Never use the top as a means of gaining extra height. lways extend the ladder to it's maximum to ensure stability. The steps should always be placed on a level surface 2 Ladders Slip, Trips and Falls B.S.B staff Ladders should be set at an angle of 4 (up) to 1 (out), and always tied at the top. The ladder should be footed, or firmly secured at ground level 3 Safety Harness Hanging B.S.B staff Harnesses must be checked for safety on an annual basis. lways inspect it and report any defects, and do not use. 4 Safety Harness Hanging B.S.B staff Harnesses should be correctly fitted. Users should not work alone, and always have ways of calling for help. Remember that death can occur in 15 minutes after a fall and suspension. Once lowered the equipment will be moved to a safe location and the Hire company will be contacted for assistance. BSB preferred hire company for MEWP is FI Ltd. B.S.B staff ll users will hold a 'IPF' PL card. The transfer of a key must only be done by a trained person. MEWPs can vary by manufactures and all users must make themselves aware of each machine's layout and method of operation B.S.B staff ll users will hold a 'IPF' PL card, to operate Type 3a and 3b MEWPS. ll operators must use the IPF Operator's Safety Guide booklet as a standard working procedure. B.S.B staff The entire machine must be inspected before and after use. This includes the power source, all working parts, the structure and safety mountings (where applicable) in accordance with the manufacturer's manual 5 Movement of MEWPs Movement in B.S.B staff and others confined areas within within the confine of a factory the site environment. 7 LOCTION: TSK/PREMISES: ny Work on/off Site Working at Height Risk ssessment SSESSED BY: DTE: SSESSMENT NO: BSB/GEN/WH REVIEW DTE: 30 Sept 2018 Safety Harnesses in Mobile Elevating Work Platforms Note for all BSB Staff Boom Type Platforms It is strongly recommended that a full body harness with an adjustable lanyard (used to provide work restraint and adjusted to be as short as possible) is used when working from a boom type Mobile Elevating Work Platform (MEWP). This would include: Self Propelled Booms (SPB), Trailer Push-arounds (TP), Vehicle Mounted Platforms (VMP26 & 100). Vertical Lifts It is not normally necessary for personnel working from a vertical lift to wear fall protection equipment, other than in exceptional circumstances. This would include: Scissor Lifts (SL) and Vertical Personnel Platforms (VPP). The need for a fall protection system will be the outcome of a job specific risk assessment undertaken prior to work commencing and taking into consideration the manufacturer s operators manual. Disconnect power before changing discs. Use correct disk for the work. Operator not to work un-supervised. 2 Disc Grinding swarf spray from the grinding wheel Persons near to the operator Hot metal swarf can travel long distances. curtain should be erected when necessary, and nearby effected people notified of the hazard. 3 Disc Grinding Risk of injury / amputation B.S.B staff ll operators must be trained in the use of grinders. Protective gloves must be worn when operating the grinder, Grinding discs are to be checked for any damage between each operation. 4 Disc Grinding Eye injury. B.S.B staff Protective eye glasses must cover around all areas of the eye. 9 ssessment Review Date: 30th September 2018 SFE ISOLTION PROCEDURE The safe isolation procedure is set out below, only qualified Electricians are permitted to carryout Electrical isolations these personnel are listed below: - C&G M1 / 236 part 1&2 / M2 / C&G Level 3 Building Regs / 17th Edition Vince Webb - Qualified to NVQ Level 3 / 16th Edition John Clifford - Qualified to NVQ Level 3 / 17th Edition. Safe Isolation will be carried out as set out below: Step 1. Identify the point of isolation for the system or circuit. Lock off, place warning label and keep the key to the lock with yourself Step 2. Select the correct and mains approved test equipment and ensure that it works correctly by testing on the proving unit or known power source. Step 3. Test the outgoing side of the means of isolation (Main Switch, MCB, Fuse, etc.) to make sure it is dead. Depending on the type of the supply you will need to complete the following tests: Single Phase installations test to confirm that there is no voltage between: - Line and Neutral - Line and Earth - Neutral and Earth. 3 Phase installations test to confirm that there is no voltage between: - L1 and L2 - L1 and L3 - L1 and Neutral - L1 and Earth - L2 and L3 - L2 and Neutral - L2 and Earth - L3 and Neutral - L3 and Earth - Neutral and Earth Step 4. Re-test the test equipment on the proving unit or known power source. Page 1 of 2

11 Procedure For Live Testing & or Connecting dapted from HSE - HSG85 (Third edition) Published 2013 (Page 20 Live working) Live Working Method Statement Identify Circuit to be worked on PPE Working rea Man Power Identify which way or circuit will be worked on, If a new MCB is to be installed select a correctly rated and suitable breaker for the circuit to be connected dditional PPE to be used over standard required PPE in the area working, you will be required to wear Eye protection and Electricians gloves during the live works The area where the distribution board will be worked on is to be cornered off with suitable barriers and no unauthorised ccess signs displayed. The DB is to have adequate lighting in the area and portable lighting to be used if required and check that no ingress of water before works start. Working on live boards will be allocated as a two man job, one person to carryout the works the second person there to be present at all times, to restrict access to the working area and assist where required. Live Working Procedure (if any of the procedures below cannot be met then works to be aborted and safe isolation procedure followed for works to commence) 1 First and foremost Identify what items are fed from the DB to be worked on and that it is possible to isolate the DB completely avoiding live work, if it is not possible to isolate the DB continue with procedure. 2 Barriers in place area cornered off and signs displayed, Working rea safe and dry especially where roof is known to leak from cladding above. 3 Correct PPE worn once working within DB Gloves and eye protection, (Skytec electrician thin gloves) 4 Permit to work obtained and works ready to commence, reinspect area for water ingress, any water found to be leaking to work area works to stop immediately. 5 Carefully Remove DB board front cover - Visably inspect the exposed board, 1. re all the relevant covers in place, 2 is board in a tidy with no visible old circuits disconnected, 3. re all isolation sliders on unused circuits in the off position.4. Use BSB Thermal imaging camera to detect any visible hotspots for potential loose cabling/mcbs 6 MCB Installation - First using an electrical dead tester confirm the bus bar pin that the MCB is being mounted to is isolated. Securely install MCB to unused circuit using insulated tools only, check that Din rail tab is clicked to lock position and terminal screw is tightened to torque setting 4 N.M. 7 Cable Dead Testing - Confirm cable is connected at field end and carryout dead testing of cable to be installed. 8 Cable connecting - Route cable tidily through board and add to existing cable harness, if possible to do so (dependant on previous installations carried out on this board) connect circuit numbers in unison i.e Live earth & Neutral in same terminal number for each relevant cable. Cables to be tightened to manufacturers recommended tongue setting. 9 Reinstate Distribution Board - reinstall front cover, mark new circuit on DB circuit Schedule, remove barriers return to normal operation. 10 Circuit Power up - The circuit can now be made live, first confirm the MCB is in the off position and then using a flat insulated screwdriver move the slider to the on position and then switch the MCB to the on position 11 Live testing - Carryout required live testing of new circuit. Larger boards is two man operation to remove front cover Manual Handling Operations 1992 ND Management of Health and Safety at Work Regulations 1999 Electric shock BSB Staff The work is only to be carried out if the distribution board is of the Schneider cti 9 or the older Merlin Gerin Isobar 4 Ranges with isolation sliders for all circuits, These boards are Minimum IP20 visible inspection that all free circuits ways have the sliders to the off position, Correct PPE - Gloves and Safety Glasses worn during works, Insulated tools rated to 1000Volts minimum only to be used. These type Boards have isolating Sliders for each circuit Only work on a Board with neat and safe cabling correctly dressed where you can see no visible old circuits removed and left hanging Isolation Sliders, these should be back when off exposing the dead bus bar CCEP & BSB Responsible Person Signed Date Page 2 of 2
