5 Manchester Swim and Racquet Club Unguarded Hours Swim Waiver I release Manchester Swim and Racquet Club Inc. (MSRC), their officers, and members and volunteers of any liability in connection with my swim activities. I also understand that in the event of an accident, MSRC will not be held liable and that my individual health insurance policy will be the primary insurer. During unguarded pool hours, a parent or other responsible adult MUST accompany their children under 18 years of age. NO EXCEPTIONS!!! I agree to and understand the following rules regarding pool use during unguarded hours. 1. All pool rules apply to unguarded hours as listed in the policy and procedure manual for all pool hours with the following exception. When there is no lifeguard on duty, my family and I will be swimming at our own risk. 2. When the lifeguard leaves at the end of a shift, unaccompanied children under age 18 MUST leave immediately. 3. Someone able to call 911 in the event of an emergency must accompany me (i.e., I will not swim alone). 4. I will not give my access card out to anyone as it is intended only for my family s use. Likewise, I will not provide unauthorized access to others by opening the gate. All adult household members should sign below.
