2 REGISTRATION ACKNOWLEDGEMENT: I understand and accept the following requirements for participation in the Annunciation Dance Ministry: 1. The dancer must be a communicant of the Greek Orthodox Church. 2. Dancers must be age 5 (Kindergarten) by September 1st. (A pre-k group may be created if enough interest) 3. The parent(s) and adult dancers should be a Steward of good standing for at a Greek Orthodox parish. 4. All Dancers MUST ATTEND Sunday School and/or Divine Liturgy on Sunday and maintain 75% attendance at Sunday School. 5. All dancers must arrive to practice ON TIME and be RESPECTFUL and maintain 85% attendance (cannot miss more than 1 practice every 6 practices). 6. The dancer/parent(s) agree to meet all financial obligations assigned to the participants in the Dance Ministry. 7. All adult dancers and parents of dancers under age 18 must participate in the fundraising activities of the DSG. There will be a $50 deposit check per dancer for the participation in a DSG fundraising event maximum of $100 per family. Check will be destroyed once volunteering has been fulfilled, if obligation is not met, then check will be used to cover shifts at a DSG fundraising event. 8. All dancers and their parents must sign and abide by the Code of Conduct. 9. All dancers are required to perform at our annual food festival and all other performances approved by the board. FEES: The cost of registration is $200 for the first dancer and $50 for each additional dancer in the same immediate family. If registration is received prior to June 1st, the first dancer will receive a $30 discount and any additional dancers will receive $10 discount after that. If the first (oldest) dancer will be in grade K-1, then registration will be $100 for the first dancer and $50 for additional dancers. Proof of grade in school may be required (ex. class schedule, ID card, anything that shows current academic year). LATE REGISTRATION: A $25.00 late fee will be assigned after September 14 th. 3 CODE OF CONDUCT The Greek Orthodox Church of the Annunciation s Greek Dance program believes that each dancer should experience a positive Orthodox Christian environment when participating in the dance program. As such, all dancers and dance directors are expected to behave in a manner that emulates the beliefs of an Orthodox Christian. Participants found to be violating the Code of Conduct rules, will be dealt with in the following manner: 1. Dance Director will have a conversation with the dancer clearly explaining what rule(s) the dancer has violated and let him/her know it is not acceptable. 2. If the dancer violates any rule again, the Dance Director will have a conversation with the dancer and the dancer s parents explaining the continued violations of rules of conduct. The parents and dancer will be told that any further violation will result in a discussion with one of the Parish Priests. The Dance Director Liaison will inform the DSG Board president of the second occurrence. 3. If a third occurrence happens, the parents, dancer, Dance Director and Director Liaison will sit down with a Parish Priest to discuss the lack of respect being shown to the other participants of the dance program. Board will be advised of resolution. Violations include, but are not limited to: Disrupting dance practice or otherwise defying the authority of the Dance Director(s); Fighting or provoking a fight; Using inappropriate language; Harassing, teasing, hazing or verbally abusing another participant; Committing an obscene act or engaging in profanity/vulgarity; Utilizing a cell phone during practice time; Destroying or defacing Church property or the property of others; including, but not limited to costume(s). Use, possession or sale of drugs, drug paraphernalia, alcohol or any other controlled substance; Sexual harassment of another including any inappropriate action that causes another to feel uncomfortable; Conduct involving dishonesty, deceit, or misrepresentation; Repeated late arrival to dance practices; Discourteous actions toward others; Dressing in an inappropriate manner including the wearing of revealing tops (girls), pants that sag or are worn so low as to make the wearer s undergarments visible, wearing flip-flop style sandals or other shoes that make it difficult for the dancers to move freely. 4 GIVING BACK DSG will be participating in various services (thiakonia). Dancers will be required to participate in various projects throughout the year, minimum of five hours are required per dancer. For example: volunteering for DSG sponsored Greek festival shifts, fundraisers, and dancing at outside events. FUNDRAISING COMMITMENT In the past we made a large portion of our funds through our fireworks booth. We are considering a replacement fundraiser(s), so we will need your help with other fundraising events such as Pasta Mania, Bingo night or possibly a Greek Dance. It is important that every family volunteer to help with these fundraisers. In order to ensure that these fundraisers are successful, we need parent/dancer participation. Therefore we are asking for a refundable $50 deposit for each dancer (maximum of $100 per family regardless of number of dancers). Failure to work at any of the DSG fundraisers will result in a $50 fine per dancer. This fine is charged in order to cover the cost of hiring people to work at the fundraisers. Please contact the DSG Board President to volunteer. I understand the fee I will be charged if I do not cover or find someone to cover my fundraising shift(s).
