1 JOB HAZARD ANALYSIS (JHA) JHA Preparation Instructions: JHAs are to be prepared by the Supervisor or Group Manager in charge of the work, with work group member participation. The goal of preparing a JHA is for those responsible for or performing any work to review the risks and possible complications of that work and to consider ways of mitigating those risks and complications. A JHA shall be completed for all work executed. The JHA## is to be incremented when a procedure requires more than one JHA form. Standing should be marked if job or task is performed regularly and work group members are always the same individuals. Supervisors are responsible for getting the work group members sign off completed and a copy sent to the safety officer for each JHA. JHA Change Instructions: If changes to the content of this JHA are required, follow the KPNO ETS Engineering Change Process. The master copy of the document must be checked out of the Document Vault, an Engineering Change form must be completed, and the Revision Table (see last page) must be updated. Note that periodic reviews of this JHA that do not result in content changes require only entry of the review date and the name of the reviewer in the Revision Table; the document revision should not be advanced in this case. 3 2100-AD JHA-JHA02 Item No. 3 BASIC JOB STEPS Remove the declination axis hard stop and the bolts that hold the assembly on the horseshoe HAZARDS PRESENT IN EACH JOB STEP ER, FB, SB 4 Remove flat-head screws FB, SB 5 Loop two 20 straps around each end of cable wrap guide and hook up straps to 5 ton crane CO, FB, SB, SL 6 Put tension on the straps with the crane FB, SB, SL 7 Remove bolts and screws FB Maneuver and lower cable wrap guide to Main Floor Move scissor lift into position, raise it back up, and remove two lifting straps Loop one 30 strap around the end of the cable chain attached to telescope, connect with other end of sling, and lift with 5 ton crane Strap 1 will be used to lower the bulk of the chain down using the 5 ton crane FB, SB FB, SB CO, FB, SB, SL CO, FB, SB, SL CORRECT AND SAFE PROCEDURES AND PRECAUTIONS -If positioned at an awkward angle to remove this heavy piece, assess all reasonable removal possibilities -Inspect bolts for rust and wear to ensure a proper fit in the equipment -Assembly must be constrained when bolts are being removed -Due to the weight of the assembly (200 lbs.), serious bodily or property injury can occur if it falls from above -Inspect screws for rust and wear to ensure a proper fit in the equipment -The cable wrap guide weighs about 250 lbs. (without cable chain or cables) and can cause serious bodily or property injury if it falls from above -Inspect bolts and screws for rust and wear to ensure a proper fit in the equipment -When lowering the cable wrap guide, the chain could cause the guide to bind up -To help prevent this from occurring, tie the chain up securely -Confirm no workers are standing underneath the scissor lift as it is being elevated Removal of Pre-DESI Declination Cable Wrap Page 3 of 6

4 2100-AD JHA-JHA02 Item No BASIC JOB STEPS Strap 2 must be disconnected and held by one or two people standing in the center section Remove bolts that hold the cable chain to the center section Note: Prepare to catch spacers when bolts are removed Lower and set cable chain on Main Floor, disconnect, and raise crane HAZARDS PRESENT IN EACH JOB STEP CO, FB, SB, SL, TH FB, SB CO, FB, SB, TH CORRECT AND SAFE PROCEDURES AND PRECAUTIONS -The chain has high potential to fall uncontrollably to the South as it is lowered down on the crane -Inspect bolts and for rust and wear to ensure a proper fit in the equipment -Keep cable chain out of workers way as much as possible to minimalize a tripping hazard Removal of Pre-DESI Declination Cable Wrap Page 4 of 6
