11 Cathedral This had a mooring attached to a chain around a coral head until it was replaced with a Helix anchor. Depth: approx. 30 ft. Sea Floor Anchor Type: Helix Anchor Problems: None Current GPS: 21º ' N, 072º ' W 3/2/13 left a new mooring set up at Big Blue as they said they would replace the mooring. 9/4/13 Replaced the chain around a coral head sea floor anchor with a Helix Anchor 11/22/13 Ball and Pickup line missing both were replaced. 11

12 Piranha Cove The was another chain wrapped around a coral head until it was replaced with a Helix Anchor Depth: approx. 48 ft. Sea Floor Anchor Type: Helix Anchor Problems: None Current GPS: 21º ' N, 072º ' W 12/1/12 Club Med Reported that the mooring has broken off. 12/20/12 Replaced the entire mooring setup used the existing chain 4/18/13 - Pick up line was missing so planned on just replacing that, but the mooring line thimble at the surface had rotated 90 degrees and was cutting the line so replaced the entire mooring setup using the existing chain. 9/4/13 Installed a new Helix Anchor to replace the chain around a coral head. 10/24/13 Mooring ball missing replaced 11/22/13 Mooring ball missing replaced with new 18 inch ball with new antichafe 6/15/14 Installed new mooring line and new 15 inch ball 12

13 Pinnacles The existing mooring is on a very solid sand anchor in about 35 feet of water It needs a proper Helix Extension installed with a shackle swivel. Depth: approx. 35 ft. Sea Floor Anchor Type: Helix Anchor Problems: Should install an extension with a shackle swivel Current GPS: 21º ' N, 072º ' W 4/18/13 - Old mooring set up was found sunk on the bottom. Replaced that with a whole new mooring set up attached to the existing Helix anchor. The mooring line has a float on the ground tackle and another about feet above the sea floor. Shackle at the top of the Helix anchor is bad as the nut holding the shackle in place has rusted out and the entire shackle needs to be replaced. New shackle will need to have an opening of 1.5 or more inches. 9/2/13 Installed a new mooring line and 18 inch buoy. Also replaced the shackle bolt 13

14 11/22/13 Inspected and looked fine nut missing on the shackle bolt, so zip ties added to keep it in place 14

15 Graceland Couldn't find the old sea floor anchor - no mooring on the site right now. Good sand there at 35 ft. - see sketch Depth: Approx. 45 ft. Sea Floor Anchor Type: Big sand patch - 35 ft deep - NEEDS A SAND ANCHOR Problems: Nothing other than no mooring Current GPS: 21º ' N, 072º W 15

16 Grouper Hole This was a mooring connected to a chain around a coral head until it was replaced with a Helix Anchor Depth: approx. 40 ft. Sea Floor Anchor Type: Helix Anchor Problems: None Current GPS: 21º ' N, 072º ' W 12/8/12 Club Med Reports that the mooring has broken off and needs to be replaced. 12/20/12 Went to replace the mooring, but the setup I brought was too short. Mooring line is there, but no ball just a bleach bottle tied to a light line attached to the polypropylene mooring line. 4/18/13 Searched for the sea floor anchor to replace the missing mooring, but it is missing. Need to install a pin before this can be replaced. 9/4/13 Replaced the chain with a Helix anchor 11/22/13 Inspected and looked fine 6/15/14 Installed new 15 inch mooring bouy 16

17 Grotto This site hadn t had a mooring for some time until a Helix Anchor was installed. Depth: Approx. 40 ft. Sea Floor Anchor Type: Helix anchor Problems: None Current GPS: : 21º N, 072º W 9/4/13 Installed a new Helix Anchor and mooring set. 17

20 Arawak This site is about 180 yards or so East of Boneyard we assume it is Arawak The mooring is a 1 inch nylon line attached to a 12 white buoy with a blue line. Pick up line is a polypropylene. The mooring is attached to the rock pin by a shackle with a swivel. Depth: Approx. 60 ft. Sea Floor Anchor Type: Rock Pin Problems: None Current GPS: 21º ' N, 072º ' W 12/20/12 Installed a new mooring on the site. 20

21 02/03/13 noticed that the mooring line should be a bit longer and that the lower float on the mooring line has nearly completely collapsed Should remove the old line laying on the bottom. 9/2/13 Installed a new mooring line it appeared that someone had just disconnected the previous one since the shackle was unscrewed and laying next to the pin. 11/22/13 Inspected and looked fine 21

22 Boneyard Depth: Approx. 55 ft. Sea Floor Anchor Type: Rock Pin Problems: None. Current GPS: 21º N, 072º ' W 2/2/13 - The coral head around which the chain is wrapped has broken half off and the remaining section appears fragile. Also, the surface float is a small orange ball with no pick up line. Mooring line itself looks fine may just need a surface float and pickup line after repairing the sea floor anchor. 11/22/13 Mooring line broke off needs an entire new set up. 6/15/14 Removed the chain from around the coral head and installed new rock pin and new mooring line set up. 22

23 Coral Cable This site is NOT on the nautical charts we have consulted. This site is near the original communication cable coming to Provo. Straight off the beach from the National Environmental Center Depth: Approx. 50 ft. 27 WE Wreck The wreck is intact, but the wheelhouse has fallen off the deck. It has also slide down the slope a bit more, now the bow is at 105 feet and the propeller in sand at approximately 175 feet. Depth: Approx. 105 ft. 31 Dolphin Grotto This site is not currently on the nautical charts. The mooring is a Helix Anchor in the sand installed in September Depth: Approx. 45 ft. Sea Floor Anchor Type: Helix anchor Problems: None Current GPS: 21º N, 072º ' W 9/6/13 New mooring pin and line installed 11/21/13 Replaced missing mooring ball with new 18 inch with better anti-chafe 31

32 Two Step This site is NOT on the nautical charts we have consulted. The existing mooring a large Helix Anchor in the sand. It is in good shape. One of 4 moorings on NW Point (Ampitheater, Eel Garden and Coral Stairway) used by the live aboard vessels. Depth: Approx. 40 ft. Sea Floor Anchor Type: Helix Anchor Problems: surface float is not an officially sanctioned buoy and needs to be replaced Current GPS: 21º N, 072º W 12/8/12 PTD Reports the mooring has gone missing and needs to be replaced. 12/9/12 Replaced the entire mooring set up except the Helix Anchor 2/3/13 Mooring looks fine 32

36 Grand Canyon This site is NOT on the nautical charts we have consulted. Rock pin is epoxied into the top of a large dead brain coral. Depth: Approx. 47 ft. 37 Shark Hotel The sea floor anchor is an epoxied U bolt installed by the Aggressor. The chain is still on the pin, but the mooring line and ball are missing. Depth: 45 feet Sea Floor Anchor Type: Stainless U-bolt Problems: Needs a swivel on the sea floor. Current GPS: 21º ' N, 072º ' W 11/27/12: The TCI Aggressor II replaced the mooring from the pin to the surface. It is a one continuous piece of Polypro line with a black 5 gallon bucket tied to the line about 30 feet from the end to serve as the surface float. 12/9/12: Replaced the black bucket with a proper white ball with a blue stripe. 37

Larger surface buoy still needed. 6/15/13 Mooring missing needs new sea floor anchor 6/24/13 new sea floor anchor (stainless u-bolt) installed by the TCI Explorer and the Aggressor to installed a new mooring set up. 9/6/13 Mooring ball had gone missing, so it was replaced. Rest of mooring line is fine. 11/21/13 Mooring ball missing so replaced with a new one with better anti-chafe 9/4/14 Replaced missing mooring buoy and added a new pickup line 39

40 Chimney Replaced the chain around the coral head with a Helix Anchor in September 2013 Depth: Approx. 40 ft. Sea Floor Anchor Type: Helix Anchor Problems: Nothing other than needs to be converted to a proper mooring Current GPS: 21º N, 072º W 2/10/13 - Some chain links are half rusted through. Line is 1 polypro that looks ok with some deterioration starting to show (brittleness primarily). Chain is destroying a nice finger coral colony on the coral head around which it is looped. 4/30/13 Missing mooring may have been taken out by the White Cloud. 9/6/13 Installed a new Helix Anchor and new mooring line set up 11/21/13 Mooring Buoy was missing so added a new one with better anti-chafe 40

41 Thunderdome Mooring is on a Helix Anchor changed from being attached to the large concrete filled tractor tires in September 2013 Depth: Approx. 30 ft. Sea Floor Anchor Type: Helix Anchor Problems: None Current GPS: 21º ' N, 072º ' W 11/28/12 - TCI Aggressor II replaced the mooring after it was destroyed during a strong storm surge. 9/6/13 Added a Helix anchor and moved the mooring line to this. 11/21/13 Mooring Buoy was missing so added a new one with better anti-chafe 41

42 The Crack Replaced the chain around a coral head with a Helix Anchor in September 2013 Depth: 40 feet Sea Floor Anchor Type: Helix Anchor Problems: None Current GPS: 21º N, 072º W 2/10/13 - found the old chain wrapped around a coral head and installed a new mooring on it. 9/5/13 added a Helix Anchor and moved the mooring line to this. 11/21/13 Mooring Buoy was missing so added a new one with better anti-chafe 9/4/14 replaced missing mooring buoy 42

43 Eel Garden Large stainless U-bolt epoxied into hard pan bottom at 40 ft. - thimble may be cutting into the line, otherwise ok - surface float is a white ball- good pickup line Depth: Approx. 40 ft. target Sea Floor Anchor Type: Stainless Steel U-Bolt Problems: None Current GPS: 21º ' N, 072º ' W 12/8/12 - PTD reports that the mooring line has sunk and needs a new float. 2/9/13 Someone has tied a round white boat bumper to the mooring line. Mooring line is fraying and the thimble at the sea floor is cutting into the line. Entire mooring needs to be replaced. 43

44 4/13/13 Replaced the entire mooring set up except the sea floor anchor. Still needs more flotation on the chain and a large surface buoy. 9/5/13 Inspected the mooring and everything looks fine. 11/21/13 New pickup line and buoy 44

45 Tiki Hut Wall Hard pan bottom. Mooring is attached to one of the large concrete blocks that were frequently used on NW Point dive sites for sea floor anchors (most of which have now been dragged over the edge of the wall) Depth: Approx ft. Sea Floor Anchor Type: Large concrete block - Needs a EPOXIED PIN at about 45 ft Problems: Concrete block needs to be replace by a rock pin. Current GPS: 21º ' N, 072º ' W 9/6/13 found the large concrete block that has served as the sea floor anchor, so a new mooring line was attached to it. 11/21/13 Mooring is missing and the concrete block could not be located. 45

46 Coral Stairway Large stainless U-bolt epoxied into hard pan bottom at 45 ft. - chain to the rope (about 4 ft) drags on bottom - Depth: Approx. 45 ft. target Sea Floor Anchor Type: Stainless Steel U-Bolt. Problems: None. Current GPS: 21º N, 072º ' W 11/1/12 Mooring is missing and u-bolt is broken 6/24/13 the pin has been replaced and a brand new mooring set up with an 18 buoy has been installed. 6/7/14 Complete new mooring line set up added 46

53 Highway to Heaven Found the original Helix Anchor and reinstalled a mooring on it. Depth: Approx ft. 54 Elephant Ear Canyon There wasn t a mooring here, so a Helix Anchor was installed. Explorer ripped the buoy off this line and has installed a new stainless steel u-bolt further inshore from the Helix. Line on the Helix may still be there, but the buoy is gone. So there are two mooring pins at this site. Depth: Approx. 50 ft. (for the Helix) Sea Floor Anchor Type: Helix Anchor and Stainless U-bolt (further inshore) Problems: Helix is missing the buoy and the lines may be damaged when the Explorer ripped the buoy off Current GPS: 21º N, 072º W 9/9/13 Installed a new Helix Anchor and mooring line set 11/1/13 Explorer ripped the mooring buoy off the Helix anchor line with its tender. 9/4/14 Installed new buoy on the line 54

57 Gully The sea floor anchor is a stainless steel U epoxied into the sea floor. The Explorer installed a new U-bolt in the same area as the last two that had pulled out. A Helix Anchor was installed slightly inshore and to the South of the rock patch where the U-bolts were installed, but the Explorer pulled the Helix out so now back to the u- bolt Depth: Approx. 51 ft. Large stainless U-bolt epoxied into hard pan bottom at 42 ft. - chain to the rope (about 4 ft) drags on bottom float to support the chain is partially deflated and needs to be reinflated to raise the chain from dragging on the bottom - otherwise in good shape - surface float is a small red float which should be replaced - the pickup line is in good shape - mooring replaced only a few months ago Depth: Approx. 45 ft. 82 Dax Canyon This site is NOT on the nautical charts we have consulted. The sea floor anchor is a rock pin epoxied into the hard pan bottom. Long chain through the pin is then shackled to a polypropylene mooring line. Chain is too long and dragging the bottom creating a large dead zone. Either remove the chain or add flotation to keep it off the bottom. Appears to be a single polypropylene line from the chain to the surface and as pick up line. Line appears relatively new. Depth: Sea Floor Anchor Type: Rock Pin Problems: The thimble on the mooring line at the sea floor has shifted 180 degrees and is cutting through the line. Chain is dragging the bottom. No swivel on the sea floor anchor and chain just runs through the eye of the rock pin, so high risk of entanglement of the chain. 82

89 G Spot The existing mooring is a attached to an epoxied U bolt it has about 8-10 feet of chain attached to the U-bolt and then to a polypro line - chain is in good shape and has a float attached to it in an effort to keep it off the bottom, but the float is partially deflated and needs more air to prevent further damage to the coral surrounding the U-bolt mooring polypro line is in ok shape, but appears to have been spliced several times near the surface 89
