5 Adult & Minor Amateur Athletic Waiver and Release of Liability: I, the undersigned do hereby volunteer my application for the attendance and participation in Warwick Shotokan Karate Championships at Central Bucks West High School on May 5, 2019 and do hereby assume full responsibility for all injuries, damages or losses that I or my child may sustain or incur, if any, while attending/participating. The undersigned agrees to defend, indemnify, and hold harmless Andrew Spivack and Warwick Shotokan its officers, managers, members, employees, agents and coaches/instructors and their successors and assigns from and against all legal liability, claims, suits, damages, losses, and expenses, including attorney fees, threatened or incurred, and arising from participation, or from any cause whatsoever. I fully realize that participation in the Warwick Shotokan Karate Championship can be dangerous and could result in serious injury or possibly death and freely assume that risk. In the event of an emergency, I give permission for Warwick Shotokan to give myself or my child first aid and to arrange for transportation to a hospital and to receive emergency medical treatment. I will assume all costs for medical care and transportation. Please list below any current or previous accidents, illnesses, or physical limitations that could impact, stop, or prevent you or your child from participating in the Warwick Shotokan Karate Championship. The below named participant is physically able to participate in the Warwick Shotokan Karate Championships without limitations (except as described above). I release permission for use of my or my child s picture for promotional material including brochures, advertisements, or warwickshotokan.com website. Names are not used in any promotional material. 6 Warwick Shotokan Karate Championship Directions to Competition: Central Bucks High School West LOCATION AND DIRECTIONS 375 West Court Street Doylestown, PA (across the street from Central Bucks Family YMCA) Central Bucks West is located off Route 202 in Doylestown. From the Pennsylvania Turnpike: Exit at Willow Grove North. When approaching Doylestown take route 611 North bypass to the route 202 South exit. At the end of the ramp go left (202 N. Main Street). Go through the light at the Hospital entrance. Take the next right onto Memorial Drive. Turn left onto Court street. From South/ West: Take Route 202 North, through Chalfont and New Britain. Continue on the same road past the Hospital. Take the next right onto Memorial Drive. Turn left onto Court street. From the North/East: Take Route 202 South, follow Route 202 by--pass around Doylestown. Follow signs for Route 202 South (Norristown & Hospital) to exit the bypass. Turn right onto State Street (202 N). Continue as above. From Lower Bucks County: Take Route 413 to Route 202 South and continue as above. or Take Bristol Road or Almshouse Road to Route 611 North and continue as above. For map information use this link:
