All Employees Review and approve all injury reports, assign / participate in accident investigations as necessary, perform trending analysis, track injury data and provide to Managers, Supervisors for their units, work with Supervisors on modified duty assignments. Hold Supervisors accountable for timely filing, and for investigations, provide funding for corrective action items that are within their area of responsibility. Report injury electronically within 24 hours, complete accident investigations assist in getting corrective action items completed, provide modified work assignments where possible. Report all injuries to Supervisor immediately when it occurs. Participate in the investigation and recommendations for corrective action.

13 Safety Program Expectations The full details of OPP written safety programs can be reviewed on the safety webpage: ** See your Supervisor or the OPP safety office for any questions about the requirements of the safety programs.

14 Electrical Safety Program (Mechanical Trades) De-energize prior to working on any electrical equipment 50 volts to ground or more Maintain flash gear in good condition, and ensure it gets tested as per the required schedule. Attend training relative to this program, and have a thorough understanding of the requirements Report and concerns related to electrical safety to supervision Only work in accordance with the approach boundaries to live parts Provide illumination for the work to be performed Remove all jewelry and conductive articles where they present an electrical contact hazard Handle tools in a manner that prevents accidental contact with live parts Use protective shields, barriers, insulating materials when working in confined spaces Use only non-conductive ladders Use barricades and warning signs to prevent access to work areas containing live parts, or use attendant Report near miss incidents

15 Electrical Safety Program (cont) Wear Appropriate clothing and PPE for troubleshooting and working with electricity. Non-meltable fiber undergarments (no nylon, polyester polypropylene or spandex shall be worn) Long sleeve non meltable shirt (cotton) 11 or 40 calorie flash gear based on incident energy level Voltage rates gloves and protectors Non-conductive hardhat, safety glasses Di-electric shoes as required in NFPA 70 E Table Inspect PPE prior to performing the work Contact supervisor or electrical engineering for any energy calculations

16 Lock Out Tag Out Program (Mechanical Trades) Identify all energy sources on the equipment to be worked on Notify any affected employees or other employees of the impending shutdown De-energize, lock and tag all potential energy sources prior to conducting any work Release or block any stored energy Verify isolation of equipment prior to starting the work Wear appropriate PPE for the task Date and name on tag Attend training relative to this program and have a thorough understanding of the requirements Only use the designated red locks for lock out tag out Remove red lock at the end of the shift. (Lock out locks are only meant to protect the employee) Notify supervisor of any need to remove someone else's lock At least annually demonstrate a good understanding of lock out tag out requirements Report any safety issues concerning equipment that is not able to be locked out Assist in the development of equipment specific lock out procedures. Obtain the proper number of locks and isolation devices for the trade

17 Hot Work Program (Mechanical Trades) Aim to avoid doing hot work when possible and use alternative methods Ensure a fire extinguisher is available and within reach of the worksite Move the work to a designated safe place when possible Fill out, date and sign hot work permit Display permit at the work site Ensure combustibles are moved at least 35 feet from the work, or use fire blankets, to cover combustibles Cover any floor or wall holes in the work zone shall be covered or protected Have fire and smoke detectors disabled prior to starting the work Establish or perform a fire watch for 30 minutes after the work is complete

18 Confined Space Program (All Trades) Complete permit for entry Use barriers to the space as needed to protect entrants and public from external hazards Maintain communication between entrant and attendant Continuously monitor the air while working in confined space (document readings) Have a rescue plan in place for each confined space job Attend and stay current with training Report all unsafe conditions, practices, or equipment to the supervisor or Safety Coordinator. Only use equipment and materials approved or provided by the supervisor. Properly wear or use prescribed protective equipment. Properly store and maintain equipment used for confined space entry. Immediately report equipment malfunctions to supervisor.

19 Fall Protection Program (All Trades) No work unprotected when working 4 or 6 feet from the ground Only use approved anchor points when using a personal fall arrest system Fall protection is mandatory on aerial lifts and elevated work platforms Participate in Fall Protection training prior to working from an elevated work surface or platform more than 4 or 6 feet in height If working under the direction of a safety monitor, obey commands Inspect all Fall Protection equipment prior to use Only those employees who are licensed and certified on aerial lifts may operate them Never work within 10 feet of the leading edge without fall protection Assist in identifying areas where fall protection is needed

20 Hearing Conservation Program (All Employees) Keep current with audiometric testing Wear appropriate hearing protection when working around noisy equipment or in noisy areas Assist in identifying high noise areas

21 Bloodborne Pathogen Program (All Employees) Follow all universal precautions when cleaning up blood or other potentially infectious materials (OPIM) Report any potential exposures to blood or OPIM Wear appropriate PPE to protect the face, eyes, mouth and skin when cleaning up blood or OPIM

22 Excavation Safety Program (Specific Trades) Maintain competent person designation for performing this work The work zone must be protected for public and pedestrian safety Air monitoring must be completed prior to entry and during if there is any work taking place that creates hazardous atmosphere. Backfill trench shields to prevent movement Standing water must be removed from the trench prior to performing any work Ensure a ladder within 25 feet of any worker Protective system must be used in trench 5 feet deep Place spoil and materials 2 feet away from excavation Barricade or fence trench to protect pedestrians Inspect trench daily with trench inspection form

23 PPE Programs (All Employees) Wear appropriate PPE as assigned by the hazard assessment for the job Wear eye protection 100% of the time while working for all trades. Wear eye protection where task requires in Custodial Services Wear appropriate footwear to work. Supportive work shoes, no open toed shoes, sneakers / tennis shoes are discouraged. Wear hard hat anytime working overhead, anytime there is an overhead hazard, mechanical spaces if there are low hanging items, equipment, etc., when visiting any jobsite or construction site Keep current with respirator clearance and annual fit testing Select and wear appropriate respirator and filter to control hazards

24 Driving and Vehicle Safety (All Employees) Adhere to OPP Vehicle Safety Guidelines Use of spotter while backing up if two people in vehicle Wear seat belt as per PA Motor vehicle law Only ride in an approved seat with a seat belt (no buckets or chairs in the back of vans) No smoking in University Vehicles Report all vehicle accidents immediately to you supervisor as well as any damage discovered on University vehicles No cell phone use while driving or fueling university vehicles

25 Injury Reporting Process (All Employees) Report all work related accidents to supervisor immediately or the day of occurrence Seek medical attention from a panel provider if needed Follow work restrictions if placed on modified duty Supply any work ability reports to supervisor after appointments Assist in accident investigations as needed Provide information to prevent re-occurrence

26 General Ladder Safety (All Employees) Follow safe ladder use as per the training received Only use correct ladder for the job Never stand on the top two steps of a ladder Use ladder that is rated for the weight of the employee and tools Report damaged ladders to supervisor

27 General Reporting of Unsafe Conditions, unsafe acts, near misses (All Employees Report any unsafe conditions to: Supervisor via safety report form safety committee representative safety office Report unsafe acts to your supervisor Assist with ideas for corrective actions Report any near miss incidents to your supervisor Provide ideas for corrective action to prevent re-occurrances
