4 RFSC PARENT CODE OF ETHICS TEAM MISSION STATEMENT RFSC utilizes progressive levels of competitive swimming to facilitate the growth and development of our membership. Our effort is to shape and strengthen character throughout one s lifetime. We emphasize individual and team growth, through all available resources, to maximize the attainment of individual and group goals in a health, safe and supportive environment. This growth is not measured only by tangible awards, but also by personal improvement and leadership. As a RFSC parent I will: Provide support, care and encouragement for my child and all the members of the team. I will leave the coaching up to the coaches Encourage sportsmanship by behaving in a respectful, kind and supportive way to all athletes, coaches, officials, volunteers and parents. Place the emotional and physical well-being of all members ahead of my personal desire to win. Support the coaches and officials working with my swimmer and consider the feelings of others so that all have a positive and enjoyable experience. Follow the proper chain of command when seeking insight and/or expressing concerns in all areas of RFSC. Participate in an environment that is free of alcohol and drug use and will refrain from any use at all events. Ensure my child has all equipment to be properly prepared for practice and swim meets. 5 RFSC RELEASE AND WAIVER OF LIABILITY AGREEMENT The undersigned: Mother (Name Printed): Father (Name Printed): of Name of Child (Printed): in consideration of and for the opportunity to participate in activities with and obtaining services from the Rolla Fins Swim Club, USA Swimming, Ozark Swimming, Inc. and any directors, officers, employees or agents of the Rolla Fins Swim Club, USA Swimming or Ozark Swimming, Inc. (hereinafter, each of the above parties are collectively referred to as the "Swim Team") hereby on their/his/her own behalf and on behalf of their/his/her child release the Swim Team, acquit and waive any claim for liability against the Swim Team, from any and all claims, demands, damages, actions or causes of action, whether or not now known or contemplated, on account of any personal injury, including death, or damages to personal property sustained or suffered by the undersigned or their/his/her child caused by or due to the negligence or fault of the Swim Team or otherwise arising from said child's participation in or obtaining services from the Swim Team, including, but not limited to, any event, travel, practice or any other activity related to or involving the Swim Team. The undersigned acknowledge(s) that they understand that from time to time and on different occasions and at different locations said child may participate in a Swim Team event, practice or any other activity related to or involving the Swim Team. It is expressly understood and agreed that this Release and Waiver of Liability and Indemnity Agreement ("Agreement ) is intended to cover and does cover not only all known injuries, losses and damages but any future injuries, losses and damages not now known or anticipated but which may later be sustained, develop or be discovered, including the effects and consequences thereof. It is further expressly understood and agreed that this Agreement shall be deemed to be a release, waiver of liability, indemnity discharge and covenant not to sue and shall bind the undersigned, their/his/her child and each of their respective heirs, executors, administrators, personal representatives and assigns. This Agreement may be pleaded as a counterclaim to or as a defense in bar or abatement of any action taken by or on behalf of the undersigned or said child. In the event it is necessary for a counterclaim to be asserted or for any other action to be taken to enforce the provisions of this Agreement, the undersigned hereby agrees to indemnify and hold harmless the Swim Team against all loss or expense incurred in connection herewith, including, but not limited to, the payment of attorney's fees and reasonable costs. It is further understood and agreed that the undersigned shall defend, indemnify and hold the Swim Team harmless from and against any claims asserted by, on behalf of or through the undersigned or their/his/her child, including claims that the negligence of the Swim Team caused or contributed to cause, in whole or in part, any personal injuries sustained or suffered by the undersigned or their/his/her child. The undersigned agree(s) that this Agreement and any claim arising out of said child's participation in or obtaining services from the Swim Team shall be construed in accordance with the laws of the State of Missouri, without regard to its conflict of laws provision and that the courts in Phelps or St. Louis County, Missouri shall be the forum for any lawsuits arising from any such claim. 6 RFSC Athlete Code of Conduct As a member of the RFSC, I understand and comply with the following guidelines set forth by the coaching staff and Board of Directors. Swimmers shall respect and show courtesy to their teammates and coaches at all times. Be on time. Be ready to get in the water on time for practice and meet warm-ups. Be prepared to swim. Have your equipment ready. Bring extra caps and goggles. Be respectful of your teammate s feelings and personal space. Swimmers who exhibit sexist, racist or inappropriate behavior toward another person will be excused from the practice or meet and will be subject to a disciplinary process. Any coach at any time has authority to ask a swimmer to exit the pool for disciplinary reasons. No dunking, foul language, hanging on lane lines, splashing, spitting, hitting, trash talking, belittling or any other behavior deemed as horseplay is allowed during practice, at a meet or at any other team activity. Swimsuits are required for practice. Skateboards, scooters, glass, tobacco, alcohol, roller blades, pets, electrical appliances, and bicycles are not allowed on deck. Swimmers are expected to exhibit good sportsmanship at practice and meets. Swimmers and Parents shall not ever confront an official or stroke and turn judge at a meet about a disqualification. This is the coach s responsibility. Swimmers and parents will be respectful and courteous to meet volunteers at all times. If you have a problem, come get your coach. Destruction or theft of property will be prosecuted under the law. The swimmer and family of swimmer will be responsible for all damages and fees incurred upon collection or prosecution of damages. Swimmers who are ill with contagious viruses and open wounds must tell their coach before entering the water. A swimmer who is injured at practice or a meet must report this to their coach immediately following an injury. Swimmers are responsible for checking in, checking postings, and heat and lane assignments, reporting to their coach and for being behind their block prior to their race. A swimmer who doesn t show up for an event they are registered to attend will still be required to pay all meet fees. Swimmers must be at practice a minimum of two days during the week before a meet. A coach has the right to scratch a swimmer from a meet if the swimmer has not been excused for their absences prior to the meet. I understand that USA Swimming and Ozark Swimming are organizations of trained volunteers and paid professional officials. Meets are operated by volunteers. I agree to treat volunteers and swim meet officials with respect. Failure to comply with the Code as set forth may result in disciplinary action, including but not limited to: 1. Dismissal from practice or competition and immediate return home. 2. Disqualification from one or more events. 3. Disqualification from future travel with the team. 7 RFSC Parent Volunteer Form Success of the Rolla Fins Swim Club depends on the volunteer support of parents. As an RFSC parent, you are expected to help the team. Not all volunteer opportunities are listed. NEW THIS YEAR: Parents who actively volunteer can earn a $50 rebate on swimmer expenses at the end of the year. Please select at least one of the following. MERCHANDISE MANAGEMENT Assist board members with placing orders for team gear (caps, shirts, decals, etc.), collect monies, and distribute uniforms. Store team merchandise. BOARD MEMBER The term is a 3-year commitment to attend monthly meetings, participate on board committee(s) and to support RFSC at its social and fundraising events. TIMER Fins parents are asked to serve as back-up timers for S&T swim meets. Training will be provided. PARTY ORGANIZER -- TWO NEEDED Chair a committee to organize a post-season swim banquet or pizza party. COMMIT Commit to help but not be in charge of a committee. Please specify committee preference. FUNDRAISING AND MARKETING Organize a fundraising event and help promote the team. NEWS COMMITTEE -- TWO NEEDED Commit to write a report covering swim meet results, send articles and/or pictures to local newspapers. Attendance at meets isn t required -- Coach Ethan can provide results and other information. SOCIAL MEDIA Assist with team posts to Twitter and Facebook. PHOTOGRAPH Take photos of Fins swimmers during practice and at meets you attend for use on team website. SPONSORSHIPS Visit local businesses to get team sponsors. SWIM-A-THON -- TWO NEEDED Organize, coordinate and distribute materials for team swim-a-thon fundraiser. NO VOLUNTEER Instead of volunteering, I will donate $200 to RFSC to be used where it is needed. Signature: Date:

8 RFSC GENERAL INFORMATION Parent Meetings Meetings will be held every other month, starting in September, and will begin at 6 p.m. in the classroom (Room G-26) on the lower level of the Gale Bullman Building. A schedule will be distributed to parents and an reminder will be sent out approximately two weeks prior to each meeting. Practice Cancellations or Changes RFSC may need to cancel or change practice times due to weather, university events or pool malfunctions. We will as soon as we know, and post on Facebook. For last-minute notifications please call Locker Room Usage and Rules Absolutely no locker room is to be used while a Missouri S&T sporting event is being held in the gymnasium. Parent Drop-Off and Pick-Up Rules RFSC is not responsible for any swimmer until they enter the pool deck promptly at the start of practice. It is the parent's responsibility to make sure the swimmer makes it from your car to the pool. Parents are to be at the practice location to pick their swimmer up immediately following practice. If other arrangements need to be made, this must be done with your swimmer s coach by the parent, not the swimmer. Parking Parking is available in the lot across from the Gale Bullman Building when S&T is not hosting a sporting event. On game day you must park in the lower S&T parking lot. The handicap drive-through can be used for drop-off or pick-up, but no parking is allowed. Team Uniform RFSC team uniform will be swim cap, team T-shirt or sweatshirt. Swimmers are expected to wear team T-shirts at all swim meets. Volunteer Responsibilities The success of RFSC depends on parent volunteerism. When your child signs up with Fins, you as a parent are signing up too. Please review the list of volunteer opportunities and sign up to participate. In lieu of volunteering, you may make a $200 donation to the team. Parents who actively volunteer during the season will receive a $50 rebate. Quitting The Team RFSC requires a written notice one month prior to leaving the team. If no written notice is provided, you are still responsible for the full year s fees. We will only refund fee for medical reasons with documentation from physician.

Please speak to the coach directly. TEAM FEES Registration fees are due at fall sign-ups. They range from $312 to $372, depending on the level your child is assigned to. The breakdown is as follows: Deposit first and last month s fees (see table above) -- $100-$160 Administrative fee covers postage costs, team equipment, T-shirt, window decal and swim cap -- $50 Pool fee covers our rental of the S&T facility -- $100 USA Swimming membership required of all swimmers, coaches and board members -- $62 Payment options -- choose one 1. Pay full registration fee. a. $312 for Team Prep 1, Team Prep 2 and Bronze 1 b. $352 for Bronze 2 and Silver c. $372 for Gold 2. Divide registration fee into three monthly installments. First installment due at registration. Second due in October. Final due in November. Installment payment will be in addition to regular monthly training fees. a. $104 for Team Prep 1, Team Prep 2 and Bronze 1 b. $118 for Bronze 2 and Silver c. $124 for Gold 3. Pay year s training cost and registration fee in full and receive a 10% discount a. $685 for Team Prep 1, Team Prep 2 and Bronze 1 (you save $77) b. $883 for Bronze 2 and Silver (you save $99) c. $982 for Gold (you save $110) We swim late August through June. Monthly invoices will be sent October through June. Additional charges will apply for participation in swim meets. Charges are typically $25-$50 per meet, depending on the meet and the number of events your swimmer enters. Our team typically enters around 16 meets each year. Meet locations include St. Louis, St. Charles, Springfield and Joplin in Missouri, and Edwardsville and Carbondale in Illinois. Once the coach determines a swimmer is ready to compete, we ask that they try to attend at least two meets per year.
