1 Authorised by: Purpose Scope Responsibility This procedure defines the processes to be followed in order to identify health and safety hazards, assess the risks associated with hazards relating to use of MANTALOCK in wet holes. This risk Assessment details the safe handling procedures for wet holes and the application there of. The use of hole plugs is considered part of the explosives loading process and as such will only be carried out under the supervision and control of a qualified Shotfirer, competent in application 1 Introduction This procedure defines the processes to be followed in order to identify health and safety hazards, assess the risks associated with each hazard and apply controls (where required) to reduce associated risks. 2 References AS Part 2 3 Risk Assessment When assessing risk, the likelihood of the hazard occurring and the potential consequences if the hazard occurred are to be considered, as described in the following tables. Selection: Select correct size slumping / MANTALOCK plug for misfire appropriate hole. 2. Preparing: locate drill cuttings and place inside hinged container leg and place additional cuttings in top 2. Use correct plug for hole range. Ensure enough weight is placed in plug and hinge is functioning correctly PPE-Gloves Clear work area Make sure plug is securely fastened RISK (2) 3. Bend plug wings in so that it travels down hole without catching 6. Lowering: Lower plug to desired location in water. Once plug meets water, slow down decent and allow plug to align 7 Activating: Once plug is in desired location pull sharply on cord to cause locking leg to engage the hole Muscular /pinch point/ damaged product Bridging/ loosing plug / snagging and misfires/ muscular D2 C3 PPE-Gloves. Check hinge to make sure it is still functioning and lugs are located inside pivot If plug gets stuck, gently pull on drop cord. Do not pull quickly as this will activate the plug causing it to lock in an undesirable location. Pull gently and lower slowly. Take slack off the drop cord and the sharply pull cord. Do this 2 of 3 times to make sure rope is free from keyhole and leg is engaging hole F3

3 Confirm: Confirm plug is locked correctly in the hole Secure: Kick in cuttings to form a base on plug for loading Detach: Once happy that plug is locked in hole detach cord and misfires/ muscular and misfires and misfires C3 High Pull gradually on cord, and hold pressure on plug, plug should not float up or move. Kick in about 1 foot of dirt into the hole. Pull gradually on cord, and keep pulling. The oring will eventually snap under 7kg force. The drop cord can now be retrieved 4 SAFETY AND HAZARDS Muscular pain from bending. Pinch Points 5 SAFE HANDLING PROCEDURES Storage: Store in dry area away from direct sunlight 6 MATERIALS / EQUIPMENT MANTALOCK Plug Drop cord (string or rope) PPE 7 SAFE WORKING PROCEDURE 7..1 Select correct size MANTALOCK plug for appropriate hole and remove from carton just prior to use Determine what depth the plug is to be placed in the hole Place drill cuttings inside the hinged container leg and some in the top 7..4 Allow leg to hang straight and pull activation rope down so that it locks in the keyhole just above the O ring

4 7..5 Prepare drop cord allowing an extra 1.5m to prevent a RSI from bending Either tie or clip the drop cord onto the O-ring 7..7 Force the top wings in so that the MANTALOCK will travel down the hole without catching 7..8 Lower the MANTALOCK plug down to the required depth Take slack off the drop cord and the sharply pull cord. Do this 2 of 3 times to make sure rope is free from keyhole and leg is engaging hole A firm upward pull on the drop line will confirm that the plug is locked in the hole Pull gradually on cord, and hold pressure on plug, plug should not float up or move While holding the cord taunt, kick in at least 2 boot loads of drill cuttings (or 1 foot) to protect MANTALOCK from falling stemming and larger debris that may impact and dislodge plug Once confirmed that MANTALOCK is holding position, pull gradually on cord, and keep pulling. The oring will eventually snap under 7kg force.

5 7..14 The drop cord can now be retrieved and the hole charged.
