1 Roof Safety Survey GENERAL INFORMATION Building 31A Roof Safety Assessment CAMPUS: Wollongong Campus BUILDING: Facilities Management Division (31 A) DESCRIPTION: Two storey metal roof building comprising of a main southern wing and smaller northern wing with a steeper pitched roof - anchor points required to clean gutters. No compliance plates or tags on anchor points. The Southern wing has a raised curved section of roof with no achor points or lifelines. High level of sap on roof. Extremely slippery. Use caution and connection to fall arrest system at all time on north side. 2 Roof Safety Survey RISK ASSESSMENT Risk Assessment / Task Location BUILDING 31 Note: The hazards identified do not include hazards that related to specific work tasks. These should be identified in the Safe Work Method Statement (SWMS) of the contractor. What is the Activity/Service Item Access to Roof via hatch Falling What are the potential Hazards What is the Risk Level Use of Roof Ladders Falling from non conforming ladders H Caution Access to Roof Unauthorised access M M List any Control Measures already Implemented Anchor Point at top of hatch, ensure 3 points of contact on ladder and ensure attach to anchor point prior to exiting from hatch All access points to roof are locked and made secure so are not accessable by unauthorised persons; Risk Control Describe what can be done to eliminate risk or reduce the harm Ensure 3 point of connection on the ladder and ensure attach to anchor point prior to exiting from hatch Install compliant ladders and maintain 3 points of connection on the ladder. Signage; Risk Assessment and Roof Safety Survey Gutter Maintenance on new roof Falling H Anchor Points Installed Ensure Anchor Points are used correctly Gutter Edge Maintenance on old roof Falling H Roof Ventilator Maintenance within 2m of roof edge Hazard Identification Risk Assessment & Control Measures Handrail, Horizontal Lifeline or a minimum of Anchor Points are required Falling & Refer to SWMS of contractor for maintenance work H Handrail, Horizontal Lifeline or a minimum of Anchor Points are required Lights Falling - Difficult Access H A minimum of fall protection if accessed from above.
