Risk Assessment: This Risk Assessment is to be used as a general guide and as such, cannot accommodate all the varying factors that may be encountered when using this equipment. Therefore, personnel are requested to conduct their own Risk Assessment before using this equipment to include any extra hazards introduced by the task performed. Operation of Centrifuge TASK PERFORMED HAZARDS 1. Severe machine imbalance leading to machine destruction and possible injury to those in the vicinity 2. Rotor spinning off the hub during operation. 3. Lid coming off during operation. 4. Exposure to biohazards/hazardous substances as a result of broken/leaking vessels RISK ASSESSMENT 1. The risk of machine imbalance is present while ever the equipment is in use. These risks are increased if the user is not trained or if the centrifuge is not regularly maintained. 2. The risk of the rotor spinning off the hub is present only when the rotor has not been correctly seated prior to operation. This risk is increased if the user is not adequately trained. 3. The risk of the lid coming off during operation is present only when the lid has become cross-threaded as it is tightened. Again the risk is increased if the user is not adequately trained. 4. 2 Page: 2 of 6 RISK CONTROL 1. Only trained staff are to use this equipment. Tubes must be balanced by weight before inserting into the centrifuge. Only tubes especially made for centrifugation are to be used. Establish a safety zone of 30cm around the centrifuge and ensure that no personnel or hazardous substances enter the zone during centrifugation. Check rotors regularly for signs of irregular wear/damage. 2. Observe the hub through the centre of the rotor as it is lowered into place. Ensure that the rotor is lined up correctly with the rests on the hub. 3. The lid must be flush with the top of the rotor after tightening. If not, then remove the lid and realign, then recommence tightening. 4. Use only specifically designed centrifuge tubes and ensure they are properly sealed before centrifugation. In the event of a spill, decontaminate machine immediately - follow spill procedures (GDP 011) and machine disinfecting instructions. Wear appropriate personal protective equipment. Use sealed rotors when centrifuging infectious or hazardous agents. 2. Calibration: N/A 3. Monitoring: 3.1 Frequency: Whenever in use. 3.2 Method Listen carefully to the centrifuge to ensure its operating sounds are characteristic to this particular machine. If there is any unfamiliar sounds coming from this equipment when it is in operation it could possibly mean that the rotor is unbalanced and must be turned off immediately. This is to avoid major damage to the centrifuge and possible safety hazards. 4. General Operation: 4.1 Read the Safety Reminder at the beginning of the Beckman Model J2-M1 Centrifuge Instruction Manual attached to this GEP. 4.2 For General Operation Instructions refer to the Operations section on page 9 of the Beckman Model J2-M1 Centrifuge Instruction Manual (Attachment 10.1). 4.3 Separate GEP s have been provided for each of the rotors available to be used with this centrifuge. Refer to the appropriate rotor GEP for instructions regarding the rotors.

Cleaning: 5.1 Frequency Cleaning must be carried out when required and immediately in the case of a spill. 5.2 Method Refer to the Cleaning section on page 25 of the Beckman Model J2-M1 Centrifuge Instruction Manual (Attachment 10.1). 5.3 Records: Record all cleaning details on the Maintenance Record Sheet (Illustration 9.1). 6. Maintenance: 6.1 Method: Refer to the Maintenance and Installation Requirements section on page 25 of the Beckman Model J2-M1 Centrifuge Instruction Manual (Attachment 10.1). 6.2 Records: Record all Maintenance details on the Maintenance Record Sheet. 7.
