2 2 of PURPOSE This standard document provides an overview of key components of a LIFT PLAN including a Lift Plan template in WORD format that can be used as a guide in the the creation of a written lift plan (or documenting of a lift plan procedure). NOTE: At Vale, a written lift plan (or procedure) is required for any mobile or fixed crane lift operation categorized as critical. And, a written lift plan, while not mandatory, is recommended for mobile and fixed crane lift operations categorized as non-standard. For further details refer to maintenance standard document: MPROC-700 Categorizing Mobile and Fixed Crane Lifts 2.0 SCOPE This guideline is available for reference by all Vale employees conducting lifting and mechanical handling activities within Vale s Ontario Operations EXCEPTIONS This guideline IS NOT applicable to: Production lifts Engineered lifts Lifts of personnel with a crane or hoist Horizontal pulls (towing) These lift categories are outside the scope of this standard.

4 4 of DEFINITIONS Critical Lift: a lift category where the lift requires comprehensive planning, including formal risk assessment, to minimize the potential of crane failure and/or catastrophic loss due to the elevated risks associated with the lift operation: i.e. characteristics of the load, path of the lift, environmental conditions the lift must be executed within i.e. a mobile crane lifting a load greater than 10 tons, or a fixed crane lifting a load where the hazards can have significant impact to the business i.e. slinging in a mine shaft. The process for categorizing mobile and fixed crane lifts is defined in maintenance standard: MPROC-700 Categorizing Mobile and Fixed Crane Lifts. Engineered Lift: a lift operation that takes into account special situations when the lifting of the load exceeds the manufacturer's hoist or crane capacity and where the lift must be carried out under controlled conditions in accordance with a lift plan prepared by a Professional Engineer. Non-standard Lift: a lift category where the lift operation cannot be accomplished using a known standard process, common method, materials or equipment. i.e., where the weight of the load is unknown and must be calculated or estimated and customized or specialized rigging of the load is required e.g. a Boom Truck Operator needing to rig and lift an awkwardly shaped item such as a large fan impeller. The process for categorizing mobile and fixed crane lifts is defined in maintenance standard: MPROC-700 Categorizing Mobile and Fixed Crane Lifts. Production Lift: a lift that is repetitive and routine, related to a production process, performed by a qualified and competent crane operator following established procedures and processes i.e. crane operator executing lifts in the Smelter converter aisle. Standard Lift: a lift category where the lift operation can be accomplished through a standard process, using a common method, materials and equipment. The load has known or evaluated weight, shape and center of gravity. The radius at which the load must be handled is within the rated capacity of the crane at that radius. The lift is performed under normal environmental conditions. Standard rigging and lifting equipment is used i.e. Mill Circuit Operator loading balls. The process for categorizing mobile and fixed crane lifts is defined in maintenance standard: MPROC-700 Categorizing Mobile and Fixed Crane Lifts.
5 5 of LIFT PLAN The purpose of a lift plan is to have control and establish safety precautions for a lifting operation. It is an important planning process that will identify all hazardous situations that might be encountered during the lifting operation. A lift plan specifies all the requirements and resources necessary to safely and efficiently carry out the lifting of a load. The following is a list of key components to be included in a lift plan. The list is not exhaustive: General Information: purpose of lift, location of the lift, details of load/item to be lifted, date of lift, lift equipment to be used, contact information of lift supervisor etc. Crane Details: manufacturer, model, maximum crane capacity (as is applicable to the crane to be used), crane inspection validations etc. Lift Calculations: the load weight, source of load weight, rigging weight, total load weight, height of lift, surface area of load, tag lines required to stabilize the load, working load limit etc. Rigging Details: slings, shackles, hitch configuration, specialty rigging, hitch/lifting points etc. Lift Team: Stakeholders Superintendent, Supervisor, Lead, Lift Plan Preparer, Crane Operator(s), Signal Person(s), Rigger(s), others as is applicable i.e., contractor(s), Engineer(s) etc. Lift Layout Sketch: a drawing to assist in clarification of crane set-up in relation to load, surrounding structures, rigging and lay-down location or direction of travel, roped off areas etc. Communication: method of communication between lift team members, method of communication to keep lift area clear of personnel not involved in lifting etc. Risk Assessment: risks/hazard identified and controls put in place to mitigate the risks Note: If a documented risk assessment is completed, i.e. JHA, it can be attached to the written lift plan Lift Day Checklist: a final checklist can be part of the lift plan to be reviewed at a prelift team meeting to ensure that all rigging and lift requirements are in place and a safe condition. Lift Plan Approvals: sign-offs indicating the lift plan has been reviewed and understand by those responsible for supervising/executing the lift operation.
6 6 of SAMPLE LIFT PLAN TEMPLATE Appendix A provides a link to a SAMPLE LIFT PLAN in a WORD template that can be used as a guide in the creation of a written lift plan (or in the documentation of a lift plan procedure) for a mobile or fixed crane lift operation.
