19 Hazard Reporting! On IRMS and via LearnLink
20 Hazard Reporting Our ultimate aim is to eliminate or control risks in the workplace before they can cause us harm. Reporting hazards and taking corrective action to eliminate them can prevent serious incidents - and possibly injuries - from occurring in the future. All DECD workers have the capacity to report hazards online. Our target is to have five times the number of hazard reports to the number of injury reports.
21 Hazards : reporting online Hazard a situation or thing that has the potential to cause harm/injury to people, property or the environment.
22 Hazards : reporting online Use this process to record a new Hazard that cannot be controlled immediately by a DECD Procedure, DECD Safe Operating Procedure or site maintenance. It is not appropriate for Human Resource or Performance Management related issues to be logged as a hazard. Where a hazard has been identified by a worker, the worker must resolve the hazard if competent and able to do so safely. If the hazard cannot be resolved immediately, the area must be made safe and others alerted. The Hazard Form should be completed as soon as possible but no later than 12 hours of discovery of the hazard.

26 Hazards terms Hazard Type = Is any mechanism that has caused harm/injury to people, property or the environment. Risk Assessment Inherent Risk = Is the level of risk with no controls in place Risk Assessment Residual Risk = Is the level of risk after controls have been implemented Risk Controls Implemented = Actions taken to eliminate or minimise the risk of harm Corrective action taken = Actions taken to eliminate or minimise the cause/s of a hazard, thus preventing its recurrence
