4 Glovers Martial Arts Academy Waiver Form STUDENT RELEASE OF LIABILITY, WAIVER OF LEGAL RIGHTS AND ASSUMPTION OF RISK In consideration for the opportunity for event participation and utilization of the dojo facilities Glovers Martial Arts Academy and their affiliates in continuance of karate, martial arts, general exercise and all other related activities. I hereby understand and agree to this Release of Liability, Waiver of Legal Rights and Assumption of Risk and to the terms hereof as follows: 1. I acknowledge that Martial Arts is an action sport and may require travel time and that such activity is subject to injury or death of its participants. I understand that there may be broken bones, internal injuries, paralysis or other fatal injury, as well as the loss of or damage to personal property while participating in the sport of Martial Arts. I hereby RELEASE AND DISCHARGE Glovers Martial Arts Academy and all related parties, other participants of Martial Arts, volunteers, directors, owners of equipment and the land and buildings used for Martial Arts activities, (hereinafter collectively refer to as Released parties ) from any and all liability, claims, demands or causes of action that I or my representatives may herafter have for injuries, loss of life and all other forms of damages arising out of my participation in Martial Arts activities, including but not limited to, losses CAUSED BY THE NEGLIEGENCE OF THE RELEASED PARTIES. I understand and acknowledge that Martial Arts activities have inherent dangers that no amount of care, caution, expertise or instruction could prevent and I expressly and voluntarily assume all of the risk of death or injury as well as any other form of damages that may occur while participating in Martial Arts activities whether or not said damages were caused by the negligence of the released parties. I further agree that I will not sue or otherwise make a claim against the released parties for any damages, losses or injuries sustained as a result of participation in Marital Arts or related activities. 7 Student Performance Contract I agree to the following rules and contract at all times: 1. To call my instructors Sensei. 2. To wear and to care for my uniform appropriately. 3. To use all of the bows when appropriate. 4. To get to class on time and stay until the end of class, unless otherwise arranged. 5. To respect my Sensei s, other students, parents and myself. 6. To use sensitivity and lean toward caution when training so no one gets hurt. 7. To not use physical techniques outside of class other than to practice or unless we are in competition or other Martial Arts related activities. 8. To NOT TEACH other students outside of the class. 9. To report any injury immediately to my Sensei. 10. To immediately stop if and when Sensei yells break. 11. To stop if there is an injury or bleeding, to inform my Sensei right away and get first aid. 12. To respect the word tap. I know I need to stop if my opponent uses it to avoid injury. 13. To not use or ingest any intoxicating or habit forming substances. 14. To remove any jewelry before class. 15. To lead by example for other students. Be a role model always exhibiting positive behavior. 16. To not shout, yell or speak harshly while in the dojo. 17. To always respond to my Sensei and others with Yes sir or Yes ma am. 18. If my Sensei is speaking, I need to stop whatever it is I am doing and pay attention to my Sensei. If it is necessary, I may need to remind others to do the same. 19. To sit and wait patiently in the designated areas before my class time until my Sensei calls me and lets me know it is time to begin. 20. Ranking students and staff deserve greater respect. I will follow their directions and example in the dojo. 21. To follow basic dojo rules and regulations for any and all Martial Arts related activities. 22. I agree to practice outside of class.
8 23. I agree to encourage like-minded others to join the dojo. 24. I agree to not chew gum in the dojo or on the mat. 25. I agree to not use profanity in or around the dojo. 26. I agree to care for the dojo and its equipment. At the beginning of each class I will make sure I am prepared. 9 Welcome Packet 1. Glovers Martial Arts Academy will help you become quicker, stronger, more confident and to be able to defend yourself better. You will gain self-confidence and self-esteem. 2. Basic Martial Arts Terminology: (More specific terminology and instruction can be found in the student workbook. a. Sensei: Teacher/Instructor b. Dojo: karate school c. Gi: karate uniform d. Kata: fighting an invisible opponent 3. Consistency is one of the keys to success. Every individual begins Martial Arts training for various reasons. Training and practice will help you to achieve your goals. Classes are held twice a week for this reason. Please try to attend each class unless you are ill or on vacation. 4. Proper attire and hygiene. One of these elements is to have a clean uniform. Your Gi tops should also have the dojo s patches in the appropriate places. Good hygiene is very important. You should always try to keep your finger nails and toe nails clean and short. 5. Our instructors take a personal interest in all of our students. Please feel free to address any and all concerns or questions that you may have with the Sensei s.
10 Notes from Sensei 1. All students should train at least twice a week. 2. Students are expected to be on time for class. Warm ups are important and aid in the prevention of injury. Tardiness also disrupts class. Please attempt to be on time. 3. There are certain requirements for promotion from belt to belt. Those will be discussed in more detail by me and can also be found in the student workbooks. However, one of the requirements is that the student must complete the minimum number of classes for his or her belt level. When a stripe or tip is given to a student it is the first indicator that you are getting ready to be tested. 4. The student is required to have a full set of sparring gear upon reaching the rank of yellow belt. Male students are also required to wear protective cups on sparring nights. All students must bring their sparring gear to every class. In order to be the best one must believe in themselves. Do not let anyone tell you that you are not able or that you do not have the ability. Only you have the ability to control the outcome of your life, but remember, sacrifices have to be made to achieve greatness -Sensei Cody Glover
11 Safety Equipment All students are required to have their own safety equipment upon reaching Yellow belt. Students must bring this equipment to each class. Students are required to have the following safety equipment at every class: 1. Head Gear 2. Hand Guards 3. Foot Guards 4. Mouth Guard 5. Protective Cup (Males Only) 6. Girls are to have their hair pulled back and secure

13 Other Programs and Events Available Many other activities are and will become available with Glovers Martial Arts Academy. Some are free and others require a small fee. Please see one of the Booster Club members for details. If you are interested in something and you do not see it listed below please just ask. Private lessons. Students that are struggling with a technique or would like extra help can see the Sensei for private classes. Private lessons are $25 per hour. You can set up a time for private lessons with either Sensei Cody or Sensei Cory. Throughout the year the Booster Club may put on such events such as movie night, game night, Christmas and or New Year s Eve parties. Please check your monthly newsletters for updates and information. Some other things to ask about and strive for would be: 1. Split Club 2. Black Belt Club 3. Academic Achievement
14 Winter Weather Closing Policy It is the goal of Glovers Martial Arts Academy to be open whenever possible. When deciding on whether or not to remain open, the safety of the students as well as staff is taken into consideration. Because our schedule has us opening in the late afternoon/evening hours the decision on whether or not we will open or not is usually made around 3pm. Many times there is winter weather early in the morning resulting in delays or closing and that is cleared up by the time our classes are scheduled to begin. We do not follow the Henderson County school system closing. Once a decision to close has been made we will do everything we can to let you know. 1. Receive a text message or phone call What not to do: If you think the weather is questionable and do not feel safe traveling in it, please do not attempt to travel. Your safety is most important. 15 How To Tie Your Belt Please make sure that your Gi s are cleaned and pressed before each competition and belt promotion ceremony. Also make sure that your patches are on the correct side and in the proper placement. Please do not wash your belts. Thank you so much! Tips for Good Personal Hygiene
16 Good personal hygiene habits include: Washing your body often. If possible, everybody should have a shower or a bath every day. Brushing your teeth at least once a day. It is very important to clean teeth after breakfast and immediately before going to bed. Come to class with clean, neat and short finger and toe nails. Keep your hair brushed and pulled back appropriately for class. Wear deodorant Washing your hands with soap after going to the toilet. Come to class with clean uniforms. Turn away from other people and cover your nose and mouth with a tissue or the hand when coughing or sneezing. If this is not done, droplets of liquid containing germs from the nose and mouth will be spread in the air and other people can breathe them in, or the droplets can get onto food. Give each other personal space when asked. Do not share food or drinks. Disinfect/Sanitize gear and mouth guards whenever necessary.
17 Student Illness Making the decision to keep your child out of class when they are ill can be difficult. Below you will find some guidelines that might be helpful. Since many illnesses are spread easily and we are in a close space in close contact with one another please take them in to consideration. Bringing your child to class when they are ill puts other children as well as our staff at risk for getting sick. Send to class or keep home? It is okay to come to class if you have any of the following: 1. Sniffles, runny nose and/or a mild cough without fever. 2. Aches, pains or fatigue. 3. One single episode of vomiting or diarrhea without any other symptoms. 4. Ear infection without a fever. Please stay home and rest if your child has any of the following: 1. Fever of 100 degrees or higher. 2. Pink eye, or any other eye related illness resulting in mucus or pus drainage. Your child will have to be on 24 hrs of antibiotic or have a doctors approval before coming back. 3. Sore Throat with a fever or swollen glands. If your child has strep throat they will need to be on antibiotic for 24 hrs before coming back. 4. Vomiting 2 or more times in a 24 hr period. 5. Diarrhea 3 or more times in a 24 hr period. 6. Rash with fever or itching. The exception being heat rashes or an allergic reaction as those are not contagious. 7. Chicken Pox. If your child has a confirmed case of the chicken pox please keep them home until 5 days after the blisters have appeared or until all of the pox are scabbed over and dry.
18 Rules and Regulations 1. Why Bow? To show RESPECT, HONOR and COURTESY. 2. How to get on the floor: The highest ranking belt must bow to you before you will be allowed on the floor. If you are the first person on the floor you simply bow yourself on. 3. What to do when a Black Belt is to be bowed on the floor: Someone will call out Black Belt on the floor. When this happens everyone must turn and face the Black Belt and bow at his or her command. 4. How to get off of the floor. You return to the area where you entered and turn facing the front, then you bow. 5. The DON TS. No smoking, food, gum, shoes or profanity on the mat. 6. Working with a partner. Always remember to bow to your partner BEFORE and AFTER you work together.
