2 Welcome Warriors & Friends, Thank you for participating in the 100 Holes of Hope Golf Marathon This information package includes everything you need to become a Warrior or Friend of this exceptional event. There are details on registration, managing and making pledges, game-day format, plus all relevant forms. Our 2014 golf marathon raised over $44,000 which has been given to the Ottawa Hospital Foundation in support of the Emilie Joinette Inspiration Fund for Adult CF Clinic patient care and support programs and the Royal Ottawa Foundation for Mental Health toward the advancement of treating mental illness. Our golf event over the last two years has raised $80,000. With your help, we re confident that we will reach this year s goal of $50,000, bringing our grand total to $130,000. If you have any questions at all, please contact either of us and we ll be happy to provide any additional information you might need. A note to all Warriors: remember to start early with your efforts to get pledges. You can reach out to your network of friends through Facebook and Twitter or use LinkedIn for business circles. 3 How to Participate in Our Event MAKE A PLEDGE You may give cash or cheque* to your Golf Warrior. To pledge online using your credit card, go to and select the MAKE A PLEDGE button and complete the 3 simple tasks: 1 Select which Golf Warrior you want to support using the drop-down menu 2 Enter the amount you wish to pledge 3 Enter your credit card information using STRIPE or PayPal Tax receipts will be issued for donations of $20 or more. Once the online transaction is complete, your charity tax receipt will be sent to you electronically the same day. * All cheques are to be made payable to Finding Hope Foundation. BECOME A GOLF WARRIOR Each golf warrior is responsible for raising money through pledges or sponsorships. A minimum pledge/sponsorship of $1,250 for one player or $2,000 for a twosome is required. To sign up, simply go to and select the BECOME A WARRIOR button. Make the commitment*, give us your name, coordinates, hat and shirt sizes and click SUBMIT. It s that easy. Once you ve signed up, you will be sent a login name and password for your personal account which will allow you to view your online pledges. Shortly after you register, we will mail your personalized bag tag, which you will need on your bag the day of the event, and 64 pledge cards (shown here) that you can hand out to potential sponsors. It has all the information they need to pledge you online. Personalized Pledge Cards * By committing to becoming a golf warrior, you are obligated to raise $1,250 as a single player or $2,000 as a twosome. Please remember that this is a charity event which has significant costs per player which is payed for by our corporate sponsors. 4 Grand Prize * Warrior Draw! 3-Day Trip for 2 to Become a Golf Warrior and raise a minimum of $1,500 in pledges and you could win a 3-day trip for two to the 2016 PGA THE PLAYERS Championship. What s more the more money you raise, the more chances you have to win! ELIGIBILITY CRITERIA Sign-up to be a Warrior in the 100 Holes of Hope 2015 Golf Marathon by June 1, Collect a minimum of $1,500 in pledges to earn 1 ticket that will be entered into the draw for your chance to win. The more pledges you have, the more tickets you can enter into the draw (see chart to the right for pledge-to-ticket ratios. All pledges (online, cash, and cheques) must be submitted before or on Monday, August 17, Must be 18 years old or older to qualify. IMPORTANT: In order to enter the Grand Prize Warrior Draw, Warriors must agree to compensate 100 Holes of Hope for any pledges made by cheque that are returned as non-sufficient funds. 5 How to Manage Your Pledges Each Golf Warrior is responsible for collecting pledges that are given in the form of cash or cheques. Please use the Pledge Sheet provided on page 12 of this package (sample below). Also, note that the cash and cheque totals have separate columns. DO NOT fill out the Online Pledge section of the Pledge Sheet. We will manage all online transactions and will complete your Pledge Sheet the day of the event. When you sign up as a Golf Warrior, you will be sent a login name and password which will allow you to view your online pledges. Online pledges using a credit card can be done at We offer STRIPE and PayPal. Please have your Pledge Sheet completed with all cash and cheques ready at the 6:00 a.m. registration. 6 Registration at the Event (6:00 a.m.) THREE THINGS YOU NEED TO HAVE FOR REGISTRATION: 1 Completed Pledge Sheet(s) with the totals of your cash and cheque pledges ONLY (example on page 5), along with the cash and cheques. We will manage your totals from your online pledges. A volunteer will greet you at registration and verify your pledge totals. 2 Signed Photography Release form (page 10) and Waiver and Release of Liability form (page 11). 3 Golf Bag Name Tag. You will be sent a personalized bag tag before the event. Please make sure it s attached to your bag when you arrive. Drop your golf bag in front of the pro shop and Loch March staff will take care of putting it on your cart. We will have a 100 Holes of Hope volunteer at the bag drop to assist you or answer any questions you may have. 7 Game-Day Format There will be one, two-person team per hole 36 warriors in total. Each Golf Warrior has their own chariot. The idea is to play hurry-up golf which will help you play 100 holes within 11 hours, so hit away when ready. Three-foot gimmie putts are allowed and will speed up play, but your playing partner must determine your gimmie. When you have been given a gimmie, it means your next shot is given, but it does count as one stroke. Warriors will play two balls each on holes #2 and #16. Each round has 20 holes and you play five rounds for a total of 100 holes. Let faster players play through and don t spend too much time looking for lost balls. Out of bound shots will be played as a lateral with a one stroke penalty (there are no mulligans). Make sure you complete your score card (sample below) before handing it in at the Pro Shop. We ll need your name, how many balls you used and the total of your best four rounds. Remember to bring your score card into the Pro Shop after the round. Your partners name will be written on the back of your golf cart sign. The combined total of the best 4 rounds out of 5 for each of the 2 person team wins. 4. Closest to the pin (2) Holes #7 & #13 (one winner per hole for all 100 holes) 5. Longest drive (2) Holes #5 & #15 (one winner per hole for all 100 holes) 6. Lowest ball number Mark your first golf ball with a #1 and when you loose this ball, and you will, mark the next #2 and so on. The fewest number of balls used by an individual at the end of the day wins. You need to have played all 100 holes to qualify. 7. Most honest golfer Total of your best 4 rounds out of 5 8. Warrior Quiz Each hole will have a puzzle, quiz or riddle to test your brain power. The highest score wins. In case of a tie, we will draw names HOLES OF HOPE 2015 GOLF MARATHON

11 Waiver and Release of Liability I, (print your name), as a participant in the 100 Holes of Hope Golf Marathon, agree to abide by the rules and regulations of the event, Loch March Golf and Country Club, and all applicable municipal, provincial and federal laws and regulations. I understand that participating in such an event is a potentially hazardous activity and can result in serious injury or death. I am aware of, and expressly assume all risks associated with participating in this event, including and without limitation the effects of weather. In consideration for being permitted to participate in this event, I, for myself and for anyone entitled to act on my behalf, hereby waive and release from any and all claims for injuries and damages I may have arising out of the event or my participation therein (including and without limitation any pre- and post-event activities), against the Finding Hope Foundation, 100 Holes of Hope, Loch March Golf and Country Club, as well as any beneficiaries, sponsors, officials, volunteers, participants, employees, directors, agents, and representatives. I intend, by this Waiver and Release, to release, in advance, and to waive my rights and to discharge all persons and entities mentioned above, from all claims for damages for death, personal injury or property damage that I may have, or which may hereafter accrue to me, as a result of my participation in this event, even though that liability may arise from negligence, carelessness, or recklessness (whether simple or gross) on the part of the persons or entities being released, from dangerous or defective property or equipment owned, maintained or controlled by them or because of their possible liability without fault. I acknowledge that, I and I alone, am solely responsible for my personal health and safety, and the personal property I bring with me.
