IN CONSIDERATION of being permitted to participate in any way in USA Table Tennis sanctioned events, I and/or minor child, our personal representatives, assigns, heirs, and next of kin: 2. ACKNOWLEDGE, agree, and represent that I and/or minor child understand the nature of Table Tennis Activities and that I and/or minor child are qualified, in good health, and in proper physical condition to participate in such Activity. I further agree that if at any time I believe conditions or equipment to be unsafe, I and/ or minor child will immediately discontinue further participation in the Activity. 3. FULLY UNDERSTAND that (a) TABLE TENNIS ACTIVITIES INVOLVE RISKS AND DANGERS OF SERIOUS BODILY INJURY, INCLUDING PERMANENT DISABILITY, PARALYSIS, DISMEMBER- MENT, AND DEATH ("RISKS"); (b) these Risks and dangers may be caused by me and/or minor child's own actions, or inaction, or the actions or inaction of others participating in the Activity, the condition in which the Activity takes place, or THE NEGLIGENCE OF THE "RELEASEES" NAMED BELOW; (c) there may be OTHER RISKS AND SEVERE SOCIAL AND ECO- NOMIC LOSSES either not known to me or not readily foreseeable at this time; and I FULLY ACCEPT AND ASSUME ALL SUCH RISKS AND ALL RESPONSIBILITY FOR LOSSES, COSTS, AND DAMAGES I and/or minor child incur as a result of my participation in the Activity. 4. HEREBY ACCEPT AND ASSUME ALL SUCH RISKS, KNOWN AND UNKNOWN, AND ASSUME ALL RESPONSIBILITY FOR THE LOSSES, COSTS, AND/ OR DAMAGES FOLLOWING SUCH INJU- RY, DISABILITY, PARALYSIS, OR DEATH, EVEN IF CAUSED, IN WHOLE OR IN PART, BY THE NEGLIGENCE OF THE "RELEASEES" NAMED BELOW. 5. HEREBY RELEASE, DISCHARGE, AND COVENANT NOT TO SUE USA TABLE TENNIS, their respective administrators, directors, agents, officers, officials, volunteers, and employees, other participants, any sponsors, advertisers, and if applicable, owners and lessors of premises on which the Activity takes place, (each considered one of the "RELEASEES" herein) FROM ALL LIABILITY, CLAIMS, DEMANDS, LOSSES, OR DAMAGES ON MY ACCOUNT CAUSED OR ALLEGED TO BE CAUSED IN WHOLE OR IN PART BY THE NEGLIGENCE OF THE "RELEASEES" OR OTHERWISE, INCLUDING NEGLIGENT RESCUE OPERATIONS; AND I FURTHER AGREE that if, despite this RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT I and/or my minor child, or anyone on my and/or my minor child's behalf, makes a claim against any of the Releases, I WILL INDEMNIFY, SAVE, AND HOLD HARMLESS EACH OF THE RELEASEES from any litigation expenses, attorney fees, loss, liability, damage, or cost which may incur as the result of such claim. 6. I HAVE READ THIS AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND HAVE SIGNED IT FREELY AND WITHOUT ANY INDUCEMENT OR ASSURANCE OF ANY NATURE AND INTEND IT TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW AND AGREE THAT IF ANY PORTION OF THIS AGREEMENT IS HELD TO BE INVALID THE BALANCE, NOTWITHSTANDING, SHALL CONTINUE IN FULL FORCE AND EFFECT. 7. By registering for the 2018 USA Table Tennis National Championships, all participants are subject to possible selection for drug testing as conducted by the US Anti-Doping Agency. 8. MEDICAL AND TRANSPORTATION RELEASE I hereby give consent for the organizers of the 2018 USA Table Tennis National Championships to provide me with athletic trainer and emergency medical treatment. This release is effective for the period of July 2-7, I swear and affirm that I am in good physical condition and I am not aware of any previous or existing diseases or injury that would result in my being injured. I further acknowledge that I have procured on my own and currently hold valid and adequate insurance for such loss, damage, or injury and if I do not have valid and adequate insurance, I accept full responsibility for the cost of the loss, damage, or injury that might occur as a result of my participation in the 2018 USA Table Tennis National Championships. I give my consent to participate in any and all emergency transportation associated with activities during the 2018 USA Table Tennis National Championships, in the event I become incapacitated or otherwise not conscious to give direct consent and I accept full responsibility for all costs associated with all emergency transportation as deemed necessary. Further, as the parent/guardian of a participating minor athlete, I hereby give my consent for complete medical treatment and emergency transportation services as deemed necessary beyond the on-site athletic trainer or emergency medical treatment provided. 9. MEDIA AND PHOTO RELEASE Any image, photographic, or otherwise, taken of tournament play or within the official venue is essentially the property of USA Table Tennis regardless of the Approved status of the recording instrument or photographer. Entrants agree to allow their voice and likeness in such images to be reproduced in connection with USA Table Tennis by way of any medium. USA Table Tennis is not responsible for nor can they control the use of camera phones inside the venue. Participants are hereby notified of this policy. Eligibility: Entries are limited to active USATT members as follows: National Championships Events: Events 1-51 U.S. Citizens-Proof of Citizenship must be submitted with the entry form. Youth National Championship Events: Junior Boys /Girls - Born on or after January 1, 2000 Cadet Boys /Girls - Born on or after January 1, 2003 Mini Cadet Boys /Girls : Born on or after January 1, 2005 Hopes Boys /Girls : Born on or after January 1, 2006 U-10 Boys /Girls : Born on or after January 1, 2008 Hardbat and Sandpaper Events: (Events 71-82) For All Above Events Please contact USATT Headquarters ( ) regarding eligibility questions. Membership: USATT Memberships must be valid through July 7th, Non-members or expired memberships must enclose membership fee with their entry forms. Rules: The 2018 USA Table Tennis Nationals will be conducted under the USATT Rules. All decisions by the Tournament Committee will be final and contestants upon entering agree to abide by them. Equipment: Tables and nets: JOOLA. Balls: Nittaku Premium 40+ (White). Floor: For the 2018 Nationals only, all courts will be matted with rubberized flooring. Rackets in all events are subject to random testing for volatile organic compounds (VOC s). Any glues containing VOC s will cause the racket to fail. EXCEPT FOR HARDBAT AND SANDPAPER EVENT S, RACKETS MUST COMPLY WITH THE RED/BLACK RULE. Clothing: The USATT Dress Code will be enforced. No white clothing will be allowed. Players must wear the assigned number or their name on the back of their shirt when competing. Names must be large and visible to spectators. It is recommended that each player come to the table with 2 distinctively different colored shirts Entries: Players may enter a maximum of 7 events but may not enter two events starting at the same time. RATING EVENTS 52-68: These events will have RR groups of 6 with 1 advancing to the SE stage. Players may enter a maximum of 6 rating events based on their rating as of 4/12/2018. Events chosen must be within any 6-sequential order that are above his/her rating used for the 2018 US Nationals. For example, if a player s rating is 1146, he/she may participate in any or all of the following: U1200, U1400, U1600, U1800, U2000 and U2200. However, he/ she may NOT participate in the U1200, U1400, U1600, AND U2200 and U2300 events since this covers a range of more than 6 events. Note that the U2200 TSRR does not count towards these 6 rating events, see FAQ for more details. Players that are over the rating limit of an event will automatically be entered in the next higher rating event. If two events with the same start time are entered, the player must decide which event they wish to participate in. Tournament Committee will choose on behalf of the player which event is to be played if the player could not be reached by phone and/or . RATING DOUBLES EVENTS: Players MUST be rated under 2500 to participate in the U4200 Doubles and under 2000 to participate in U3200 Doubles. Rating Adult and Junior Events: Players entering the Rating Junior events must be born on or after 1/1/2000 (18 and under). Players entering the Rating Adult events must be born on or before 12/31/1999 (19 and over). Doubles Partners: Players MUST enter with a partner in all doubles events. USA Table Tennis will NOT arrange for a doubles partner. USATT cannot be responsible for the nonparticipation of your selected Doubles Partner. It is the responsibility of the players to contact his/her doubles partner about participating in the event. Players without a rating at the Qualification time can t enter rating doubles events. Partners withdrawing before June 1, 2018 may be replaced provided the replacement partner has a lower rating than the original partner, partners withdrawing after June 1, 2018 may not be replaced. If you have not found a doubles partner as of June 1st, you will be dropped from the event. For all doubles events the prize money is for the entire doubles team, NOT per person. Adding or Dropping Events: Events may be added or dropped up to 6/1/2018. After 6/1/2018, no additional events may be added to your registration, events may only be dropped. Unrated Players: players without USATT rating: Players may enter any championship event for which they are otherwise eligible. Players who are unrated on the seeding date (6/14/2018) will not advance to the single elimination stage of rating-restricted RR events. Players who enter a rating-restricted event without a rating and who acquire a rating between the qualification cutoff (4/12/2018) and the seeding (6/14/2018) will be permitted to advance provided their new rating meets the event s requirements. Unrated players are not permitted to enter ratingrestricted single elimination events. Matches: All matches at the 2018 USA Table Tennis National Championships will utilize 11-point game rules with the exception of Hardbat and Sandpaper events (see Hardbat and Sandpaper info). Men s and Women s Championship events will be best of seven (11-point games) from the round of 32. All RR events will be the best of five (11-point games). All other events will be the best of five (11-point games), single elimination, unless otherwise indicated. Event Scheduling: Events are not guaranteed to be completed on the same day that they begin. No refunds will be given for early departure. All events will be completed by Saturday evening. Any event may be cancelled and the entry fee for that one event refunded if the event contains less than four players or pairs, as appropriate. In the case of event conflicts, the player is responsible for notifying the Control Desk as soon as possible. In case of an inadvertent error, liability is limited to the refund of the event fee for the event(s) involved. Each event will begin on the date/time designated on the form, and it will continue at regular intervals until a champion has been determined. An entrant s signature on the tournament entry form acknowledges that the dates are acceptable for participation in the registered events regardless of any holidays, federal, state, local, religious, or foreign. Championship Age Events: Players must be over 30, 40, 50, 60, 70, 75, 80 and 85 as of December 31, Players in the 21 & Under Men s and Women s Singles must be born on or after January 1, Proof of age must be readily available at the tournament. Withdrawal/Refunds: A 45 processing fee will be charged to any participant who withdraws from the tournament prior to May 25th, There will be no refunds given after this date except for injured players with a signed doctor s note. Withdrawal notification must be in writing at USATT Headquarters in order to receive a refund. PLAYING FORMATS: Youth Events: (Events 6 13) First two stages will be RR groups and stage 3 (main draw) will be played in a single elimination format. Men s and Women s Singles Championship Events: In the Men s Singles, the top 16 players will be seeded into the main draw of 64 players. In Women s Singles, the top 8 players will be seeded into the main draw of 32 players. Depending on the number of entries, all other players will be drawn into qualification RR groups to advance into the main draw. The main draw will be played in a single elimination format. All matches from the round of 32 will be played best of seven games. All other matches will be played best of five games. The US Nationals is the second senior and second youth qualifying competitions (National Ranking Tournaments) for the 2018 TTTeam USA season. It is part of the new USATT National Team Program and Selection Procedures that are effective starting January 2018 and can be found here The new USATT National Team Program and Selection Procedures include, among others, a system of points that players will earn through their participation and results in different qualifying competitions like the US Nationals in Las Vegas. Junior Boys and Girls Teams: Corbillon Cup System: 4 singles and 1 doubles. A-X, B-Y, doubles, A-Y, B-X Playing system will be Single Elimination or Round Robin depending on number of entries. DEFAULTS IN YOUTH EVENTS OR MEN S AND WOMEN S Singles and Doubles CHAMPIONSHIP EVENTS: (Events 1 13, 88 91) An athlete who defaults in any Youth and/or Men s or Women s Championship Event is considered to have completely, fully and finally defaulted the opportunity for any further participation whatsoever in the 2018 US National Championships. However, in cases where the default is due to illness or injury, an athlete may appeal to the Tournament Director for the right to be permitted to participate in a separate Youth and/or Men s or Women s Championship Event for which the athlete has previously registered other than the Event in which the athlete defaulted. The Tournament Director is vested with the sole authority, without opportunity for appeal, to determine whether the athlete s default was due to a verifiable injury or illness and whether said athlete should be permitted to compete in a said separate Event. Seeding: Seedings in the Youth Events, the 21 & Under Men s and Women s Singles Events and the Men s and Women s Singles Championship (Events 1, 2, 6 13, 17, 18) are based first on the June 2018 ITTF World Rankings and then on USATT Rating List generated on 6/14/2018.

8 Prospectus PLAYING FORMATS (Continued): U1700 Sandpaper and Open Sandpaper, Events 81 82: The same rules as Hardbat (see below) will apply except the equipment will be an all wood blade with a sandpaper only cover on the playing surface. All players must be US Citizens and proof must be submitted with the entry form. 38mm balls will be used. Quarterfinals through Final will be 3 out of 5, 21 point games; all other matches will be 2 out of 3. USATT ratings will be used for seeding. Event 82 - Eligibility and seeding for the U1700 Sandpaper RR Singles event will be determined by the player's hardbat rating (or USATT rating at the seeding cutoff date of 6/14/2018, if the player does not have a hardbat rating). Hardbat Events: Hardbat rules, as adopted by the USATT Classic Table Tennis Committee, will be used for all hardbat events. See for the complete list of official rules. As in all events, a question of racquet legality is the decision of the Tournament Referee. Rackets complying with all requirements of the rules for hardbat rackets shall be legal. These events do not count for USATT ratings points. Both eligibility and seeding for the hardbat events will be determined by the player's hardbat rating (or USATT rating at the seeding cutoff date of 6/14/2018, if the player does not have a hardbat rating). If the participant enters the U-2000 hardbat RR, U-1800 hardbat RR, or U-1500 hardbat RR and there are insufficient entries, he/she will automatically be moved into the U-2000 RR, U-1800 RR, or U-1500 RR, if eligible. All hardbat matches will be played to 21-point games. All matches will be the best of 3 games, except for the quarterfinals, semifinals, and final of events 71, 72 and 81, which will be best of 5 games. Balls: Nittaku Premium 40+ (White). Entry will be free and seating will be first come first served. VIP package participants will have access to reserved seating. Match Check-in: Players must register with tournament officials at least 30 minutes before their first match and be ready to play each match 10 minutes before the scheduled starting time. See Earliest Starting Times in the Schedule of Events. Publications: Display and/or distribution of any brochures, posters, literature, or written material at the tournament venue is strictly prohibited unless previously approved by USA Table Tennis. Films/videotapes: Anyone wishing to make films or videotapes of the matches must receive written permission from USATT Headquarters. You may record yours or your family member s own matches provided you get permission from the opponent first. No videotaping of the Men s and Women s Semi-Finals or Finals matches is allowed. Cancellation Without Fault: USATT will refund 50% of all entry fees paid by participants if the U.S. Nationals event is cancelled or partially cancelled due to acts of God, force majeure, war within the continental U.S., governmental authority, disaster strikes, civil disorder, curtailment of transportation services that prevent at least 50% or more of USATT participation from attending the event.
