Qualification and participation in the U18 US National Championships is a key marker in athletic development and truly an accomplishment in and of itself. With that said, this championship event is a great opportunity to display the strength and depth in the Rocky Central Region amongst the other regions in the country. You have worked hard and fought to get here, now it s time to show the US what Rocky Central is all about! 2 Athlete Travel/Arrival: Please book flights in and out of the Denver International Airport. Coaches flying in will pick up vehicles at the Airport to transport athletes and their gear. In order to be picked up at the airport you must provide your flight information here: Information about the airport pick up will be communicated once travel itineraries are set. All RMD athletes should plan to travel to Copper on their own. Lodging and Meal Plan: Women s Team in Central Village and Men s Team in East Village, Copper Mountain Resort Room assignments will be provided at or before the first Rocky Central team meeting either at Central Reservation or through Ian Dunlop and/or Darlene Nolting. The lodging will have kitchens available for cooking and storing food. Meal plan includes Dinner at Double Diamond Restaurant, next door to the Copper Station Base Lodge in East Village, then a voucher to be used for lunch at any Copper Mountain Food Court. Rooms will also be stocked with breakfast food items. Tuning space will be available at a location near the team lodging. Expenses: Total Project Fee is based on participation in either Tech Only or All Events. Beyond this there will be no prorated discounts. Prepayment is required for all USSA projects. If you are flying we suggest that you carry your ski boots on the plane and carry your money with you in case your luggage is lost. Please note that you will be responsible for any overweight or other baggage charges, so pack appropriately and come prepared. Miscellaneous: - All athletes participating in USSA projects must complete some form of concussion screening program/service prior to attendance and supply the contact information for this service. This is a part of project registration in case it is necessary to help a medical physician determine skiing status in case of a possible head trauma resulting in a concussion. This information may be helpful as a tool by a medical physician towards when a return to training and physical activities may begin. Coaches will not diagnose severity of a head trauma as the law dictates a certified medical physician is required to make such diagnosis. - In an emergency, please call Ian Dunlop at (435)

5 Prep Project - U18 US National Championships Rocky Central Training Copper Mountain, CO February 21 st,22 nd, 25 th, 26 th Hello Rocky Central Athlete, Rocky Central is providing training opportunities to help you prepare for the U18 US National Championships. Since the tech events are scheduled right away, this will be our best opportunity to acclimate and come prepared to rip! We will conduct two sessions a day. Early Speed or GS training sessions followed by a Slalom training session. All training will be conducted on the race venues. This training opportunity is in addition to the U18 US Championship event and is not required, however, strongly encouraged. I would like to see everyone attend some portion of the training so if problems/concerns arise, please don t hesitate to contact Ian Dunlop. Athletes must have all U18 Rocky Central Team US Nationals paperwork, registrations and payments completed (see U18 RC Team Selection Announcement) in order to participate in the training. 7 February 2015 Dear Parents, Congratulations on your athlete qualifying for the USSA U18 Alpine National Championships; this achievement represents a significant milestone in their ski- racing career. Participation in this Championship event is an honor that the athletes have earned through hard work and support from coaches and parents. USSA seeks to provide the athletes with an opportunity to compete with their peers from around the country in a safe environment, therefore compliance with the USSA code of conduct is expected, and will be enforced. By signing the Team Agreement below, athletes commit to conducting themselves according to USSA s core values of Team, Loyalty, Integrity, Respect, Perseverance, and Accountability. USSA staff expects all athletes to comport themselves in a sportsmanlike manner, and athletes will be held responsible for their actions while attending or participating in all USSA activities. Failure to comply with the team rules, including any of the points below, will lead to disciplinary action being taken by the Western Region director and regional head coach: Not being in the assigned lodging at curfew Possession of illegal drugs Possession of alcohol Transgression of rules may lead to athletes being sent home immediately at the parents expense. 8 USSA Regional Alpine Development Department Team Agreement Team Rules Members of USSA teams attending competitions on the national quota, traveling with or as part of a competition trip organized by the USSA Regional Development Staff, or attending any training camp or project as part of a USSA Group are required to abide by the USSA Code of Conduct. Additionally, team members agree to: Pay in full the project fee before the first day of arrival of the project, unless a payment plan is worked out at the time that the team is named; Fully participate in the organized accommodations; Attend all team meetings and training activities on time and fully prepared; Observe any curfew established by the coaching staff; Not use or possess illegal drugs; Not illegally use or possess alcohol; Not use or possess alcohol in the team accommodations, the team vehicles, or in the presence of team members who are not of legal age; Reside in the room to which they are assigned and will share responsibility with their roommates for damages, cleaning charges, and rule violations that occur in the room when it is not possible to establish individual responsibility; Observe any additional rules established by the head coach of the project Be filmed and photographed and to have his/her image and voice otherwise recorded in any media by the USSA s official photographer(s), film crew(s), and video crew(s) and by any other entity authorized by USSA and grants to USSA the irrevocable, fully paid up, worldwide right and license to use, and to authorize third parties to use, in all media, Athlete name, likeness, picture, voice, and biographical information for: (1) news and information purposes; (2) promotion of USSA and the specific competitions in which Athlete competes; (3) promotion of USSA through its fundraising mission; and (4) to support USSA s educational and philanthropic efforts through the production of educational and training videos, DVDs and other media. Violations of these rules may result in suspension from the team and in loss of the opportunity to participate in future teams or quota team preparation or competition projects. Agreement I, the undersigned USSA competitor, have reviewed these rules and the USSA Code of Conduct, and agree to abide by it and all applicable rules and procedures during in my participation in any national training or competition project during the current season. I agree that a violation of the rules may result in my immediate suspension from a team, and in the loss of all team services including representation at Team Captains and Jury meetings, training, housing, meals, and transportation. I realize that I will receive no refund for the unused portion of these services. If suspended, I will be prepared to make whatever arrangements are necessary in order to continue to compete in the event and/or return home. I realize that actions that are violations of the law may result in my arrest and that it will be my responsibility in such a case to notify my parents or legal guardians and/or to make such arrangements as may be necessary for my release and return home. This agreement shall be valid for all projects in the training and competition season. Athletes under 18 years of age must have the following section signed by either parents or legal guardians, if applicable. We, the undersigned, parents or legal guardians of the above Athlete, having read and understood the above, agree to allow our child to participate in USSA training or competition teams. We understand and agree that if our child violates these rules and procedures, he/she may be immediately suspended from the USSA project with the loss of all team services and without refund of fees paid. We agree that in any case where our child s actions result in his/her arrest, that sole responsibility for notifying us rests with our child and the local law enforcement officials, and that we, and not USSA or USSA Western Region, are solely responsible for such actions as may be required to secure his/her release and return home. This agreement shall be valid for all projects in the training and competition season. 9 USSA Regional Alpine Development Department Hold Harmless and Indemnity Agreement For and in consideration of the provision of ski racing instruction and training by USSA, I ( athlete ), and if athlete is under the age of 18 years, the parents or legal guardians of athlete, do hereby covenant and agree on behalf of ourselves, to hold harmless, release, defend, and indemnify USSA and any of its employees, or volunteer workers of and from any and all claims arising from athletes participation in USSA Regional events, including, but not limited to racing instruction, racing competition, or traveling to and from races or training camps. We specifically represent that we have read and have understood that this release is intended to serve as a general release of all legal claims against USSA and USSA/Regional Alpine Development projects, is to be construed as broadly as possible in favor of USSA, and releases USSA from all claims, including, but not limited to, those arising from the negligence of USSA itself. 11 Code of Conduct This Code of Conduct is based on common sense principles. Each athlete or staff member shall conduct themselves in such a manner as to bring respect and dignity to the organization and themselves. Basic Principles While this document provides a general outline for expected behavior, it is not practical to list rules for every situation that may arise. However, the following principles should be carefully noted. 1. The safety and welfare of athletes, coaches, Copper Mountain s guests and staff is of primary importance, and all precautions should be taken to insure their safety. 2. Athletes and coaches are to be responsive and respectful to Copper Mountain s guests, staff and property at all times. 3. Athletes and coaches should display the highest level of personal responsibility while at Copper Mountain. 4. Each athlete is expected to conduct him or herself in a sportsman-like manner at all times and is responsible for his or her own actions while at Copper Mountain, on or off the hill, indoors or out. Rules of Conduct 1. Be responsive and courteous to other program participants, coaches, parents, and Copper Mountain s guests and staff. 2. Treat all athletes, coaches and Copper Mountain s guests and staff with respect. 3. Respect Copper Mountain equipment and facilities. Vandalism to public or personal property is punishable by law and appropriate actions will be taken. 4. Each athlete will adhere to the Ski and Snowboard Responsibility and Smart Style Codes. 5. Quiet hours are from 10 pm to 8 am. Noise will be kept to an appropriate level during these hours. 6. The lodging unit will be left free of all debris; all garbage shall be placed in appropriate trash containers. If the unit requires excessive housekeeping additional fees will be applied to the team s final bill. 7. Any damage to the unit or property will be subject to extra fees for maintenance and replacement. This fee will be determined by Copper Mountain s Maintenance and Housekeeping departments and will be added to the team s final bill. Ski and Snowboard Responsibility Code 1. Always stay in control, and be able to stop or avoid other people or objects. 2. People ahead of you have the right of way. It is your responsibility to avoid them. 3. You must not stop where you obstruct a trail, or are not visible from above. 4. Whenever starting downhill or merging into a trail, look uphill and yield to others. 5. Always use devices to help prevent runaway equipment. 6. Observe all posted signs and warnings. Keep off closed trails and out of closed areas. 7. 12 COPPER MOUNTAIN RESORT MOUNTAIN SPORTS RELEASE SKI AND RIDE SCHOOL, WOODWARD COPPER AND COMPETITION/RACE ACTIVITIES WARNING, ASSUMPTION OF RISK, RELEASE OF LIABILITY AND INDEMNITY AGREEMENT Adult means the undersigned adult participant taking part in any Copper Resort - Mountain Sports Activity, being at least 18 years old, signing on behalf of himself/herself and/or the undersigned parent or legal guardian, being at least 18 years old, signing on behalf of the minor named below ( Minor ) so that the minor will be permitted to participate in an Activity. Undersigned means the Adult and Minor collectively. The person actually taking part in the Activity is referred to herein as Participant. Activity means participation in any or all activities at Copper Mountain Resort and any sponsored off site programs. An Activity includes but is not limited to skiing, snowboarding, free skiing, skiing and snowboarding competitions, races, race training, all Ski and Ride School programs, all Woodward Copper programs, all Competition Department programs, instructional activities or events, other special events, demonstrations and performances, strength and endurance training, tubing, swimming, on mountain air bag use, skateboarding, in-line skating, use of skateboards, scooters, longboards, parkboards, parkskis, snowbikes, trampolines, slackline, tumbling, stunting, being spotted, use of foam boards, trampoline boards, trampoline skis, foam pits and air bags, ramps, rails, boxes, bowls, half-pipes, spotting rigs, skier/snowboarder extraction units, trampoline/ bungee, jumps, digglers, BMX and mountain bikes and/or any other uses of the venues, facilities, activities, or equipment of Copper Mountain Resort (the Resort ). UNDERSIGNED AGREE THAT THIS AGREEMENT APPLIES FOR EACH AND EVERY DAY PARTICIPANT ENGAGES IN ANY ACTIVITY DURING THE SKI SEASON AND PRE AND POST SEASON TRAINING WITHOUT REQUIRING UNDERSIGNED TO SIGN AN ADDITIONAL FORM FOR EACH DAY AND/OR EACH ACTIVITY UNTIL UNDERSIGNED REVOKE IT IN WRITING AND THAT WRITING IS ACCEPTED IN A WRITING SIGNED BY THE RESORT S AUTHORIZED REPRESENTATIVE. Risks of Activity. Undersigned agree and understand that participating in the Activity is inherently dangerous, and Undersigned fully realize the dangers and risks of participating in the Activity, which dangers and risks include, without limitation, THE RISK OF PHYSICAL INJURY AND/OR DEATH. The Colorado Ski Safety Act defines a "skier" as any person using a ski area for the purpose of skiing, which includes, without limitation, sliding downhill or jumping on snow or ice on skis, a toboggan, a sled, a tube, a snowbike, a snowboard or any other device; or for the purpose of using any of the facilities of a ski area, including but not limited to ski slopes and trails, and terrain parks. UNDER COLORADO LAW, A SKIER ASSUMES THE RISK OF ANY INJURY TO PERSON OR PROPERTY RESULTING FROM ANY OF THE INHERENT DANGERS AND RISKS OF SKIING AND MAY NOT RECOVER FROM ANY SKI AREA OPERATOR FOR ANY INJURY RESULTING FROM ANY OF THE INHERENT DANGERS AND RISKS OF SKIING. In addition to the inherent dangers, OTHER DANGERS AND RISKS, including, without limitation, falls from heights and level surfaces both inside and outside facilities, drops, high elevation, marked and unmarked obstacles, avalanches, wildlife encounters, slick or uneven surfaces, objects and surfaces covered with ice and snow, falling trees and limbs, unstable ice and snow, rugged mountainous terrain, dehydration, overexertion, acts of other skiers, equipment malfunction, encounters with snowmobiles and/or other motor vehicles and equipment; lift loading, unloading, and riding; separation from instructors, coaches or groups; limited access to and/or delay of medical attention; mental distress from participation in the Activity; and negligence of others. Dangers and risks also include the decisionmaking and conduct of ski area operator employees, including, but not limited to, the risk that a coach, instructor, or counselor may misjudge participants abilities, conditioning, or misjudge weather, terrain, snow conditions, route selection, location, or some aspect of Participant s mental, emotional, or physical condition that may make a certain portion of the Activity appropriate or inappropriate for Participant. Undersigned further understands that coaching or other supervised participation in an Activity does not in any way eliminate the dangers and risks of an Activity. NOTWITHSTANDING THE FOREGOING, ADULT ACKNOWLEDGES, UNDERSTANDS AND AGREES THAT BY SIGNING THIS AGREEMENT, ADULT FOR HIM/HER SELF AND, IF APPLICABLE, ON BEHALF OF MINOR, IS VOLUNTARILY ASSUMING ALL DANGERS AND RISKS OF PARTICIPATING IN AN ACTIVITY, INHERENT OR OTHERWISE, AND IS WAIVING RIGHTS, AND RELEASING CLAIMS IN CONNECTION WITH DANGERS AND RISKS ABOVE AND BEYOND THOSE ADDRESSED BY THE ACT. Participant accepts the responsibility of maintaining control at all times while participating in the Activity. Adult agrees to read, to have Minor read and, if necessary explain to Minor all posted signs and warnings including instructions on use of lifts and equipment and Undersigned agree that Participant will obey those signs and warnings located on the property or at any facility or premises of the Resort and to obey instructions from Resort s staff. Adult agrees and understands that Minor will be using ski lifts without a ski instructor, coach, Resort representative or other adult present. Undersigned understand that Participant must have the physical dexterity and knowledge to safely load, ride and unload the lifts. Undersigned understand that entering or skiing in a CLOSED area is illegal and more dangerous to Participant and others than skiing in open areas of the ski area. Undersigned understand that, although Participant may be wearing a helmet, a helmet cannot guarantee Participant s safety and no helmet can protect the wearer against all potential head injuries or prevent injury to the wearer s face, neck or spinal cord. UNDERSIGNED FURTHER AGREES THAT ANY MINOR PARTICIPATING IN SKI SCHOOL PROGRAMS, ANYONE REGARDLESS OF AGE PARTICIPATING IN WOODWARD SKI/ RIDE AND SKATE BOARDING ACTIVITIES/FREE STYLE TERRAIN PARK PROGRAMS OR ANYONE PARTICIPATING IN A COMPETITION/RACE EVENT SHALL BE REQUIRED TO WEAR A HELMET APPROPRIATE FOR THE SPORT. Undersigned agree to inspect all equipment and facilities before use and ask questions of Resort employees if Participant does not fully understand how to use the equipment and facilities, undersigned accepts equipment and facilities in their "AS IS" condition. As a participant in the Activity, Participant may be involved in jumps, racing, boardercross, race training, competition, performance, demonstration, free style skiing or special event programs. Undersigned recognize, understand and agree that participation in such programs is more hazardous and dangerous than recreational skiing. The Undersigned further agree that as a participant in jumps, racing, competition, performance demonstration, free style or special event program, Participant is a competitor at all times, whether warming up, practicing for competition, free skiing or in competition. Participant agrees that, prior to any training, competition, event, or race, Participant shall always conduct a reasonable inspection of the training, competition, event or race course and make his/her own decision about whether to participate, given the conditions at the time and taking into account, without limitation, Participant s skill level and experience in courses or venues of similar difficulty. Undersigned understand, agree and represent that Participant will always engage in reasonable inspection and decision-making pertaining to participation in any Activity. Undersigned freely assume all risk of all course and venue conditions including but not limited to course/ venue design, construction, layout, terrain, jumps, features and obstacles and course maintenance. Participant may be involved in travel to and from an Activity or events over snow or ice covered

13 roads in inclement weather and Undersigned accept such risks. Where Participant is involved in Woodward programs, Undersigned agree that Participant may attend offsite outings or field trips planned by Resort unless and until Undersigned notifies Resort in writing that Undersigned does not wish for Participant, if a Minor, to participate in a field trip or offsite outing. Undersigned further understand that Participant will travel in Resort vehicles within and outside of Summit County, Colorado for field trips and offsite outings. By signing this Agreement Adult on his/her own behalf and, if applicable, on behalf of Minor acknowledges the risks and dangers associated with the Activity and the use of the Resort s facilities generally and, as a condition to Participant engaging in any Activity and agrees to (1) ASSUME ANY AND ALL RISKS OF INJURY OR DEATH to Participant while or as a result of participating in any Activity; (2) WAIVE, RELEASE, and NOT SUE, MAKE ANY CLAIMS OR FILE ANY ACTIONS against Powdr-Copper Mountain LLC, Powdr-Woodward PA LLC, Powdr Corp., the United States and The Village at Copper Association, Inc., and each of their insurance carriers, subsidiaries, affiliates, officers, directors, shareholders, members, representatives, assignees, employees, volunteers and agents, as well as any Activity sponsors and Equipment manufacturers and distributors (collectively, the Indemnified Parties ) that are based on, arise or result from, in whole or in part, participation in any Activity; and (3) INDEMNIFY, DEFEND AND HOLD THE INDEMNIFIED PARTIES HARMLESS, from any and all claims, demands, actions, causes of action, losses or liabilities whatsoever arising from or related to participation in any Activity and any loss, damage or injury, including death, that may be sustained by Participant or caused to others or their property by Participant. Undersigned agree to pay all costs, including reasonable attorneys fees and disbursements, incurred by any Indemnified Party in defending an investigation, claim or suit brought by, on behalf of, against, or as a result of Undersigned. Undersigned gives Resort or any third party authorized by Resort, including, without limitation, any sponsors of the Resort, permission to take and use photographs, video recordings, or movies of Participant taken during an Activity for any purpose in promoting the Resort or Resort activities in print, brochures, advertisements, films or videos and on broadcast presentations of any sort. Undersigned further agrees and provides permission for Woodward and Copper to copy and retain any photos, video, voice, film, or any other digital media resulting from the Woodward Digital Media Programs. Resort has the right to confiscate tickets or passes or revoke the privileges conferred by a ticket/pass where in the sole judgment of its representative, Participant acts inappropriately in any fashion. A ticket/pass is NOT TRANSFERABLE and CANNOT BE RESOLD. A ticket/pass may be confiscated with no re-issue, if in the sole judgment of a Resort representative; it is used in a fraudulent, reckless or dangerous manner. Reissued passes may be subject to a replacement fee. Undersigned acknowledge their affirmative duty to immediately notify Resort if Participant's ticket/pass is lost or stolen. Adult warrants that Participant is physically capable of safely participating in the Activity and is in good health. For those participants involved in Ski and Ride/ Woodward programs Adult agrees there are no special needs of participant which has not been disclosed in writing to the Resort. Undersigned authorize the Indemnified Parties and/or their authorized personnel to call for medical care for Participant, or to transport Participant to a medical facility or hospital if, in the opinion of such personnel, medical attention is needed. Adult consents to care given to Participant by an emergency care provider or under the instruction and direction of a licensed physician. Reasonable efforts will be made to contact Adult when care is undertaken for Minor. Undersigned agree that upon Participant s transport to any such medical facility, ambulance or hospital that the Indemnified Parties shall not have any further responsibility for Participant. Further, Undersigned agree to pay all costs associated with such medical care and related transportation provided for Participant and shall indemnify and hold harmless the Indemnified Parties from any costs incurred therein. In consideration for accessing the facilities of the Resort, ADULT agrees for him/herself and MINOR that ALL claims arising from or related to any Activity, including injury to person or property and/or death shall be GOVERNED BY COLORADO LAW, without regard to conflicts of law principles, and that EXCLUSIVE JURISDICTION shall be in the District Court of Summit County, Colorado or in Federal Court for the State of Colorado. UNDERSIGNED VOLUNTARILY AND IRREVOCABLY WAIVE ANY OBJECTION TO SUCH LAW AND JURISDICTION. This Agreement shall be binding to the fullest extent permitted by law. If any provision of this Agreement is found to be unenforceable, the remaining terms shall be enforceable. WITHOUT PARENT OR GUARDIAN SIGNATURE PARTICIPANT, UNDER PENALTY OF FRAUD, REPRESENTS THAT HE/SHE IS AT LEAST 18 YEARS OF AGE. THE UNDERSIGNED PARENT OR LEGAL GUARDIAN REPRESENTS AND ACKNOWLEDGES THAT HE/SHE IS ENTITLED TO AND IS SIGNING THIS AGREEMENT ON BEHALF OF MINOR AND THAT MINOR WILL BE BOUND BY ALL THE TERMS OF THIS AGREEMENT. UNDERSIGNED UNDERSTAND AND AGREE THAT IF THIS AGREEMENT IS NOT SIGNED ON BEHALF OF MINOR, MINOR WILL NOT BE PERMITTED TO PARTICIPATE IN ANY ACTIVITIES. This Agreement shall be binding upon Undersigneds assignees, subrogors, distributors, heirs, next of kin, executors and personal representatives.
