1 JOB HAZARD ANALYSIS (JHA) JHA Preparation Instructions: JHAs are to be prepared by the Supervisor or Group Manager in charge of the work, with work group member participation. The goal of preparing a JHA is for those responsible for or performing any work to review the risks and possible complications of that work and to consider ways of mitigating those risks and complications. A JHA shall be completed for all work executed. The JHA## is to be incremented when a procedure requires more than one JHA form. Standing should be marked if job or task is performed regularly and work group members are always the same individuals. Supervisors are responsible for getting the work group members sign off completed and a copy sent to the safety officer for each JHA. JHA Change Instructions: If changes to the content of this JHA are required, follow the KPNO ETS Engineering Change Process. The master copy of the document must be checked out of the Document Vault, an Engineering Change form must be completed, and the Revision Table (see last page) must be updated. Note that periodic reviews of this JHA that do not result in content changes require only entry of the review date and the name of the reviewer in the Revision Table; the document revision should not be advanced in this case. HAZARDS PRESENT IN EACH JOB STEP SB, SA CB, CBY, CI, CO, FB, SA, SB, TH 4 Install the stay bar CB, CBY, CI, CO, SA, SB 5 Ensure pin on crane arm is engaged CO, CW 6 Make sure the telescope is in position at dec of 45 before moving the crane in place 7 Wearing safety harness FB 8 Clip in to lanyard FB SA, SB 9 Flip the lift step into place CBY, CO, CW, SA, SB 10 Flip the lift step out of the way CBY, CO, CW, SA, SB CORRECT AND SAFE PROCEDURES AND PRECAUTIONS Make sure dome enclosure and floor are free of objects in the telescope movement path Make sure not to exceed 45 in dec or the telescope will strike the handrail at the stairs that access the cass cage Failure to turn off breakers may result in unwanted telescope movement by other personnel unaware of instrument change Failure to install the stay bar may result in unwanted telescope movement by improper telescope balance during installation If arm is not locked in place the arm can move freely If the telescope is not in position first it will hit the crane arm when moving into position Inspect harness for unusual wear Wear for the duration USE FALL PROTECTION Inspect for unusual wear Flipping the step too early will cause the step to hit the secondary ring Failure to flip the step will cause the step to hit the secondary ring PROTODESI INSTALLATION PROCEDURE JHA Page 2 of 5
