2 1.0 Scope The procedure identifies the minimum level of PPE required along with work/task specific PPE requirements 2.0 Requirements 2.1 Standard PPE The standard PPE issued to each person and which they are required to wear is as follows: Head protection; Eye Protection; Hand Protection; High Visibility Clothing; Foot and Ankle Protection. Appendix 1 contains guidance on the recommended types and makes of hand and eye protection provided as standard PPE for triio personnel. This list is periodically updated. The list doesn t cover all high risk operations, where a specific risk assessment shall be carried out to identify the correct type of hand and eye protection to be provided. 2.2 Non triio Personnel Subcontractors, plant hire personnel and suppliers shall follow this procedure. They may substitute any specified item of equipment with alternative equipment providing it meets the relevant current British Standard and provides the level of protection required. In the case of a one off delivery driver not being in possession of the requisite PPE then it may be expedient for the equipment to be provided by triio. The wearing of shorts on site is prohibited. 2.3 Variations to the Mandatory wearing of PPE Where a formal risk assessment has been carried out and the findings of the risk assessment identify that the wearing of any of the mandatory standard PPE places the wearer or others at greater risk, then the Site Manager shall complete an exemption form (appendix 3) identifying the person(s) exempted. The Site Manager shall specify in writing the timescale this exemption may be in place. A copy of the exemption shall be kept with the risk assessment. Example - where work involves standing in a hazardous material e.g. wet cement products during a concrete pour, then the hazard is greater from skin contact with cement. In these circumstances, the use of wellington boots is of greater benefit than standard lace up boots. Plant operators working within an enclosed cab, e.g. a crane driver, are not required to wear the standard head, eye or hand protection or high visibility clothing whilst protected by the confines of the cab. Depending on the operation, if windows are down, light eye protect maybe required. Delivery drivers off loading or loading goods or equipment in a safe office or stores environment may be excused from the wearing of standard PPE where an assessment of the risk shows such provision to be unnecessary. 2.4 Maintenance and Storage of PPE All PPE shall be cleaned, inspected and maintained by the user of the PPE. This shall be in accordance with the manufacturers instructions. All sites shall be equipped with suitable drying and changing rooms where PPE may be stored. 2.5 Defective Equipment Where any defective PPE is found, it shall be removed from service immediately, quarantine and a replacement sought. 2.6 New and Replacement Issue All new and replacement issues of PPE shall be recorded on The PPE Record of Issue Form. The reason for issue shall be recorded on the form, along with the date. These records shall be archived in accordance with the company procedures. All PPE provided shall be suitable for the individual wearer. Different sizes and styles are available. Some PPE is also adjustable to fit the user e.g. Hard Hat. Page 2 of 9

3 All PPE shall be tested for fit and seal as appropriate. If the PPE/RPE is of poor fit/seal then it shall not be used and the correct size obtained. 2.7 Hierarchy of control The use of PPE shall follow the HSE guidelines on the hierarchy of control published within the MHSWR. This specifies that priority must be given to collective measures over individual control measures (see appendix 2). 2.8 Risk Assessment triio has introduced a minimum standard of PPE to be worn by all personnel on its construction sites following analysis of its accident, injury and near miss data. However, the need to use other PPE may be identified as part of the control measures within a specific risk assessment. 2.9 Mandatory PPE Mandatory PPE to be issued and worn unless specified in a risk assessment. Head Protection triio head protection shall be printed with The triio logo on the front above the peak. All safety helmets shall conform to BS EN 397. Head protection will display the wearers name and have a completed ICE tag attached. As a member of Build UK, the following safety helmets colours must be used on all triio sites from January Prior to January 2017, projects may follow previous project standards or adopt the new Build UK standard early Black Supervisor Orange slinger/signaller Blue Visitor/young person/inexperienced worker Yellow optional for identifying management as separate to competent workforce Previous standard: White General Duty Yellow Site Supervision Red Fire Marshal Green First Aider Blue Rail Operative For Network Rail project the blue helmets are required for: A Track Visitor Permit A PTS card with a green square symbol on it or New starters under the mentoring passport scheme White helmets are used in all other incidents Green, red and yellow helmets shall not be permitted for use on rail projects. Eye Protection Eye protection to BS EN F must be worn by all persons on construction sites. This includes those persons who wear prescription spectacles. Standard eye protection may be provided by the use of safety glasses or helmet mounted eye shields. Where a risk assessment deems it necessary, persons exposed to the risk of eye injury from particular activities are required to wear eye protection to the appropriate standard. In the event a person is required to wear ordinary glasses, prescription safety glasses will need to be worn (Appendix 4). Hand protection The type of glove worn shall be suitable for the task. This will result in a number of different gloves being available for different tasks/trades. Page 3 of 9

4 High visibility coat/jacket/waistcoat High visibility clothing should conform to ISO Foul Weather Wear Coats and trousers shall conform to Class 3 and BS EN 343 for ultimate protection (breathable and waterproof). Safety Footwear Safety Boots shall conform to BS EN ISO (200 Joules protection). All safety footwear shall have steel toecap, steel mid sole and provide adequate ankle support. This precludes the wearing of safety shoes, trainers. PPE to be issued and worn as a result of a risk assessment 2.10 Specific PPE Hearing Protection Hearing Protection shall conform to BS EN 352 Hearing protection may be identified through site-specific risk assessments. Hearing protection may be in the form of earplugs or earmuffs, which may be helmet mounted. All hearing protection shall be selected based upon the attenuation required to reduce the noise level dosage to a level below the statutory action level Respiratory Protective Equipment (RPE) Respiratory protection shall conform to the standard required based upon the type of hazard encountered. This may vary from solvents to asbestos fibres. The RPE must provide adequate protection against the identified hazards. Careful selection of RPE is required to ensure that the correct type and level of protection is provided and that the relevant substance work exposure level is not exceeded. Where RPE is required the user must have successfully passed a quantitative face fit test in accordance with HSE guidelines (HSE282) Where RPE is personally issued as a standard item of kit i.e. in for gas engineering teams, a risk based face fit testing frequency must be established for re-assessment. Other PPE Other PPE shall be identified from specific risk assessment. These shall identify the relevant standard (BS EN) for the equipment. This PPE shall be used, inspected and maintained in accordance with the manufacturers instructions 2.11 Highways When working on the highway, the type worn will vary depending upon the speed of the road. When working on a construction site, short sleeve versions are acceptable. On roads under construction (not live ) yellow florescent hi-vis vest (no sleeves) must be Class 2 or 3 as per standard. Greenhams Order No: On highways (live roads) with speeds of above 50mph requirements are Class 3, colours as above but vests or jackets must have sleeves. Greenhams Order No (vest), (coat). Outside of protected areas, full length fluorescent trousers to be worn Railways Rail Infrastructure - Florescent Orange Minimum hi-vis vest Class 2, with elasticated side fronts and centre back waist with side seams and shoulder stud fastening. 5 Other versions are acceptable e.g. body warmers and t-shirts provided they are to Class 2 with reflective tape over shoulders and around body. When working on or near the line all PPE must be fastened securely Personnel must wear high visibility clothing of a pattern and an approved shade of orange colour when working on or near the line or when in sight of train drivers. PPE must be correctly and where there is the possibility of PPE or clothing being confused with signals, the colours red or green shall not be worn. Appendix 1 Catalogue Codes Standard PPE Issue to be worn when on site The catalogue codes below are from our preferred supplier Greenhams. Head Protection: Standard head protection to be worn Centurion 1125 Helmet Eye Protection: The eye protection listed below provides low energy impact protection. They cannot be used for use with percussion or high speed tools or where there is a chance that debris may be discharged at high speed. In these circumstances the operator of the equipment and any person who may be affected by the work shall wear eye protection that gives medium or high level of impact protection as identified in the risk assessment as usual. Keep Safe Spitfire (available in clear or smoke tint). Keep Safe Typhoon (available in clear or smoke tint) Uvex I-VO (available in clear or amber tint). Any person who has to wear prescription glasses will also need suitable eye protection. Any permanently employed staff or operatives wishing to be provided with safety prescription spectacles should apply to his Site Manager (see appendix 4). Keep Safe Hunter Lens Cleaning Station It is recommended that a lens cleaning station is also provided on site to enable all persons to clean their eye protection. It should be placed in an accessible and prominent position on site. Uvex Lens Cleaning Station Uvex Lens Cleaning Station small (fluid and tissue) Hand Protection: As with any PPE a good fit and suitability for the task is essential. It is essential that the glove fits the wearer; not only for comfort, but also to ensure they get the best performance and use out of the glove. Keep Safe Yellow A general purpose glove, hat has good all round grip and abrasion resistance. Made from latex (a few persons may have an adverse reaction to this material. An alternative is available; see Green Nitrile Coated Palm Poly Liner Showa 350R). Page 5 of 9

6 Anticipated uses include manual handling operations, brick and block work, general labouring and other similar work. May be used during the heavier plant maintenance operations. Available in difference sizes. Green Nitrile Coated Palm Poly Liner Showa 350R Can be used as an alternative to Keep Safe (yellow) for those who have sensitivity to latex products. Available in different sizes. Keep Safe Insulated Liner (Grey) As Keep Safe (yellow) glove, has a fleece lining. Anticipated use as Keep Safe (yellow) but suitable for working in much colder weather conditions. Available in different sizes. Keep Safe Blue Rubber Palm Coated on Kevlar or Blue Latex Palm Coated on Kevlar Very high level of cut an abrasion resistance made with Kevlar. Anticipated uses include handling very sharp or serrated material where normal gloves would not be sufficient e.g. sheet metal or glass. Available in different sizes. Polyflex (Grey) A lightweight close fitting glove. Gives reasonable dexterity when worn. The level of protection provided by this glove not as high as the Reflex gloves. Anticipated uses include, general site work, joinery, finishing trades, scaffolding, setting out, supervisors and visitors, etc. it may be used for some M & E process work. Use should be restricted to areas where no hard abrasion can occur or is minimised. Available in different sizes: Keepsafe Gauntlet Rubber gauntlet, with grip and fleece liner. Anticipated uses included wet trades such as concreting and mortar mixing etc, refuelling and associated lubrication activities. Available in different sizes: 8½ 9½,10½. Foot Protection: Wellington Boots Shall normally be knee boots with steel toecap and steel midsole to BS EN ISO Available in different whole sizes Safety Boots These shall normally be lace up with steel toecap and steel midsole to BS EN ISO Different styles are available. Rockfall Quartz safety boot with midsole 6 6-hole leather safety boot. Thinsulate lined. Available in black or brown. Available in different whole sizes Large Sizes available: Solemate Padded Leather High leg safety boot with midsole 6 hole leather safety boot. Water repellent. 9 Appendix 4 Ordering prescription eye protection Where a person is eligible for the company to provide prescription eye protection, the following process will be followed: 1. Request prescription spectacle form from triio 2. Safety advisor to verify person is eligible for prescription safety spectacles. Verification to Safety Department Admin. 3. triio order form sent to individual. 4. Individual attends opticians (except spec savers) with order form for completion by the optician and orders prescription safety spectacles. 5. Re-attend opticians for collection. 6. All fees paid via Greenhams. Safety Department to keep log of issue of order forms and pass to Procurement once issued. Page 9 of 9
