2 Welcome to the Huskies Basketball Program Congratulations on your son making the Horizon Basketball team! This will be an exciting opportunity for him to not only improve his basketball skills, but to also develop a strong work ethic, sportsmanship, and time-management skills. As a member of the Horizon Huskies Basketball team, he will be a part of a spirited, cohesive group where he can develop strong friendships and make memories that will last a lifetime. The Husky Hoops Booster Club is the parent support group for the Horizon Basketball program. We play a critical role in helping the coaches throughout the season. This club is completely run by parent volunteers who are committed to helping our boys succeed both on and off the court. We assist the coaches in organizing the financial commitments, as well as the many activities that take place throughout the season, including the Horizon Invitational Tournament, end-ofseason awards banquet, and publication of the full-color commemorative Basketball Player Guide. We also purchase equipment and Husky apparel for the players, as well as all food and drink supplies to stock the snack bar for the home games and Horizon tournament. None of this would be possible without the hard work and dedication of all our parent volunteers. To simplify the process and make the season run as smoothly as possible, we have prepared this packet of Parent Action Items that requires your immediate attention. Please set aside at least one hour to complete the attached Parent Checklist before our mandatory Parent Meeting. This meeting will be held from 6:30 to 8:00 p.m. on Tuesday, Nov. 13 th in the Horizon small gym (Dance Room). Please read the packet thoroughly, complete the checklist, and bring all required paperwork and payments with you to the meeting. It is mandatory that at least one parent attend the meeting. Of course, all parents of players are welcome to attend. When you arrive at the meeting, check in at the Husky Hoops tables where you can turn in all your paperwork and payments. Each table will correspond with items on the Parent Checklist. When you visit a table and complete all requirements, those item will be marked as completed on your Parent Checklist. We will have a limited supply of Husky basketball logo T-shirts, polos, hoodies and car decals for sale at the meeting, so bring your checkbook or cash and do some early holiday shopping. 3 Basketball Parent Checklist Parent Name: Circle One: Frosh - JV - Varsity #1 Complete Parent Contact Info & Volunteer Opportunities Form With your permission, parent and cell phone numbers will be included in a parent roster and shared with other players parents to facilitate volunteering and other activities. Only coaches will have player information. Also, consider volunteering for one of the open Husky Hoops positions shown on the attached form. The Husky Hoops Club is completely run and supported by parents and wouldn t exist without our dedicated volunteers. Complete the attached Parent Contact Information form and bring it with you to the Parent Meeting. #2 Pay $250 Husky Team Player Fee The Husky Hoops Booster Club requires funding in order to perform its basic functions throughout the entire season and to organize important player events. Here is a partial list of how the Team Player fee is used: Host annual Invitational Tournament at Horizon Host end-of-season awards banquet Host Senior Night celebration Purchase new NIKE polo, T-shirt, shorts and shooting shirt and backpack with embroidered last name for all players Stock all food and drink items for home games and Horizon Tournament Pay fees for: o Great Shootout Tournament at Cactus Shadows High School o Game day videos o Website maintenance o Husky Basketball banners displayed in the gym and snack bar Design and print full-color commemorative Basketball Player Guide Provide HUDL access for team and players Please make your check payable to Husky Hoops Club in the amount of $250. Our club is a nonprofit C Corporation, so consult your accountant about potential tax deductions. Note: This fee is not tax-credit eligible. #3 Pay the District Athletic Participation Fee The Paradise Valley Unified School District requires all students participating in athletics to pay a $200 AIA Participation Fee for each individual sport. The entire fee goes directly to the school district. Directions for payment are included on the attached handout, Paying Your District Participation Fee. Page 3 of 19

4 If your child participates in several sports/activities or you have more than one child in public school activities, you may have already reached the family maximum of $400 per school year. If your family has already paid this maximum amount, contact Pam Houle at to confirm (M-F, 7:30 a.m. to 3:30 p.m.). If paying this fee creates a financial hardship, contact Asst. Principal Shelley Strohfus at sstrohfus@pvlearners.net to discuss options. #4 Have Your Son Complete the Player Profile Questions for Our Annual Basketball Player Guide As part of your Team Player Fee, you will receive a copy of our annual full-color commemorative Basketball Player Guide. Included in this booklet will be the official game schedule, team pictures, as well as an individual photo and profile of each player. Please have your son fill out the attached Player Profile Questions form and bring it with you to the Parent Meeting. #5 Sign Up for Four Volunteer Shifts for the Season The Husky Hoops Club is completely run by parent volunteers. Each family is required to sign up for a minimum of four volunteer shifts at the snack bar during home games and/or the Horizon Invitational Tournament. Directions on how to sign up via Sign Up Genius are included in the attached Sign Up for Volunteer Shifts handout. Sign-ups are on a first-come, first served basis, so don t delay in logging on to Sign Up Genius using the links below. For your convenience, these links are also on the homepage of the huskyhoopsclub.com website. Snack Bar Sign-Up during Home Games: Shift Sign-Up during Horizon Invitational Tournament (12/20 to 12/22): #6 Complete PAWS Membership Form and Pay $30 Fee All volunteers at Horizon High School are required to be members of the Horizon Booster Club, PAWS (Parents Association Working for our School). PAWS carries insurance that covers the operation of all parent booster groups. If you haven t yet paid your annual PAWS fee, please complete the attached PAWS Membership Application and bring the form and payment to the parent meeting or sign up and pay online at #7 Complete the District Site Volunteer Application Every parent who volunteers at Horizon High School must complete the attached Site Volunteer Application and Agreement form. This is required by the Paradise Valley Unified School District. Each parent must fill out a separate application. Complete the attached application and bring it with you to the Parent Meeting. #8 Complete the Husky Hoops Website Release Form Go to the HuskyHoopsClub.com website and click on Release Form in the upper right corner of the page. Please fill out all fields and click Submit Information when you re done. Page 4 of 19

5 ADDITIONAL WAYS TO SUPPORT THE HORIZON BASKETBALL PROGRAM #9 Consider Making an AZ Tax Credit Donation to Horizon Boys Basketball A Win-Win for All You can receive a dollar-for-dollar tax credit through the Public Schools Tax Credit when you make contributions and pay extracurricular fees directly to an AZ public school. (The maximum credit allowed is $400 for married filing joint filers and $200 for single, heads of household and married filing separate filers.) If you have not already paid $400 in public school tax-credit eligible contributions and fees for the school year, please consider making a donation designated specifically for our Horizon Boys Basketball Program. Here s an example: If you paid the $200 District Participation fee for your son s basketball and a $25 fee for your daughter s Robotics Club, you may be eligible to make a Public School Tax Credit donation for the difference: $400 family max. minus $225 total fees = $175 donation to Horizon Basketball. This would give you a dollar for dollar $400 tax credit with no out-of-pocket cost. AZ Tax Credits give you a FULL refund of your money. To gain this tax benefit and support Horizon Basketball, please visit the Horizon Webstore and follow the instructions on the attached handout, Reduce Your Tax Bill. This year our varsity team will have brand new NIKE uniforms that were paid for using the Public School Tax Credit donations that parents made directly to our basketball program. #10 Purchase Husky Apparel and Show Your School Spirit One of the easiest ways to support your son is by proudly wearing your green and gold Horizon T- shirts or hoodies as you cheer them on this season. You can also customize a T-shirt or sweatshirt for you or your son by adding the player s name to the back of the shirt. If you would like to order Husky apparel, car decals, or water bottles, please complete the attached Husky Apparel Order form and bring it with you to the meeting. Additional Husky items will be available at the Parent Meeting. Order deadline is Saturday, Nov. 17 th. Prices are based on bulk quantities. We only place one large order per season, so order your Husky swag today! #11 Join the Green & Gold Husky Sponsorship Club We are grateful for all our generous parents who go above and beyond to help our players. If you would like to make an additional Husky Hoops Club donation, you will become an exclusive member of our Green & Gold Husky Sponsorship Club. In return for your generosity, you will receive valuable Husky Hoops gifts worth up to $200 and recognition in our Basketball Player Guide and HuskyHoopsClub.com website. Ad deadline is Saturday, Nov. 17 th. #13 Become a Business Advertiser in Our Basketball Player Guide If you re a business owner or work for a business that would like to advertise with us, please complete the attached Place a Business Ad form. Advertising with us will help you attract more customers and be viewed as a Horizon community partner. Husky parents love doing business with Horizon advertisers! Ad deadline is Saturday, Nov. 17 th. #14 Download the Remind App to Your Mobile Phone Our goal is to establish clear, timely communication among parents, coaches and our Husky Hoops Board. To simplify communication, we are asking each parent to download the free Remind app before the parent meeting by visiting your App Store. This app will allow us to send reminder texts to parents throughout the season. After you download the app, you can join our Husky Hoops Parent group by texting the to the following phone number: If you would like to opt out of receiving Remind texts, please let us know at the Parent Meeting or us at horizonhoopsboard@yahoo.com. If you have difficulty setting up the Remind app, our board members will assist you at the Parent Meeting. (Player contact information will NOT be shared.) This list can be used to facilitate volunteer shift exchanges and carpools. PLEASE PRINT. 8 Checklist #3 - Pay Your Player s District Participation Fee (Must be paid before uniform is issued) Option #1: Pay the Fee Online To make payments as easy and efficient as possible, the Horizon Bookstore highly recommends that you pay online with a credit card. Just follow these simple steps: Visit the Horizon website at and scroll down to Find It Fast. Click on Webstore at the bottom of the page in the left-hand column under Parents. (If you re using a mobile phone, click on Find It Fast and then scroll down and click on Webstore in the left-hand column.) Use your player s username and password to log in at the bottom of the page. Username is your student s ID number. Password is your child s last name (First letter is capitalized; all other letters are lowercase. e.g. Smith.). Note: if you changed the password and don t remember it, go to the Contact Us page and the Bookstore for help. Select your student s name, then choose Items at Student s School under the Shop heading. o o If you are NOT paying with a tax credit, select ATHLETIC PARTICIPATION FEE NON- TAX CREDIT. (Do NOT select Tax Credit Donations Athletics ) When the page opens, select Participation Fee Non-Tax Credit Basketball, and then click on the Buy button. Click Checkout and confirm that the cart contents are correct. If you ARE paying with a tax credit, select ATHLETIC PARTICIPATION FEE TAX CREDIT. When the page opens, select Participation Fee Tax Credit Basketball and then click on the Buy button. Click Checkout and confirm that the cart contents are correct. What is a Tax Credit? Basketball activity fees can be paid with an Arizona State Tax Credit donation, which can be used toward certain extra-curricular school programs. If you pay using a Tax Credit, the FULL amount you pay will be subtracted from the Arizona taxes you owe for Unlike a tax deduction, a tax credit is a FULL refund of your money. This tax credit is available to all Arizona taxpayers; individual taxpayers can contribute up to $200 each year using the Public School Tax Credit; married couples can contribute up to $400 per year. After you complete the above steps, confirm your purchase by clicking on Checkout Step 1: Additional Info. Fill out parent s name and address on the next screen, then click on Checkout Step 2: Payment. Fill out all fields for credit card payment, including your billing address, then click on Pay Now. Follow the prompts and be sure to include your address, so that a receipt can be ed to you usually within a few minutes of completing the above steps. If you don t receive an receipt, check your spam/junk folder. If you can t find it, go back to the Horizon Webstore and log in again. Under Reports, select Purchase History. A list of your recent purchases will appear. Print a copy of your Participation Fee receipt. Option #2: Pay the Participation Fee in Person at the Horizon Bookstore Visit the Horizon High School Bookstore during normal business hours to make your payment. (They accept cash, checks and credit cards). Note: There is no guarantee that the bookstore will be able to process the payment while you or your student is waiting. If they are unable to process it when you drop off the payment, you may have to return later to receive the receipts for your payments. It is likely this may happen, as the Bookstore remains busy throughout the day. The Horizon Bookstore strongly recommends that you pay online using Option #1 instructions above. Page 8 of 19

This full-color guide is a beautiful keepsake and can be shared with friends and family for many years to come. 10 Checklist #5 Sign Up for Volunteer Shifts The success of this program depends on EVERY player s family volunteering their time. We need each player s family to sign up for a minimum of 4 volunteers shifts. These shifts range in duration from one to two hours and can be completed either at the snack bar during a home game and/or during the Holiday Hoops Invitational Tournament on Dec. 20 th to 22nd. We are using Sign Up Genius to organize all of our volunteer shifts. Use the links below to sign up. For your convenience, these links are also on the homepage of the huskyhoopsclub.com website. Home Game Snack Bar Sign Up: Shift Sign Up during Horizon Invitational Tournament (12/20 to 12/22): Upgenius.com/go/5080D4CA5A82F4-husky Haven t used Sign Up Genius before? Here s how to register and volunteer: Go to one of the Sign Up Genius links listed above. Register for an account using your address. Don t worry; it s safe you won t get any spam or marketing from Sign Up Genius. No s are shared. Find an open slot that fits your schedule from the many options that are offered. You will see names already listed for those slots that are already filled. See a friend s name? Sign up with them! Click the Sign Up button for all the slots you want. Enter your user name and password to log in OR create an account by completing a few simple fields. Click the Sign Up button and YOU RE DONE! Sign Up Genius has options for you to add the volunteer slot time to your electronic calendar and it will also automatically send you an a few days in advance to remind you of all slots for which you signed up. Sign-ups are on a first-come, first-served basis, so don t delay in signing up! Remember your family should sign up for a total of 4 shifts between the two Sign Up Geniuses. Example #1: A husband and wife work two snack bar shifts together to complete all four of their family s four volunteer slots. (Keep in mind that most parents like to volunteer when their son isn t playing for example, a freshman or varsity game, if their son plays on JV). Example #2: One family member sells tickets at a Holiday Hoops Invitational game while another family member keeps score to complete two of their family s volunteer slots. The next day they both work a Holiday Invitational Snack Bar shift to complete the other two of their family s four volunteer slots. Example #3: A player s mom works a snack bar shift during three different season home games and also sells tickets at one Holiday Invitational game to complete all four required volunteer slots for her family. The deadline to sign up for your FOUR slots is Saturday, Nov. 17th. If your family has not signed up for four slots by that time, we may assign slots to you from those that remain...so it s in your best interest to sign up yourself for the ones that best fit your schedule and interest. If you are unable to work the shift assigned to you, please refer to the team roster we will be providing and find someone to trade/work your shift for you. Please note that no one under age16 is allowed to work or be inside the snack bar, so if you have little ones, you ll need to make arrangements for someone to watch them while you re working your shift. 14 Checklist #9 - Reduce Your Tax Bill Make an AZ Tax Credit Donation to Huskies Basketball Looking to pay less state tax? Make an AZ Public Schools Tax Credit donation that will go directly to the Horizon Boys Basketball Program. In return, you will receive a reduction in your tax bill while supporting the Horizon Huskies Basketball Program. If you have not already paid the $400 maximum* in Public School Tax-Credit eligible contributions and fees for the school year, please consider making a donation designated specifically for our Horizon Boys Basketball Program. Follow these easy steps to make your tax credit donation to the Horizon Basketball Program (takes less than 10 minutes): 1. Go to the Horizon High School website: 2. Scroll to the bottom of the page and click on Webstore in the left column. 3. Log in using your son s User Name (his Student ID #) and password, which is his last name (First letter is uppercase and all other letters are lowercase. e.g. Jones). 4. Choose your student s name and select Items at Student s School under the Shop heading. 5. Select Tax Credit Donations Athletics. 6. Scroll down to Boys Basketball and type in $200 in the second white box (or whatever amount you have remaining to meet the $400 maximum. If you have paid only the $200 basketball participation fee towards the tax credit, then you have $200 remaining to donate to our basketball program.) Then click the Buy button. 7. Click Checkout and confirm that the contents in your cart are correct. 8. Click on the blue box Checkout Step 1: Additional Info and fill out the parent s name and address on the next screen. Then select Checkout Step 2: Payment. 9. Fill out all fields for credit card payment, including your billing address. Then select Pay Now. 10. Print out a receipt and file the appropriate tax form to receive your tax credit. It s a winwin situation for you and the Husky Boys Basketball Program! What is a tax credit? A dollar-for-dollar reduction in the actual tax you owe. For example, if you owe $800 in state income taxes and you donate $400 to a public school for extracurricular activities, you can subtract $400 from your tax bill and pay the state just $400. *The maximum amount to donate for the AZ Public Schools tax credit is $400 if you are married filing jointly. If you are single or married but filing separate returns, each spouse can claim one-half of the credit that would have been allowed on a joint return. Questions?
