Name: Jim Baetge PGA Classification: A-14 Facility: Traditions Club Year Elected to Membership in the PGA: ) Please briefly describe your involvement and significant leadership contributions through mentoring and sharing with other PGA Professionals about growing the game of golf. As the Director of Instruction it is my responsibility to not only teach, but to instruct others how to teach and run a successful Player Development Program. In addition to our own professional staff, we also hire PGM Interns throughout the year. Each member of the team is encouraged take an active part in all of our programs from shadowing lessons to coordinating camps & clinics. Growing the game is specifically emphasized with our Ladies and Junior Programs. No matter where our staff and interns go, these two demographics will always need attention in order to grow the game. 2.) Please describe each of the programs you have conducted to grow the game of golf at your facility, in your community, in your Section and nationally. Free Junior Golf Clinic To open the golf season, we offered a FREE to the public junior golf clinic to kids ages 6 and up. During the clinic, we also hosted an Operation 36 Orientation for the parents. Operation 36 Junior Golf Academy (open to the public) This year we will have conducted both spring and fall After School Programs (20 weeks total, 3 classes/week) and a Summer Academy (11 weeks, 3 classes/week). Participants also had twenty 9 hole match opportunities in addition to the classes. Traditions Club Junior Golf Camps (open to the public) This year we conducted two PeeWee Golf Camps (ages 5-8, 3 days), two Junior Golf Camps (ages 8-18, 4 days) and one Combo Camp (ages 5-18, 3 days). Operation 36 Ladies Golf Academy (open to the public) The Ladies Academy is conducted in the spring (12 weeks) and in the fall (9 weeks) each year. Classes are 1.5 hours each week with an additional seven 9 hole matches testing their skill from yardages starting at 25 yards and moving out Brazos Valley Golf Association Junior & MiniTour The BVGA is a 501(c)3 not-for-profit organization that I run to grow junior golf in our community. As part of it operation, we conduct 18 junior golf tournaments throughout the year. (Don t hold it against me - we send qualified players to the STPGA) Participation is generally for beginning and intermediate juniors with an interest in learning to play.

3.) Please indicate the number of consumers and/or members of your facility that have attended your various growth of the game programs (list by each program conducted). Operation 36 Overview (includes BVGA Members) 192 Families & 471 Golfers Operation 36 Junior Golf Academy (all) 112 participants Traditions Club Junior Golf Camps (all) 47 participants Operation 36 Ladies Golf Academy (all) 20 participants Brazos Valley Golf Association Junior & MiniTour Members - 57 Rounds Played ) Please describe what you have done to measure the impact of these programs in areas of player retention, increased rounds played, increased customer satisfaction, and impact on growing revenues and decreasing expenses. The bulk of our programs use the Op36 framework for registration and communication. The response from ALL of our participants has been outstanding. Since it is our first year in the system and we have vastly expanded our opportunities for players, it would be impossible to compare to previous years. However, this year blows away what we have done in the past. As a result, each of my assistants has nearly doubled their income from last year with less planning on their part. There is an expense to the Op36 program; however, it is offset by a yearly registration fee that seems to also help with retention. There are several stats that the system tracks, but the most interesting ones include: # of Families registered (192), # of Golfers registered (471) & # of Strokes Lost (student improvement) (18914 this year to date).

5.) Please describe your best practice idea(s) that have been successful in your growth of the game programs. Two programs that have grown the game in the Brazos Valley are the Brazos Valley Golf Association and Operation 36. The Brazos Valley Golf Association is an not-for-profit organization established to promote the game of golf by helping to make it more affordable and accessible to junior golfers. Charitable donations are used to provide FREE junior golf clinics, scholarships for junior golf camps/clinics and tournament entry fees. Additionally, donated clubs are provided directly to deserving junior golfers as well as the local school golf teams. Operation 36 was originally designed to be a junior golf program. However, its platform has been extremely useful in developing our Ladies Golf Program as well. The idea of starting from the green and working backward has attracted 10 beginning golfers to our program this year. While that number is not huge, it nearly doubles the number of ladies in our program. In addition to the Ladies Program, the Op36 Junior Program had been fully embraced by our juniors and their parents. Participation and retention is at an all time high. Operation 36 is a 6 level program with 12 curriculum within each level teaching different aspects of the game all designed to shoot even par (36) from designated yardages working progressively back from the green. All students monitor their progress through an app that keeps track of all of their activities.

6.) Please briefly describe your level of service to your PGA Section and/or to the PGA of America. I was not active on any section committees this year. Please keep me in mind for future opportunities. 7.) 5 Name: Tirrel Baymon PGA Classification: A-14 Facility: Cypresswood Golf Club Year Elected to Membership in the PGA: ) Please briefly describe your involvement and significant leadership contributions through mentoring and sharing with other PGA Professionals about growing the game of golf. Personally I always make it a point to reach out to fellow professionals and seek different teaching methods and styles. I also work with other PGA professionals to develop PGA Junior League conferences and TXGA State Team League Play Conferences. Within our company Foresight Golf Management I was influential in helping all the pros at our properties receive the PGA HOPE training from Judy Alvarez that is required to run the program. Since that time Cypresswood and The Republic Golf Club have ongoing PGA HOPE programming at both facilities. 2.) Please describe each of the programs you have conducted to grow the game of golf at your facility, in your community, in your Section and nationally. I have conducted several programs. We have a PGA Junior League team, PGA HOPE programming. We do our version of Get Golf Ready which is called Introduction to Golf. We have a Junior Golf Academy that features individual, group lessons, and playing opportunities. We also work with the county and do a Harris County Fun for Seniors program. Twice a month we conduct the LPGA-Girls Only Golf program. All these programs are ongoing programs that we try to execute throughout the year. Our goal is to continue to have an offer that brings people into the game and then have a secondary offer that will transition them into regular golfers.

3.) Please indicate the number of consumers and/or members of your facility that have attended your various growth of the game programs (list by each program conducted). PGA Junior League - 12 Introduction to Golf - 72 PGA HOPE - 70 to 80 Harris County Fun for Seniors - 43 LPGA- Girls Only Golf - 15 Junior Golf Academy ) Please describe what you have done to measure the impact of these programs in areas of player retention, increased rounds played, increased customer satisfaction, and impact on growing revenues and decreasing expenses. We have a loyalty reward program that consist of a card that is issued to our participants. They use the card to get points that lead to free golf. This card allows us to track customer spending. Before we execute any of our programs, I will create the loyalty reward card for each participant along with establishing their specific customer class. This allows us to track all activity at the facility based on the specific type of customer. 5.) Please describe your best practice idea(s) that have been successful in your growth of the game programs. Our ongoing Introduction to Golf class has been a great success. I ve found that in order to bring new people into the game we have to market into a place where current golfers are not present. We also use a price point to allow customers an opportunity to commit yet not fully invest until they are truly interested. We use Groupon for this class and since we started it a couple years back we have introduced over 250 people to the game. From there we find that constant communication via phone calls and s keep people engaged along with funnel type programming. So we try to transition our nre golfers into one of our current programs.

6.) Please briefly describe your level of service to your PGA Section and/or to the PGA of America. I execute every PGA Reach initiative along with the LPGA - Girls Only Golf initiative. These program components require a lot of time and pre scheduling to properly execute. 7.) Please list any Section or National PGA awards the nominee has received and list the year received: (Please state whether National or Sectional and list Section.) I have not received any awards in the past. The STPGA Awards Committee strongly recommends that the application for this award be typewritten and will not accept additional materials beyond the answers to the questions on the application. 18 Name: Eujone Kim PGA Classification:A6 Facility:Axis Golf Academy and Fitting Center Year Elected to Membership in the PGA: 5/24/11 1.) Please briefly describe your involvement and significant leadership contributions through mentoring and sharing with other PGA Professionals about growing the game of golf. The best way to be involved in leadership and mentoring is to host other golf pros who are interested in the teaching and coaching aspect. Having them shadow us for a day or two is truly a unique experience. It allows the visiting coach to ask questions as well as learn hands on and have the ability to ask questions right then and there. Also it shows the dynamic of the student athlete relationship, how is the coach acting and behaving, what are his or her mannerisms like when coaching. These are things that can t be taught but can be learned by watching. We do have occasional visitors come shadow us and we welcome anyone to come and do it; it truly is the best way to receive the information. One of our independent contractors was a PGM student who shadowed us through the summer and she has joined us because she enjoyed the energy at our facility and the fact that she can grow as a teacher and coach here. I have also contributed at conferences at the PGA Show regarding junior clubfitting to my peers. 2.) Please describe each of the programs you have conducted to grow the game of golf at your facility, in your community, in your Section and nationally. PGA Junior Golf League has been our biggest program when it comes to grow the game initiative. I have been part of the PGAJLG since its inception and we have been fortunate to be going back to Arizona for the National Championship for the second year in a row. Ben Willman who is captain and I the coach have worked really hard to promote and grow our program. We are also very fortunate that most of the kids involved in our post season are our students as well. We will be representing Texas and hopefully bring Texas its first national championship. We also this year have been approved to be a Girls Golf Rocks facility. Having my own daughters I feel that the female golf programs are lacking and with the help of the LPGA I feel that we can have a stronger impact on growing the game for the girls and ladies. For our summer program we have the Nike Golf Camps, that we conduct as well. We have many outsiders of Houston who participate and a large number of the kids are usually visiting their grandparents who are in the local area. During the school year, we are a Off Campus Physical Education site for the local school districts, Conroe, Montgomery, and Magnolia.

3.) Please indicate the number of consumers and/or members of your facility that have attended your various growth of the game programs (list by each program conducted). PGA Junior League 46 Girls Golf Rocks 15 Nike Golf Camp 50 Off Campus PE 15 4.) Please describe what you have done to measure the impact of these programs in areas of player retention, increased rounds played, increased customer satisfaction, and impact on growing revenues and decreasing expenses. Being that we are at located at a Public Facility, every time someone participates in one of our programs, there is an opportunity to increase rounds at Woodforest Golf Club. We also have a special rate for our Axis juniors that wish to go out and walk the course. Being from a private facility this was not the case. Non-members were not allowed to go and play at the facility. Once the program has completed they must leave the property. Working at a public facility the participants may stay and have lunch or they may go out and play 9 holes. So the opportunity to increase F&B, merchandise and golf rounds definitely increase with each one of our programs. A great example is when our parents drop off their children for Off Campus PE, the parents usually will purchase dinner or have a glass of wine while waiting for their children. 5.) Please describe your best practice idea(s) that have been successful in your growth of the game programs. Course time is extremely vital in the game development. Golf is one of the only sports that we do not practice where we play. So camps, lessons, or clinics, it is extremely important that the athletes are executing what they had learned in class. Commitment is another vital part in the process of growing your own game. Being honest with oneself as far as how much time they can commit to the game is important. From there on a game plan can be developed based off of their allotted time in a week. So programs like Get Golf Ready might be the right amount of golf interaction or it might be too much. That way a realistic goal for yourself can be set.

6.) Please briefly describe your level of service to your PGA Section and/or to the PGA of America. I try and be active when it comes to programs that help promote the game, especially with juniors. I am currently not on the board of any section. However I plan to join the Junior Golf Committee to become more involved with the section. 7.) 22 Name: Robert F. Scott PGA Classification: A-8 Facility: Royal Oaks Country Club Year Elected to Membership in the PGA: ) Please briefly describe your involvement and significant leadership contributions through mentoring and sharing with other PGA Professionals about growing the game of golf. While being a PGA Professional has been a rewarding achievement in my life, I get just as much gratification helping other PGA professionals and apprentices learn how to teach and build an instruction business. I have encouraged other instructors to shadow me as I teach private lessons and have also had the opportunity to share programs that my facility runs. I am able to discuss programs that have worked such as Operation 36. Operation 36 is a program that all ages can benefit from. The program helps students, develop skills that can be used to shoot 36 from their desired tee. I also have the opportunity to discuss programs that have not worked in the past such as drop in clinics. These clinics were set up to allow members to drop into a clinic during a particular time frame to receive help. This program did not work for us due to the structure and loose time frame. Some students would show up late and others would forget the days and times we would conduct classes. We also discuss how to provide value to their programs so that they may continue to grow. My other contributions include helping PGA professionals and apprentices understand how to conduct clinics in an organized manner and delivering the correct content. This skill requires the teacher to understand their audience and the skill levels of each group. 2.) Please describe each of the programs you have conducted to grow the game of golf at your facility, in your community, in your Section and nationally. At my facility we run a lot of our programs through the Operation 36 platform. Operation 36 uses an instruction component and a playing component where students can be measured on how well they are retaining movement and also how those skills are transferred to the golf course. The skills learned include grip, posture, ball first contact, alignment, putting, green reading, fitness and nutrition and some knowledge. The playing portion of the program is conducted over various yardages. All players in the program start from 25 yards and then work their way back to a further yardage once they shoot 36 for 9 holes. The overall goal of the program is to help them shoot 36 from their regular tee markers. Another program that I conduct is a Biggest Loser program. Each participant is allocated 3 private lessons a month at a discounted rate. The goal of the program is for the student to reduce their handicap over the course of 1 year. The student is able to receive consistent instruction monthly where the coach is able to monitor progress and make small adjustments. It also allows the student to set a more routine practice schedule to improve.

3.) Please indicate the number of consumers and/or members of your facility that have attended your various growth of the game programs (list by each program conducted). These numbers reflect this year s participation. Operation 36: Juniors: 50 Women: 30 Men: 10 Biggest Loser: Men: 20 Wowen: 5 Juniors: 2 4.) Please describe what you have done to measure the impact of these programs in areas of player retention, increased rounds played, increased customer satisfaction, and impact on growing revenues and decreasing expenses. We have various ways of measuring the impact of how these programs increase participation at our facility. Each time we conduct matches, each participant pays a cart fee for the 9-hole round. The cart fees are built into the package of the program. Once the matches are completed, we are able to bill the cart fees and track the revenue at the end of the term. We also keep a spreadsheet of attendance for each class. We can track how many classes students are attending and also how many sessions throughout the year they are attending. Ladies from our programs play 9 holes after our morning clinics. Once they are done playing, they will have lunch afterwards, which contributes to the Food and Beverage revenue. Our programs continue to grow as we have increased our participation over the last year. We have continued to tweak our programs and make them more inviting to members that may be apprehensive about receiving instruction. Providing more value to the programs has assisted in growing the programs also. 5.) Please describe your best practice idea(s) that have been successful in your growth of the game programs. Leap Frog: This drill works great for beginner golfers teaching them how to control the speed of their putts. I usually start with 2 lines of yarn on one end of the green. One is spaced out 3 ft. from the other. The student will have to hit their first putt from the first string of yarn past the second string. Their next objective is to leap their first ball with their second. They will have to execute 10 jumps to pass. Random Practice Sessions: I will usually set out a white board with various tasks to complete based on the subject for the day. This allows the students to mix up the clubs and shot selections while practicing. Block Practice Sessions: I usually use this practice session to allow the players to work on technique with one club over multiple swings instead of varying their club selection.

6.) Please briefly describe your level of service to your PGA Section and/or to the PGA of America. I have yet to serve the PGA of America in any capacity however, I try to attend and participate in as many of the section events and education seminars as possible. 7.) Please list any Section or National PGA awards the nominee has received and list the year received: (Please state whether National or Sectional and list Section.) I have yet to be recognized for any awards on the Section or National level. 26 Name: Doug Strawbridge PGA Classification: A-14 Facility: GOLFTEC Upper Kirby Year Elected to Membership in the PGA: ) Please briefly describe your involvement and significant leadership contributions through mentoring and sharing with other PGA Professionals about growing the game of golf. As GOLFTEC s Houston Regional Director of Instruction, I have had the opportunity to influence teaching throughout Houston and the surrounding metro area. I have mentored over 75 professional instructors and trained 2,000 unique students during my 16-year career. In the last 12 months as a full-time instructor with GOLFTEC, I have taught over 1,800 lessons and during my career have taught over 27,000 lessons. This year alone, the team of 20 instructors I mentor has taught over 35,000 lessons in the Houston area which resulted in students lowering their handicaps by an average of 6 shots. From I served the Southern Texas Section as the Committee Chair of the Player Development Committee. This committee s responsibility was to help grow the game in the area of adult golf. We helped trade best practices to create more involvement in the game. I now serve on the Section Education Committee helping our professionals explore new ways to educate themselves in various areas from technology to instruction. We want to make the info readily available for those wanting to better their craft as a PGA Professional. 2.) Please describe each of the programs you have conducted to grow the game of golf at your facility, in your community, in your Section and nationally. Our Player Development program is a blending of teaching, coaching, and relationship building. To be successful in developing golfers, one must be an expert in all three of these components. The first component is good training in the technique of hitting a golf shot. My beginning instruction focuses on controlling the contact of the ball. By controlling the low point in the swing and the balance, my players quickly start striking the ball more consistently. Then I move to controlling the curve by educating and training players in the face and path relationship. Lastly, I educate them on how to create enough speed in the swing to propel the ball far enough to play the game. The second component in my development program is coaching. I first simulate a round of golf in our bays with our ball flight technology. Next, I move to coaching players on course. This happens in multiple ways. First, hands on training in the field helps ensure that players leave with a better Golf IQ. Second, there is also a time to be a coach on the sidelines watching how your players complete to be to uncover ways to help players score more effectively. Lastly, this coaching environment helps players learn the rules and etiquette as well as the art of playing golf. Finally, to be able to teach and coach the game, you must build a great relationship with your students. I don t believe in just being a facilitator of information, but I build a personal relationship with each student. That way I can inspire them in their golf journey. This blending of teaching and coaching with a customized approach to each student s needs is what leads to a 96% success rate exemplified by decreasing students handicaps by an average of 6 shots during their first year of instruction. In combination to my standard development program I work to offer unique secondary events tailored to the different playing abilities of my students. One of the most challenging parts of developing players is to help translate what they learn in the lesson to the course and from the course to competition. To achieve this, I have developed three programs to address this. Get GOLFTEC Ready This is a takeoff on the PGA s National Player Development Program, Get Golf Ready. The program is designed to help beginners in our teaching facilities transition from the lesson tee to the course with other players around. These players consist of men and women, young and old, who have taken lessons but have never played a round of golf. Our participants are divided into groups consisting of three beginners and one coach in a 9-hole playing experience. The day starts off with an introduction of how to book tee times, check into the pro shop, choose proper attire, and develop pre-round warm ups. Once onto the course the coach starts by covering basic course layout and strategy,

etiquette, club selection, and rules. After laying the groundwork of the game over the first three holes, I move into teaching the game from 50 yards and in. The coaching moves into putting, chipping, pitching, bunkers, and club selection around the greens. Finally, to complete the instructions, participants get the opportunity playing tee to green for three holes. During these three holes, the instructor coach s players on shot and club selection, pace of play, order of play, and basic technique. Students leave with smiles on their faces and coaches with the joy and satisfaction of creating a new golfer. GOLFTEC Texas Cup Over 10 years ago, I initiated and developed a tournament for students to transfer their lessons into competition. I started the GOLFTEC Texas Cup which is an annual Pro-Am tournament designed for pros to teach in a competitive environment. Three students are paired with one professional. The event started with 5 teams and has now grown to include the participation of all the GOLFTEC teaching facilities across the state. The event is now in its tenth year with over 20 teams. Each professional who competes with his students spends the 3-day event educating their players on how to use a practice round to prepare for the competitive event, control the emotions of competition, play to one s strengths, deal with errant shots, and course management. This event gives our instructors the opportunity to coach players through all the details of competition and helps create long lasting coach/student relationships. Performance Teams I believe that developing elite players is not just about teaching and coaching. There is a time and place to change swing techniques, and there is a time to adjust a strategy or coach through a situation. But the 3 rd aspect of developing elite players is through competition. To help players grow, I help organize playing groups for my best players by introducing them to each other and setting up play dates. As players start participating in these scheduled practices, they become friends and often begin playing together regularly. I feel that when advanced players are matched with average players, the better player at some point stops advancing his development. However, when they compete with players better than themselves, they recognize their weaknesses. Usually, their competitive nature helps motivate them to work harder to improve. 3.) Please indicate the number of consumers and/or members of your facility that have attended your various growth of the game programs (list by each program conducted). Over the previous 12 months in the GOLFTEC Upper Kirby facility, in which I coach full time, we have had over 1,000 participants in our development programs. Combining all 6 of the facilities I directly oversee, we have had over 4,000 participants in the last 12 months. We are constantly changing and adapting our content provided to our customer base. With the 3 most popular programs listed above, here is our participation over the last 12 months: 1. Get Golf Ready 192 participants 2. GOLFTEC Texas Cup 68 participants 3. Performance Teams 100 participants 4.) Please describe what you have done to measure the impact of these programs in areas of player retention, increased rounds played, increased customer satisfaction, and impact on growing revenues and decreasing expenses. Tracking participation and results is critical to the student s and professional s success. I utilize an internal client relations management system to measure results and participation. I also measure customer satisfaction through semi and annual client surveys and net promoter scores from quick surveys that go out to customer right after a lesson. NGF (National Golf Foundation) states of the 22 million golfers in the US only 3.6 million of them participate in golf lessons. Those students who do participate average only 3 lessons per year. My team at Upper Kirby is quadrupling that average at 12 per year. We measure success at GOLFTEC Upper Kirby in the following three ways: - Surveys o Annual survey 96% of customers state they are playing better than before starting with GOLFTEC Students indicate they drop 7 shots on average in their first year. o NPS (Net Promoter Score of 86. That rivals and beats companies like Apple, Amazon, Starbucks, and Disney. - Participation o Average student takes 12+ lessons a year (This includes someone that comes in for a one time visit to those most dedicated students who might work 50+ times with their coach).

o Total lessons given: 11,688 o Average coach gave 1,900 lessons last year o Number of club fittings: 553 o Number of new students acquired this year: 265 o Lessons Given: Grown 10 years in a row o Grew customer participant/base: 17% (142 Students from LY) - Sales o Grown by over 10% this year in total sales o Retention Ratio of over 75% o Renewal rate of 60%. This is measured by once a student finishes his initial package of lessons, he or she renews by purchasing a 2 nd series of lessons 5.) Please describe your best practice idea(s) that have been successful in your growth of the game programs. Like many others, I started in player development to help pass on the knowledge I learned as a player and from others that mentored me. As an instructor, I started by learning how to fill my own lesson book and to develop programs that will be well attended, help elevate the level of play in my students, and retain my customer base. Once I achieved a full lesson book, due to daily time constraints, I felt that there was a limit to the scope of influence I could have on the game and to golfers. As other professionals achieve this same scenario, the best practice I can offer is how to scale high quality instruction. Organizing a group of professionals together with a common goal can and will successfully help grow the game on a much bigger level. Currently I lead a team of 6 coaches directly at GOLFTEC Upper Kirby and 14 others indirectly in the Houston Market. Training a team of instructors in which we all have common beliefs about how to develop players from a technique, coaching, and relationship mentality allows me to affect the games of more players that I could ever influence on my own. This strategy has been effective in introducing, engaging, and retaining thousands of golfers annually. I enjoy knowing how many people across the market are getting good quality instruction due to these efforts. I also enjoy the time spent training and mentoring instructors and equipping them with the knowledge that will aid them in becoming a better instructor. Today golfers want the latest technology, competent instructors, and significant improvements in their game in a time efficient manner which is exactly what we provide. Our lessons utilize the latest technology available including video, motion measurement, and launch monitor technology. To meet the students time restraints, lessons are typically more frequent and for shorter lengths. To provide retention, students utilize their personal GOLFTEC website built and updated by their instructor after every lesson. This information can also be viewed mobile as well. Once the student starts making progress indoors, we encourage our players to participate in one of the three programs listed above in question 2, a playing lesson, or a short game clinic to help coach our players to succeed outdoors. The balancing of indoor technical training and outdoor coaching creates success for our students as well as success for our local outdoor partners in which we drive revenue, rounds, and members back to the green grass facilities. 6.) 30 Name: Ben Willman PGA Classification: A6 Facility:Axis Golf Academy at Woodforest GC Year Elected to Membership in the PGA: ) Please briefly describe your involvement and significant leadership contributions through mentoring and sharing with other PGA Professionals about growing the game of golf. At our Section Annual Meeting last year I presented with my business partners about my Academy and shared with fellow professionals what has worked well for us and how we have become a successful Academy. I also presented at the Proponent Group Summit earlier this year in Orlando on the topic of Club Fitting for Juniors. I explained to fellow professionals from around the country how to properly fit juniors with golf clubs. I am the Captain of Axis Junior League Golf team this year. We will be traveling to the Nation Championship in Arizona this November for the second year in a row. I ve consulted several local professionals at the Academy on better teaching/coaching practices. Axis Golf Academy held a Prep for college night last year where we had five college coaches and a mediator discussing what college coaches are looking for in potential students. Subjects such as which tournaments high school kids should be playing and specific skills coaches look for in students were covered. 2.) Please describe each of the programs you have conducted to grow the game of golf at your facility, in your community, in your Section and nationally. DCP clinic This clinic is 90 mins long and divided into three 30 min sessions which specifically get kids ready for the Drive/Chip/Putt tournament. There are only 4 clinics in total. Summer/Holiday Camps These half day (9:00-12:00) and full day (9:00-3:00) camps teach kids everything about golf from the full swing to the putting stroke. Lunch is followed by playing holes in the afternoon. Axis Little Tykes Tour This is a once a week hourly clinic designed to teach the fundamentals of the game of golf to kids between 6-9 years of age. Axis Junior Tour - Once a week hourly clinic designed to teach the fundamentals of the game of golf to kids between years of age. Kids generally graduate from the Little Tykes Tour into this clinic. Axis Performance Tour Similar to Little Tykes and Junior tour but for kids aged More Advanced Junior League Golf This is an STPGA program. Off Campus PE After school program lasting either 90 mins or three hours. Linked to school Home School Golf Clinics designed for home school kids to learn golf and socialize with other kids. Aim Point - Clinic designed to improve putting Girls Golf Rocks Clinics designed to improve girl golfers High Performance Camp Full day camps designed for more advanced kids.

3.) Please indicate the number of consumers and/or members of your facility that have attended your various growth of the game programs (list by each program conducted). DCP clinic 65 kids Summer/ Camps 80 kids Axis Little Tykes Tour 25 kids Axis Junior Tour 35 kids Junior League Golf 41 kids Off Campus PE 8 kids Home School Golf 12 kids Aim Point clinics 30 adults/kids Girls Golf Rocks New program for the Academy. Clinics start next week High Performance Camps 10 kids Speed Clinic 18 kids Ladies Speed Clinic 8 4.) Please describe what you have done to measure the impact of these programs in areas of player retention, increased rounds played, increased customer satisfaction, and impact on growing revenues and decreasing expenses. I am in charge of all the finances in our Academy so I know exactly which programs are popular and which aren t. If the program is growing from one year to the next, I will keep it going. If not, then I will think about reasons why it isn t so popular. One reliable source of information regarding to customer satisfaction are the reviews that our students can write on our online scheduler. 5.) Please describe your best practice idea(s) that have been successful in your growth of the game programs. At my previous job, I started a program we called Junior Tour. It was for kids under 12 that wanted to get better at golf. The clinic was an hour long followed by playing holes. The number of holes depended on how good/experienced the player was and varied between 1-9 holes. The program grew, so I decided to do it twice a week. As more and more kids attended it became obvious that I had to split the age groups into 6-9 (Little Tykes Tour) and and run them on 4 different days. As the kids began to age out of Junior Tour, it became obvious again that they still wanted to participate in a group setting so I started Performance Junior Tour. In my opinion, the key to the success of these programs have been getting the kids onto the course after the clinic. The parents generally come and watch and everyone has a great time. When the kids can get on the course on a weekly basis, they will always improve quicker even if they are only playing a couple of holes. Junior League Golf is another massive program at the Academy. The atmosphere created by the team, as well as the camaraderie play a significant role in the success of this program.

6.) Please briefly describe your level of service to your PGA Section and/or to the PGA of America. I m currently a member of the Tournament Committee. With all the kids that come through the Academy doors, I funnel hundreds of kids into all the STPGA programs. I m Captain of the Axis Junior Golf League Team, which will be representing Texas in November at Nationals at Grey Hawk in Arizona. 7.) Please list any Section or National PGA awards the nominee has received and list the year received: (Please state whether National or Sectional and list Section.) Player of the year 2014 Player of the year The STPGA Awards Committee strongly recommends that the application for this award be typewritten and will not accept additional materials beyond the answers to the questions on the application.
