3 2014 NATIONAL TEAM U.S. Paralympics will nominate a cycling National Team for 2014, comprised of the following: National A Team National B Team National Development Team Each team will be nominated based on athlete performances from the following events: National A Team 2013 UCI Para-Cycling Road World Championships 2014 UCI Para-cycling Track World Championships National B Team 2013 UCI Para-Cycling Road World Championships 2014 UCI Para-cycling Track World USA Cycling selection event (Chattanooga, TN) 2014 USA Cycling Para-cycling National Road Championships National Development Team 2014 USA Cycling Para-cycling National Road Championships 2014 USA Cycling National Handcycling Criterium Championships 2014 UCI Para-cycling Track World Championships 2014 U.S. Paralympics Cycling National Track Championships 2014 UCI Road World Cup Events Qualification per the 2013 Athlete & Sport Program Plan No Track World Championships were held in 2013, and are therefore not utilized to nominate any teams for U.S. Paralympics 2014 National Cycling Team status applies for one or more of the following terms: Spring Term: January 1 April 30, 2014 Summer Term: May 1 July 31, 2014 Fall Term: August 1 December 31, 2014 National Team status and benefits are only conferred upon athletes who accept, sign and remain current with the U.S. Paralympics National Team Athlete Agreement. Athletes named to the team for the Spring or Summer term, will retain their National Team status through the last date of the term for which they are named, and may retain their team status through the end of the following term provided they have met the minimum performance standards per the athlete performance plan. Athletes named to the team for the Fall term will retain National Team status through 12/31/2014. Athletes nominated to the 2014 National Team(s) for any term will be added to the USADA Registered Testing Pool and will be responsible for remaining compliant with all forms, updates and tutorials as required by USADA. More information regarding USADA can be found in Attachment E. Athletes nominated to the National Team(s) must be internationally classified and hold a Paralympic-eligible sport class as per the UCI master list, and hold a current season international license from USA Cycling. 4 2014 National A, B, and Development Team Minimum Performance Standards Athletes nominated to the National Team are required to maintain minimum performance standards during each term of the cycle in order to maintain status at any level for the next term. Athletes who do not meet the minimum performance standards during a term will be moved one level lower effective the first day of the next term, or for athletes on the Development Team will be removed from the team effective on the first day of the next term. Minimum performance standards for each athlete will be outlined in the Athlete Performance Plan, authored by the Head Coach or VA Cycling Coach, U.S. Paralympics Cycling and signed by the following parties: Athlete Athlete's Personal Coach Head Coach or VA Cycling Coach, U.S. Paralympics Cycling All Athlete Performance Plans must also be approved and signed by the High Performance Director, U.S. Paralympics Cycling National Team Standards (Attachment A) ROAD 2014 National Team Road standards are created using the following system: Men: Average of 3 rd place finisher s individual time trial result for each sport class at the following events: 2011 UCI Para-cycling Road World Championships 2012 Paralympic Games 2013 UCI Para-cycling Road World Championships Women: Standards are calculated at 116% of the men s standard for each respective sport class. This calculation is used to ensure equity across all women s classes due to limited field sizes. TRACK 2014 National Team Track standards are based on the average of the 3 rd place finisher s results for each sport class from the following events: 2011 UCI Para-cycling Track World Championships 2012 UCI Para-cycling Track World Championships 2012 Paralympic Games Track standards for sport classes with fewer than 3 entrants at any of the above events are calculated at 105% of the current world record or world s best in the respective sport class. 5 2014 NATIONAL A TEAM QUALIFYING CRITERIA Spring Term: Athletes who won one or more medals in individual events at the 2013 UCI Para-cycling Road World Championships will be nominated to the National A Team for the 2014 Spring Term. Summer Term: Athletes who won one or more medals at the 2014 UCI Para-cycling Track World Championships will be nominated to the National A Team for the 2014 Summer Term. Fall Term: Athletes who are nominated to the roster for the 2014 UCI Para-cycling Road World Championships and who met the National Team road time trial standard at both the USA Cycling selection time trial in Chattanooga, TN and the USA Cycling Para-cycling Road National Championships Time Trial Championships in Madison, WI will be nominated to the National A Team for the 2014 Fall Term NATIONAL B TEAM QUALIFYING CRITERIA Spring Term: Athletes who placed in the top 50% in the road race or time trial where the field for the individual sport class included 8 or more finishers at the 2013 UCI Para-cycling Road World Championship will be nominated to the National B Team for the 2014 Spring Term. Athletes who were part of a medal winning team event at the 2013 UCI Para-cycling Road World Championships will be nominated to the National B Team for 2014 Spring Term. Summer Term: Athletes who place in the top 50% in an individual event where the field size is 8 or more finishers at the 2014 UCI Para-cycling Track World Championships will be nominated to the National B Team for the 2014 Summer Term. Athletes who are part of a medal winning team event at the 2014 UCI Para-cycling Track World Championships will be nominated to the National B Team for the 2014 Summer Term. Fall Term: Athletes who are nominated to the roster for the 2014 UCI Para-cycling Road World Championships and who met the National Team standard in either the road time trial at the USA Cycling selection time trial in Chattanooga, TN or the USA Cycling Para-cycling Road National Championships Time Trial Championships in Madison, WI will be nominated to the National B Team for the 2014 Fall Term. 6 2014 NATIONAL DEVELOPMENT TEAM QUALIFYING CRITERIA Spring Term: Athletes who met the Talent Pool Standard in the individual time trial at the 2013 UCI Para-cycling Road World Championships will be nominated to the National Development Team for the 2014 Spring Term. Athletes who met the Talent Pool standard in an individual race at the 2013 U.S. Paralympics National Track Championships will be nominated to the National Development Team for the 2014 Spring Term. Athletes who won a medal in an individual or team race at a 2013 UCI Para-cycling Road World Cup event will be nominated to the National Development Team for the 2014 Spring Term. Summer Term: Athletes who met the Talent Pool standard in an individual race at the 2014 UCI Para-cycling Track World Championships* will be nominated to the National Development Team for the 2014 Summer Term. Fall Term: Handcycle and Tricycle athletes who win the National Criterium or Road Race Championship in a sport class with 6 or more finishers, will be nominated to the National Development Team for the 2014 Fall Term. Athletes who meet the Talent Pool Standard at both the USA Cycling selection time trial in Chattanooga, TN and the USA Cycling Para-cycling Road National Championships Time Trial Championships in Madison, WI will be nominated to the National Development team for the third term of / 2014 NATIONAL DEVELOPMENT TEAM QUALIFYING CRITERIA Athletes who did not meet the above criteria for the 2014 National A Team, National B Team, or National Development Team, but were named to the National Team during 2013 and are still within the 13-month period as outlined in the 2013 plan, will be named to the 2014 National Development team until: A ) The 13-month period expires, or B) They meet the criteria (above) for one of the 2014 National Teams, whichever is sooner. Athletes in this category who are named to the Development Team are subject to the following, adjusted benefits: $166 per month in Direct Athlete Support No Supplemental Athlete Support No eligibility for EAHI * In the event that the 2014 UCI Para-cycling Track World Championships are cancelled, or are not held prior to 4/30/2014, U.S. Paralympics will conduct an alternate UCI international track event, which will be utilized to name athletes to the National Team for the 2014 Summer Term. Specific NT qualifying criteria will be included in a revision of this document pending the final determination and/or scheduling of Track World Championships by the UCI National Team Standards (Attachment A) Road Road standards are based on the 3 rd place finisher's individual time trial result for each sport class at the the following events: 2011 UCI Para-cycling Road World Championships 2012 Paralympic Games 2013 UCI Para-cycling Road World Championships Track Track standards are based on the 3 rd place finisher's results for each sport class from the following events: 2011 UCI Para-cycling Track World Championships 2012 UCI Para-cycling Track World Championships 2012 Paralympic Games Page 6 of 25

7 DISCRETIONARY NOMINATION Discretionary nomination to the 2014 National Team(s) may be used if an athlete who has demonstrated international competitiveness during any of the qualification periods for the 2014 National Team(s) was ill, injured, or otherwise unable to compete or perform at a level to qualify him/her for the team; but it is expected that the athlete will return to internationally competitive form during the term to which they are named. The Director, Paralympic Cycling High Performance in consultation with the Team Leader will make discretionary nominations. All athletes nominated to the 2014 National Team(s) through discretionary selection must have been on a previous National Team during the preceding two years/seasons ( ) NATIONAL TEAM U.S. Paralympics will nominate a Cycling National Team(s) for 2015, based on the 2015 Athlete and Sport Program Plan. NATIONAL TEAM BENEFITS National Team athletes may be eligible for the following benefits: 1. USOC Direct Athlete Support (Attachment B) 2. Cycling Additional Athlete Support Programs (Attachment C) 2. USOC Elite Athlete Health Insurance program (Attachment D) 3. Air transportation (or ground equivalent), lodging, ground transportation, meal per diem, and coaching support at designated U.S. Paralympics training and competition activities. 4. National Team athletes will be provided cycling competition and casual apparel for training and competition. Guidelines for apparel are outlined in the U.S. Paralympics Cycling National Team Uniform Agreement which all NT athletes must sign and adhere to. Page 7 of 25

8 TALENT POOL ATHLETES Athletes (military and non-military) who are eligible for Paralympic competition in a UCI sport class, and who met the Talent Pool standard in 2013 or during the 2014 season, at a USA Cycling or UCI-sanctioned event* may be invited to participate in selected U.S. Paralympics Cycling Program activities. Invitation is at the discretion of the Director, Paralympic Cycling high Performance Talent Pool Road and Track standards (Attachment A) are calculated using 105% of the National Standards. EMERGING ATHLETES Athletes (military and non-military), who are eligible for Paralympic competition in a UCI sport class, and who met the Emerging standard in 2013 or during the 2014 season, at a USA Cycling or UCI sanctioned event*, may be invited to participate in selected U.S. Paralympics Cycling Program activities. Invitation is at the discretion of the Director, Paralympic Cycling High Performance Emerging Road and Track standards (Attachment A) are calculated based on 115% of the National Team Standards. MILITARY ATHLETES Military and Veteran athletes who are eligible for Paralympic competition in a UCI sport class, and who met the Military standard in 2013, or during the 2014 season, at a USA Cycling or UCIsanctioned event*, may be invited to participate in selected U.S. Paralympics Cycling program activities. Invitation is at the discretion of the Director, Paralympic Cycling high Performance Military Road and Track standards (Attachment A) are calculated using 120% of the National Standards. PROOF OF PERFORMANCE Forms for submitting proof of performance (applies to athletes who need to verify they have met the Talent Pool, Emerging, or Military Standard), are available via the Para-cycling section on the U.S. Paralympics website: * Time trials utilized by athletes to meet standards must be on a course that is at least 15km long, on an out/back or looped road course (no closed circuit track courses) Page 8 of 25

9 2014 PROGRAM ACTIVITIES The 2014 program activities are outlined on Page 2. Athletes will be required to attend all 2014 activities included in their athlete performance plan which will be developed by the U.S. Paralympics Cycling Head Coach and/or Military & NT Coach in collaboration with the athlete s personal coach. All athlete performance plans will be approved by the Director, Paralympic Cycling High Performance. Each athlete is responsible for individual arrangements (including expenses) to attend these activities (i.e. travel, lodging, accommodation, entries) unless otherwise indicated in the activity listing. National Team athletes are required to obtain a 2014 USA Cycling International Competition License. Camps Consideration for attendance at camps listed on page 2 will be determined by the Director, Paralympic Cycling High Performance in consultation with the U.S. Paralympics Cycling Head Coach, Military / NT Coach, and the Director, Paralympic Emerging Sport & Talent ID. Competitions For UCI international competitions in 2014, athletes will earn TeamUSA roster nominations (regardless of National Team status) based on the published selection criteria for the event. Competitions that are Nations Events (open only to athletes nominated to the official TeamUSA roster by U.S. Paralympics), or where U.S. Paralympics will enter athletes as part of the National Team, are indicated by a TeamUSA annotation on the event schedule on page OPERATION GOLD U.S. Paralympics will award Operation Gold funds in Cycling based off of the results at the 2014 UCI Para-cycling Road and Track World Championships as outlined below. -OR- In the event that there is no UCI Para-cycling Track World Championship event for Cycling in 2014, the UCI Para-cycling Road World Championships ONLY. In non-olympic/paralympic years, Operation Gold Awards are limited to one award per athlete in a program year. Athletes who qualify in multiple events during a non-olympic/paralympic year will only receive the highest award amount for which he/she qualified at the specified competition. For Paralympic sports in non-paralympic years, individuals must finish in one of the top six (6) places and among the top 50% of the athletes who started the event to qualify for Operation Gold Awards. 12 Attachment B 2014 U.S. Paralympics Cycling Direct Athlete Support 1. DAS is processed monthly on the first Friday of each month. 2. Athletes are eligible for DAS at the beginning of the month following nomination to the National Team(s). 3. DAS is provided to athletes that are in compliance with 2014 Athlete Agreement obligations and athlete performance plan obligations. If an athlete is not current with the agreement/plan obligations and/or USADA on the first day of the month in which the payment is being made, the athlete s payment will be withheld until he/she is compliant. 4. U.S. Paralympics will provide DAS for multi-sport athlete in ONLY one sport. In conjunction with the respective NGB/HPMO High Performance Director(s) the multi-sport athlete shall designate the primary ( A ) sport in which they wish to receive DAS. 5. Athletes sanctioned by USADA, WADA and/or the respective sport IF for a doping violation are not eligible for DAS, regardless of any National Team(s) status, during the period of such sanction. 6. Athletes sanctioned by the IPC and/or the respective sport IF for classification Intentional Misrepresentation are not eligible for DAS, regardless of any National Team(s) status, during the period of such sanction. 7. DAS will only be paid once the Athlete Support Designee Form has been completed and submitted (annual basis). 8. Athlete support is paid directly to the athlete. National Team athletes are responsible for understanding and maintaining their own eligibility status with other organizations where applicable (i.e. NCAA, NFHS, etc.), and may choose to decline direct athlete support payments in order to retain high school or collegiate eligibility. 9. 13 COACHING SUPPORT Attachment C 2014 U.S. Paralympics Cycling Additional Athlete Support Programs Coaching Pool U.S. Paralympics Cycling will create a coaching pool derived from the list of coaches hired for National A Team athletes. The coaching pool may also include additional coaches at the discretion of U.S. Paralympics. All coaches must hold a USA Cycling Level 2 coaching license or higher, and must undergo a USOC background check. Agreements (including payment terms) with all coaches will be negotiated by the Director, Paralympic Cycling High Performance. National A Team Athletes Athletes named to the National A Team will select their own personal coach, who will be paid a monthly coaching fee by U.S. Paralympics. National A Team athletes also have the option of choosing U.S. Paralympics Cycling staff coaches as their personal coach (subject to availability). National B Team Athletes Athletes named to the National B Team will receive coaching support and will select a coach from the coaching pool (including staff coaches, if available). National B Team athletes who wish to supplement this with additional, personal coaching support may utilize their SAS funds to hire an additional coach (see Supplemental Athlete Support below). National Development Team Athletes Athletes named to the National Development Team are eligible to hire a personal coach using SAS or personal funds. Development Team athletes who choose to hire a coach from the U.S. Paralympics Cycling Coaching Pool, can do so at a discounted rate. Page 13 of 25

14 SUPPLEMENTAL ATHLETE SUPPORT Supplemental Athlete Support (SAS) is a new athlete benefit for 2014 that provides athletes with a flexible system to support their training and racing. Athletes can submit receipts for reimbursement up to the amount outlined in the table below (based on team status) for expenses in the following categories: Personal Coaching. In addition to, or in lieu of using a staff coach, or a coach from the preferred coaching pool. Discounted Coaching. Development team athletes can receive discounted coaching when using SAS to hire a coach from the preferred coaching pool. Equipment. Any equipment related to training or competing as a Para-cyclist. Travel. Any non-national Team travel expenses to USA Cycling or UCI cycling competitions (including entry fees) or pre-approved training trips. All noncompetition travel submitted for reimbursement must be part of the Athlete Performance Plan Team Status Level 2014 SAS Annual Amount* National A Team $4,000 National B Team $2,500 National Development Team $1,500 * In the event that an athlete changes to a different team status level during 2014, their SAS will be pro-rated based on the number of whole months at any given status level. 20Proposed 2017 U.S. Paralympics does not fund National Team athletes to attend Trials/National Championships/selection events; however, attendance at specific USA Cycling and/or UCI competitions may be required as per the athlete s Athlete Performance Plan. PERFORMANCE INCENTIVE SUPPORT Additional performance incentives will be provided to athletes who attain results at UCI Para cycling C1 events and Road World Cup events including results for team competitions when competing as a member of TeamUSA*. UCI C1 Events (Overall finish ranking within sport class) 1st Field with > 2 participants $ $ nd Field with > 3 participants $ $ rd Field with > 4 participants $ $ World Cups (Place within sport class per individual race) *No incentives will be paid to athletes who race at UCI events as independents or as part of a trade team. 15 OTHER SUPPORT Equipment National Team athletes may have access to NT team equipment, including but not limited to: Bicycles / Trikes / Handcycles Components and Wheels Power meters Specialty helmets Race-specific competition apparel Trainers and rollers All NT equipment will be inventoried and distributed based on a) Team status, and b) Medal potential. Equipment will be issued at the discretion of the Director, Paralympic Cycling High Performance in consultation with the Head Coach and Military/NT Coach, U.S. Paralympics Cycling. All NT athletes utilizing equipment owned by U.S. Paralympics will be required to sign the National Team Equipment Agreement Form and will be responsible for any damage or loss to the equipment. Special Pricing National Team athletes may have access to special pricing from official partners and suppliers of U.S. Paralympics Cycling. Details and policies will be communicated to athletes on a case-by-case basis, by the Director, Paralympic Cycling High Performance throughout the 2014 season. Page 15 of 25

16 Attachment D 2014 U.S. Paralympics Cycling Elite Athlete Health Insurance (EAHI) The USOC agrees to provide up to 16 EAHI slots to athletes who meet the eligibility and criteria. The approved EAHI eligibility and criteria for EAHI is as follows: Athletes must be currently training and competing to receive EAHI and they must sign the 2014 Athlete Agreement and Athlete Support Designee Form. Criteria 1: All 2014 National A Team athletes are eligible for EAHI. Criteria 2: Athletes who are named to the 2014 U.S. Paralympics Cycling National B Team or National Development Team may be eligible for EAHI as determined by the Director, Paralympic Cycling High Performance and Team Leader. Selection for EAHI will be based on current performance at a level that indicates a positive trajectory of performance with a chance to medal in the Paralympic Games or World Championships between 2014 and Should there be more athletes than slots available after Criteria 1 and or Criteria 2, the decision to award EAHI will be based on which athlete(s) have the best opportunity to medal at the Paralympic Games in 2016 or Should there be any available slots after all eligible athletes [who request and] have been awarded EAHI, those slots will remain unused. MEDEX Insurance Every athlete enrolled in the Elite Athlete Health Insurance Program also receives MEDEX Plus as part of the benefits. MEDEX Plus provides: Medical Assistance Services (i.e., locate nearest doctors or hospitals, facilitate hospital payments, and monitor case) ** Travel Assistance (i.e., replacement of lost/stolen travel documents, translation services, travel arrangements) ** Destination Services (i.e., pre-travel information, travel & health information, real-time security intelligence) ** Personal Security Services (i.e., political evacuation, security evacuation) ** Medical Evacuation - In the event of a medical emergency, MEDEX will arrange & pay for a medically supervised evacuation to the nearest medical facility or return to point of origin. Repatriation Services - In the case of an injury or an unexpected illness resulting in death, MEDEX will coordinate & pay for the return of remains to your home country. **This is assistance only and does not cover the cost of any services provided. MEDEX Assist also offers a benefit referred to as TravMed Abroad. This supplemental benefit is offered at an additional cost to you of $3/day and can only be purchased when traveling outside the U.S. What does TravMed Abroad provide? Team USA Career Program Opportunities The Team USA Career Program is in its early stages and is anticipated to grow over time. Currently, there are limited employment opportunities with USOC Sponsor, Hilton Hotels, Inc. Eligibility Criteria 1. Only athletes who are currently training and competing in Olympic and Paralympic events/disciplines will be considered for eligibility into the Team USA Career Program. Page 19 of 25

Athletes participating in Olympic/Paralympic events/disciplines must be a member of the U.S. National Team (senior level), or the equivalent, if the athlete s sport does not have a National Team. *There may be cause for an exception to the Team USA Career Program eligibility criteria because of an extenuating circumstance where a deviation from the criteria best meets the needs of the athlete and NGB or the Team USA Career Program employer. These deviations will be handled on a case-by-case by the USOC s Athlete Support Working Group, the athlete s NGB and the Team USA Career Program employer. ** This eligibility criteria will be reviewed at least once per quadrennium by the USOC (with input from the Athlete s Advisory Council (AAC) and NGB Council) to ensure fairness and consistency. Allocation of Team USA Career Program Opportunities By meeting the above eligibility criteria, athletes are qualified for consideration for an employment opportunity with Hilton or any other participating USOC Sponsor-Employer in the Team USA Career Program. Meeting the athletic eligibility criteria established by the USOC and U.S. Paralympics does not guarantee a position within the TeamUSA Career Program. Hiring decisions will be made independently by the Team USA Career Program Employer with information provided by both the USOC and Adecco, the USOC s Official Career Transition Management Sponsor and Administrator of the Team USA Career Program. Annual Eligibility Review Process Summer sports will be reviewed annually in the October/November timeframe to verify they still meet the Team USA Career Program Eligibility Criteria. Winter sport athletes will be reviewed annually in the March/April timeframe to verify they still meet the Team USA Career Program Eligibility Criteria. Annual Eligibility Review Process (continued) Athletes no longer meeting the athletic eligibility criteria will be removed from the Team USA Career Program by the USOC. During these review periods and anytime throughout the athletes employment, the USOC sponsor-employer may terminate the athlete s employment if he or she fails to meet mutually agreed upon and established work criteria. Athletes are expected to keep working in their Team USA Career Program position until they are notified by the USOC, Adecco and/or their employer that they no longer meet the eligibility requirements. 22 Attachment H 2014 U.S. Paralympics Cycling Resident Program U.S. Paralympics Cycling will re-introduce a Resident Program at the Colorado Springs (CSOTC) and Chula Vista (CVOTC) Olympic Training Centers. The core of the program will be at the CSOTC beginning January 2014 and continuing through the 2016 Paralympic Games. Athletes accepted as part of the program will reside in dormitories at the CSOTC. Specific training and competition will be geared to individual athlete needs but generally will include 8-14 hours per week of training with other resident athletes, or independently. Eligibility: Be at least 15 years of age (OTC policy). Be a current US Citizen eligible to represent the USA in the Paralympic Games. High School age applicants must be enrolled in high school and maintain a 2.7 GPA. Program staff will strongly recommend online courses or enrolling in private school to best facilitate both school and training requirements. Applicants who take college courses must maintain a 2.0 GPA. Be willing to submit to a complete health profile coordinated by USOC Sports Medicine including thorough medical history and comprehensive physical examination. Requirements: (including but not limited to) Commitment to reside in CO Springs from January 1 December 31, with a review every 6 months and renewal application each calendar year Strict adherence to the Athlete Performance Plan Sport science testing Sport psychology sessions Nutrition counseling Community outreach and involvement Media training Acceptance & Application Acceptance to the Resident Program will be at the sole discretion of U.S. Paralympics and all positions available in the Resident Program will not necessarily be filled. Evaluation of athletes for acceptance to the Resident Program will consider a wide range of factors including: Space availability. Meeting and maintaining a minimum emerging standard (Attachment A) per the athlete s specific event/sport class. Athlete s current home training environment. Potential for the athlete to medal at the World Championships/Paralympic Games. Projected effect/impact of the Cycling Resident Program on the athlete s medal potential. Athlete s desire and willingness to commit fully to the training Resident Program. Training compatibility with other resident athletes.
