Accountability Supervisors are responsible to ensure all person involved in towing/recovering vehicles and machines have completed the assessment included with this SWP. All persons co-ordinating the recovery/towing are to complete a risk assessment before commencing. 6. Training Requirements Only trained Supervisor or Maintenance supervisors are coordinate towing heavy equipment. 7. Document Storage The electronic master version of this document, together with the associated Forms and Templates, is stored on the Safety Managers computer. 3 Note these Requirements A risk assessment is to be completed when making the decision as to which method shall be used to recover the equipment. All recovery equipment on site shall be marked Towing Only (painted a distinct colour) and shall not be used for lifting. The recovery equipment shall be designed to withstand the maximum drawbar pull of the largest piece of equipment on site. A fibre sling or wire rope sling shall be used for the recovery of bogged equipment. A static bar (solid tow bar arrangement) shall be used to tow equipment that has had mechanical failure. A 55T SWL Shackle, shall be used for towing and recovery and shall be marked accordingly. Packing and / or padding shall be used around shackles and sharp edges. MVDC SWP011 May 2008 Page 3

General Property Damage Serious injury In the event of an operator of heavy plant becoming bogged, the operator shall notify their Shift Supervisor immediate and await further instructions. The Shift Supervisor and Maintenance Supervisor shall assess the situation to ascertain which procedure shall be used. The equipment operator is to stay in the equipment at all times until equipment is out of the bog, unless told otherwise by their Shift Supervisor. When moving the equipment out of the bog, the equipment operator is to ensure that the seat belt is securely fastened. The operator is to have a good posture facing forward and have a firm grip on the steering wheel, to reduce possible jolting The Maintenance Supervisor shall inspect the equipment following the recovery, prior to the equipment being returned to service. Towing the Equipment Property Damage Serious injury A spotter/supervisor is to be used with radio contact to the equipment operator and shall stand in a position where they can be easily seen by the operator, so the operator does not have to twist in the seat to see the spotter/supervisor. When it is required to tow a dump truck or other heavy machinery a fibre sling or wire rope sling shall be used between the vehicle being towed and the vehicle requiring towing. Select the correct length and size sling for the task. If in the situation where damage may be caused to the machinery, the Maintenance Supervisor shall be contacted to assess the situation and assist with the towing procedure. Lay the sling in position using the correct manual handling procedures. If the sling is too heavy to move, use a machine eg. Grader or Loader, to place the sling in position. Position the loop of the sling onto the tow hitch of the vehicle. Gloves are to be worn when manual handling the sling. Use a minimum of 2 people to lift a sling into position. The D shackle used shall be of the appropriate size. An approved pin must be used for towing. The manufacturer of the machine being towed usually supplies the appropriate pin. The Supervisor shall be in control of the area and only personnel directly involved with the towing procedure shall be allowed within 50 metres of the area. Ensure the people lifting the slings and D shackle use the correct manual handling procedures, and they are capable of lifting such weights MVDC SWP011 May 2008 Page 7

When the towing is about to start no personnel other than the 2 operators in the 2 machines shall be in the area. All other personnel are to be at a radius of no less than 50 metres away from the machines. This is in case of the sling breaking. To release the sling after towing is complete, drive the bogged machine forward to release tension on the sling, remove the pin and/or shackle. Disconnect the sling from both machines. Return the towing equipment (slings, shackles etc.) to the correct storage location. This is in case of the sling breaking. MVDC SWP011 May 2008 Page 8
