All forms are due to Coach Kluttz NO LATER THAN Thursday, May 12 th. It is your responsibility to see a doctor for a physical prior to your arrival at tryouts. If a current physical is not in the hands of the coach PRIOR to the first day of tryouts, you will NOT be allowed to participate. (Make sure the doctor notes the date of the exam, including the year, and SIGNS the form!) Feel free to use this letter as a checklist to ensure you are ready to go on the first day of tryouts. Please understand that CMS cheerleading is both time-consuming and physically demanding, and, therefore, must not be entered into lightly. I look forward to seeing you at tryouts May 16 th! 3 CMS CHEERLEADING SPIRIT GUIDELINES I) Mission Statement A) It is the primary goal of the cheerleaders to positively promote school spirit, support all athletic teams of the school, and successfully involve the crowd at all events. Cheerleaders will assume leadership roles, setting an example of exemplary behavior, promoting friendship, and demonstrating the benefits of teamwork, while maintaining their priority to be a STUDENT athlete. II) Purpose A) It is the purpose of the cheerleading squad to: Promote and uphold school spirit Establish qualities of responsibility, tolerance, and confidence Demonstrate teamwork Grow as adolescent athletes III) Squad Make-up A) Cabarrus County Schools allows coach s discretion in determining how many members will be on the squad. The Concord Middle School cheerleading program will intend to consist of two squads, 7 th and 8 th grade, consisting of 15 or fewer members each. BOTH squads will cheer at all home football games, which will take place every Wednesday during the season. 8 th grade cheerleaders will cheer all home games of the 8 th grade basketball season. 7 th grade cheerleaders will cheer all home games of the 7 th grade basketball season. Travel to basketball tournament games will be determined at a later date. All cheerleaders will participate in other activities (pep rallies, fund-raisers, etc.) at the discretion of the coach. The coach may alter squad makeup if there are not enough eligible candidates for two teams. IV) Tryouts A) Cheerleading tryouts will be conducted over four (4) days, May 16 th 19 th, On the first two days, candidates will practice arm positions, jumps, and tumbling, and learn two chants and a dance. A review of the material and stunt practice will take place on the third day. On the fourth day, candidates will practice showmanship and performances of the material they have learned. Depending on the number of candidates, a cut may be made after the third day at the discretion of the COACH. Though judges may be brought in daily during tryouts to evaluate candidates, Cabarrus County Schools policy states that the use of outside evaluators is NOT mandated and the coach has final discretion on who makes the team. Evaluators will share their evaluations with the coach, who will take them into consideration when selecting the final members of the team. There will be a mandatory parent meeting for all new team members following the last day of tryouts on Monday, May 23 rd for fittings, payments, and fundraiser kick-off. 7 th grade cheerleaders from the previous year are NOT guaranteed a spot on the 8 th grade squad. All candidates will go through the same tryout process. Tryouts are closed to outside viewing. 1) ***Candidates must dress in appropriate attire while attending practices and the tryout: shorts, t- shirts, athletic shoes, NO jewelry, hair back and out of face, and no hard plastic accessories can be in the hair. Candidates should wear CMS school colors of black, gold, and white. No Cheerleader/Cheerleading logos or apparel should be worn in order to prevent any bias in evaluating. Fingernails must be short (no longer than the fingertips) and unpolished for safety purposes. 2) ***Each candidate will be evaluated in the following categories: (a) Responsibility paperwork turned in promptly, attendance and punctuality, academic eligibility (b) Attitude, Effort, and Growth progress should be evident and obvious (c) Crowd Appeal and Projection fitness, neatness, facial expressions, energy, confidence 1 P a g e

4 CMS CHEERLEADING SPIRIT GUIDELINES (d) Jumps form, height, flexibility (e) Motions precision, arm placement, technique (f) Cheer, Chant, and Dance precision, timing/rhythm, coordination, memorization, execution (g) Tumbling level of difficulty, execution, technique (h) Stunting flexibility in position, technique, reliability V) Requirements A) In order to tryout for the Concord Middle School cheerleading team, each candidate must have the following: Been in attendance 85% of the current semester (missing no more than 13 days in a semester). Passed 4 of 6 classes, 3 of which must be CORE classes from the previous semester. All paperwork completed, signed by the parents, and returned to the coach by the designated deadline. A current physical form from a doctor. Keep in mind that these are MINIMAL requirements! The coach may choose to have stricter guidelines. B) Once on the squad, members must: Maintain eligibility by: o Being in attendance 85% of the time (missing no more than 13 days in a year). o Passing 4 of 6 classes, 3 of which must be CORE classes, each semester. Attend all scheduled practices and camps. Cheer at all games for football and basketball season. Actively participate in all other activities set forth by the coach, to include fundraisers and special events. Buy shoes, briefs, and other various items. Be a positive role model for others. Abide by all rules and regulations provided by the coach. C) ***Not fulfilling the above requirements can result in ineligibility and/or removal from the cheerleading squad. D) Any candidate who is more than fifteen (15) minutes late being picked up after practice during the week of tryouts will be subject to elimination from tryouts. E) ***If a candidate already has several afternoons taken up by other commitments, he/she should not tryout, unless prepared to quit some of those activities. Unlike other sports, we do not have a bench from which to pull replacements. Every member of the squad is extremely vital. When one member is not present, it negatively affects the entire squad. If you have a conflict, contact the coach to discuss options prior to tryouts. VI) Monetary Commitments A) While the school provides uniforms and pompons for the squads, each individual is responsible for paying for other clothing items and fees. These items and their approximate costs are listed below. There may be optional items available as well. There will be fund-raising to alleviate the cost of some of these items. However, it is not guaranteed to provide full coverage for these costs. B) Items you must purchase: 1. Camp Fee approximately $ Camp Shirts (3), Bloomers (1), and Hair Bow (1) approximately $50 3. Final totals will be tallied once the items have been ordered ***Costs will be broken down into either one or two payments due May 26 th and June 8 th, ***NO CANDIDATE WILL BE TURNED AWAY DUE TO FINANCIAL HARDSHIP!!! VII) Fundraisers A) In an effort to reduce the overwhelming monetary costs of cheerleading at CMS, fundraising will be a mandatory commitment on behalf of ALL cheerleaders. B) All funds will be distributed as deemed necessary by the coach to alleviate as much of the above listed costs as possible. Funds may also be used to cover any and all expenses incurred by the cheerleading program as a whole. C) All cheerleaders are expected to attend and actively participate in any and all fundraising activities. Thirty-One/Premier Designs Jewelry sales will begin immediately following tryouts. Autobell Carwash tickets will go on sale during the month of August. A Children s Cheer Clinic may be held on one Saturday in October. Spirit ribbon sales will take place on the morning of every game. Cheerleaders will need to be at school at 8:35 on these days. The coach may deem it necessary to have a fundraiser aside from those listed above depending on the needs of the squad. VIII) Expectations A) Concord Middle School cheerleading must take priority over all other extracurricular activities. Each member is expected to be at ALL practices and performances. Because we do not have a bench from which to pull replacements, every member plays a vital role in our performances and as a member of the squad. Attendance at summer camp at UNC Chapel Hill August 5 th 7 th is mandatory for all cheerleaders, as this is where we will begin to establish stunt groups, which cannot be designed without the entire team present. B) Concord Middle School cheerleaders must be positive role models at all times. This includes having good sportsmanship, citizenship, attendance, grades, behavior, and appearance. These qualities should be shown at ALL TIMES. Deviation from exemplary behavior will not be tolerated. C) CMS cheerleaders are students before they are cheerleaders. Grades must be kept in compliance with eligibility requirements. Each school day, fifty percent attendance is mandatory for involvement in a practice, game, or other cheerleading activity. In order to participate in a day s event, a cheerleader may not check-in any later than 12:30 pm. Likewise, a cheerleader may not check-out any earlier than 12:30 pm. D) Cheerleaders are expected to take care of all clothing, especially uniforms, pompons and shoes should be kept clean. These clothing items will be used throughout the entire year. Uniforms and pompons must be returned for use from year to year. E) When in uniform, cheerleaders must have all of their hair up and out of the face (with a ribbon) and remove ALL jewelry (even tiny studs and ankle bracelets). 6 CMS CHEERLEADING SPIRIT GUIDELINES F) Members are to be picked up promptly from all practices and games. Pick-up times will be determined, in advance, by the coach. Cheerleaders who are consistently late being picked up from events will be subject to consequences that may result in removal from the squad. G) While an accurate event calendar is provided in advance, occasional additions and/or changes may occur. All members of the squad are expected to follow these changes and be as flexible as possible with their schedules. (This is especially in regards to basketball tournaments, as we often will not know if/when/where we will play until the day before.) H) Cheerleaders and parent(s)/guardian(s) are to provide accurate contact information and check/respond to those methods of contact within a reasonable time frame. Frequent communication is VITAL for updating parents and cheerleaders! VI) Practices A) Coach will provide calendars of mandatory practices. B) To prevent injury, cheerleader must dress appropriately for practice. NO jewelry or hard plastic hair accessories are allowed, and nails are to be kept no longer than the length of the fingertips and unpolished. Hair must be pulled up TIGHTLY and off of the face. Camp clothing will become practice uniforms throughout the year and cheerleading shoes are to be worn at all times. C) Cheerleaders will be dismissed at second dismissal and must be dressed and in the practice area at the appointed time. D) Stretching is a must before every practice. A thorough stretch before cheering will prevent injuries. E) Stunting, jumping, and tumbling (anything where feet are off of the ground) without a coach present are ABSOLUTELY PROHIBITED. F) Cell phones and tablets are to be completely inaccessible during practices to promote full focus to the tasks at hand and eliminate possible distractions. G) Members are to be picked up promptly from all practices and games. Pick-up times will be determined, in advance, by the coach. Cheerleaders who are consistently late being picked up from events will be subject to consequences that may result in removal from the squad. VII) Games A) On game days, cheerleaders may be released at 3:45 for football season and 3:30 for basketball season. They will have ten minutes to dress and ready themselves for pre-game practice. B) Members must attend all football and basketball games. C) Members must be dressed appropriately for all games; (i.e. uniform, briefs, shoes, hair). D) Members must stay in their designated area during the duration of the game. E) Members must focus on cheering for the game and avoid any socializing. F) Members will stand with hands on hips or sit with hands in laps during down time. G) There is to be ABSOLUTELY NO ARGUING OR DISCUSSIONS ABOUT ANYTHING during games. We will focus on the game at hand and maintain our professionalism. H) If traveling to away games the cheerleaders will ride the bus with the players (unless prior arrangements have been made), and parents will pick them up at the away location at the conclusion of the game(s). I) During game time, the only time a cheerleader is permitted to eat is during half-time and between games. Drinks may be consumed at anytime to promote proper hydration. J) Members are not to use or be in possession of his/her cell phone or other electronic devices at any time during a game. Phones may be used at half-time, after their performance, or between games. K) Members must be in the designated area two minutes prior to the start of a game and one minute prior to the end of half-time. L) Sportsmanlike conduct is to be demonstrated at ALL times. Cheer Wars are prohibited. M) Cheerleaders are not to speak to family and/or friends during the game unless there is an emergency. Parents may send messages/drinks to their cheerleader(s) through the coach if necessary. 4 P a g e

7 CMS CHEERLEADING SPIRIT GUIDELINES VIII) Safety A) Concord Middle School and Cabarrus County Schools has adopted the National Federation of State High School Associations Spirit Rules. This rule book lists all steps that must be taken to ensure safe practices and performances. Safety is our main goal, but the stunts and tumbling we do are difficult and accidents may occur, causing injuries. B) The coach is both certified by AACCA in safety as well as tested on the NFSHSA Spirit Guidelines. C) Although mats are used to develop and perfect tumbling/stunting techniques, the NFSHSA Spirit Rules do not require the use of mats during routines. If the coach does not believe a stunt or tumbling pass can be safely executed without a mat, said stunt or tumbling pass will not be performed. This is up to the discretion of the coach. D) In order to prevent accidents and injuries, we will incorporate strength training (conditioning) into daily practices. These exercises will increase both strength and stamina of all squad members. E) The rules and regulations set forth in the Concord Middle School Cheerleading Spirit Guidelines must be followed at all times to ensure each member s safety. IX) Consequences and Coach s Decisions A) While the goal of cheerleading is to have fun, we know that certain rules must be followed for the season to be a safe success. We believe that the above rules, expectations, and responsibilities are reasonable and fair. To ensure that our goals are met and our season is safe and successful, disciplinary procedures will be implemented at the coach s discretion. B) ABSENCES AND TARDIES If, at any time, a cheerleader must be absent from or tardy to an event or activity, he/she or his/her parent must contact the coach via verbal notification, written note, telephone message, or one day PRIOR to the event. This is to allow plenty of time for the coach to adjust their plans accordingly as well as account for the whereabouts of cheerleaders. Cheerleaders are to attend 100% of every event. Any last-minute and/or extenuating circumstances must be brought to the attention of the coach immediately and at least by the 12:00 noon on the day of the event itself. This will give the coach time to make last-minute adjustments prior to the event. If a cheerleader is absent from the practice before a game, he/she is ineligible to be in the half-time routine for the next game. If a cheerleader is absent from an event/activity without properly notifying the coach, he/she is subject to disciplinary action that may result in removal from the squad. Frequent tardies to school are unacceptable for a CMS cheerleader. (a) Every four (4) tardies recorded by the front office will be counted as one absence. Chronic absences and/or tardies will not be tolerated and may result in removal from the squad. Any cheerleader who is more than fifteen (15) minutes late being picked up three (3) times will be subject to removal from the team. Parents will be notified after each instance, with the third notification requiring a mandatory conference with the coach, athletic director, parent, and administrator. Cheerleading is a nearly year-round commitment and must not be entered into lightly. One hundred percent dedication is mandatory to produce a successful team. C) ATTITUDE AND BEHAVIOR Cheerleaders are expected to be on their best behavior at all times. This includes their interactions with fellow cheerleaders and their coach. Disrespectful attitudes and behavior toward fellow cheerleaders and/or coach, as well as classroom teachers will not be tolerated. Offenses of this nature will be subject to disciplinary action that may result in removal from the squad. 5 P a g e

8 CMS CHEERLEADING SPIRIT GUIDELINES During stunting, there is to be absolutely NO TALKING OR LAUGHING. Stunting can be very dangerous, and distractions of this nature can lead to serious, life-threatening injuries. If a cheerleader cannot abide by this rule, he/she risks being prohibited from stunting. D) All disciplinary actions will be taken at the discretion of the coach. Each instance will be dealt with individually. The coach will do what is best for the team as a whole. E) ACADEMICS Although a D is passing according to the county policy, such grades are unacceptable for a CMS cheerleader. Any cheerleader receiving a grade less than a C (70) will immediately be placed on probation. (a) The student will still be expected to attend all practices and events. However, the student will not be allowed to participate. He/she will be in a study hall throughout the duration of the practice or event until the grade has improved to a C (70) or above. (b) The coach will be in constant communication with the teachers and parent(s)/guardian(s) of the cheerleader at academic risk. According to county policy, any child who fails to maintain eligibility will be removed from the team. F) Teachers are given a list of cheerleaders at the beginning of the school year. Any disciplinary issue brought to the coach by a cheerleader s teacher will be addressed immediately. The consequences of such are as follows: First Offense: Warning and conditioning Second Offense: Cheerleader will write a letter of apology to the teacher; and conditioning Third Offense: Cheerleader will write and recite a letter of apology to his/her coach and teammates. Cheerleader will be placed on a one-week probation and prohibited from participating in any practice or event that will take place on the day the offense was committed, though attendance at the practices and events is mandatory. Fourth Offense: Any offense immediately taken to the administration, whatever consequences are handed down from administration, will be dealt with accordingly. Fifth Offense: Immediate suspension from all practices and events, which may result in removal from the squad, pending consideration and investigation by the coach. G) Any cheerleader assigned to serve time in ISS (In School Suspension) as a disciplinary consequence will be prohibited from participating in and attending any practice or event scheduled for the day(s) student was physically in ISS. ISS penalties are as follows: (a) First Offense: See above (b) Second Offense: 2 week cheerleading suspension and letter of apology written and recited to teammates and coach. (c) Third Offense: Removal from squad H) In accordance with Cabarrus County policy, any athlete receiving ANY number of days of OSS (out of school suspension) will immediately be removed from the team. I) If, at any time, a cheerleader is suspended from activity for reasons other than ISS or OSS, or is injured but physically able to attend, he/she is still required to attend all events in uniform, and sit on the sidelines with the rest of team. ***Please be advised that CMS Cheerleading operates as a team. When just ONE member of that team is missing for ANY reason, the ENTIRE team is negatively affected. All CMS cheerleaders are advised to consider this carefully before misbehaving or allowing grades to fall below par. 6 P a g e

9 CMS CHEERLEADING SPIRIT GUIDELINES Prospective cheerleaders and their parents are required to read and understand the expectations set forth by these guidelines PRIOR to cheerleading tryouts. Please read the agreement below, sign, and date. The signed agreement must be returned to the coach along with the attached paperwork on or before Thursday, May 12 th, Please keep the Spirit Guidelines for your records. I understand that if these rules and regulations are not followed, consequences will result. As a cheerleader, I will be part of the cheerleading team based on the Concord Middle School Cheerleading Spirit Guidelines. I will do everything I can to make the year a positive and successful experience. 10 Participation Contract for CMS Cheerleaders Attending J.N. Fries My child,, will be attending J.N. Fries while participating in Concord Middle School cheerleading in the school year. It is my responsibility to transport my child to and from athletic events and practices on time. I am aware that my child will not be allowed to enter the building before 3:30 on game days or before 4:00 on practice days, as there will be no one available to supervise my child. Tardies and absences will be dealt with at the coach s discretion as outlined by the coaches prior to joining the team. It is my responsibility, and that of my child, to communicate regularly with the coach either by or phone. This is necessary for updates, changes, and cancellations. It is my responsibility, and that of my child, to share any behavior indiscretions at J.N. Fries with the Concord Middle School coach immediately. I understand that my child is subject to the consequences of any indiscretions as outlined by the coaches prior to joining the team. It is my responsibility, and that of my child, to share my child s grades with the coaches at each progress report and report card. I understand that my child is subject to the consequences of unacceptable grades as outlined by the coach and the Cabarrus County Eligibility policy prior to joining the team. I understand that failure to accept these responsibilities could result in the removal of my child from the team at the discretion of the coach. I have been given a copy of the rules, which I signed prior to joining the team, and am fully aware of the consequences outlined by the coach. Street City: State: Zip: INSURANCE: Student Accident Insurance must be taken unless the insurance waiver option is checked below by the parent indicating adequate personal insurance and releasing the Board of Education and its employees from responsibility for any claim due to injuries received while participating in a schoolsponsored athletic program. This requirement also includes team tryouts and out of season workouts. 1. There are limitations in the Student Accident Insurance coverage. It will not always pay all charges for every accident. Read the description of the current Student Accident Insurance carefully and be sure that you understand 2. Neither the Board of Education nor any of its employees will assume responsibility for claims resulting from injury to your child while he or she is participating in this program. This means that you will have to pay for any necessary medical treatment not covered by the Student Accident Insurance or any personal insurance coverage that you might have. 3. School Board Policy requires that all students who participate in athletics be adequately covered by medical or accident insurance. Choose one of the following options: (a) I have adequate personal insurance and release the Board of Education and its employees from any responsibility in this matter. (b) My Son/Daughter is already enrolled in the Student Accident Insurance Program and I understand that I am responsible for payment of any charges not covered by this policy. (c) I need to purchase Student Accident Insurance. Information can be obtained online at or requested at the school. MEDICAL AUTHORIZATION: Permission is hereby granted to the Athletic Director, Athletic Trainer, or Coaching staff at our Cabarrus County School to proceed with ANY needed medical treatment, ambulance notification, and/or x-ray for the above named student. In the event of serious illness or injury, need of surgery, or accidental occurrences, I understand that an attempt will be made by the coaching or medical staff to contact me by phone. 17 STUDENT ATHLETE CONTRACT CABARRUS COUNTY SCHOOLS I understand participating in athletics is a privilege and not a right. I have certain responsibilities that I must maintain since I will be representing my school. Conditions for participating in athletics are as follows: I will meet ALL eligibility requirements set forth by the North Carolina High School Athletic Association. DRUG and ALCOHOL - I commit to staying drug and alcohol free. All court consequences must be resolved before petitioning to return to the athletic program. *See Criminal Charges Policy located on Athletic Pledges and Additional Policies Page. 2 nd Offense 365 days suspension from athletic participation 3 rd Offense Suspension from athletic participation for the remainder of the student s high school career Any student who has continuing charges or unresolved court consequences on the first day of tryouts for that particular season will NOT be allowed to participate for that season. Non-Charged or Non-School Related Alcohol/Drug Offenses: Suspension from participation for the remainder of the current season. When out of season, any student involved with drugs or alcohol will not be allowed to participate in off season workouts for that current season. If a second offense occurs within a one year time period of the first offense, the student athlete will be suspended from ALL athletic participation for 365 days. I understand that my participation is governed by Board of Education Policy 3620 and that my participation may be restricted if I (1) am charged with criminal conduct; (2) violated the student conduct standards found in the 4300 series of policies; or (3) have violated school rules for conduct. Any student who has continuing charges or unresolved court consequences on the first day of tryouts for that particular season will NOT be allowed to participate for that season. I represent the athletic department and my actions reflect on the school, community, and my family. Any actions that bring discredit to me or these institutions will be dealt with by the administration and the athletic department and may include dismissal from the team. This includes ALL social media sites. I commit myself to continuously working toward the goal of being a contributing member of an athletic team at my school. Once I begin a sport, I may not try-out for any other sport until that team s season is completed. If I decide to quit a sport, I must immediately notify the coach and return all uniforms/equipment issued. I will be charged for all uniforms/equipment not returned and prevented from entering the next sport. If I quit a team during a contest, I will be suspended from that team for the remainder of the season. I may also be suspended from athletics for up to one year and must have the permission from the principal and athletic director to rejoin the athletic program. I understand that to letter in a sport I must fulfill all requirements of the athletic department and the head coach. I must be counted present at school for at least half a day on game day/practice day. Exception: Under extraordinary circumstances, the parent can petition the school (Principal/A.D.) in writing for permission to play. (e.g. Immediate family funeral; College/University visit) I must ride to and from the game in which an activity bus is provided. (Athletes are to ride back after the contest under the supervision of the coach unless their parent(s) receive permission from the head coach (in person) at the end of the contest to ride with them. Parents must sign off that there child is riding with them). I pledge to be a positive role model to my fellow student athletes/teammates and help them abide by all of the athletic department s rules and policies. If I am removed from a team for disciplinary reasons, I may be prohibited to return to any event involving that team or sport for the remainder of the season. In keeping with the sportsmanship and team spirit/chemistry necessary to have a successful season, it is of the utmost importance that all athletes conduct themselves in a positive manner supporting their team. If deemed that any actions are considered detrimental to the success of the team, that athlete may face disciplinary action as deemed fit by the coach including removal from the team. I accept my responsibility to model good sportsmanship that comes from being a student-athlete. NCHSAA Parent Pledge - As a parent, I acknowledge that I am a role model. I will remember that school athletics is an extension of the classroom, offering learning experiences for the students. I must show respect from all players, coaches, spectators and support groups. I will participate in cheers that support, encourage and uplift teams involved. Using inappropriate language and taunting are contrary to the spirit of fair play and the good sportsmanship our school and conference and the NCHSAA expect. NCHSAA Sportsmanship/Ejection Policy - We acknowledge that we, both the student and parent, have read and understand the NCHSAA Sportsmanship/Ejection Policy. We understand that the following types of behavior will result in an ejection from an athletic contest: fighting, taunting or baiting, profanity directed toward an official or an opponent, obscene gestures, disrespectfully addressing an official, flagrant contact. 1st ejection: 2 game suspension in all sports except 1 game in football Ejection for fighting doubles the penalty: 4 games in all sports except football, which is a 2 game suspension 2nd ejection: Suspended for remainder of sport season. 3rd ejection: Suspended from ALL athletic competition for 365 days from date of 3rd ejection. Felony Policy - The NCHSAA Policy on felony charges states that any student who is subject to the 8 semester rule who (1) Is convicted of a crime classified as a felony under North Carolina or federal law or (2) is adjudicated delinquent for an offense that would be a felony if convicted by an adult; is not eligible to participate in the NCHSAA sports program effective immediately through the end of the students high school career. Criminal Charges Policy - As the parent of a student-athlete, I certify that there are no pending criminal charges against my child. If at any time during the school year, my child is charged with a crime, I must immediately notify the school which he/she attends. Failure to do so will result in the student-athlete losing his/her athletic eligibility for up to the remainder of his/her school tenure with Cabarrus County Schools. According to CCS Board Policy 3620, student participation in extracurricular activities may be restricted due to criminal charges (other than minor traffic violations). If the student is acquitted of the charge(s) or the charge is dismissed, he/she may apply to the principal for reinstatement of the privilege to participate in extracurricular activities. Suspension from extracurricular activities for criminal charges shall continue until court involvement is completed, including but not limited to the following: Charge is dismissed; Student is acquitted; or Court consequence(s) are completed (probation is completed; fines are paid; community service is completed) In addition, suspension from extracurricular activities for subsequent criminal charges shall result in the following consequences if student was convicted of prior charges: 2 nd Offense (if convicted) 365 day suspension from extracurricular activities 3 rd Offense (if convicted) suspension from extracurricular activities for the remainder of the student s high school career Transportation To/From Extra-Curricular Activities - Students must ride to and from the event in which an activity bus is provided. EXCEPTION: With prior written permission and the coach s permission, parent/guardian may take his/her student home from the event. Parents and students are responsible for providing their own transportation to and from all practices and events where an activity bus or other school vehicle is not offered or available. Coaches and staff members shall not transport students in their own private vehicles. Residency Requirements - A student is eligible to participate at the school to which he or she is assigned by the local board of education, within the administrative unit of residence. Residence as used for athletic eligibility purposes is defined as the equivalent of the term domicile as applied by the courts of North Carolina. Under no circumstances can a student have more than one residence for eligibility purposes. It is the obligation of the school to know the residence status of each athlete and to require compliance with these requirements. Except as otherwise provided, the residence of any student shall be deemed to be (a) that of his or her parents (if he/she lives with both parents); (b) that of the sole parent (if he/she lives with only one parent); (c) that of the parent with whom he/she spends the majority of nights during the school calendar year (if he/she has two parents who do not live together); or (d) that of the legal custodian if his/her custody has been awarded to a non-parent by a court of competent jurisdiction. In the event the parents are separated or divorced, the residence of the student shall be that of the parent or other adult to whom primary physical custody has been awarded by a court of competent jurisdiction. If no custody order has been entered, the residence shall be deemed to be that of the parent with whom the student spends the majority of nights during the school calendar year. Any change in residence must be bona fide. Determination of what constitutes a bona fide change of residence depends upon the facts of each case. In order for a change of residence to be considered bona fide at least the following facts must exist: (1) The original residence must be abandoned as a residence; that is sold, rented or disposed of as a residence, and must not be used as residence by any member of the family; (2) The entire family must make the change and take with them the household goods and furniture appropriate to the circumstances; (3) The change must be made with the intent that it is permanent. No non-parental guardianship will be recognized where a student has a living parent unless: there has been a determination of abandonment of the student by such parent(s) or a determination that the student is a dependent juvenile as defined in G.S.7A-5 17(13) or comparable statute by a court of competent jurisdiction; or the student has been judicially declared a ward of the court or has been identified as an orphan or placed in a foster home by the Division of Social Services (or a comparable agency if out of state) and custody (not guardianship) has been determined by the court or social service agency. A student whose custody has been established by court order or social services agency decision is eligible for participation at the school to which he or she is assigned by the Board of Education of the local unit where the custodian resides. The residence of a student who is emancipated shall continue to be his or her residence as of the time of emancipation, unless an exception is granted under the procedures established by the Hardship Rule. A student who resides out of state but attends a member high school could be eligible, if the student attends a member non-boarding parochial school or is a member of a parochial church and submits an authorized pastor verification form; or if the out-of-state student is attending a member school as an employee benefit because a parent is working at the school, the school must request a hardship waiver of the residence rule with appropriate documentation as a result of the employee status. The residence of a student who is in an organized and recognized foreign exchange program shall be considered to be the place to which he or she is assigned by that program, and is participate at the school to which he or she is assigned by the local education agency. Foreign students who are not part of an organized and recognized foreign exchange program must present extenuating circumstances through the procedure established in the Hardship Rule. A foreign exchange student is eligible only during the first year of residence in the United States. With the exception of this residence rule, foreign exchange students are subject to all other eligibility requirements, including but not limited to, the prohibition on participation after graduation or eligibility for graduation from high school. In no case will any exception be made to the following principles: No student may participate at a second school in the same sport during the same sport season, except in the event of a bona fide change in residence of the parent(s) or legal custodian; change of schools must be contemporaneous with change in residence. Documents purporting to establish guardianship or custody issued by a notary public, an attorney, a clerk of court, or any entity other than a court of record with competent jurisdiction will not be accepted. 19 ASSUMPTION OF RISK INFORMATION WARNING TO STUDENTS AND PARENTS The following is in compliance with requirements to inform all athletes that any activity involves the potential of injury, which is inherent in sports. I/We acknowledge that even, with the best coaching, use of the most advanced equipment and strict observance of the rules, injuries are still a possibility. On rare occasions these injuries can be severe as to result in total disability, paralysis or even death. I/We acknowledge have read and understand the warning. SERIOUS, CATASTROPHIC AND PERHAPS FATAL INJURY MAY RESULT FROM ATHLETIC PARTICIPATION By its nature, competitive athletics may put students in situations in which SERIOUS, CATASTROPHIC and perhaps FATAL ACCIDENTS may occur. Athletics can be one of the most rewarding aspects of any student s scholastic career. We are proud that your son/daughter has chosen to be a student-athlete and has accepted the responsibility of that DUAL ROLE. It is our hope that you will share in your son/daughter s athletic experience as a spectator and, more importantly, as a source of encouragement at home. Many forms of athletic competition result in violent physical content among players, the use of equipment which may result in accidents, strenuous physical exercise, and numerous other exposures to risk of injury. Students and parents must assess the risks involved in such participation and make their choice to participate in spite of those risks. No amount of instruction, precaution, or supervision will totally eliminate all risk of injury. Just as driving an automobile it involves choice of risk, athletic participation by middle/junior and senior high students may also be inherently dangerous. There have been accidents resulting in death, paraplegia, quadriplegia, and other very serious permanent physical impairment as a result of athletic competition. By granting permission for your child to participate in athletic competition, you, the parent or guardian acknowledge that such a risk exists. By choosing to participate, you, the student, acknowledge that such a risk exists. Students will be instructed in the proper techniques to be used in athletic competition and in the proper utilization of all equipment worn or used in practice and competition. Students must adhere to that instruction and utilization and must refrain from improper uses and techniques. As previously stated, no amount of instruction, precaution, and supervision will totally eliminate all risk of serious, catastrophic, or even fatal injury. If any of the foregoing is not completely understood, please contact your school principal, athletic director, coach or athletic trainer for further information. ASSUMPTION OF RISK - FOOTBALL ONLY Football is a contact sport and injuries will occur. The coaches working in our program are well-qualified professional people. Fundamentals related to playing football will continually and repeatedly be emphasized on and off the field. The information contained within the list of rules and procedures is to inform the young men/ women in our football programs of the proper techniques to practice for maximum safety in the contact phase of the game. Serious head and neck injuries leading to death, permanent brain damage or quadriplegia (extensive paralysis from injury to the spinal cord at the neck level occurs each year in football. The toll is relatively small (less than one fatality for every 100,000 players and an estimated one non-fatal severe brain and spinal cord injury for every 100,000 players) but persistent. They cannot be completely prevented due to the tremendous forces occasionally encountered in football collisions; they can be minimized by the manufacturer, coach, and the player compliance with safety standards. TACKLING, BLOCKING AND RUNNING THE BALL By rule, the helmet is not to be used as a ram. Initial contact is not to be made with the helmet. It is not possible to play the game safely or correctly without making contact with the helmet when properly blocking and tackling an opponent. Therefore, technique is most important to prevention of injuries. Tackling and blocking techniques are basically the same. Contact is to be made above the waist but not initially with the helmet. The player should always be in a position of balance, knees bent, back straight, body slightly bent forward, HEAD UP, target area as near to the body as possible with the main contact being made with the shoulder. Blocking and tackling by not putting the helmet as close to the body as possible could result in shoulder injury such as separation or a pinched nerve in the neck area. The dangers of not following the proper techniques can be from minor to disabling to even death. The reason for following the safety rules in making contact with the upper body and helmet is that improper body alignment can put the spinal column in a vulnerable position for injury. If the head is bent downward the cervical (neck) vertebrae are in a bind and contact on the TOP OF THE HELMET could result in a dislocation, nerve damage, paralysis or even death. If the back is not straight, the thoracic (mid back) and lumbar vertebrae are also vulnerable to injury with similar results if contact again is made to the TOP OF THE HELMET. Cabarrus County s daily football workout includes exercises to develop strength in the neck muscles. Strengthening the neck muscles is one of the best methods of preventing neck injury and enabling an individual to hold his head up even after getting tired during the workout or contest. BASIC HITTIING (CONTACT) POSITION AND FUNDAMENTAL TECHNIQUE If the knees are not bent, the chance of knee injury is greatly increased. Fundamentally, a player should be in the proper hitting position at all times during live ball play and this point will be repeated continually during practice. The danger is anything from strained muscles, to ankle injuries, to serious knee injuries with may require surgery. The rules have made blocking below the waist (outside the two-yard by four-yard area next to the football at the line of scrimmage) illegal. Cleats have been restricted to no more than ½ inch to help further prevent knee injuries. A runner with the ball however, may be tackled around the legs. In tackling, the rules prohibit initial contact with the helmet or grabbing the facemask or edge of the helmet. These restrictions were placed in the rules because of serious injuries resulting from Non-compliance to these safety precautions. Initial helmet contact could result in a bruise, dislocation, broken bone, head injury, internal injury such as kidneys, spleen, bladder, etc. Grabbing the facemask or helmet edge could result in a neck injury, which could be anything from a muscle strain to a dislocation, nerve injury, spinal column damage causing paralysis or death. Illegal play by participating athletes shall not be tolerated and all players are repeatedly reminded of the dangers of unsportsmanlike acts. FITTING AND USE OF THE EQUIPMENT Shoulder pads, helmets, hip pads, pants, including high pads and kneepads, must have proper fitting and use. Shoulder pads which are too small will leave the shoulder point vulnerable to bruises and separation; it could also be too tight in the neck area, will slide on the shoulders making them vulnerable to bruises and separations. Helmets must fit snugly at the contact points: front, back and top of the head. The helmet must be safety approved NOCSAE stamped (certified seal); the chin straps must have four contact points to the helmet and must be fastened, and the cheek pads must be the proper thickness. On contact, too tight a helmet could result in a headache. Too loose a fit could result in headaches, concussion, and a face injury such as a broken nose or cheek bone; a blow to the back of the neck causing a neck injury could possibly be quite serious such as paralysis or death. This report does not cover all potential injury possibilities in playing football, but it is an attempt to make the players aware that fundamentals, coaching and proper fitting equipment is important to the safety of those playing football in Cabarrus County. The above information has been explained to me and I understand the list of rules and procedures. I also understand the necessity of using the proper technique while participating in the football program. I understand not to use the helmet to butt, ram, and spear an opposing player. This is a violation of the football rules and such use can result in severe head or neck injury, paralysis, or death and possible injury to opponents.
