2 Parent s & Player s Code of Conduct: (Must be read and signed below before allowed on ice) All players and parents will be aware of abuse & harassment and will understand the consequences by not abiding to Redwater Minor Hockey Association s bylaw. Abuse is any form of physical or emotional mistreatment or lack of care which causes physical injury or emotional damage to a player, coach, referee or spectator. Harassment is defined as conduct, gestures or comments, by way of social media, which are insulting, intimidating, humiliating, hurtful, malicious, degrading or otherwise offensive to an individual or group of individuals, and which create a hostile or intimidating environment sport activities. Those involved with Hockey Canada in providing hockey opportunities or participants understand and agree that abuse or neglect, as defined above, may be the subject of a criminal investigation and/or disciplinary procedures. 3 Waiver and Release of Liability I,, hereby release the Redwater Minor Hockey Association, its instructors, its agents and volunteers, and the Town of Redwater from any claims for loss, injury or damage to person or property either directly or indirectly from the attendance, including participation in any activity scheduled or unscheduled, including travel to and from any location. 4 Player Information Release Form This information is collected and distributed in accordance with the Freedom of Information and Protection of Privacy Act, Section 32, 33 and 37. This Consent Form must be signed to allow us to: 1. Release player s names for recognition of achievement in athletics or community involvement. 2. Use player s name, address and telephone number in Redwater Minor Hockey Association directories. 3. Use player s name, photo and comments in newsletters, annual or other publications. 5 Registration Information Registration and Fees Attend the Annual General Meeting on April 30, 2018 and pre-register your child to be entered into a draw for a free registration. Players registering after May 1, 2018 will be subject to a $ late fee. Decisions to accept late registrations will be made by executive at a regularly scheduled meeting. The registration fee and fundraising deposits must be received in full prior to the player being allowed to participate. o Payment for the registration fee may be made in the form of a single cheque made payable to RMHA and can be post-dated no later than September 1, 2018 or cash. o Individuals wishing to utilize a payment plan must provide on or before May 1, 2018, 5 equal payment by post-dated cheques payable with dates of May 1st, June 1 st, July 1 st, August 1 st, September 1 st. NSF fee $ o Please refer to the following IMPORTANT INFORMATION FOR HOCKEY FAMILIES DOCUMENT for information regarding fundraising activities and cheque requirements. o Failure to provide the fee payment and fundraising cheques by September 1 st will mean that the registration is deemed not received. o Failure to make full payment by September 1, 2018 will mean that the registration is deemed, not received. All players must have at least one parent certified in the parent Respect In Sport program. Players new to Redwater or who have never played hockey must provide a copy of their Birth Certificate with their registration. Players previously registered in a Local Minor Hockey Association other than Redwater must provide the name of their previous Local Minor Hockey Association, the RMHA Registrar will request the release.
