2 INTRODUCTION Height Dynamics is an innovative height safety company that provides a diverse range of products and services. This is supported by an experienced rope access/height safety team with a background in the construction industry enabling them to perform repairs and maintenance on multi-story buildings. To ensure our work is of the highest quality, Height Dynamics enlists the services of specialist companies such as Structural Engineers and Protective Coating experts to offer technical advice and assist as required in the completion of repairs. We also have tradesmen who can work off rope including electricians, builders and painters. Working at heights is a potentially dangerous practice; Height Dynamics is here to assist you in: Inspections and Non-destructive testing on structures and spans. Provision of rescue supervision of persons whilst working at heights. Rope Access technicians for work at heights. Hire of equipment and appropriate levels of supervision to allow your personnel to carry out more specialized tasks. Nationally Accredited Training specifically suited to your organization. Development and Installation of Height safety systems on new or existing structures. Site Assessment or Suitability reports. Asset management including annual inspection and certification of height safety systems and equipment. Height safety equipment sales and maintenance. To ensure supplied equipment and height safety installations are maintained and re certified for safe use as per AS/NZS 89.4:009 and AS/NZS 4488.:997, an asset management tracking system unique to Height Dynamics is in place generating notification of due inspections and post inspection reports to ensure industry compliance and product serviceability. Height Dynamics competent inspectors boast specific equipment brand training to ensure the highest inspection standards. Height Dynamics adopts an innovative approach to Height Safety. We ensure that Australian Safety Standards are never compromised while fulfilling the needs and expectations of our customers. RESPONSIBILITIES It is the responsibility of the building owners or management, to ensure the safety of its workers by maintaining currency and integrity of installed height safety anchors and all associated components within any systems currently in use with the building. This responsibility includes certifying systems annually, through regular periodic inspections performed by certified, competent technicians to ensure systems and components are to a safe standard for rope access workers to perform relative tasks COMPLIANCE INFORMATION Height Dynamics use the below documents for the use of reference and compliance to the required standards. 3 February 7, 08 Queensland University of Technology Attn: Peter Neale Po Box 434 Brisbane QLD 400 Dear Peter, RE: 08 Anchor Inspection Report - Gardens Point Campus As part of our ongoing service to customers Height Dynamics have compiled a comprehensive overview of the height safety inspection recently completed for Queensland University of Technology with regards to the Gardens Point Campus at George Street, Brisbane QLD The height safety systems have been inspected on the following areas of the above address: B Block C Block D Block E Block H Block M & O Link & O Block Level 5 O Block P Block Q Block X Block Y Block Z Block Ferry Bridge Old Government House The height safety system inspected on R Block at the above address incorporates fifteen (5) free fall arrest surface mounted anchor points positioned in the approximate locations as per the layout included in this report. The height safety system inspected on A Block at the above address incorporates two () free fall arrest surface mounted Height Dynamics inspectors have identified the following issues: One () custom diversion anchor point on P Block has failed inspection as the O ring is rusted and must be replaced Two () diversion anchor points at Y Block have failed inspection as the O rings are rusted and must be replaced Five (5) of the Retrofit Anchor Points on Z Block have failed inspection on as they were removed to install an antenna and are covered by flashing Please note the height safety systems on S Block were not inspected as the building is a construction site with new height safety anchors being installed and the existing ones are currently certified by another company. 4 Please note the following points: It is the recommendation of Height Dynamics that the height safety system described in this report is not used for anything except that which it is designed for. The correct use of the safety system remains the responsibility of the owners of this building. Prior to use of the system, a risk assessment must be undertaken for the identification and control of hazards encountered when working at heights and undertaking manual tasks. Operators using the system must provide a specific Safe Work Method Statement to cover identified risks and hazards. The Safe Work Method Statement must include a detailed and practiced rescue plan. The user of the anchor system shall review the documented access procedure prior to use to ensure appropriate legislated skill level and selection of the appropriate Fall Arrest equipment. A copy of this report must be made available for review by system users prior to access to roof. Any persons using the inspected safety system for working at height should do so only if they have received professional training to a Nationally Accredited level on the safe use of height safety equipment and systems relative to the task they are performing. Height Dynamics accepts no responsibility for the safety of inadequately trained / supervised personnel using the height safety system mentioned in this report. The safety systems that have passed inspection have been certified for safe use. The next inspection will be due in twelve () months for the height safety system in accordance with AS/NZS A record of the inspection has been placed on a tracking program unique to Height Dynamics, to monitor and inform customers as inspections near the due date. A notification will be ed at least a month prior to the inspection date to help keep in line with the specified Australian Standards.
