A DOMS Impact Assessment Form (DOMS IAF) is the control method for Scottish Water to authorise access to all parties onto its water distribution network. The DOMS IAF must be completed for all planned and unplanned intervention on the water distribution network. Scottish Water s DOMS Impact Assessment Forms must be completed for any valve operations or hydrant operations where there is an impact or risk to the following: Water quality The continuity of supply or actual Interruption to Supply To maintain water supply hygiene A DOMS IAF must be completed for the following scenarios where there is the potential for or actually any valve operations or hydrant operations etc are required for any planned or un-planned emergency work on the water distribution network: 1. All mains repairs or any mains intervention on the water distribution network that requires or has the potential for valves to be operated. This includes any mains repaired under pressure which still need valve(s) to be operated or hydrant / scours / washouts opened to reduce the pressure to fit a repair collar. 2. Service reservoirs that require compartments or tanks to be isolated for whatever reason. 3. Isolation of treated water storage points that require compartments or tanks to be isolated for whatever reason. 4. Mains and hydrant flushing as part of a planned / regular flushing program or as an emergency flushing program as part of an incident. te Any Mains Flushing as part of any MST does NOT require an IAF but should be updated onto the Operation Log Pre and Post Flushing. 5. De-commissioning or re commissioning of mains 6. Tankering or bowsers being filled as part of an incident. 7. n Intrusive Pipe sampling or New Connection above 32mm to the live distribution mains. (Under Pressure branch or Bores) 8. Under Pressure bores / connection either 25mm or 32mm will only require a DOMS IAF if the work is to be undertaken on any main deemed a Complex shut or at SW discretion. All under pressure 25mm or 32mm connection will be completed as per the generic and/or any site specific Risk assessment or method statement. 9. All trial excavations must be checked, agreed and validated in advance with Water Operations staff. Any trial excavation on or near (SW discretion) any water main deemed to be a Complex shut. Requires a valid / completed DOMS Impact Assessment Form. A DOMS IAF is required to be completed for all burst repairs including any communication pipes and service repairs. The only exception is where the individual supply can be isolated without carrying out any valve / hydrant operations on the distribution network. Page 1 of 14

2 If there is any doubt regarding the isolations of any Communication pipes or the Service pipe s supply a DOMS IAF must be completed with the appropriate information supplied for the isolation of the main with the valves cleaned and marked. The following is a step by step guide for the completion of a DOMS IAF for both Complex and n Complex using paper copies. The DOMS IAF is available electronically with built in drop down and pop up box for guidance on the correct population and completion Section 1. Within SW this will generally be the Field Response NSO when raising a mains repair work order (n Complex) Emergency Complex Burst mains the Field Response NSO will complete a DOMS IAF to the best of their Knowledge for issue to Water Operation NSO. Company name or Business function Confirmation on who will be carrying out the work i.e. SW Customer Ops / Leakage Delivery /Water Response / SW Solutions / Clancy Dowcra / SLO s / RPS etc For Customer Ops NSO`s when raising the work this will normally be left blank as OMC are the planners of the work and they will know who will be issued the work request. This can be populated and updated by either SW staff after the OMC assigns the work pack to either a Water Ops NSO / Water Response or by the framework contractor. For all other work types this should be already populated by the person requiring access to the network. I.e. Mains rehab work contractor. Valve Operator Name & Valve Operator contact number These details should / will be added updated prior to the valves being operated on site. For Customer Ops especially any emergency Burst repairs out of hours on stand by this will generally be the person, NSO raising the work. CSD staff the valve operate name shall be updated if different by the person carrying out the valve Ops on site Page 2 of 14

3 EUSR Skills number This shall be populated at the same time as the valve operators name for customer Ops work once known. For all contractors requesting work on the network they will know in advance who will be doing the valve ops on their behalf i.e. mains Re-Hab Customer Care operative etc. For all contractors this section must be checked by Scottish Water staff giving their authorisation for any contractor being allowed to operate valves etc on the network. Their DOMS & National Water Hygiene cards must be checked by Customer Ops that they are correctly trained and have a valid cards and training. Service Request Number SW NSO populates when raising a work request / IAF Job work order Number This is added if already known or updated when issued by OMC CID Auto code & Project Name This is section is completed by contractor Planned start date and time The DOMS IAF should be completed with the view of the work being completed within a 7 day window of the DOMS IAF being Authorised and signed off being by Customer Ops staff. I.e. Monday 13 th Oct 08 then the DOMS IAF is valid for 1 week / 7 days If the work does not go ahead by the 20th Oct 08 then a new DOMS IAF is required either re written or counter signed after all details have been re-checked by CSD. WOA name The Water Operational Area name WSZ name The Water supply Zone name DMA name The district metered area name This information shall be checked and populated by SW as part of the authorisation process. Description of work to be undertaken I.e. 4 Burst Main Repair / Flushing Program / DMA valve zoning / Burst comm. pipe / Mains Re-Hab with description Pipe Bursting / Slip lining /Main laying / New main installation etc Diameter Confirm the pipe diameter either Imperial or Metric Material Type Confirm Material type i.e. Cast Iron MDPE / HPPE / Asbestos Cement, Ductile Iron, Steel Main. Location of the work This should include the site location of the actual excavation and any other affected areas or street names if known. Planned Complex Operations Network Control forums have been set up to review all Planned COMPLEX shuts. All Planned Complex shuts or network activity must be submitted to the local Network control meeting for review and authorisation These forums allow more visibility of work being carried out and ensure rigorous contingency plans are in place. We also review works to allow us to learn from our successes and failures and share best practice. Through these forums we will reduce the risk to water quality and interruptions to supply and also improve customer satisfaction. 14 C.A.L.M. Initiative REMEMBER C.A.L.M!! WHEN WORKING ON SCOTTISH WATERS DISTRIBUTION WATER NETWORK I.E For a clockwise opening valve CHECK Check the valve position and status turn the valve fully clockwise until it stops, always count the turns to this point, this will confirm the valve is full open. ACTUATE It is important to understand that closing a valve using the correct technique is as important as when the valve is being opened. When actuating the valve the following should be followed; Closing The valve should be closed at a constant slow pace until 75% closed. The operator should then close the remaining 25% using a 5 stage approach e.g. 5% wait 30 secs and so on. Remember the last 25% will stop 50% of the flow. Opening From the fully closed position the reverse of the above technique should be followed e.g. First 5% until the valve stops sounding then increments until 25% open. Pause 30 secs then open the remainder slowly and consistently. LISTEN Listening/sounding the valve throughout it s operation is critical to understanding the affects of its operation MONITOR It is important to be observant when carrying out valve operations, you should constantly monitor the situation for example; Before we are committed to work Be sensitive to customers who come out to complain of an interruption from the wider area. Take pressure pre and post valve ops confirming affected area. Call customer contact centre checking if there are any unplanned issues associated with you operations. Reminder for all staff on good practice for the correct method when operating valves and hydrants etc on the Water network. Network Control forums have been set up to review and Authorise all Planned COMPLEX shuts. COMPLEX SHUT CRITERIA is any shut off required which meets any of the following criteria 1. Any Main 200mm or above in size? 2. Is a Strategic Main going to be affected? 3. Is it a multiple valve change job which requires NSO support (generally 5 Valves or more)? 4. Is Back-feeding required? 5. Is a District Metering Area (DMA) or Boundary Valve going to be changed? 6. Are any Key Customers, Hospitals or Prisons going to be affected?
