2 STANDARD PROCEDURE INSTRUCTION PURPOSE: T prvide directin fr the handling f nuclear gauges including entry t a vessel n which a nuclear gauge is installed. CONTEXT: Numerus lcatins acrss the utilize nuclear gauges within their prcesses. These nuclear gauges are well suited t the harsh applicatins in which they are applied. Althugh the gauges are well designed t allw nrmal peratin f the radiactive surce withut harm t persnnel in the area, expsure t inizing radiatin must be cnsidered when wrking arund a nuclear device. PERSONNEL Canadian Nuclear Safety Cmmissin (CNSC) Canadian regulatry authrity fr nuclear material. Radiatin Safety Officer (RSO) r Designate The Radiatin Safety Officer rle will be held by the Instrumentatin Engineering Supervisr as apprved by the CNSC and is the nsite persn respnsible fr the fixed gauge radiistpe license and regulated activities. Authrized Emplyee: Persn that has cmpleted the curse titled Radiatin Safety Training fr Nuclear Gauges within the past tw years and is permitted t wrk n r handle nuclear gauges as dcumented by the RSO. Envirnment and Health E&H manages the dsimetry prgram. Handles new dsimeter requests and maintain recrds. EQUIPMENT Persnal Prtective Equipment (safety glasses, wrk glves, mask, etc.) Persnal Dsimeter Radiatin Survey Meter LIMITS Annual whle bdy inizing radiatin dse f 1 msv (100 mrem). Radiatin field is maximum 0.05 msv/hr r 50 micrsv/hr (5 mrem/hr) at 30 cm frm the surce head with the shutter clsed Strage Area: maximum dse rate f 2.5 micrsv/hr (0.25 mrem/hr) at any ccupied lcatin arund the area. 3 ALARA POLICY Vale feels that the health risks assciated with fixed nuclear devices can be mitigated with prper prcedures and training. The prcedures in this SPI will be readily available t all emplyees via the cmpany intranet and shuld be sufficient t ensure that radiatin expsure f the emplyees is kept as lw as reasnably achievable. The Radiatin Safety Officer will be respnsible fr ensuring that persnnel are aware f lck-ut and emergency prcedures. Peridic training cvering basic radiatin safety, prcedures, and regulatry cnditins will be prvided t wrkers when there is a sufficient change in persnnel t warrant such a curse. In additin t this the RSO, by fulfilling his respnsibilities ensures that regulatins and licence cnditins i.e., (signage, inventry cntrl, leak testing etc.) are cmplied with. GENERAL RULES The Substatin Operatr and Surface First Aid Attendant shall be prvided with the names f the current RSO, alternate and Authrized Emplyees. All emplyees wh perfrm Radiatin Surveys will be in pssessin f and will use, a suitable persnal dsimeter during the perid f these duties. All emplyees wh perfrm radiatin surveys will be familiar with the lcatin and use f a radiatin survey meter. Vale is nt authrized t perfrm maintenance n the surce hlder. Maintenance n the detectr can nly be perfrmed if the surce shutter is clsed, lcked and verified with a survey meter and recrded. All nuclear gauges shall be stred in the assigned area. Any exceptin t this practice must be apprved by the RSO. All surce hlders are stamped with a radiatin warning sign and istpe details. A separate sign r label cntaining these details must be psted n r near the device. All areas where gauges are in use r in strage shall be psted with a separate radiatin sign including emergency cntact details. Persnal dsimeter recrds may be viewed by the emplyee n request. Vessel drs shall be lcked in the clsed psitin. The supervisr f the Authrized Emplyee(s) fr the area shall keep the key. Only authrized emplyees as dcumented by the RSO r emplyees under the direct supervisin f an authrized emplyee may install, munt r dismunt a fixed nuclear gauge. Minimize expsure t inizing radiatin by limiting time spent handling r near surce cntainers. Prir t installatin, munting r dismunting inspect the surce hlder and verify that the surce hlder shutter is in the ff/clsed psitin. The surce hlder shutter must remain in this psitin until the wrk is cmplete. During installatin r dismunt DO NOT place yur hands n the shutter pening until yu have verified shutter clsure using a calibrated survey meter. Keep surces in the shielded (clsed) psitin unless being used fr testing purpses. DO NOT direct radiatin beam twards anther individual r int nrmal ccupied areas during handling r mving. During installatin whenever pssible, gauges shall be munted such that the radiatin beam is pinting away frm the directin f nrmal traffic. All areas where gauges are in use r in strage shall be psted with a handling sign referring t this SPI. Vessel drs shall have a radiatin warning sign psted directly n the dr. SPI 34-36Page 3 f 15

RADIATION SAFETY REQUIREMENTS FOR INSTALLATION, DISMOUNT OR ENTRY TO A VESSEL ON WHICH A NUCLEAR GAUGE IS INSTALLED File a Nuclear Fixed Gauge Wrk Permit (attached) prir t cmmencing wrk n the device, with the RSO. Cmplete a NUCLEAR SOURCE SHUTTER CLOSURE VERIFICATION FORM (attached) during the wrk. Radiatin measurements must be taken befre and after clsing the shutter. Nte: measurements must be taken in a lcatin that indicates a significant drp in dse rate after clsing the shutter. Often the nly lcatin where shutter clsure can be verified is n the detectr side. Send a cpy f the NUCLEAR SOURCE SHUTTER CLOSURE VERIFICATION FORM t the RSO. SPI 34-36Page 4 f 15

5 GAUGE ISOLATION Rnan SA-1 Lcate metal lever n tp f surce hlder. Tggle metal lever s that it is perpendicular with the face f the surce hlder (parallel t the radiatin beam). Insert persnal lck (with persnal key) thrugh lever and eyehle n the surce hlder. Tag ut with apprpriate tag. TN5201 and TN5202 Lcate metal lever n the right side f surce hlder. Push metal lever away frm the marked ON psitin. Insert persnal lck (with persnal key) thrugh hle n the surce hlder t impede the mvement f the lever. Tag ut with apprpriate tag. Berthld LB7442 Lcate metal lever n back (ppsite t radiatin beam) f surce hlder. Tggle metal lever s that the lever arrw pints at the clsed psitin. Lck ut with manufacturer installed lck (nly ne key shuld be available and shuld be held by persn ding the lcking ut). Tag ut with the apprpriate tag. Ohmart SH-F1B Lcate metal lever n tp f surce hlder. Tggle metal lever s that it is in the OFF psitin Insert persnal lck (with persnal key) thrugh hle n the surce hlder t stp lever mvement. Tag ut with apprpriate tag. INSTALLATION, MOUNTING AND DISMOUNTING PROCEDURES Rnan SA-1, Ohmart SH-F1B, TN-Tech 5201 and 5202, Berthld LB7442 Pipe Installatin Mve the detectr int the desired measurement psitin. Hld the detectr in this psitin using either temprary (hist, ect) r permanent (bracket, ect) means. Mve the surce hlder int the desired measurement psitin ensuring that it will be installed s that the radiatin beam is pinted away frm nrmal traffic areas. Hld the surce hlder in psitin using temprary r permanent means. Insert blts thrugh the fur hles in the surce hlder and detectr and secure t pipe. Tighten s as t nt damage the pipe r pipe insulatin. Bracing and brackets must be put int place t supprt the weight f the surce hlder and detectr. These supprts must nt be in the path f the radiatin beam. Remve any temprary bracing. Cmplete detectr wiring and calibratin as per the specific detectr manual. SPI 34-36Page 5 f 15

6 Vessel Installatin Mve the detectr int the desired measurement psitin. Hld the detectr in this psitin using permanent bracing. The detectr must be as clse t the vessel as pssible while in a psitin where the detectr will nt mve in relatin t the vessel r surce. Surce hlder and detectr shuld be aligned alng a chrd f the vessel (nt the diameter) t allw fr prper measurement. Mve surce hlder int the desired measurement psitin ensuring that it will be installed s that the radiatin beam is pinted away frm nrmal traffic areas. Hld the surce hlder in psitin using permanent bracing s that it will be as clse as pssible t the vessel but will nt mve in relatin t the vessel r detectr. D nt munt surce hlder t vessel in such a way that the vessel integrity will be cmprmised. Cmplete detectr wiring and calibratin as per the specific detectr manual. Pipe Dismunt Secure surce hlder and detectr using temprary r permanent means t ensure that they will nt shift r fall nce the blts are remved. Und and remve the fur blts that hld the surce hlder and detectr tgether. Lwer surce hlder t the grund and mve t RSO apprved strage lcatin. Surce hlder is nt t be dispsed f except by the RSO t a qualified cnsultant. Lwer detectr t the grund and remve frm wrk area. Vessel Dismunt Secure surce hlder and detectr using temprary r permanent means t ensure that they will nt shift r fall nce they are remved frm the permanent bracing. Remve all cnnectins between the surce hlder and permanent bracing s that it may be mved. Lwer surce hlder t the grund and mve t RSO apprved strage lcatin. Surce is nt t be dispsed f except by the RSO t a qualified cnsultant. Remve all cnnectins between the detectr and permanent bracing s that it may be mved and remve frm wrk area. RECEIPT OF PACKAGES The RSO will be required t sign ff n the purchase rder fr any new nuclear devices that are t be brught t site. Radiistpe license amendment is required befre btaining any nuclear surce nt already listed in sectin IV f current license. Inventry will be updated if any nuclear surces are acquired, transferred r dispsed f. Inventry will be updated if surces are mved t anther lcatin within cmpany prperty. Radiactive packages will nly be pened by Vale persnnel wh have attended Transprtatin f Dangerus Gds class 7 training r by a qualified cnsultant. Any damaged packages received will nt be handled and will be immediately reprted t the shipper emergency cntact phne number listed n the Dangerus Gds Shipping Dcument. CONTROL AND SECURITY Nuclear gauges will nly be handled and serviced by persnnel wh are an Authrized Emplyee r under the direct supervisin f an Authrized Emplyee. SPI 34-36Page 6 f 15

7 Security cverage at each f the plant entrances will be prvided 24 hurs per day, 365 days per year. Gauges that are used t measure the prcess will be mnitred by peratrs during nrmal peratins and any device malfunctins will be investigated immediately as they are nticed. In the case f theft the fllwing steps will be taken: The persn t discver the theft will immediately infrm the RSO, prviding the RSO with their name, emplyee number and details f the incident including the riginal lcatin f the gauge. The RSO will immediately infrm the Canadian Nuclear Safety Cmmissin, the lcal plice and plant security. The RSO will assemble the persn t discver the incident as well as the site peratr and freman t perfrm an investigatin. Plant security will search each vehicle as it exits the plant site while the investigatin is nging. Stred gauges will be handled as belw. STORAGE OF NUCLEAR GAUGES When nt in use nuclear gauges must be stred in the apprved strage facility lcated abve the Refinery Instrumentatin Shp in the Central Shps building r in a temprary shrt-term strage lcatin that is accessible nly t authrized persnnel (lcked) and must be apprved by the RSO. When a surce has t be mved ut f the way f a jb in prgress fr a shrt perid f time and then reinstalled, permissin may be btained frm the RSO. Prper handling techniques are t be used and prper signage is t be psted n matter hw shrt this time perid may be. DISPOSAL OF NUCLEAR DEVICES N emplyees n the plant site are qualified fr transprtatin r dispsal f nuclear devices. The RSO must be infrmed f intent t dispse f any nuclear surces frm the plant site s that they can be dispsed f by a qualified persn. Transfer r dispsal recrds will be btained fr any surces remved frm cmpany prperty and given t the RSO. LEAK TESTING All fixed gauges that are in service will be leak tested by an apprved cnsultant accmpanied by the RSO r alternate nce every twelve mnths under nrmal perating cnditins. All fixed gauges that are in strage will be leak tested by an apprved cnsultant accmpanied by the RSO r alternate at least nce every twenty fur mnths under nrmal perating cnditins. Any device that has been invlved in an emergency incident will be leak tested befre being returned t service. Any device that is returned t service after being in cntinuus strage will be leak tested befre being returned t rutine use. Leak test sampling is t be perfrmed by an apprved cnsultant accrding t the manufacturer s instructins. Vale persnnel will nly perfrm a wipe test under the direct supervisin f an apprved cnsultant. 8 The leak tests will be analyzed by an apprved cnsultant t determine if any radiactive material has leaked ut. The results will be returned t the RSO and filed by the RSO. Any results abve the acceptable levels will be immediately reprted t CNSC. INSPECTION PROGRAM Audits shall be cnducted every three mnths by the RSO r an alternate t ensure each device cmplies with CNSC requirements. The results f the audit will be filed by the RSO in the Radiatin Cmpliance binder. The audits will include: Verify nuclear inventry and lcatins Verify prper signage at gauge lcatins Verify all field gauges are in wrking rder Verify gauges have n bvius damage Verify that a cpy f the license and prcedures are available at each plant that huses nuclear gauges Verify the lcatin map f nuclear gauges is available at each plant that huses nuclear gauges Verify device nameplates are legible r that replacements are in place In additin every secnd year and in cnjunctin with the Radiatin Safety Training fr Nuclear Gauges training the fllwing audits will als take place: Radiatin Survey f nuclear gauge strage area Verify that dsimeters are being wrn crrectly Verify that radiatin survey meters are being used crrectly and are calibrated Verify maintenance recrds fr gauges. Authrized Emplyees Authrized Emplyees are permitted t install/munt, dismunt and lck ut fixed nuclear gauges. Authrized emplyees will be required t cmplete the Nremtech Radiatin Safety Training fr Nuclear Gauges every secnd year. Upn cmpletin f this training the emplyee will be cnsidered authrized fr the next tw years. This training will include: Prperties f radiatin Dse equivalents Dse limits Types f radiatin Lng term and acute affects Radiatin prtectin Emergency prcedures Regulatry requirements Installatin, dismunt and vessel entry prcedures Transprtatin verview RSO and Alternates The Radiatin Safety Officer and alternates will be required t cmplete the curse titled Radiatin Safety Officer Training Sealed Surce Users prvided by Stuart Hunt and Assciates Ltd. Recrd f this training must be entered int VIP. The Radiatin Safety Officer and alternates will als be required t cmplete the training fr Authrized Emplyees every secnd year. Nn-Authrized Emplyees Nn-authrized emplyees are anyne wh is emplyed by Vale and will be ding wrk n the plant site. Emplyees are required t cmplete a cmputer based training package prir t beginning wrk n the plant site. The cmputer based training will include: Radiatin expsure and effects Signage and identificatin f nuclear devices Overview n hw gauges wrk Emergency respnse prcedures Visitrs and Cntractrs Visitrs and cntractrs will be required t cmplete the same training as nn-authrized Vale emplyees if they will be visiting an area with a nuclear gauge and will nt be accmpanied by a trained Vale emplyee. RADIATION SURVEY METERS Calibratin and Maintenance Vale emplyees will nt cnduct any radiatin survey meter calibratin r maintenance beynd battery installatin. All maintenance and calibratin will be dne by qualified cnsultants. All radiatin survey meters must be sent ut fr calibratin annually. D nt use a radiatin survey meter that is ut f calibratin. Ludlum Mdel 3 with Mdel Detectr Care Stre Survey meter in a hard plastic case in a lcatin that is dry and away frm nrmal traffic. Cleaning f detectr can be perfrmed with a damp clth (water nly) after the instrument has been ff fr at least ne minute and the detectr is discnnected. Use Battery installatin Ensure range selectr is in the OFF psitin. Push dwn n the battery lid and turn the quarter-turn thumbscrew cunterclckwise. Install tw D sized batteries nting the crrect plarity. Cnnecting the detectr Cnnect ne end f the detectr cable t the detectr by firmly pushing the cnnectrs tgether while twisting clckwise ne quarter turn. Repeat the prcedure n the instrument end. Battery test Test batteries prir t use. Mve the range multiplier switch t the BAT psitin. Ensure the meter deflects t the battery check prtin f the meter scale. If meter des nt respnd check battery installatin and replace if necessary. Instrument test Test the instrument with a knwn check surce prir t using. Turn instrument range switch t the X100 psitin Turn the AUD ON-OFF switch t ON SPI 34-36Page 9 f 15

Battery installatin Install a standard 9-vlt alkaline battery in the battery cmpartment in the lwer rear f the detectr. Instrument test Set the tp switch t micr seiverts per hur. Set the bttm switch t ON Wait 30 secnds fr a shrt beep t indicate the detectr is initialized. Measuring Hld instrument ne t six inches frm surface f the device t be tested. 11 RECORD KEEPING The RSO will maintain all recrds in accrdance with the Nuclear Safety and Cntrl Act and all assciated regulatins.
