1 JOB HAZARD ANALYSIS (JHA) JHA Preparation Instructions: JHAs are to be prepared by the Supervisor or Group Manager in charge of the work, with work group member participation. The goal of preparing a JHA is for those responsible for or performing any work to review the risks and possible complications of that work and to consider ways of mitigating those risks and complications. A JHA shall be completed for all work executed. The JHA## is to be incremented when a procedure requires more than one JHA form. Standing should be marked if job or task is performed regularly and work group members are always the same individuals. Supervisors are responsible for getting the work group members sign off completed and a copy sent to the safety officer for each JHA. JHA Change Instructions: If changes to the content of this JHA are required, follow the KPNO ETS Engineering Change Process. The master copy of the document must be checked out of the Document Vault, an Engineering Change form must be completed, and the Revision Table (see last page) must be updated. Note that periodic reviews of this JHA that do not result in content changes require only entry of the review date and the name of the reviewer in the Revision Table; the document revision should not be advanced in this case. 3 Item No. BASIC JOB STEPS HAZARDS PRESENT IN EACH JOB STEP CORRECT AND SAFE PROCEDURES AND PRECAUTIONS Ensure cage pins are aligned properly with cage bottom 3 Unclip from 20 SRLs, clip in to 9 SRLs CO 4 Align cage bottom with guide pins SA 5 Unclip from 9 SRLs CO 6 Lower lift CO 7 Move ramp west CO Switch to SRLs with access to the exterior of the cage Failure to line up the pins properly will cause the cage bottom to jam on the pins The 9 SRLs are not long enough to allow workers to stay attached and take the lift all the way down Lower lift slowly until 100% sure cart is free Watch ramp to make sure it doesn t bind on the track 8 Pull the net out CI, TH Make sure not to trip on the net 9 Attach safety straps to net CO 10 Unhook the 20 SRLs from inside the cage CO, SA 11 Unhook the 9 SRLs FB The net straps are not strong enough to hold the weight of the net by themselves. The net will sag over time without the safety straps Be careful of wires fed along upper side of cell Beware of low head clearance Leaning too far out could cause a fall to the net below 12 Remove Lockout/Tagout (LOTO) from the RUPS Breaker, Turn on RA and Dec breakers CB, CBY, CI, CO, FB, SA, SB Failure to turn off breakers may result in unwanted telescope movement by other personnel unaware of instrument change METER CASS CAGE INSTALLATION Page 3 of 5
