1 2018 State Playoffs JV/High School Attached you will find documents providing the schedules, hotels and other information. Price of the tournament. Because of less than 8 teams in both Division 1 and Division 2 there are 3 different prices depending on where you are placed. The assignment of teams to each of the divisions was based on which Division your team is. As to the placement of teams your position on either National or American was assigned by a blind draw at the State Tournament meeting. The pricing is as follows: Ice price is $405 per game for 2 hours and includes click operator/score sheet. All teams in Division 2 (players from the same school) is $810 per team. Division 1 teams National conference will pay $607 per team. Division 1 teams American Conference will pay $760 per team. Checks are to be made payable to MAHA and sent to Joe Spedowski, Mile Rd, Big Rapids, To date I have received 2 team pictures. I need all teams within the next 14 days to include them in a program. We will have both clothing and photo vendors on site. If you have any questions, please send them to me or call me. HOST ASSOCIATION MAHA ALLOWED CHARGES: (see supporting calculation sheet) PER TEAM $: 590 Per USA Hockey Rules: In order to eligible to host a national bound State Playoff or a USAH National Championship, all programs, leagues, etc. at the host facility must be USA Hockey registered and the facility must be a member of the USA Hockey S.T.A.R. rink program. 3 STATE TOURNAMENT INFORMATION SHEET MAHA FORM 11 Organizations hosting a M.A.H.A. State Playoff are required to provide certain features and services. The manner in which these requirements are carried out and the addition of any of the optional items will greatly affect the quality of your tournament. Direct questions to the Chairperson of the State Playoff Committee. REQUIRED ITEMS: Explain if unable to comply. 1. Experienced, mature, time keeper scorekeeper for each game. 2. Public address announcer for each game. 3. Mature penalty box attendants for each game. 4. Certified athletic trainer, EMT, paramedic, or medical or osteopathic physician at each game. 5. Large standings board maintained up to date throughout the tournament. 6. Printed tournament program. 7. Distribute MAHA player packages. 8. Display MAHA banners / signs. 9. Individual player introductions before the championship game. 10. Hotel Rooms available that are sufficient to meet the number of teams entered. 11. Locker rooms for each team and the referees. All rooms with showers. 12. Host to provide Apparel & Photo vendors. They must use the approved MAHA logo. Note: MAHA will supply pins, champions hats, trophies, medals and referees. OPTIONAL ITEMS: 1. National Anthem played before the first game of each day. 2. National Anthem played before the championship game. 3. Referees introduced before each game. 4. Team name posted at their respective bench area. 5. A welcome sign at the arena entrance. 6. Teams separated when leaving the ice after each game. 7. Personnel for crowd control.
