2 Mission FlipCity South Tumblers Program Description The mission of FlipCity South is to create a tumbling program for children of all ages; 4 and up. Our tumbling levels start at Intro Tumblers and goes all the way up to Elite Tumblers. Our goal is to become one of the first teams to compete in the U.S. Olympics. FlipCity South encourages and strengthens great scholars and good behavior within our Gyms. FlipCity South will teach children the fundamentals of Tumbling, Life, Leadership, Sportsmanship and Integrity Who We Are... FlipCity South is a tumbling program formed in Lithonia, Georgia, now located in Stockbridge GA. FlipCity South Tumblers are the brother program to Flip City in Newark, New Jersey. Flip City has won a number of team titles since 1979; they have won numerous world titles and world records. Flip City has performed for a number of companies like, The New Jersey Nets, New York Knicks, LA Lakers, Philadelphia 76er s, Microsoft, Six Flags, and the 1996 U.S. Olympic to name a few. In the year, season, FlipCity South won 15 gold medals and has, doubled the number of medals in the season. FlipCity South has opened for Wal-Mart Health Fair, local churches, and have several shows scheduled this year. Intro to Tumbling Classes Designed to teach our athletes the basics needed to become great tumblers. Course Features: Front Rolls, Backrolls, Cartwheels, Roundoffs, Basics of Back Handspring Instructors: Frank Riley, Latifa Riley, Valentine Mba, Garrett Wheeler Back Handspring Classes Designed to teach the athlete the fundamentals of a back handsprings from the beginning stages to sub advance. Course Features: Beginning Back Handspring, Intermediate Back handspring, Advance Back Handspring (without a spotter), Classes are offered once a week. (Multiple classes are offered at a discounted rate.) Instructors: Frank Riley, Valentine Mba, Garrett Wheeler FlipCity South Advance Tumbling Classes Course Features: Advance Back Handsprings, Tuck, Whip, Brandy, Full, Ruby, Randys, Double Fulls Classes are offered once a week. (Choose a class, multiple days are offered at a discounted rate.) Instructor: Frank Riley FCS Competitive Team (invitation only) Our Competitive team is an invitational only team, that competes AAU, USAG, & USTA. Instructor: Frank Riley Private Lessons are available by appointments only.

Everyone must wear deodorant. All attire must smell and look clean. Open Door Policy There is no revolving door policy, there cannot be any taking days off of practice just because Practice is mandatory for competition and performance team. If your child is going to miss practice please call or either Coach Frank or or

5 as soon as you are aware. If there are any problems with Coach, Judge or Parent, it is to be discussed with Coach Frank or Latifa ONLY. No parent is allowed on the gym floor during practice or competition. Note: If any team member s show up to competition inappropriately dressed they will not be allowed to compete. 9 Absolute Release of Liability I fully understand that FlipCity South staff members are NOT Physician s or Medical Practitioners of any kind. With this in mind, I hereby release FlipCity South to render temporary first aid to my child or children in the event of injury or illness, if deemed necessary by FlipCity South, or call our doctor or 911 or FlipCity South staff member volunteer to transport my child to any healthcare facility or hospital or to arrange for an ambulance for said child should FlipCity South staff deem necessary. Parents should make their child (ren) aware of the possibility of injury and encourage their children to follow all safety rules. Waiver and Release I am fully aware of the risk or catastrophic injury, paralysis and even death as well as other damage and losses associated with participation in tumbling, events or competitions. I further agree that FlipCity South and the sponsor(s) of any FlipCity South event(s), along with the employees, agents, officers, directors, and volunteers of these organizations shall not be liable for any losses or damages as a result or the intentions of reckless conduct of one of the organizations or individual identified above. 10 FlipCity South Participation Agreement FlipCity South LLC. Power Tumbling Team, recognizing it is our obligation to make our students and their parents aware of the risks and hazards associated with the sports of tumbling, trampoline, and double mini. Students may suffer injuries, possibly minor, serious, or catastrophic in nature. Tumbling, trampoline and double mini can be dangerous and can lead to injury! While FlipCity South LLC. Power Tumbling Team maintains safety rules, it is the parents responsibility to warn the child about the dangers of gymnastics and injury. Parents should make their children aware of the possibility of injury and encourage their children to follow all safety rules and the coaches instruction. Therefore, in consideration of my child s membership in FlipCity South LLC. Power Tumbling Team and my child s participation in FlipCity South LLC. Power Tumbling Team classes, events, and activities, I (parent), agree to be bound by the following: 1. Eligibility: I agree to comply with the rules of FlipCity South LLC. Power Tumbling Team. 2. Readiness to Participate: My child will only participate in those FlipCity South LLC. Power Tumbling Team training, events, competitions and activities for which I believe he/she is physically and psychologically prepared. 3. Medical Attention: I hereby give my consent to FlipCity South LLC. Power Tumbling Team to provide through a medical staff of its choice, customary medical/athletic training attention, transportation, and emergency medical services as warranted in the course of my child s participation. 4. Waiver and Release: I am fully aware and understand that participation in the programs at FlipCity South. Tumbling & Trampoline involve motion, rotation, and height in a unique environment which carries a risk of injury, including catastrophic injury, paralysis, and death, as well as other damages or losses associated with participation in gymnastics and other physical activities. I further agree that FlipCity South LLC. Power Tumbling Team, along with the employees, agents, officers, and directors of this organization shall not be liable for any losses or damages occurring as a result of my child s participation in the event, except where such loss or damage is a result of willful, wanton, or reckless conduct of one of the organizations or individuals identified above. 5. Medical Insurance: I agree and understand that as a participant in FlipCity South LLC. Power Tumbling Team, my child must be covered by a health/medical insurance obtained by myself. I represent that I and my child are covered by primary health/medical/accident insurance through:. I further understand that FlipCity South LLC. Power Tumbling Team maintains insurance that is only secondary in nature for the purpose of

11 covering claims not covered by my own primary insurance. I understand that this insurance does not cover co-payments or deductibles and that FlipCity South LLC. Power Tumbling Team and/or their insurer will not be liable to reimburse me for any copayment or deductible. 6. Severability: In the event that any section or portion of this agreement shall be invalidated by legal declaration, it shall have no affect on the validity and legality of any other portion or section of this agreement not invalidated. I understand the degree of danger and seriousness of risk of injury and assume responsibility for discussing such dangers with my child. I certify that my child has sufficient insurance to cover any injury sustained during participation in FlipCity South LLC.
