1 Bonnyville Minor Hockey Association 2014/2015 Online Registration Package This package applies to existing BMHA players who were registered with BMHA in the previous hockey season. If you are a registering with BHMA for the first time, please refer to the 2014/2015 First Time Registration Package what can be found on the BMHA website under the registration tab. Print this document Go to Registration tab under Bonnyville Minor Hockey website and click on REGISTRATION to complete on-line registration. Print your on-line Receipt Drop off the following to: BMHA Drop Box located in the C2 Hub, the Bonnyville Minor Hockey Office located in the southwest corner of the RJ Arena(Outside Access at the Southwest Doors) or mail to BMHA by May 30 th, 2014: Birth certificates (must be provided 1 st year in Atom only) Payment for registration Receipt for online registration 3 undated $200 security cheques payable to BMHA (if updated cheques are required) $600 cheque if you choose not to work any Special Events Please make Cheques or Money Order payable to: Bonnyville Minor Hockey Association (BMHA) Mailing address: BMHA Attention: Registrar Unit 1002, Avenue Bonnyville AB T9N 0B4

2 Important Notes Physical Address Changes If your rural physical address (not mailing address) has changed since last season, please edit your Hockey Canada profile by completing the following steps BEFORE starting your player's registration: 1. Click on My Account on the home screen 2. Click on Edit My Profile 3. Edit new rural land location in Address (1) *Please Note: This must be your Legal Land Location (ex: NW W4) NOT your Rural 911 address. 4. Click Update Zero Tolerance Policy and Parent/Guardian Pledge Bonnyville Minor Hockey Association has a Zero Tolerance Policy. Any verbal, emotional and/or physical or harassment towards players, officials, volunteers and fans will not be tolerated. Offenders will be instructed to leave the premises immediately. This Policy is now part of your online registration questions.

3 Security Deposit Cheques BMHA will now be keeping your previous year's security deposit cheques in your player's file. You only need to submit new security cheques IF the information on your cheques has changed (address, name, banking information, etc)

4 Bonnyville Minor Hockey Association 2014/2015 Registration Information In order to register your child with Bonnyville Minor Hockey (BMHA) You will need to have completed the Respect in Sport. Incomplete registration forms will not be accepted and will be returned to registrant until all information is complete. This includes, payment, bond cheques, birth certificates and Player declaration/verification forms. PAYMENT All fees must paid by May 30 th by either one of the following options: Post-dated check dated for August 1 st, 2014 Bank Draft Credit Card Cash during BMHA office hours *NSF cheques immediately VOID the player s registration and the registration process will need to be completed again with a $35.00 NSF fee and late registration charges being applied if applicable. PLAYER REGISTRATION POLICY DEADLINE TO REGISTER IS May 30 th, 2014 Players must reside within BMHA boundaries. Any players residing outside BMHA boundaries are subject to BMHA approval and appropriate provincial association approval. Initiation players must be 5 years old by Dec 31 st of the current registration year. In the event that team rosters are too full at May 30 th registration deadline, players will need to tryout for the team. If they do not make the team, a full refund will be issued or players will be released to other communities A $250 late fee will apply after May 30 th. BMHA reserves the right to accept your registration based on availability within the age appropriate division. Fee does not apply to first time players in BMHA and imports A waitlist will be created for those who register after May 30 th, but are not guaranteed a spot on a team to play. *Any required releases, concessions, or other documentation must be submitted to BMHA before the player s first ice-time, and is the responsibility of the player. BMHA will, to the best of their ability, try and ensure that players are aware of all required documentation at the time of registration. However, it is ultimately the parent s responsibility to read registration policies and complete any required documents. Please contact the BMHA

5 REQUIRED FORMS Can be found online at SPECIAL EVENTS POLICY Option 1 - Security Deposit Cheques Each parent(s) or guardian(s) must work 3 Special Events per registered child throughout the year. If unable to work, YOU are responsible to make arrangements for your replacement. Please submit 3 undated $200 Security cheques with each child s registration. NOTE: Cheques will only be cashed for if workers are a NO SHOW or have not participated in the event. Option 2 - Opt out fee You may choose not to work any of the Special Events and pay a higher registration fee. Please add $600 to your player registration cheque if you choose this option. Our special events for the year include: Live Free For a Year Raffle Sobeys gift card- you must sell a minimum of $200/ per child in BMHA Special event to be determined at the discretion Bonnyville Minor Hockey including but not limited to tournament volunteer requirements, Allumni Games, etc. In addition to registration at this time, please ensure that you are registered for any information updates sent by Bonnyville Minor Hockey by signing up for the Newsletter Signup on This is the best way to ensure that you are in the loop for the start of the hockey year. You are also able to enter more than one as well (ie work and personal) to ensure that you do not miss any Important information.
