5 Page H Record-keeping & Honors Recognition Guidelines & Special : A Year of Achievement It all began September 1, 2015 and will end on August 31, It was another 4-H year full of achievements. One in which you set your goals, completed project work, and realized your successes...and a few shortcomings. Now it is time to summarize it in your Project Record forms and the overall report we have called the Honors form. At the county fair, you can now submit your Project Record. If you do not submit it at that time, you can certainly do it at the time you turn in your Honors form to earn your project disk (s) for the project work you completed this year. You can complete a project record for each project you did this past year. At the same time, it is time to complete and turn in your 4-H Achievement/Honors forms, summarizing all your leadership, citizenship, and individual skill achievements in 4- H. In the past, we called this form a Junior or Senior Honors Form. In 2012, the honors form was revised, changed to make it simpler for you to complete. There is only one honors form to be used by both Junior members ages 9-13 and Senior members ages It is now known as the Kentucky 4-H Achievement Application. It replaces the old Jr. and Sr. Honors Form. Clover Buds, we have not forgot you. Again this year, we will recognize 4-H ers members under age 9 who have participated in 4-H activities in Our under age 9 youth are called Novices or Clover Buds. We have a report form for you to complete. It is called the 4-H Clover Bud Report Form. You can visit our website to download it to your computer; request a copy on a CD; or the good-old fashioned paper copy from our office. We also will have easy, simple, project forms especially for Clover buds to complete. What s the deadline? The deadline is September 9, Each year we recognize those who bring to a close or completion their project by completing a project record and the Kentucky 4-H Achievement (Honors) Application. All of this is done at our 4-H Awards Banquet which will be October 27, If you want to be a part of the recognition, then it s time to get busy! First you need to know the following definitions: Junior Member - A 4-H member who is/was 9-13 years old as of Jan. 1, 2015, typically 4th 8th graders. Senior Member - A 4-H member who is/was years old as of Jan. 1, 2015, 9th to 12th graders. The following awards are easy to apply for. The deadline is September That gives you two whole months to work on it. All you have to do is complete a Kentucky 4-H Achievement (Honors) Application.
