In the event the Player quits and/or leaves the Club anytime during the season, from the first regular season game and or beyond, there will be no League and or Club Refund. Any violation of the League and/or Club Policies could result in suspension, loss of playing privileges and/or dismissal from Club, with or without any Refund at the discretion of the NOJHL Commissioner POWASSAN VOODOOS Player Agreement Standard Player Agreement

3 P a g e 3 THIS AGREEMENT MADE THIS.. DAY OF AMONG: POWASSAN VOODOOS (Hereinafter called the Club)OF THE FIRST PART AND (Hereinafter called the Player ) AND (Hereinafter called the Parents ) WHEREAS the Club is a registered Junior A Hockey Club playing in the Northern Ontario Junior Hockey League and operating out of the city/town of Powassan in the Province of Ontario Purpose The purpose of this agreement is to provide an outline and guide lines for all Players that play for the Powassan Voodoos Hockey Club as to what is allowed and expected with the participation within the NOJHL and the Powassan Voodoos Hockey Club. This agreement will be enforceable by the Commissioner of the NOJHL and all sanctions and discipline rulings are not appealable to the NOHA, OHF or Hockey Canada. This agreement is set out in the following section: NOJHL Player s League Fee: $1, HST =$1, Showcase Fee: $ HST =$ The player will pay this amount $1, to the Club who will then transfer this amount to the League Office. Equipment Game and practice jerseys and socks Every player will receive an a equipment bag to carry their hockey equipment The equipment mentioned above will remain the property of the Powassan Voodoos Player s Club Fee: $2, HST = $3, This amount does not include the NOJHL Player s League Fee

4 P a g e 4 Player s Club Fees include: games (average 2-3 per week) 2. Average of 3 practices per week 3. Private home dressing room with stalls and equipment storage 4. Players lounge 5. Shower towel, shampoo and liquid soap supplied 6. Transportation to and from all away games by highway coach 7. Road trip meals and hotel lodging (2 players per room) 8. Voodoos winter traveling coat 9. Warm up gear (top and shorts) and track suits 10. Free admission to all North Bay Battalion home games 11. Player s in their final year due to age (overage), at season s end will be presented with either their home or away jersey Billet Fees: $ per month paid by cheque/cash on the first day of camp and every month on the same day payable to the billet family (billet coordinator will collect cheques). If your cheque is returned NSF or failure to pay on time you will be suspended until the problem resolved. Billet Fees Refund Formula ($ / days of month = refund) House Rules 1. Keep your room tidy and put your dirty clothes in laundry basket provided in your room. 2. Keep your showers SHORT. Respect others in the home who have to shower 3. Assist in chores around the house if asked to do so.( e.g. put out the garbage, shovel snow, help with dishes and any reasonable request) 4. Serve as a positive role model for younger children in the billet home and in the community. 5. Team curfews must be respected. They will not lie for you. They will turn you in 6. No female companions in the billet home without permission. 7. No alcohol to be consumed in the billet home without permission. 8. No smoking/tobacco chewing or drugs allowed in the billet home 9. If Billets aren't home, no other players or guests are allowed in the billet home. 10. Respect the Billet's rules at all times!!

5 P a g e 5 Billet House Courtesies 1. When you are going to be late coming in from road trips, practices etc., make every effort to be as quiet as possible. 2. When you are going to absent/late for dinner, please call billets, so they can make alternate arrangements.( Not 10 minutes before dinner) 3. Make sure when you re using the laundry room, washroom, or kitchen facilities, you clean up after yourselves. 4. Be aware of the billet family, and their rules, dinner times, rest times and schedules. Community Service: 1) Community events and fundraisers i.e.: School Visits Remembrance Day 2) Minor Hockey Practices 3) Hockey Schools: Summer/March Break/Christmas Break (optional) CLUB code of conduct: 1) No Drugs, alcohol or performance enhancement drug will be permitted. 2) No bullying 3) No profound Language 4) No Sexual Harassment Payment Plans: Option A: Full payment of the NOJHL Player s League Fee, Showcase Fee and Player s Club Fee upon signing both agreements. Option B: Player s Club Fee payable by first league game September 09/2016 Option C: League and player fees can be paid by a jersey sponsor ($2, for home jersey or $2, for away jersey or $4, for both home and away + HST) If the team supplies these jersey sponsors fees are NOT refundable to either the sponsor and or the player.

6 P a g e 6 Dressing Room Rules: 1) 2 alternating players per week will be responsible for cleaning the dressing room chosen by the captain on Mondays 2) No loitering around in the dressing room 3) Dressing room will be open 2.5 hours before game time and 1 hour before practice 4) No Voodoos Jersey is to ever touch the floor 5) Voodoos logo in the dressing room is never to be walked on 6) Only players and staff are to be in the dressing room 7) No video or photography is allowed in the dressing room 8) Cell phones etc not allowed in shower or dressing room 9) Cell phones etc will be kept by the coach in a cell phone rack in the players lounge. Curfew Call: 1) 1 Night before a game day 10pm 2) Regular night 11pm (unless approved by coaching staff) 3) Random curfew calls will be conducted by the coaching staff Player Discipline: The parties confirm, acknowledge and agree that any violation of the terms of this Agreement will constitute a breach of the fundamental principles of the Northern Ontario Junior A Hockey League and/or the Club. A violation of any of the provisions of this Agreement, may result in the immediate suspension of the Player from participating in Club and/or Northern Ontario Junior A Hockey League activities, including, but not limited to, practices and/or games, for a period of not greater than twelve (12) months as determined by the NOJHL Commissioner and /or the Club Management or if constituted, the committee or individual(s) empowered by the Commissioner and the Club to oversee such matters. All decisions are final and may not be appealed to the NOHA, the OHF or Hockey Canada.
