3 Job Safety Analysis ELITE SHOPFITTING AUSTRALIA PTY. LTD. 1.0 DEFINITIONS SWMS / JSA Safe Work Method Statement / Job Safety Analysis: means a statement that: describes how work is to be carried out, and identifies the work activities assessed as having safety risks, and identifies the safety risks, and describes the control measures that will be applied to the work activities, and includes a desciption of the equipment used in the work, the standards or codes to be complied with, the qualifications of the personnel doing the work and the training required to do the work. Consultation: - A conference at which advice is given or views are exchanged, this means the sharing of relevant information about HSE and welfare with employees; that employees be given the opportunity to express their views and to contribute in a timely fashion to the resolution of HSE and welfare issues at their place of work; and, that the views of employees are valued and taken into account. SDS: A Safety Data Sheet tells you: - the ingredients of a product the health effects of the product and first aid instructions precautions to follow when you use the product, and safe handling and storage information. Day labour: Day labour is labour done where the worker is hired and paid one day at a time. There will be no certainty that more work will be available in the future. Day labour is a form of contingent work. Competent person: Is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees and who has authorization to take prompt corrective measures to eliminate them. Tagout: Placing a tagout device on a plant, equipment or energy isolating device, under an established procedure, to indicate that the plant, equipment or energy isolating device and what it controls can t be operated until the tagout device is removed. Lockout: Placing a lockout device on a plant, equipment or energy isolating device, under an established procedure, to ensure the plant, equipment or energy isolating device and the equipment it controls can t be operated until the lockout device is removed. INDUCTION (the following definitions may vary in some jurisdictions) 1. Work Activity Induction: must include at least the following health and safety topics (Must be carried out by Sub-contractor): common hazards, risks and control measures involved in carrying out the work activity participation in the hazard identification, risk assessment and control process relevant HSE legislative responsibilities, codes of practice and their application to the work activity an industry sector overview of HSE work activity performance using data such as workers compensation/injury records, lost time etc. to identify high risk activities or hazards work methods to be used including control measures being implemented to prevent injury correct use, handling, storage and transport of plant (including tools, equipment and personal protective equipment) in accordance with the manufacturer s recommendations correct use, handling, storage and transport of materials and hazardous substances, including the provision and use of Safety Data Sheets electrical safety 3. Site Induction: must include at least the following health and safety topics (usually carried out by Principal Contractor and/or Client): any site specific hazards and risk control measures involved in carrying out the work as identified by the risk assessment process regulatory requirements or codes of practice relevant to any site specific hazards site orientation including location of safe access, amenities, first aid site specific safety rules or procedures including notification of changes to the work site accident, emergency and evacuation procedures and associated equipment on site. 5 2.0 INTRODUCTION Objectives of the Health Safety Environment Management Plan The aim of the Health Safety Environment Management Plan is primarily to eliminate all accidents in the workplace. It is recognised that the Management Plan cannot achieve this on its own, as it requires support, commitment and participation by all personnel in the workplace. Elite Shopfitting Pty Ltd therefore expects contractors, subcontractors suppliers and its own workforce to work together to develop and implement appropriate safe working procedures. Given the Objective of this Health Safety Environment Management Plan is the elimination of safe systems of work to ensure reduction as far as practicable, of incidents and accidents, it is therefore part of the business of Elite Shopfitting to ensure the appropriate welfare of all personnel in the workplace. Management HSE Responsibilities The HSE responsibilities relevant to each level of management are as follows: DIRECTORS The directors are responsible for the review and implementation of the Company s Health Safety Environment policies and procedures. This includes: A commitment to health safety and management including support to Project/Site managers in the management of the workplace. Review of Health Safety Environment targets and monitoring of changes to Health Safety Environment requirements. Review of Health Safety Environment reports submitted by Project/Site managers and/or appropriate delegates. Review major accident reports with the Project/Site mangers to ensure all actions have been taken to prevent reoccurrence and that appropriate prevention measures have been implemented. Oversee the performance of the Project/Site Managers and their staff to ensure that they fulfil their allocated Health Safety Environment requirements. The directors may wish to have an appointed delegate to assist in the implementation of the above responsibilities. Assist in the monitoring, resolving and preventing of significant HSE issues (eg fatality, serious injury, incident, non-compliance) as serious incidents or injuries occur. Elite Shopfitting Directors will visit sites on a regular basis to openly discuss HSE issues. HSE Site specific issues are to be discussed and documented in Site Team Meetings. The Directors or their delegate (HSE Manager) shall implement regular HSE Management review meetings dealing with: Effectiveness of the Health Safety Environment Plan. Review of work Place Safety Management. Implementation of accident prevention programs Any changes to safety standards, legislation, WorkCover requirements etc. Major safety issues, non-conformances etc. Potential improvements in the Health Safety Environment management system. 6 The Safety Management Review Meetings shall be minuted as follows: Date, time and place of meeting. Position, title and name of participants. Agenda. Action taken, responsible parties and target dates. DESIGNATED COMPANY HEALTH SAFETY AND ENVIRONMENTAL MANAGER the company HSE Manager shall have the following responsibilities: Establish, implement and maintain the HSE management system (HSEMS) is accordance with the Standards, AS/NZS 4801 Assist management in the implementation and maintenance of workplace safety systems. Assist site safety representatives in the setting up of the site safety management plans. Report directly to the Directors and Project Managers regarding HSE performance against the project activities, and injury or incident that occur Assist in any required investigations following a serious injury of incident, including near miss incidents Liaise with the area WorkCover Inspector regarding relevant site related issues Conduct internal audits throughout the project life to determine that the HSEMS is being implemented on site Assist in the monitoring, resolving and preventing of significant HSE issues (eg fatality, serious injury, incident, non-compliance) as serious incidents or injuries occur Organizational wide HSE issues are identified and captured through regular site visits, incident investigations and reporting of non-conformances. Lessons learnt are detailed to all employees and subcontractors through the issuing of companywide alerts. The Directors or their delegate (HSE Manager) shall implement regular HSE Management review meetings dealing with: Effectiveness of the Health Safety Environment Plan. Review of work Place Safety Management. Implementation of accident prevention programs Any changes to safety standards, legislation, WorkCover requirements etc. Major safety issues, non-conformances etc. Potential improvements in the Health Safety Environment management system. PROJECT MANAGERS the responsibilities of the Project Managers shall include the following: Implement HSE matters in construction design and planning. Elite Shopfitting Page 6 03/09/15

7 Ensure that subcontractor tender packages incorporate all relevant Safe Work Method Statements. In conjunction with the letting of the subcontract tender packages, to ensure that subcontractors have made full allowance for performing works in accordance with correct Health Safety Environment procedures. Assist in the monitoring, resolving and preventing of significant HSE issues (eg fatality, serious injury, incident, non-compliance) as serious incidents or injuries occur. In conjunction with the HSE Manager, and behalf of the project team to set up the site safety plan. Elite Shopfitting Project Managers will visit sites on a regular basis to openly discuss HSE issues. HSE Site specific issues are to be discussed and documented in Site Team Meetings. SITE MANAGER/FOREMAN the responsibilities of the Site Managers/Foremen shall be: Work in Conjunction with the Project Manager in setting up the site safety plan. Monitor full compliance with the site induction format. Ensure all Safe Work Method Statements have been received prior to the commencement of subcontractors and that they are both accurate and relevant. Maintain the management of safety on site, including the set up of regular site safety inspections by the elected HSE committee Be a member of the sites HSE Safety Committee as a Management Representative Consistently consult with the elected Health and Safety Representative to ensure persons are aware of forth coming activities and changes to work activities and conditions Ensure the Site HSE Committee inspects the site weekly and records are taken record findings and subcontractor ratification. Ensure a induction process is established for the project to ensure all persons are site inducted prior to commencing work on site Report directly to the Project Manager. Assist in the monitoring, resolving and preventing of significant HSE issues (eg fatality, serious injury, incident, non-compliance) as serious incidents or injuries occur SITE SAFETY REPRESENTATIVE Where no safety officer has been appointed these responsibilities shall be the site supervisor s responsibilities. To manage compliance with HSE legislation, regulations, codes and standards; To ensure that hazards and risks to health and safety are identified and that adequate risk controls are implemented; To ensure that safe working rules practices and procedures are implemented and adhered to; To acquire and disseminate HSE information; Review work method statements and job safety analysis issued by contractors on site; To ensure that all plant and equipment brought on site is in a safe condition, and that it has proper guards and safety devices in place and evidence of regular maintenance. To ensure that a regular programme of maintenance is established and adhered to; Elite Shopfitting Page 7 03/09/15

8 Through regular workplace and plant inspections, identify potential problems and hazards. When necessary, arrange assessment of possible hazards and institute control measures; To quarantine unsafe work areas, materials, plant and equipment; To encourage reporting of all injuries, incidents, accidents, dangerous occurrences and near misses. To investigate these and to initiate corrective (preventative) actions; To support and assist with the rehabilitation of employees who have been injured at work, by encouraging their early return to normality through work based rehabilitation programs; To assess and determine first aid needs and to appoint trained first aid officers in accordance with the Code of Practice for First Aid; To develop and implement emergency procedures. To ensure that suitably trained persons are appointed and trained as Site Supervisor and Safety Officer HSE Project Design Review and Risk Management Where Elite Shopfitting is involved in the design or has input into the design, the Elite Shopping Design Managers along with the Design Consultant Team are to ensure risk assessments are undertaken at the design stage to identify, assess and manage HSE buildability issues that may arise during construction. This assessment will be carried out at various stages of design to identify risks, assess risks, and determine the likelihood and consequence of the adverse risk. Where Elite Shopfitting has no input into the design, design related buildability hazards are identified, assessed and managed pre-construction phase. All medium to high level risks will have mitigation strategies developed. Safety in Design will be an Agenda Item at the Design Meetings. It is also a requirement that consultants undertake their own in house safety in design review. Design Change Design changes during the construction phase shall be reviewed and assessed to determine risks to health and safety. This review aims to identify hazards in order to be eliminated or to reduce the risk of harm to its lowest possible level following the Hierarchy of Control including elimination, substitution, engineered controls, administrative controls and personal protective equipment. During this review the trade responsible to complete the works will be required to provide work methods for review by Elite Shopfitting, prior to the work being commenced. Elite Shopfitting will review the work procedures; consider the outcome of the changes and effects on the site or other construction activities that have commenced. Existing drawings will be stamped superseded, new drawings will be reviewed to highlight the change, and issue to subcontractors concerned with completing the works. 9 Subcontractor Selection HSE requirements for selection of subcontractors: Interview the shortlisted tenderers in order to ensure that the relevant subcontractors are fully aware of the following obligations: Compliance with all statutory HSE requirements. Preparation of submission of Safe Work Method Statements (SWMS). Preparation and submission of plant and equipment verification records, such as log books and the like. Preparation and submission of safety data sheets (SDS). The sub-contractor must supply Elite Shopfitting Pty Ltd with evidence that all employees of the sub-contractor have been inducted into Company works procedures and all relevant SWMS. o Selecting subcontractors for each project shall take due regard of an evaluation of the perceived commitment of the relevant subcontractor to comply with the Company s HSE requirements. The selection criteria is based upon: Reputation Recommendation Availability Ability to supply Price Terms and conditions Insurance provisions Compatibility with client Consultation Elite Shopfitting Pty Ltd will endeavour to consult directly with its employees, or through the elected Health and Safety Representatives, or independent Contractors. All employees will be encouraged to express their views on all health and safety matters that directly affect them at their workplace. Elite Shopfitting Pty Ltd shall take those views in to account. Management / Health & Safety Representatives / Safety Committees will meet as required to review or discuss all aspects of site work or other issues relative to HSE. Minutes or all official notices relative to on-site HSE issues will be displayed on site notice boards to assist in the consultative process. Elite Shopfitting Page 9 03/09/15

10 Elite Shopfitting Pty Ltd recognises that some external safety related training may be required by some employees to perform specific tasks in the course of their employment. Management recognises this requirement and will allocate sufficient time off with pay to undertake that training. Company Auditing The Directors or their delegate, with the HSE Manager, shall conduct audits on the effectiveness of the HSE Management Systems, and shall produce reports accordingly. Persons conducting the audit are to have relevant auditing training. By scheduling and completing HSE MS Audits the Auditor can verify whether activities and processes comply with procedures and work instructions, and have been properly implemented and maintained. Following the review, and in the event of any identified deficiencies, relevant Action Plans shall be developed by the HSE Manager and issued to the site team for close out. The actions taken will be assessed and signed off as complete by the projects Project Manager and Site Manager. Audits will be scheduled quarterly, but may be conducted more frequently or additional audits may be scheduled in response to non-conformances or concerns. The Health Safety Environment Audit Form can be found within the HSE management System. This form can be used by any party undertaking an audit of the establishment of our safety systems on any project. In addition to company audits, the project may be audited by the client, or their representative as requested. Amendments to HSE Management Plan The Health Safety Environment Management Plan is available for review by all persons working on site on a controlled basis. It is emphasised that the Health Safety Environment Management Plan specific to Tradelink is considered to be a living document, which shall require constant review and updating, particularly as a consequence of: Amended legislative requirements Improved Health Safety Environment knowledge General perceived improvements in the method of implementation of the Health Safety Environment policy. To ensure all companywide required amendments are issued in a controlled fashion, amendments shall only be prepared and issued as an Opportunity For improvement, by following the Elite Shopfitting OFI Procedure; Elite Shopfitting Page 10 03/09/15

11 Identify HSE issues, procedural problems, internal audit findings, training or safety issues, etc, as areas where improvements can be made or potentially prevented. The recommended amendments are to be brought forward to the Directors for consideration by the HSE officer and/or Project Managers, along with the reasons for change. Upon approval by the Directors of the proposed amendments, one master copy of relevant amended pages are to be prepared, and held in a controlled fashion at Head Office. The amended pages are to clearly denote the following : - Issue number. - Issue date. The amended pages are then to be used by either the Directors, the HSE Officer or a nominated delegate, with the amendments issued under a covering memorandum clearly detailing the following: - A summary of the agreed amendments - A listing of all pages subject to amendment - A listing of all new pages, including the relevant issue number and Issue date. Immediately upon receipt, all amended pages are to be inserted into each controlled copy of the Health Safety Environment Management Plan, and superseded pages destroyed forthwith. Record Keeping as detailed in the Elite Shopfitting Procedures for Document and Records Control. Identify HSE records which are required to be collected, filed and disposed of these records are listed in the Records Register. The responsible person, identified in the Records Register, prepares and collects the relevant records and ensures that the records are legible and identifiable. Accessing the Management Review, contracts and personnel records is restricted to Directors, and to staff authorised by the Directors. Accessing all other records shall be unrestricted. Transfer records into archive boxes and label with the details of the records enclosed and the time period involved. Store the archive boxes as listed in the Records Register. After the expiry of the retention period, destroy the records as detailed in the Records Register as directed by a Director. Back ups are maintained by the I.T. Department. The back ups are completed on a daily, weekly and monthly basis. Retain the records for a minimum period as defined in the Records Register. 3.0 SAFETY POLICY ELITE SHOPFITTING is responsible for: Completing the Safety Plan and providing a copy to the Project Manager before work commences on site. Maintaining an up to date version of the Safety Plan. All obsolete pages will be destroyed. Elite Shopfitting Page 11 03/09/15

12 Providing an updated copy to the Project Manager whenever changes occur. Reviewing the Safety Plan when there is a change and/or event affecting our work activity and at intervals of not more than one month to ensure it is up to date. At ELITE SHOPFITTING our Health, Safety Environment Policy is based on a belief that the well-being of people employed at work, or people affected by our work, is a major priority and must be considered during all work performed on our behalf. People are our most important asset and their health and safety is our greatest responsibility. The public shall be given equal priority to that of our employees. The objectives of our Safety Policy are: To achieve an accident free workplace. To make health & safety an integral part of every managerial and supervisory position. To consult with our workers and the Principal contractor and to ensure health & safety is considered in all planning and work activities. To involve our employees in the decision making processes through regular communication, consultation and training. To provide a continuous program of education and learning to ensure that our employees work in the safest possible manner. To identify and control all potential hazards in the workplace through hazard identification and risk analysis. To ensure all potential accident/incidents are controlled and prevented. To provide effective injury management and rehabilitation for all employees. The success of our health & safety management is dependent on: Pro-active planning of all work activities with due consideration given to implementing HSE controls that are suitable to each given situation. Understanding the total work process and associated HSE risks. Ensuring the work team is totally committed to achieving our objectives. 13 Tag Out Process/Lock Out Process Purpose This establishes Elite Shopfitting s policy for protecting employees who must do service or maintenance on machines or equipment (electrical and non electrical) and who could be injured by an unexpected start-up or release. Service or maintenance includes erecting, installing, constructing, repairing, adjusting, inspecting, unjamming, setting up, trouble-shooting, testing, cleaning, and dismantling machines, equipment or processes. This policy will ensure that machinery or equipment is stopped, isolated from all hazardous energy sources, and properly locked or tagged out. Elite Shopfitting is responsible for implementing and enforcing this policy. All employees must comply with this policy. Supervisors must enforce the use of lockout and tagout devices when employees do service or maintenance work and may be exposed to hazardous energy. Employees who do service and maintenance work must follow the lockout/tagout procedures described in this policy. Employees who work in areas where lockout/tagout procedures are used must understand the purpose of the procedures and are prohibited from attempting to restart machines or equipment that are locked or tagged out. 15 Roles and Responsibilities Defined ELITE SHOPFITTING will provide the following key personnel on site. Their roles and responsibilities regarding safety on site are outlined below. Project Manager Is responsible for safety on the project and duties include: implementing the company Health Safety Environment procedures; using the principles of the Hierarchy of Controls in all design, fabrication and construct activities to minimise the risk to all personnel in the workplace; carrying out a design review with the Principal Contractor s project team to assist in the identification of further risk reduction control measures. participating in the planning and design stages of trade activities; stimulating a high level of safety awareness at all times; identifying safety training needs; leading by example; ensuring safe equipment and plant is provided and maintained; insisting on correct and safe work practices at all times; assisting in the identification and preparation of safe work procedures; reviewing safety reports and inspections and initiating rectification where necessary; participating in accident/incident investigations; participating in safety meetings and programs; monitoring compliance with safe work methods (controls); SITE SUPERVISOR Is responsible for safety on the project and duties include: implementing the company Health Safety Environment procedures; observing all HSE requirements and Statutory rules and regulations; ensuring that all works are conducted in a manner safe and without risk to employees health and safety; planning to do all work safely; providing advise and assistance on HSE to all employees; participating in the planning and design stages of trade activities; ensuring current HSE and other relevant legislative requirements are met in the workplace; identifying HSE training programs in advance and allow for employee/s identified as requiring training to attend the training; actioning safety reports and carrying out workplace inspections; preparing and participating in safety meetings and safety programs; facilitating the preparation of Safe Work Method Statements and Job Safety Analysis for the trade; insisting and ensuring on safe work practices at all times; investigating hazard reports and ensuring that corrective actions are undertaken; conducting project inductions, toolbox talks and daily team briefings; participating in accident/incident investigations; leading by example and promoting HSE at every opportunity; supervising and ensuring compliance with safe work procedures; providing suitable employment to assist rehabilitation initiatives; stimulating a high level of safety awareness at all times; Elite Shopfitting Page 15 03/09/15

16 SITE SAFETY COORDINATOR Is responsible for safety on the project and duties include: assisting the Site Supervisor to develop and implement the Health Safety and Environment procedures communicating safety performance to the Site Manager; providing advice and assistance on HSE to all employees; participating in the planning and design stages of trade activities; monitoring HSE legislative requirements for the trade package; monitoring compliance with safe work procedures; co-ordinating rehabilitation / return to work for injured employees; reviewing safety reports and inspections; preparing and participating in safety meetings and programs; facilitating Tool Box Talks on a regular basis; insisting on correct and safe practices at all times; preparing and conducting project safety inductions; investigating and developing new HSE initiatives for the trade; conducting accident/incident investigations; leading by example and promoting HSE at every opportunity; stimulating a high level of safety awareness at all times; communicating with the HSE Site Manager on matters relating to health and safety; facilitating the maintenance of all records as required under this SubbyPack; participating in regular workplace inspections and ensure that any improvements resulting from such an inspection are actioned in the required time frame. HSE Manager Is responsible for safety on the project and duties include: Supports, leads and directs the internal and external resources in order to support the HSE programs by providing advice and guidance to the Company employees. Continually maintains an appropriate level of awareness, knowledge and preparedness across the company to create a culture that prioritizes effective HSE. Review internal HSE policies and practices to ensure they are current and appropriate, as well as conducting hazard and risk assessments and change existing, or develop new recommendations from these assessments. Address regulatory agency compliance issues, third party safety & liability consultants and subcontractor representatives. Provide regular written reports, as well as other production-related results and updates to the Company employees. Act as a support agent to the project and construction teams in investigation of all project and construction incidents (Recordable Incidents, Near Miss and First Aid) and ensure proper records are completed and submitted. Ensure any incidents are communicated throughout the Company and are incorporated into future learning material. Devise, supervise, and coordinate training programs or media which will increase proficiency in safe practices and promote HSE consciousness. Schedule and hold safety meetings across Company locations to review HSE performance and policies. Conduct site visits/audits Elite Shopfitting Page 16 03/09/15

17 5.0 SITE MANAGEMENT Project will strive for Zero Lost Time Injuries All persons to be site inducted prior to commencing work All persons to work on site to have completed or be booked into Industry Induction Report all incidents immediately as per Serious incident Response Checklist HSE Manager to ensure all reportable incidents or serious injuries are reported to WorkCover within the required 48 hour time frame Safety Walks are to be performed weekly as a minimum and items identified must be closed out Separation by placement of hoarding, fencing, barricading will be maintained through the life of the project to ensure a physical separation between construction activities and members of the public All programmed target dates as agreed to with the client are to be met Site Establishment The project team is to follow the requirements of the Procedure for Site Establishment Prior to commencement of works on site, it will be the Project Manager s, Site Manager s and Safety representative s responsibility to ensure that all HSE controls have been prepared. The Site Manager, Foreman and HSE representative, with assistance from the HSE Manger, shall work together in actioning the following tasks: Ensuring that the site establishment trades have prepared and submitted Safe Works Methods Statements relating to their temporaries installations (i.e. electrical connection, drainage and hydraulics connections). The preparation of a Site Plan. Concurrent with all of the above, the Site Manager / Foreman, HSE Representative, shall in all cases instigate a thorough inspection of the site to identify potentially hazardous areas, and implement appropriate measure to make safe Site Safety Plan The Project Manager, Site Manager / Foreman and the HSE Officer, or his delegate, has the responsibility for the development of this Site Safety Plan. The Site Safety Plan will include the following items: Elite Shopfitting HSE Policy, Site boundary and amenities layout. This layout is to incorporate the following: - Fences, hoardings and gantries. - Amenities including first aid facilities. - Temporary services routes - Location of construction and worker access roads - Traffic Management Plan for work being undertaken. - Public access (if applicable). - Emergency procedure marshalling area. - First aid stations (if other than first aid amenity). Emergency procedures: - Evacuation Plan and accompanying instruction of duties of HSE Elite Shopfitting Page 17 03/09/15

18 Team. - Emergency services contact numbers. Medical Treatment: - First aid instruction. - Hospitals / doctors / medical centres / contact numbers. Site HSE Committee - Role and company of trade representative details. - Safety walk and meeting time details. Note: The above four dot point items are to be on display in the Site Office, First Aid amenity, and where applicable in the common tea room area. Copies are contained within the site specific copy of the Elite Shopfitting HSE MS. Site Safety Actions and Subcontractors Requirements Prior to any work being commenced on site all subcontractors will be required to submit their site specific Safe Work Method Statements specific to their scope of works to Elite Shopfitting. Subcontractors are required to provide evidence of the certificate of currency for both Public Liability and Workers Compensation insurances. Each subcontractor is required to ensure all of their workers have been inducted into their site specific Safe Work Method Statements by their company supervisor or foreman. All workers will be site inducted by Elite before commencing work on site. Public and Visitor Safety It is the responsibility of the Project Manager and the project team to ensure that prior to commencement of works on site, detailed plans and adequate provision have been made for both public and visitor safety. Control measures shall be put in place to address the following: Unauthorised access to the site. Site fencing. External loading / unloading arrangements. Hoardings and gantries. Construction work performed on footpaths (i.e. external to the project). Internal public / pedestrian access (if applicable). Overhead cranes of loads. Separation of construction zones from completed operational areas. Courier deliveries. Establish Visitor Sign In Register. Elite Shopfitting is fully aware our scope of works are within an existing and trading shopping centre, and the use of hoardings, barricading and signage is important in ensure safe access between the constructions works and pedestrians. 19 Emergency Preparedness and Response The Elite Shopfitting project team must ensure all site specific potential emergencies are identified, risks evaluated and a site specific emergency plan has been developed. The emergency plan must include the Elite Shopfitting arrangements, systems, strategies and procedures relating to the response and management of emergencies. The plan must be regularly reviewed for its suitability. An emergency response procedure must be developed to: Identify the responsibilities of the site team in response to an emergency situation Create an evacuation plan Identify emergency contact phone list All designated emergency personnel to receive training and practice in emergency procedures. All emergency response arrangements must be communicated to all personnel attending induction. All Emergency systems must be monitored for their adequacy and effectiveness. Site specific emergency response exercises shall be planned and carried out by the site team where necessary to determine the effectiveness of the emergency response procedures. The Emergency Evacuation Observers Checklist must be used during an emergency exercise to document and investigate the outcomes and observations of the drill. Site HSE Committee Elite Shopfitting Pty Ltd will establish, on site a safety committee to discuss on site health safety and environment requirements and problems. This committee may consist of any of the following: Project Manager, Foreman/Supervisor, Safety Coordinator and nominated workplace safety representatives. These meetings will be held on a weekly basis to inspect the workplace, identify any issues for rectification. The Committee will also have the discretionary power to convene a meeting of the HSE Committee whenever circumstances may dictate, e.g. safety issues, serious accidents, etc. The function of the Committee is as follows: Discuss and review the Site Safety Plan. Review Risk Assessment and Safe Work Method Statements in critical areas. Assist management through the identification unsafe acts and conditions by offering solutions. Assisting in health and safety disputes. Participate in regular safety walk inspections. Attend meetings following the safety walk inspections and minute accordingly. The safety walk meeting minutes are to be posted on the site notice board for review and notice by all. A notice displaying the members of the site HSE Committee shall be placed on display in the Site Office, First Aid room, and workers amenity area. The Elite Shopfitting HSE Manager will regularly attend site to discuss HSE issues and inspect the site. Copies are contained within the site specific copy of the Elite Shopfitting HSE MS. 20 HSE Induction The Site Manager / Foreman shall be responsible for implementing the Company s HSE Site Induction to the project. Prior to their commencement of work on site, it is the Company s requirement that all subcontractors and employees participate in the Company induction session delivered by the nominated person. Prior to inducting individual employees, it shall be necessary for the employer of each employee to have presented (and had approved) the following documents relating to the respective scope of works: Safe Work Method Procedures. Risk Assessment. Evidence must be provided that all sub-contractors have been inducted into their own procedures. (Further details regarding Safe Work Method Statements are described in Section 5 of this Plan.) In the event of failure, non-compliance or inadequate description of Safe Work Method Statements, the Site Manager / Foreman and / or his delegate are to request the resubmission of the Safe Work Method Statements to an accepted standard. This may require working with the subcontractor in developing a point form Safe Work Method Statements. The following items are to be addressed during the site induction: Qualifications of those operating plant on site. Trenching operations. Working at heights Demolition activities (if any). Elevated work platform requirements. Lift and shafts works policies. Access ways. Emergency procedures. Hazard reporting procedures. First aid facilities. Incident / accident reporting. HSE Committee Safety Walks and Meetings. Electrical Safety requirements (i.e. tool, leads, etc). Use of oxy/acetylene equipment. Wearing of hard hats, hearing protection, safety boots, etc. Explosive power tools. Copies are contained within the site specific copy of the HSE MS. On-site HSE Management As a standard procedure, each site shall have on display the following information: If you are injured poster Statutory signage where applicable (i.e. hard hats, Wearing of safety vests, safety boots, etc). Equipment hazards (i.e. operation of plant and equipment). Elite Shopfitting Page 20 03/09/15

21 WorkCover information and Alert Bulletins. HSE Committee details. Site safety rules. Safe Delivery and Storage of Materials and Equipment It will be the Site Manager s / Foreman s responsibility for the planning of safe delivery and storage of materials and equipment on site. The following issues are to be considered in their implementation. Delivery Access to the site and appropriate signage. Maintaining access roads. Unloading methods (i.e. carnage, the use of gantries, etc.). Storage Safe storage of dangerous goods, chemicals, etc. To be located away from pedestrian access ways and site personnel. Accessibility in relation to manual handling. Plant & Equipment Prior to commencing work on site the subcontractor will be required to complete a Plant & equipment register and a Plant & Equipment induction. This process ensures Elite Shopfitting is aware of all equipment on site, and that persons scheduled to operate the equipment have the required certificate to operate the item of plant. Subcontractors are to provide proof the Plant and equipment brought to site either owned or hired is regularly serviced and the operator assigned to the machine is properly trained and instructed to operate the machine in a safe manner. Purchasing Elite Shopfitting must ensure that suitable consideration is given when purchasing equipment, materials or substances which may have an adverse impact on persons health or safety. The following needs to be considered to determine if an item is hazardous: Could a significant injury or incident occur in the use, storage or transport of the item? Are there any specifications which are required to ensure safe operation? Does the item need to comply to legislation, code of practice and/or Australian Standards. Items such as plant and equipment require formal risk assessment to identify any hazards posed by the item and ways as eliminate/reduce hazard. All Hazardous materials and dangerous goods are to be purchased with a current copy of SDS, to be referred to by workers using the hazardous materials or dangerous goods. All SDS are to be stored on site. All purchased PPE must comply with relevant Australian Standards and be worn as described in relevant SDS and SWMS. Elite Shopfitting Page 21 03/09/15

22 The most effective method of reducing the risk of a hazard in the workplace is through the process of eliminating the hazard from the workplace and these risks are to be considered through the purchasing process. Legal Requirements It is the responsibility of the Elite Shopfitting employees to promote and ensure compliance with all applicable laws and regulations relevant to their area of employment. Employees are advised of and have access to all current health and safety legislation, standards, and other documentation relevant to health and safety. The Site Team must ensure all construction activities are in line with the applicable Acts and Regulations and relevant Industry Standards. Elite Shopfitting is responsible to keep the HSE MS up to date against current legal and other industry related regulations and guidelines. The ensure industry knowledge is obtained, Elite Shopfitting will ensure industry membership is maintained with the Master Builders Association in New South Wales. ELITE SHOPFITTING will not commence construction work at a place of work unless: The Principal contractor has provided ELITE SHOPFITTING with a copy of the parts of the workplace HSE management plan that are relevant to the sub-contractor, and ELITE SHOPFITTING has undertaken an assessment of the risks associated with the work to be carried out and provided to the principal contractor a written SWMS/JSA that includes a copy of the assessment of risks, and ELITE SHOPFITTING has completed induction training with respect to Health Safety Environment as specified in the Health Safety Environment management plan for the place of work. ELITE SHOPFITTING will maintain and keep up to date the sub-contractor s SWMS/JSA for a place of work, and must provide the principal contractor with any changes made to the safe work method statement. ELITE SHOPFITTING will identify the potential hazards of the proposed work, assess the risks involved and develop controls to eliminate, or minimise, the risk. The risk management process is to be carried out in consultation with the workers. Identify Hazards: All potential hazards the job will be broken down into activities, which follow the sequence of construction. These activities are provided in a Safe Work Method Statement (SWMS) / Job Safety Analysis (JSA), which is a list of job procedures, and other work related practices. The SWMS/JSA details how the Scope of Work will be carried out. For each of the work activities and associated job steps identified in the SWMS/JSA provided ELITE SHOPFITTING will identify potential hazards. To assist this process resources such as the following will be used: - WorkCover and trade based Codes of Practice and other publications, e.g. safety alerts; Hazard Profiles for specific trade groups; Workplace experience; and Consultation (e.g. Tool Box Talks) with workers experienced in the task to be undertaken. 23 Assess Risks: For each potential workplace hazard identified a Risk Class will be determined by referring to the categories below. The attached Risk Class management tool will be used to determine the requirement for management of the risks identified. 6.0 WORKING AT HEIGHTS How to identify fall hazards You must identify all locations and tasks that could cause injury due to a fall. This includes access to the areas where work is to be carried out. Tasks that need particular attention are those carried out: on any structure or plant being constructed or installed, demolished or dismantled, inspected, tested, repaired or cleaned on a fragile surface (for example, cement sheeting roofs, rusty metal roofs, fibreglass sheeting roofs and skylights) on a potentially unstable surface (for example, areas where there is potential for ground collapse) using equipment to work at the elevated level (for example, when using elevating work platforms or portable ladders) on a sloping or slippery surface where it is difficult for people to maintain their balance (for example, on glazed tiles) near an unprotected open edge (for example, near incomplete stairwells) near a hole, shaft or pit into which a worker could fall (for example, trenches, lift shafts or service pits). Inspect the workplace Walk around the workplace and talk to your workers to find out where work is carried out that could result in falls. A checklist may be useful in this process. Key things to look for include: surfaces: o the stability, fragility or brittleness o the potential to slip, for example where surfaces are wet, polished or glazed o the safe movement of workers where surfaces change o the strength or capability to support loads o the slope of work surfaces, for example, where they exceed 7 degrees. levels where levels change and workers may be exposed to a fall from one level to another structures the stability of temporary or permanent structures the ground the evenness and stability of the ground for safe support of scaffolding or a work platform Elite Shopfitting Page 23 03/09/15

24 the working area whether it is crowded or cluttered entry and exit from the working area edges protection for open edges of floors, working platforms, walkways, walls or roofs holes, openings or excavations which will require guarding hand grip places where hand grip may be lost. In some situations, advice may be needed from technical specialists, such as structural engineers, to check the stability of structures or load bearing capacity. Review available information, including incident records You should check your records of previous injuries and near miss incidents related to falls. Information and advice about fall hazards and risks relevant to particular industries and work activities is also available from regulators, industry associations, unions, technical specialists and safety consultants. How to assess the risk Each Subcontractor will do their own site/risk assessment and will then provide a copy to have the Site or HSE Manager review and make any necessary changes. Creating a site/risk assessment this will help you determine the following: what could happen if a fall did occur and how likely it is to happen how severe a risk is whether any existing control measures are effective what action you should take to control the risk how urgently the action needs to be taken. A risk assessment is unnecessary if you already know the risk and how to control it. When assessing the risks arising from each fall hazard, the following matters should be considered: the design and layout of elevated work areas, including the distance of a potential fall the number and movement of all people at the workplace the proximity of workers to unsafe areas where loads are placed on elevated working areas (for example, loading docks) and where work is to be carried out above people and there is a risk of falling objects the adequacy of inspection and maintenance of plant and equipment (for example, scaffolding) the adequacy of lighting for clear vision weather conditions the presence of rain, wind, extreme heat or cold can cause slippery or unstable conditions the suitability of footwear and clothing for the conditions the suitability and condition of ladders, including where and how they are being used the adequacy of current knowledge and training to perform the task safely (for example, young, new or inexperienced workers may be unfamiliar with a task) the adequacy of procedures for all potential emergency situations.
