1 LOCAL PAGEANT REQUEST FORM SOUVENIR BADGES MISS & TEEN COMMITTEE MEMBERS Please return to: MISS OKLAHOMA ORGANIZATION, INC. DO NOT FAX! A South Mingo Road, #287 Tulsa, OK or to: NOTE: Souvenir badges for Local Pageant Committee and/or your Teen Committee members will be will be prepared only upon written authorization from the Local Pageant Director. It is therefore most important that you send in the names of all pageant personnel requiring badges. REMEMBER you must have a badge for admittance to the Mabee Center for rehearsals, meetings, all visitations, events requiring a badge etc. A check, made payable to the Miss Oklahoma Organization, in the amount of $25.00 per badge must accompany this form, if ordered & postmarked by April 30 th. Any Badge Requests ordered between May 1 st & May 10 th will be $30.00 per badge. Badges ordered after May 10 th will be $50.00 each. A check must be included. Replacement badges will be $50. NO BADGES WILL BE PROCESSED DURING WEEK OF MISS OKLAHOMA. The following Miss & Teen Pageant Committee Members will attend the Miss Oklahoma 2017 in Tulsa, Oklahoma. Badges are the property of the Miss Oklahoma Organization, Inc. and can only be used by the person to whom it is issued. Badge sharing is a serious security breach resulting in confiscation of the badge and removal of both parties from the event. Miss Oklahoma officials may at their discretion, revoke the use of a badge at any time. 3Anyone attending rehearsals must be wearing a current Miss Oklahoma badge.
