1 ONEOK Cnstructin Prject s Cntractr ESH Expectatins At ONEOK, we are cmmitted t pursuing a zer-incident culture by cntinuusly wrking twards mitigating risk and eliminating incidents that may bring harm t ur emplyees, cntractrs, the public and the envirnment. Building and maintaining ur assets utilizing a diverse wrkfrce f bth Cntractrs and emplyees requires clear cmmunicatin between all parties regarding ESH expectatins. ONEOK strives t hire qualified, respnsible Cntractrs t help us execute ur prjects, and per the terms f ur Cntractr agreements, all Cntractr wrk peratins are required t be cmpliant with all applicable ESH laws and regulatins and execute n the fllwing expectatins. REQUIRED CONTRACTOR WORK PRACTICES The cntractr must have had its S&H prgrams evaluated and apprved by ISNetwrld fr cmpliance and adequacy against ONEOK S&H expectatins. ONEOK expects that all Cntractr(s) and any hired subcntractr(s) t strictly abide by the Cntractrs previusly apprved S&H prgrams and assciated wrk practices. Required Cntractr S&H Prgrams and/r prcess shuld include the fllwing as applicable t the scpe f wrk: Abrasive Blasting Bldbrne Pathgens Cnfined Space Entry Crane & Lifting Equipment Operatins Discipline Excavatin Trenching & Shring Excavatin Cmpetent Persn Training Drug & Alchl Electrical Safety Fall Prtectin Fire Prtectin/ Prtable Fire Extinguishers First-Aid/CPR Ladder Safety Lckut-Tagut Hearing Cnservatin Persnal Prtective Equipment Respiratry Prtectin Rigging & Material Handling Scafflds Welding, Cutting, and Ht Wrk Equipment Inspectin Heavy Equipment Operatin Heavy Equipment Maintenance Prgram Undergrund & Overhead Pwer Line Safety Hand and Pwer Tls Traffic Cntrl and Flagging (Cmply with Hazard Cmmunicatin MUTCD n public rad ways) There may be prtins f prject activities that take place n r near existing ONEOK perating assets. In these instances, the Cntractr may be required t bserve ONEOK business unit specific ESH Prgram affiliated with the perating asset such as Wrk Authrizatin Permitting r equivalent, excavatin, prcess safety management (PSM), and thers. These applicable ESH Prgrams/Prcesses will be identified and cmmunicated t the Cntractr in the bidding and/r pre-wrk prcess. As a pint f emphasis and fcus regarding Cntractr S&H wrk practices, the fllwing S&H Abslutes highlight sme f the areas f specific S&H wrk practice requirements: A cmpetent persn fr excavatins will inspect all excavatins t be ccupied that day (greater than 4 feet deep), after each rain, and if cnditins change. Excavatin daily inspectins must be dcumented by the cntractr verseeing the ccupied excavatin and n site fr the duratin f the ccupied excavatin. If there is an incident as defined under the Incident Reprting and Investigatin sectin f these expectatins, the cntractr must prduce this excavatin daily inspectin during the incident investigatin and prvide a cpy in the incident reprt. 2 ONEOK Cnstructin Prject s Cntractr ESH Expectatins (Cn t) All ccupants must wear a seat belt while perating vehicles and equipment with Rll Over Prtectin Structures (ROPS). ANSI apprved hard hats are required n all prject sites (exceptin while welding nly). ANSI apprved safety glasses with side shields are required n all prject sites. Apprpriate level f safety ftwear is required n all prject sites. Face shield and safety glasses are required fr all grinding activities. Apprpriate welding helmets and cutting gggles are required (Nte: safety glasses are nt cutting gggles). Flame Resistant Clthing (FRC) requirements and areas requiring FRC must be identified prir t wrk activities. All individuals will dn the required FRC prir t entering the identified areas. Nte: Where required, flame resistant clthing shall have a minimum arc rating f 8 cal/cm 2 (FRC Categry 2) Flame-resistant clthing can include ne-piece cveralls r pants and shirt. All flame- resistant clthing shall have lng sleeves. Nncnductive tag lines are required fr lifting equipment, pipe, and supplies. D nt leave suspended lads unattended and/r under stress. Unattended equipment/vehicles shall nt be left running. Hses shuld be prperly secured t prevent whipping in the event f failure. Sitting, walking r standing n tp f the pipe is prhibited. This desn t apply t individuals prperly dnning and tying ff with fall prtectin equipment in a facility pipe rack. The use f telephnes and ther electrnic devices are prhibited while perating equipment (e.g. crane, backhe, track he, truck, dzer, trencher vehicles and etc.). D nt fuel equipment r vehicles n public radways. Firearms, alchlic beverages, and illegal drugs are strictly prhibited. Dgs and ther pets are prhibited n the prject site. Overhead pwer line barricades must be installed and maintained at all times when there is a risk f cntacting an energized pwer line. D nt exceed the right-f-way speed and/r bundaries. N hrse play r fighting. All incidents regardless f severity must be reprted. Incidents include any injury, near misses, significant events, prperty damage, spill and/r releases. Utilize equipment sptters t reduce the risk f an incident in cngested areas, tight areas, inclement weather, adverse rad cnditins, and thers as necessary. As a pint f emphasis and fcus regarding Cntractr envirnmental wrk practices, the fllwing Envirnmental Best Management Practices highlight sme f the areas f specific envirnmental wrk practice requirements and are the minimums t be implemented n every prject t avid and mitigate envirnmental risks assciated with prject activities: Cnfine all prject activities t the designated, apprved wrk areas. Stre and dispense all hazardus and petrleum prducts at least 100 feet frm wetland and water bdies bundaries, and 150 feet frm wells. Frequently inspect tanks and equipment fr leaks and deteriratin. Leaking tanks and equipment must be immediately repaired r remved frm the wrk site. Maintain spill kits in readily accessible lcatin(s) as apprpriate fr each prject site. In the event f a spill r release, immediately stp the surce and cntain the extent f the spill r release, ding s in a manner that cnsiders persnal and public safety. Immediately reprt all spills t apprpriate prject persnnel and ONEOK persnnel. Equipment must be clean and free f tracked and trapped materials including sils and vegetatin prir t entering each new prject site. OKSesh2.301ADD (V1) - Cntractr Expectatins 2 P age

3 ONEOK Cnstructin Prject s Cntractr ESH Expectatins (Cn t) Where pssible, minimize clearing and grading t the area needed fr safe pipeline installatin and repair activities. Segregate and preserve tpsil as required by prject permit cnditins r landwner requirement. Install and maintain sediment and ersin cntrl measures prir t r immediately after grund disturbance t prevent discharges t wetlands, water bdies and drainage channels. As applicable, cntinue t maintain cntrls until disturbed areas are stabilized. Maintain clean and safe cnditins at all public rads (e.g. remved tracked mud, vehicle parking, equipment crssing, etc.) Where applicable, utilize cnstructin mats t minimize rutting and sil-mixing in wetlands. Direct water discharges t well-vegetated upland areas, if available, r discharge t getextile filter bags, straw bale dewatering structures, r equivalent energy dispersin and sediment retentin devices in accrdance with applicable permits and/r apprvals. Keep hse inlets ff the trench bttm. As applicable, restre disturbed areas as sn as pssible upn cmpletin f prject activity. Respread segregated tpsil during final grading. All waste must be prperly cntained, cllected, transprted and dispsed. If any histrical r cultural resurces (e.g., arrw pints, artifacts, bnes r human remains) are uncvered during prject, immediately suspend all activity in the area and establish a minimum 100 ft buffer t islate the area frm further disturbance. Immediately ntify the apprpriate prject persnnel f the discvery, including the ONEOK Envirnment Prject Manager in rder t receive further instructin. Fllwing ntificatin, prject activity may resume utside the 100 ft (r greater as necessary) buffer arund the discvery. If prject activity encunters r ptentially impacts any species prtected under an applicable envirnmental law, such as the Endangered Species Act r Migratry Bird Treaty Act, immediately suspend prject activity in the area and cntact apprpriate prject persnnel, including ONEOK ESH persnnel. Envirnmental prtectin is everyne s respnsibility. PROJECT MEETINGS Cntractr management representative, ESH and key persnnel will participate in a Prject Kickff Meeting as scheduled by ONEOK befre executin f field based tasks (e.g. right f way staking, envirnmental surveys, right f way clearing, trenching, pipeline/plant cnstructins, and etc.) assciated with the cnstructin f an asset. Meeting items might include: ONEOK ESH Management System ONEOK Partner s Operating Cmmitment & Key Drivers Prject Overview Cmpany s ESH requirements and expectatins fr the wrk Safety Abslutes and review f majr prject ESH hazards Envirnmental Best Practices Prject specific permit cnditins and plans (e.g. Cntractr Safety and Health Management Plan, Air Permit Apprvals, Spill Preventins Cntainment and Cuntermeasure Plan (SPCC), Strm Water Pllutin Preventin Plan (SWPPP), Emergency Plan, hydrstatic testing discharge permits/apprvals, endangered species and/r migratry bird act mitigatin plans) Plans fr briefing f Cntractrs/Subcntractrs wh were nt able t attend the meetings OKSesh2.301ADD (V1) - Cntractr Expectatins 3 P age

4 ONEOK Cnstructin Prject s Cntractr ESH Expectatins (Cn t) Incident reprting and investigatin requirements including methds f cmmunicatin and expectatins Prcesses and/r prgrams related t specific wrk activities Cntractr management representative, ESH and key persnnel will participate in Weekly Fllw-up Meeting(s) as scheduled by ONEOK t review any changes t the prject scpe and any new cnditins, prject status, ESH Key Perfrmance Indicatrs (KPI), ESH incidents and cmpliance, inspectin/audit results, lessns learned, utilizatin f prcesses/prgrams related t upcming wrk activities, and ther items as necessary. Cntractr management representative, ESH and key persnnel will participate in a Prject Clsure/Final Meeting as scheduled by ONEOK as sn as practical after prject cmpletin t review verall prject perfrmance, ESH perfrmance (KPI), lessns learned, ideas fr imprvements mving frward, and ther items as necessary. ORIENTATIONS Cntractr will ensure that all cntract persnnel have received applicable ESH training pertinent t the task they will perfrm n the prject as reflected in the Cntractr s written ESH Prgram. Dcumentatin f the successful cmpletin f this training may be required t be prduced upn request by ONEOK r designated representative(s) prir t and/r during the prject. Cntractr will ensure that all cntract persnnel, visitrs and bservers wh will access r perfrm wrk n the prject site have received a prject ESH rientatin. The prject ESH rientatin (including PSM Cntractr rientatin when applicable) will be received prir t starting any wrk and dcumented as evidence by the date f delivery and persnnel signatures. Cntractr persnnel will be issued a hard hat sticker, identificatin card r similar prcess that signifies the successful cmpletin f the prject ESH rientatin. Cntactr will immediately require persnnel discvered nt pssessing the dcumentatin that signifies the successful cmpletin f the prject ESH rientatin t leave the prject site. Prject ESH rientatin will cnsist f a thrugh review f the fllwing items as applicable: review f these expectatins, prject specific ESH risks, hazard cmmunicatin infrmatin, lcatin f Safety Data Sheets (SDS), Envirnmental Best Management Practices, and safety abslutes. Prject ESH rientatin sessin will address specific issues, permit cnditins n an as-needed basis, r t satisfy an agency requirement t hld a pre-cnstructin meeting fr the purpses f ntificatin, review and/r training. Cntractr is respnsible fr translating all ESH material t a language that is understd by its persnnel. Cntractr shall cnduct and dcument an ESH meeting at least nce per week in which all prject cntractr persnnel shall participate (attend, listen, remain present). Additinal ESH meetings may be scheduled r required by ONEOK as apprpriate t address additinal risks. Dcumentatin f these ESH meetings shall include: date and time f meeting, name f persn(s) cnducting the meeting, tpics discussed, and signatures r names f attendees. Dcumentatin shall be maintained by the Cntractr and made available immediately t ONEOK Representative(s) upn request fr the duratin f the Prject. As applicable, the fllwing tpic shuld be presented and discussed at these ESH Meetings: Cmmitment t ESH (Leadership, Management, All Persnnel), ESH tpic that is apprpriate fr the safety hazards r envirnmental cnditins/permits present n r in effect fr the prject, review f recent jb site ESH inspectins; review f recent ESH incidents including any required crrective actins, review f ESH alerts/bulletins frm ther prjects, spreads, r activities deemed relevant, and answer questins frm attendees as apprpriate. 5 ONEOK Cnstructin Prject s Cntractr ESH Expectatins (Cn t) INCIDENT REPORTING & INVESTIGATION The Cntractr is respnsible fr reprting and investigating all incidents pertaining t the prject wrk it is perfrming. All jb related incidents invlving wrker injury, envirnmental spill r release, envirnmental nncmpliance issue, prperty damage, r significant near miss shall be reprted immediately by telephne t apprpriate persnnel as designated during the Pre-Wrk Meeting. A written incident reprt and/r STOMP (Safety Track fr ONEOK Majr Prjects) system entry, cntaining the items listed belw are due t the designated ONEOK Representative(s) and ther apprpriate prject persnnel as instructed during Pre-Wrk Meeting and ESH rientatin within 24 hurs and ptentially earlier if required agency ntificatins are t be made in the prescribed regulatry timeframe. All agency ntificatins f reprtable incidents shall be made by the apprpriate ONEOK Representative(s). All reprtable incidents shall be investigated by the Cntractr as sn as pssible t identify ptential causative factrs and implement apprpriate crrective actins. The results f Cntractr s investigatin shall be shared with ONEOK Representative(s) in a timely manner. IMPORTANT NOTE: If the cause f the incident is unknwn, uncnfirmed r unable t be determined cmpletely at the time f the reprt, then there shuld be n speculatin as t cause included in the reprt. Only include thse facts which are knwn and abslute as t the causatin, d nt include and statements f suppsitin, speculatin, r ptential causes. Reprts can be amended later nce the cause f the incident in knwn. Recmmended actin(s) t prevent recurrence f a similar event ESH PERFORMANCE REPORT The fllwing infrmatin shall be entered int STOMP by the Cntractr n a weekly basis: Any updates t previusly reprted wrker ccupatinal injuries, illnesses, first aids, vehicle incidents, envirnmental spills r releases, prperty damage r significant near misses that ccurred n the prject OSHA DART and Lst Time Days Hurs Wrked OSHA recrdable incident rate (prject-t-date) First-Aid case incident rate (prject-t-date) Vehicle Miles Driven Vehicle Preventable Rate and ANSI Recrdable Rate Near Misses A shrt descriptin f any external regulatry inspectins that ccurred n the prject Any deficiencies fund as a result f ESH jb site inspectins perfrmed by either the Cntractr r ONEOK persnnel and the status f any utstanding crrective actins OKSesh2.301ADD (V1) - Cntractr Expectatins 5 P age

Deficiencies fund will be recrded, prper crrective actin taken by the Cntractr and cmmunicated t the designated ONEOK Representative(s). All Cntractr weekly ESH inspectin alng with assciated crrective actin dcumentatin will be stred by the Cntractr n site and subject t review by ONEOK Representative(s) as warranted fr the duratin f the prject. ESH ROLES & RESPONSIBILITIES Cntract Field Management - Cntractr management respnsibilities include: Establish ESH Prgram fr its Cntractr wrkfrce Prmte wrkplace ESH cmmitment and excellence Require cmpliance with the established ESH Prgrams with keen fcus n the S&H Abslutes and Envirnmental Best Management Practices as required per these expectatins. Establish a written prcess fr crssing verhead and undergrund pwer lines Establish a prcess fr cmpleting and updating One-Calls Cnduct weekly ESH meetings fr the Cntractr wrkfrce Cnduct daily tailgate ESH meetings fr the Cntractr wrkfrce Verify jb site ESH inspectins are perfrmed weekly and any crrective actins generated are addressed in a timely manner Crrect unsafe acts and cnditins reprted by the Cntractr wrkfrce Reprt all incidents immediately as required per these expectatins. Remve cntract persnnel, visitrs and Cntractrs frm the prject/wrksite that d nt meet established ESH requirements Cntract Wrkfrce - The Cntractr wrkfrce is all f the wrkers emplyed by the General Prject Cntractr and any Sub-Cntractrs.
