2 Dear Parents; Thank you for allowing your daughter to become part of the Miss Oklahoma Organization, Inc. Oklahoma Star Mentoring program. The Oklahoma Star program originated several years ago and has developed into a very successful and beneficial program not only to the titleholders but also to the girls that they are mentoring. We are so excited that your daughter is going to be part of our Oklahoma Star Mentoring Program. Enclosed is a packet of information for you to complete and return to our office along with your payment. We hope your involvement with our program gives you a better understanding of the Miss Oklahoma Organization, its purpose and benefits. We strive to give young women the opportunity for personal growth and the skills needed to excel in life. Through this Oklahoma Stars Mentoring Program we extend this unique opportunity to your daughter. Besides the mentoring program each Oklahoma Star will have the opportunity to make an appearance on-stage at Miss Oklahoma 2018 and her photo will appear in the Miss Oklahoma Program Book. Each Star will receive an official t-shirt, an official Oklahoma Stars crown, have a special lunch with the titleholders on Thursday, June 7 th plus have a special luncheon on Friday, June 8 th just for the Oklahoma Stars. Each Oklahoma Star will also receive two Season Ticket Packages to Miss Oklahoma, which includes two tickets to each prelim and to the finals of Miss Oklahoma, plus 2 badges (the tickets and badges include one for the Oklahoma Star and one for a parent). The cost to participate in the Oklahoma Stars program is $ for girls new to the program or $ for those that are continuing and already have an official Oklahoma Star Crown. Information regarding additional tickets to Miss Oklahoma, tickets to the Teen Pageant will be available later. Kent Parham, our music director and choreographer, is developing another outstanding show. Each Oklahoma Star will take part in the Evening Wear Production Number during Miss Oklahoma The Miss Oklahoma Competition will be held in Tulsa, Oklahoma June 3 thru June 9 with the stage competitions being held at the Paradise Cove in the River Spirit Casino Resort, June & 9. Each Oklahoma Star is to provide her own solid white dress. The dress should fall between the knee and the ankle. The dress must NOT touch the floor. The dress can be of any white fabric and your choice of style, with age appropriate shoes. We do ask that the dresses not have excessive amounts of rhinestones and beading and no dresses with lots of petticoats. The dress may be purchased or made from any style pattern. Each Oklahoma Star will need to be at rehearsals on the day that her titleholder has Evening Wear and again on Saturday, June 9. A schedule for the week s activities and additional information will be available in May. Each Oklahoma Stars photo will appear in the 2018 Miss Oklahoma Program Book. Please

3 one (1) 300 dpi color photo (any size from wallet to 5x7) or submit a CD 300 dpi. The photo needs to be vertical head & shoulders only with no hats or crowns. Please mark your CD or with her name & local organization she is representing - Please no hats or crowns in the photo. All photos must be received by March 10, Contestants Day is scheduled for Saturday, April 7 th. This is a day when all of the local titleholders and directors gather to meet each other and to obtain information about the week of Miss Oklahoma. In the morning each Miss contestant will perform her talent that she will present during the week of Miss Oklahoma. After lunch the Teen contestants will perform their talents while the Miss Contestants will be involved in a production rehearsal. The talents rehearsals will be open for you to attend. We would like to extend an invitation for each Oklahoma Star to attend the talents sessions if she would like. This is not mandatory but as in the past some Oklahoma Stars like to attend to cheer their titleholder on as she performs her talent. Contestants Day is open to anyone wanting to attend the talent sessions. Contestants Day will be held at the River Spirit Event Center Riverside Parkway, Tulsa, OK. The titleholders are divided in two groups. All Miss Talents should be completed by 12:00 noon. We plan to take a group photo of those Stars that are in attendance during the morning. The dress is casual. More information about the day will be sent later. We look forward to having you and your daughter as part of Miss Oklahoma, its mentoring program and special events. Please contact our office anytime with your questions. Remember the number of Oklahoma Stars is limited so please mail your completed application, attached forms and payment in as soon as possible. The deadline for the completed application and supporting materials is February 28, A minimum non-refundable payment of $ must be included with the application. Or you may choose to submit the full payment with the application and supporting materials. 4 Oklahoma Star Mentoring Program The Oklahoma Stars Mentoring Program is for young ladies ranging in age from five to ten (must be 5 years old and no older than 10 years of age as of March 1 st, 2018). The purpose of this program is to mentor these young ladies by providing them with a young woman as a role model who will encourage them to achieve their best and to make wise choices and who have an interest in competing someday in the Miss Oklahoma Scholarship Pageant. The current Miss Oklahoma local titleholders will serve as mentors to the Oklahoma Stars. Each Oklahoma Star will have the opportunity as provided by the local pageant director and local titleholder to be a part of the titleholder s year. The local titleholder is encouraged to engage the Oklahoma Stars in any activity that she can to make her feel a part of her year as she prepares for the Miss Oklahoma competition, represents her local franchise at various events throughout the year and be a part of her platform and community service activities. The Oklahoma Stars are present during the week of the Miss Oklahoma. Each Oklahoma Star will participate during the evening gown segment the night that her titleholder has Evening Gown Competition. All Oklahoma Stars will also appear during a special Production on Saturday evening. Goals: Encourage younger, active involvement in Miss Oklahoma. Provide forum for personal growth. Mentor using positive role models. Have fun! Oklahoma Star Dress & Crown Requirement: Dressy, SOLID WHITE dress (any style or type of fabric. The dress should fall between the knee and the ankle. The dress must NOT touch the floor. We also ask that the dresses not have excessive amounts of rhinestones and beading and no dresses with lots of petticoats. White, silver or clear shoes (age appropriate). Due to safety concerns, the shoes should be secure when climbing stars. No backless shoes please. Your choice of white socks or tights. Nude pantyhose or bare legs are acceptable for older girls. Must wear official Oklahoma Star crown only. 3

6 GENERAL RULES and PRECEPTS OF CONDUCT FOR THE OKLAHOMA STARS The Precepts of conduct for Oklahoma Stars Program are the Foundation upon which the image and reputation of The Miss Oklahoma Organization, Inc. has been built and will continue to be the basis for its operation. The Oklahoma Star Chairmen, the Miss Oklahoma Executive Director and the Executive Producer have complete charge of all activities during pageant week. She/he has full authority to make decisions governing contact with the Oklahoma Stars. 1. Oklahoma Stars will conduct themselves at all times with dignity, grace and good manners of young women who exemplify the integrity of American youth. 2. From the time an Oklahoma Star arrives in Tulsa until the end of Miss Oklahoma week, she is under the Jurisdiction of the Miss Oklahoma Organization, Inc. and must abide by its Rules and Regulations. Any Oklahoma Star whose conduct is deemed unbecoming by the Miss Oklahoma Organization will be asked to withdraw from the Oklahoma Star Mentoring Program. 3. No one is permitted backstage at any time, except Security and stage personnel and Pageant Officials. No one may enter the Oklahoma Stars area at any time, except authorized personnel. 4. An Oklahoma Star will not participate in any interview, picture, and press contact or any types of publicity unless a member of the Oklahoma Star Committee is present. 5. All problems or complaints should be reported to the Oklahoma Star Committee Chairmen s or Miss Oklahoma Executive Director. Regardless of how small of large a complaint, please report it. A PROBLEM CANNOT BE SOLVED IF NO ONE KNOWS ABOUT IT. 6. All Oklahoma Stars must wear her name badge AT ALL TIMES. Only official Oklahoma Star crowns are allowed. No sashes or banner allowed at any official Miss Oklahoma event. Transportation, Housing & Meals: You must provide your own transportation while participating in The Oklahoma Stars Mentoring Program. All Oklahoma Stars, their families and friends are responsible for their housing and meals during Pageant activities for their stay in Tulsa. Our Headquarters Hotel is the River Spirit Casino Resort - located at 8330 Riverside Parkway, Tulsa, OK. The Miss Oklahoma rate is $ per night plus taxes. 8 OKLAHOMA STARS MENTORING PROGRAM PARENT OR GUARDIAN PERMISSION FORM AND AUTHORIZATION FOR TREATMENT TO MINORS In the event I/we cannot be reached, we, the undersigned parent(s)/guardian(s), give permission and do hereby authorize any x-ray examination, anesthetic, dental, medical or surgical diagnosis or treatment by any physician or dentist licensed by the State of Oklahoma and hospital service that may be rendered to said minor under the general, specific or special consent of the Miss Oklahoma Organization, Inc. Volunteers and/or Staff to seek any medical attention for the temporary custodian of the minor; whether such diagnosis or treatment is rendered at the office of the physician or dentist, or at a hospital licensed by the State of Oklahoma. I/We authorize the physician or dentist to call in any necessary consultants, at his/their discretion. We further authorize said physician or dentist to exercise his/their discretion in authorizing the disposal of any severed tissues or member. 9 OKLAHOMA STARS MENTORING PROGRAM STATEMENT OF UNDERSTANDING As parents of we are submitting her application for approval. We agree to conduct ourselves at all times in a professional manner as to not embarrass our daughter or The Miss Oklahoma Organization, Inc. We will ensure that our daughter will conduct herself in the proper manner at all times. We understand that this is not a competition, but one of a mentoring and learning process conducted by The Miss Oklahoma Organization, Inc. and agree to abide by all decisions made by The Miss Oklahoma Organization relative to The Oklahoma Star Mentoring Program. HOLD HARMLESS AGREEMENT Oklahoma Stars Full Having a residence at (Full Address) As the parent of a Oklahoma Star participate, I acknowledge this application for my daughter to participate in the Miss Oklahoma Organization, Inc - Oklahoma Star Mentoring Program, with production rehearsals, preliminary competitions and final show during the week of Miss Oklahoma June 3 through June 9, 2018 at the River Spirit Casino Resort, Tulsa, OK and all locations used for appearances of the Miss Oklahoma Organization/Oklahoma Stars. As part of her participation in this event, it is the Oklahoma Stars responsibility to confine and amend her performance to adapt to the constraints of the production requirements. The Oklahoma Stars hereby agrees to indemnify and hold harmless The Miss Oklahoma Organization, Inc., The Miss Oklahoma Organization Committee and any other entities which might be construed as liable from and against any and all claims for personal injuries, death, damages, cost and/or other expenses including reasonable attorney s fees, arising from or in any way connected with the use of the facilities as above noted. The Oklahoma Star further agrees to be solely responsible for all equipment, stock-in-trade, personal property, etc., utilized in connection with the use of the premises, and Oklahoma Star does hereby wave any right of recovery against The Miss Oklahoma Organization, Inc., the Miss Oklahoma Organization Committee and any other entities which might be constructed as being responsible for loss, damage, theft of such equipment, stock-in-trade, personal property, etc. This agreement is subject to termination by The Miss Oklahoma Organization, Inc., without advance notice or penalty at any time. This agreement is personal to the Oklahoma Stars Mentoring Program and may not be transferred or assigned. Please submit the following by March 10, One Head shot - this photo will be used in the program book and on souvenir buttons. Be sure background does not have trees or bushes coming out of head. This is your official photograph so should reflect age-appropriate clothing. No strapless dresses, glitz, hats, crowns, flowers, trophies, etc. MUST be submitted electronically or on CD labeled with Oklahoma Stars name and local organization name. If you are submitting professional photograph you must submit a photo release from the photographer. School photos do not require a photo release. 13 Miss Oklahoma Organization, Inc Salute Page Ads Congratulations on being part of Miss Oklahoma. As an Oklahoma Star you have many fun and exciting months ahead. Your titleholder is working hard and is thrilled to have you as part of her experience with Miss Oklahoma. As an Oklahoma Star your photo will appear in the Miss Oklahoma Program book. However, we also want to give you the opportunity to purchase a Salute Page Ad or ads if you would like. The following is a brief description about the Salute Page Ads. Also, each titleholder has been mailed a complete packet of information regarding this campaign. If you have questions, your titleholder can give you more details and show you samples or feel free to contact our office. What is a Salute Page Ad? It is an advertisement page in our official program book. The Salute Page Ad is a wonderful way for hometown businesses, friends, family and relatives to show their support to you and your titleholder. What is the cost for a full page? A full-page black & white ad is $350. or full-page color ad is $750. Each page can have one sponsor or several sponsors. How many people will view this ad? Thousands of people will see the ad not only in Oklahoma but Kansas, Texas, Arkansas, Missouri, and throughout the entire country! What do I do if I want to purchase a page(s)? a. Contact your titleholder and let her know the number of pages you would like to purchase. She has all of the paperwork and details on preparing your page. b. Have your titleholder complete the order form & salute page checklist. c. Give your titleholder your check for the designated amount due per ad page. d. Select photo(s) to be used on page(s) and give to your titleholder. It is always fun to have your photo with your titleholder in the program. Make sure your name is on the CD or back of your photo. What is the deadline for the ads? All ads, checks, and paperwork must be turned in no later than Contestants Day Saturday, April 7,
