2 Welcome to the Husky Basketball Season! Congratulations on your son making the Horizon basketball team! The coaches will ask a great deal of your son on the court this season, but parent support behind the scenes is equally important for the success of this program. The Husky Hoops Club is the parent support group for the Horizon Basketball Program. We help the coaches by organizing the many activities and financial commitments required during the basketball season. To simplify the process, we have prepared this Parent Packet, which highlights information about the program, as well as action items that need your immediate attention. (See Parent Checklist on next two pages.) Please read the packet thoroughly, complete the checklist, and bring it and all required paperwork and payments with you to the Parent Meeting from 6:30 to 8:00 p.m. on Tuesday, Nov. 14 th, in the small gymnasium (Dance Room). It is mandatory that at least one parent attend the meeting. All parents of players are invited to attend. When you arrive at the meeting, check in at the Husky Hoops tables where you can turn in all your paperwork and payments. Each table will correspond with an item on the Parent Checklist. When you visit a table and complete all requirements, that item will be marked as completed on your form. When all items are checked off, you can turn in your checklist at the last table. To aid us with our bookkeeping, we ask that you write a separate check for each of the checklist items. We apologize for the inconvenience, but greatly appreciate your assistance in keeping our finances organized. We will have Husky basketball logo T-shirts, polos, hoodies and car decals for sale at the meeting, so bring your checkbook or cash and do some early holiday shopping. 3 Complete the two checklist pages and any necessary actions and bring them to the Parent Meeting Boys Basketball Parent Checklist Checklist Item #1: Pay $200 Basketball Participation Fee Checks are no longer accepted by the Husky Hoops Club for this fee, so you must complete this task BEFORE the Parent Meeting. Directions for payment are included on the sheet in this packet titled PAYING YOUR BASKETBALL PARTICIPATION FEE. Coaches will not issue a uniform to players until this payment is completed. *If paying this fee creates a financial hardship for your family, contact Assistant Principal Kandie Kelley at kkelley@pvlearners.net to discuss options. *A family should not pay more than the maximum of $400 each year in PVUSD Participation Fees. If your family has already paid that amount this year for other sports/district activities, contact DeDe DeMoss at (M-F between 7:30am and 3:30pm). Checklist Item #2: Bring Snack Bar Donation To ensure that we have a full supply of items in the Snack Bar, each player is required to bring a designated item to the Parent Meeting. Each team has been designated to bring the following: Freshmen: Each family should bring three 8-packs of Gatorade (plastic 20-oz size ONLY) Approved Flavors: Orange, Fruit Punch, Lemon-Lime, Grape or Mountain Berry Blast (blue) Jr. Varsity: Each family should bring three 12-packs of soda (12-oz cans ONLY) Approved Flavors: Coke, Diet Coke, Dr. Pepper or Sprite ONLY Varsity: Each family should bring at least 1 box/package of 48 (or more) items of the following: Sour Punch Kids (2-oz), M&Ms (2-oz plain or peanut), Snickers (1.86-oz bars), Skittles (14- oz bag), or Starburst (2.07-oz) Don t want to shop? Donate $20 at the Parent Meeting instead (Make checks* payable to Husky Hoops Club). Checklist Item #3: Sign Up for 4 Volunteer Slots for the Season Each family is required to sign up for a minimum of 4 volunteer shifts at the Snack Bar (during a home game) and/or the Holiday Hoops Invitational Tournament held at Horizon. Directions on how to sign up via Sign Up Genius are included on the sheet titled VOLUNTEER REQUIREMENTS. Signups are on a first-come, first-served basis, so don t delay in logging on to Sign Up Genius using the links below: Snack Bar Sign Up during Home Games: Holiday Hoops Tournament Sign Up: Don t forget you must sign up for a minimum of 4 shifts. Checklist Item #4: Complete the Player/Parent Contact Information Form With your permission, parent and cell phone numbers will be included in a parent roster and shared with other players parents to facilitate volunteering and other activities. Only coaches will have player information.
4 Checklist Item #5: Complete PAWS Membership Form and Pay $30 Fee All volunteers at Horizon High School are required to be members of PAWS, the Horizon Booster Club. PAWS carries insurance that covers the operation of all parent booster groups, including the Husky Hoops Club. If you are already a member of PAWS, no action is required. If you aren t, please fill out the form in your packet titled PAWS MEMBERSHIP APPLICATION and bring it and a $30 check* payable to Horizon High School Boosters, Inc. to the Parent Meeting with you. Checklist Item #6: Complete the Husky Hoops Website Release form Log on to the HuskyHoopsClub.com website and follow the instructions on the form titled HUSKY HOOPS WEBSITE RELEASE FORM in this packet. Checklist Item #7 (optional): Become a Kennel Club Sponsor We are asking each family to consider becoming a Kennel Club Sponsor by making a donation to our Husky Hoops Booster Club. This money will help pay for uniforms, backpacks, equipment, travel expenses and an end-of-season banquet for our players. The more donations we receive, the less fundraising parents and players will need to do during the season. Anyone who joins will receive recognition in our full-color Media Guide, as well as valuable gifts such as Husky Snack Bar Bucks, basketball logo T-shirts, car decals and a copy of our commemorative Media Guide. (Benefits vary depending on the level of sponsorship.) To become a Kennel Club Sponsor, simply fill out the form in your packet titled KENNEL CLUB SPONSORSHIPS and bring it the Parent Meeting, along with a check* payable to Husky Hoops Club. Thanks in advance for your support and generosity! Checklist Item #8 (optional): Purchase a Spirit Ad for Your Player in Our Media Guide and/or Become an Advertiser in Our Media Guide Our commemorative, full-color Media Guide is produced each year as a fundraiser and keepsake for the players and their families. Player profiles are included for every team (boys and girls varsity, JV and frosh), as well as information about the coaches, schedule, season outlook and sponsors. The guides will be sold at all home basketball games. Each year we offer family members and friends an opportunity to purchase Spirit Ads that recognize and honor their player. The full-color Media Guide is a beautiful keepsake and an easy way to show your Husky pride. If you would like to purchase an ad, fill out the flyer in your packet titled HONOR YOUR PLAYER WITH A SPIRIT AD and bring it with a corresponding check* payable to Husky Hoops Club to the Parent Meeting with you. AND/OR If you own a business, placing an advertisement in our Media Guide is an outstanding way to promote your products or services to the Horizon community AND position yourself as a strong community partner. If you would like to join our exclusive group of Horizon business supporters, fill out the form titled MEDIA GUIDE ADVERTISING FORM and bring it with a corresponding check* payable to Husky Hoops Club to the Parent Meeting with you. * To aid us with our bookkeeping, we ask that you write a separate check for each of these checklist items. 5 Paying Your Player s Basketball Participation Fee (Must be paid before team uniform is issued) Option #1: Pay the Fee Online To make payments as easy and efficient as possible, the Horizon Bookstore highly recommends that you pay online with a credit card. Just follow these simple steps: Visit the new Horizon Webstore at and scroll down to Find It Fast. Click on Webstore near the bottom of the page in the left-hand column under Parents. (If you re using a mobile phone, click on Find It Fast and then scroll down and click on Webstore in the lefthand column.) Use your player s Username and Password to log in at the bottom of the page. Username is your student s ID number. Password is your child s last name (First letter is capitalized; all other letters are lowercase. e.g. Smith.). Note: if you changed the password and don t remember it, go to the Contact Us page and the Bookstore for help. Select Items at Student s School under the Shop heading. o o If you are NOT paying with a tax credit, select ATHLETIC PARTICIPATION FEE NON-TAX CREDIT. (Do NOT select Tax Credit Donations Athletics ) When the page opens, select Participation Fee Non-Tax Credit Basketball, and then click on the Buy button. Confirm that the cart contents are correct and click Checkout. If you ARE paying with a tax credit, select PARTICIPATION FEE TAX CREDIT. When the page opens, select Participation fee Tax Credit Basketball and then click on the Buy button. Confirm that the cart contents are correct, and then click Checkout. What is a Tax Credit? Basketball activity fees can be paid with an Arizona State Tax Credit donation, which can be used toward certain extra-curricular school programs. If you pay using a Tax Credit, the FULL amount you pay will be subtracted from the Arizona taxes you owe for Unlike a tax deduction, a tax credit is a FULL refund of your money. This tax credit is available to all Arizona taxpayers; individual taxpayers can contribute up to $200 each year using a Tax Credit; married couples can contribute up to $400 per year. After you complete the above steps, confirm your purchase by clicking on Checkout Step 1: Additional Info. Fill out parent s name and address on the next screen, then click on Checkout Step 2: Payment. Fill out all fields for credit card payment, including your billing address, then click on Pay Now. Follow the prompts and be sure to include your address, so that a receipt can be ed to you usually within a few minutes of completing the above steps. If you don t receive an receipt, check your spam/junk folder. If you can t find it, go back to the Horizon Webstore and log in again. Under Reports, select Purchase History. A list of your recent purchases will appear. Print a copy of your Participation Fee receipt. Option #2: Pay the Participation Fee in Person at the Horizon Bookstore Visit the Horizon High School Bookstore during normal business hours to make your payment. (They accept cash, checks and credit cards). Ask the Bookstore to provide you with a hard copy of the receipt, so you can bring it to the Parent Meeting. Please note: There is no guarantee that the bookstore will be able to process the payment while you or your student is waiting. Please be understanding if they are unable to process it when you drop off the payment, and know that you may have to return later to receive the receipts for your payments. It is likely this may happen, as the Bookstore remains busy throughout the day. This is why the Bookstore strongly recommends that you pay online using Option #1 instructions above.
6 Volunteer Requirements The success of this program depends on EVERY player s family volunteering their time. We need each player s family to sign up for a minimum of 4 volunteers shifts. These shifts range in duration from one to two hours, and can be completed either at the Snack Bar (during a home game) and/or during the Holiday Hoops Invitational Tournament on Dec , We have 63 Snack Bar slots to fill and 90 slots to fill for the Invitational Tournament, which is why everyone is expected to do their part. We are using Sign Up Genius to organize all of our volunteer shifts. You should have already received two separate s with Sign Up Genius links: one for the Snack Bar and one for the Invitational Tournament. Can t find the ? H e r e a r e t h e l i n k s : Snack Bar Sign Up: Tournament Sign Up: Haven t used Sign Up Genius before? No problem. It s easy to register and volunteer. Here s how: Go to one of the Sign Up Genius links listed above. Register for an account using your address. Don t worry; it s safe you won t get any spam or marketing from Sign Up Genius. No s are shared. Find an open slot that fits your schedule from the many options that are offered. You will see names already listed for those slots that are already filled. See a friend s name? Sign up with them! Click the Sign Up button for all the slots you want. Enter your user name and password to log in OR create an account by completing a few simple fields. Click the Sign Up button YOU RE DONE! Sign Up Genius has options for you to add the volunteer slot time to your electronic calendar and it will also automatically send you an a few days in advance to remind you of all slots for which you signed up. Sign-ups are on a first-come, first-served basis, so don t delay in signing up! Remember: your family should sign up for a total of 4 shifts between the two Sign Up Geniuses. Example #1: A husband and wife work two Snack Bar shifts together to complete all 4 of their family s four volunteer slots. (Keep in mind that most parents like to volunteer when their son isn t playing -- for example, a freshman or varsity game, if their son plays on JV). Example #2: One family member sells tickets at a Holiday Hoops Invitational game while another family member keeps score to complete two of their family s volunteer slots. The next day they both work a Holiday Invitational Snack Bar shift to complete the other two of their family s four volunteer slots. Example #3: A player s mom works a Snack Bar shift during three different season home games and also sells tickets at one Holiday Invitational game to complete all four required volunteer slots for her family. The deadline to sign up for your FOUR slots is Sunday, Nov. 12th. If your family has not signed up for 4 slots by that time, we will need to assign slots to you from those that remain...so it s in your best interest to sign up yourself for the ones that best fit your schedule and interest. If you are unable to work the shift assigned to you, please refer to the team roster we will be providing you (it will include contact information for each family) to find someone to trade/work your shift for you. Please note that no one under 16 is allowed to work/be in the Snack Bar, so if you have little ones, you ll need to make arrangements for someone to watch them while you re working your shift.

We plan to create a player/parent roster that will include your player s name, and parent names and contact information. You can use the rosters to facilitate volunteer shift exchanges, carpools or any other reason you may need to reach another player s family. 13 Get Ready for Our Annual Shoot-a-Thon Our Biggest Fundraiser of the Year! Saturday, Dec. 9th Be on the look-out for an informational packet that will come home with your player very soon regarding our annual Shoot-a-Thon fundraiser. The Shoot-a-Thon is the main source of revenue for our basketball program; all players benefit from it. We are counting on your son to do his part. Funds raised help pay for shooting shirts, warm up pants, game-day polos, backpacks, the end-of-season banquet, summer league fees, and other basketball-related expenses. OUR FUNDRAISING GOAL FOR EACH PLAYER IS TO RAISE A MINIMUM OF $250. Each player will need to collect pledges from people who agree to make a donation, either based on the number of shots made (e.g., $1 per shot made) or the form of a flat donation. At the Shoot-a-Thon, each player will shoot 100 free throws and 25 three-point shots. Prizes will be awarded to the top three players on each team who raise the most money. The more the player gets in pledges, the better his chance of winning a prize! We are giving you a heads up now because your son will have a very short timeframe in which to contact people to contribute to the fundraiser, so he will get started soon. The coaching staff will be encouraging and reminding the players to find pledge donors and stay on track with reaching their goal. Players will collect all the money and turn it in at practice on Monday, December 18th. In order to make this fundraiser go smoothly, we will need 12 parent volunteers to help out on the day of the event. We ask that every parent visit the website and complete the Release Form so we can have accurate information on the site, along with team communication and alerts for any updates and schedule changes. This will also give us permission to post photos of your son on our website. Please complete the online Player Information and Release form by November 10 th using the link below: This online form only needs to be completed ONCE. If your child is a returning player and you have already completed this online form you do not need to complete it again unless information has changed.
