flr lading requirements; Crane, placing bm and ther equipment peratins; Site access planning, methds and cntrl including deck access; Lkuts and ther platfrms; All ther prject specific cncrete expsures. TRAFFIC AND PEDESTRIAN CONTROL Subcntractrs whse wrk impacts pedestrian and vehicle traffic must supply Pwer with a written Traffic and Pedestrian Cntrl Plan prir t starting this wrk. Signs shall cnfrm t the requirements f 29 CFR and ANSI Z Wrk n r adjacent t radways must be cnducted in accrdance with Illinis Statutes and the current versin f the Manual f Unifrm Traffic Cntrl Devices (MUTCD). IDOT Certified Flagger cntrl must be prvided in accrdance with Illinis Revised Statutes Chapter 121, Par Sufficient chain link fencing r range "barricade" fencing must be installed as apprpriate t separate active cnstructin areas r hazardus areas frm active ccupied wrk areas and t prtect the public. The fence must be sufficiently supprted and marked with apprpriate signage. Signs and barricades must be remved immediately when n lnger applicable r required. 19 Appendix A CCIP Safety Prgram Requirements SAFETY EDUCATION WORKSHOPS Subcntractr principals, Prject Managers, Superintendents, Safety Representatives and Site Supervisrs are required t attend a CCIP and safety expectatin wrkshp prir t cmmencing wrk at the prject. The purpse f the wrkshp is t prvide a clear understanding f the safety and risk management expectatins fr the prject, review the insurance prtcls fr the prject and address any cncerns in advance f said subcntractr s mbilizatin. Wrk will nt cmmence until a precnstructin meeting has ccurred at the prject with said subcntractr. INCIDENT REPORTING AND RETURN TO WORK PROGRAM Subcntractrs must reprt all injuries, ccupatinal related illnesses, prperty damage and ther prject related incidents t a Pwer supervisr immediately. All subcntractrs must prvide a written reprt with facts frm the incident investigatin t the Prject Safety Supervisr within 24 hurs. Failure t reprt a claim immediately and r t submit written reprt within 24 hurs f an ccurrence may result in a $5000 penalty per ccurrence. All subcntractrs understand that having a cmprehensive and effective return t wrk prgram is a precursr t bidding the prject. The return t wrk prgram must be able t immediately accmmdate any and all reasnable medical restrictins that ccur as a result f incident n the prject. Only cntractr emplyees invlved in an incident n the Pwer specific prject are eligible fr return t wrk / "light duty" restrictins n the prject; On an as needed basis, cntractrs may be required t identify, in writing, ptential return twrk jb tasks with crrespnding physical requirements that can be reviewed by a treating physician; Return t wrk crdinatin with Pwer, the carrier and all ther applicable parties is required as sn as reasnably pssible after an incident. Each subcntractr is required t prvide prvisins t accmmdate restricted duty fr wrk related injuries and illnesses as part f a cmprehensive Return t Wrk Prgram. Subcntractrs wh fail t accmmdate restricted duty will, at their cst, be subject t ne r mre f the fllwing (per ccurrence): Utilize a placement service fr restricted duty accmmdatins in crdinatin with Pwer and its insurers. Be respnsible fr all csts assciated with temprary ttal disability payments fr wrkers n restricted duty plus administratin cst. Subject t terms in accrdance with the subcntract agreement.
