2 How to Order Safety video programs are loaned only to policyholders of Midwestern Insurance Alliance and MWIA Insurance Services, and are loaned for no more than a 28 day period (commencing on the date the safety video is delivered to your location). Each employer is limited to 1 video on loan at any given time. Placing your order Online Go to and complete each of the required fields. Late and Non-Returned Videos Your company (MIA-insured policyholder) will be charged a $50 restocking fee if a safety video program is returned 10 days or more past the due-date. Your company (MIA-insured policyholder) will be charged an amount determined by the safety video lending library service (NRSC) to replace any DVD that was lost or damaged, or has not been returned within 30 days or more past the due-date. Rush Orders Most videos will be received within 7 days of the date on which the request was made. If a rush delivery is needed, we may be able to accommodate for a fee of $35.
