3 REQUIRED TEXTS: Medical Terminology: The Language of Health Care, Marjorie Canfield Willis, Lippincott, Williams and Wilkins, Second Edition, ISBN-10: , or ISBN-13: Stedman s Medical Dictionary for the Health Professions and Nursing, Sixth Edition, Lippincott, Williams and Wilkins ISBN: [For students with limited funds: This dictionary is available for reference at the Reserve Desk in the OCC Library] GRADING: Your grade is based solely on the number of points you accumulate. Possible points are as follows: 5 EXAMS (multiple choice) (60 points each) 300 points 5 QUIZZES (fill-in) (12 points each) 60 points 11 MEDICAL RECORD ASSIGNMENTS (4 points each) 44 points Total 404 points Grades are based on: Grading scale for 60 point section exam: % = A = A = A 80-89% = B = B = B 70-79% = C = C = C 60-69% = D = D = D below 60% = F 241 & below F 35 & below F RULES: 1. YOU WILL BE ALLOWED TO MAKEUP ONE SECTION EXAM if you are absent at test time. Ten percentage points will be deducted from the makeup exam score regardless of the reason for your absence. THE MAKEUP EXAM WILL BE GIVEN IMMEDIATELY AFTER THE FINAL EXAM AT THE END OF THE SEMESTER. 3
4 2. THERE ARE NO MAKEUPS FOR CLASS QUIZZES Miss the quiz, miss the points 3. Medical Record Homework Requirement: Record your answers to each question in the spaces provided in the text. Tear the pages of the completed assignment from the book or photocopy intact pages and staple together as necessary. Print your name and seat number in the upper right hand corner of the assignment and hand it in prior to class on the date due. 4. No medical record assignments will be accepted early or late. 5. Each section exam will include information covered in Chapters 1-4. You must sit for all section exams to receive credit for the course. It will be necessary for you to purchase five (5) ParSCORE TEST FORM scantrons (F-289-PAR-L) for testing purpose. They will be collected at the beginning of the second class meeting. ACADEMIC DISHONESTY/CHEATING: An incidence of cheating will be reported to the Dean of Students with a recommendation for disciplinary action consistent with the Academic Honesty Policy and the Student Code of Conduct. Examples of cheating are: 1. obtaining information from another student during an examination 2. communicating information to another student during an examination 3. knowingly allowing another student to copy one s work 4. offering another person s work as one s own 5. taking an examination for another student or having someone take an exam for oneself 6. sharing answers for a collaborative project and take-home unless specifically authorized by the instructor 7. using unauthorized material during an examination 8. altering a graded examination or assignment and returning it for additional credit 9. having another person or company do research writing and/or writing of an assigned paper or report 10. misreporting or altering the data in laboratory or research projects Stay on target and have a great semester!! 4
