1 Procedure Type: Creation Date: Revision Date: Standard 12/22/ /19/2016 Purpose: Ensure that the removal of an anthill around a wellhead is performed in a safe and controlled manner. Scope: This SOP is intended to be used as a guideline for Brown's Backhoe Service, Inc. employees who oversee or perform the removal of anthills around a wellhead. Requirements: Before starting work, the site shall be thoroughly inspected by the work team to determine if special safety measures must be taken. If so, supervision must be contacted before work starts. 2 1) Assess location prior to commencing work. a) Perform 360 walk around location to identify hazards and verify pumping unit components are in place and working properly. If not, Stop Work and call your Supervisor immediately. b) Use Quad Gas Monitor to detect possible gases from wellhead on location. Continue to monitor the area until the job is complete. No documentation needed. c) Place orange cones in mandatory areas: wellhead, AOJ/Jog post, ASD, chemical tank, power pole and water riser. Lime green cones must be directly 20 feet out from bull plug, chemical line, and valves. Any remaining orange cones must also be used to isolate or secure work location. 2) Lock-Out/Tag-Out Review Lock-Out/Tag-Out SOP 01 prior to performing any procedure You must contact your Supervisor when shutting down a well that is currently active. You must contact your Supervisor before the Lock-Out/Tag-Out procedure is started when ASD is present. Page 2 of 8

3 3) Inspect Electric Jackhammer a) Make sure Electric Jackhammer is not plugged into the Portable Generator. b) Lubricate the bit shank. c) Retract locking sleeve and insert the bit shank d) Ensure bit is properly engaged. e) Inspect the electrical cord, the cord must have the Quarterly tape present prior to using cord or equipment. This inspection does not eliminate the need for inspection prior to the use of the elctrical equipment. If tape is not present or the electrical cord contains any cuts, abrasions, broken prongs it must not be used. Contact your Supervisor immediately. The Quarterly tape colors are as follows: Red: January - March White: April - June Blue: July -September Green: October - December Remember extension cords should not be draped over or near exposed production or steam lines. This could cause cord jacket/cover to melt and cause a short. Equipment and cords should be unplugged by the plugs, not by pulling on the cord itself. Page 3 of 8

4 4) Inspect Portable Generator Generator will have hot surfaces after operation Never fuel your equipment while HOT. Allow your equipment to cool down for 10 minutes before fueling. a) Check oil level before each use on a level surface with engine stopped. Remove the oil filter cap and wipe the dipstick clean. Check the oil level by inserting the dipstick into the filler neck without screwing it in. If the oil level is low, fill to the top of the filler neck with the recommended oil. Any spillage of motor oil must be cleaned up immediately. b) Start generator c) Test Ground Fault Circuit Interrupter (GFCI) Press the GFCI test button you will deactivate the circuit and the reset button will pop out. If reset button does not pop out, stop work and call your Supervisor immediately. d) Push the reset button to re-energize the circuit. The "pig tail" GFCI will need to be used if no GFCI is present. The "pig tail" GFCI works the same manner as the built in GFCI. If these are used, they must be tested as well. Do not use two GFCIs in the same circuit. Any electrical problems with generator is to be reported to supervisor immediatley. If generator is taken out of service, tag the equipment and do not use until the issue is fixed. Never overload the outlet on the generator or exceed the total wattage rating. 5) Prepare work area a) Remove wellhead guards by using team lift and place guards away from immediate work area. REMINDER! Ensure that all the hazards have been mitigated and that all safeguards are in place. (i.e. Lime Green Cones) Page 4 of 8

5 6) Clear the anthill and surrounding area around the wellhead and the trombone out to the 20 foot valve. This will require the use of the Electric Jackhammer, shovels, rakes, and digging bar. It is required that job rotation take place every 30 minutes to prevent any repetitive motion stress and fatigue. Do not dig beyond any undisturbed ground, unless instructed to by your Supervisor. Remember, all valves and pipes must be exposed so that the pumping unit can properly be maintained and functioning. DO NOT cross the Lime Green cones while any work is being performed. 7) As spoils accumulate, use the wheelbarrow to move spoils to a location that is at a minimum of 5 feet away from fixed objects, roadways, embankments, and outside the 10 foot Overhead Power Line curtain/bubble/safety-window. Position the wheelbarrow out of the line of fire from the valves, bull plugs, and chemical lines. Never fill the wheelbarrow completely full. It should be filled half way to three-quarters full. 8) Once anthill and surrounding area has been cleared, pick up and secure all equipment on the vehicle. Page 5 of 8

6 9) Prepare a Journey Management to a Satellite location. 10) Arrive at satellite location. a) Use hand tools to load up wheelbarrow and truck bed with clean material to place around the wellhead and trombone. b) Once enough clean material has been loaded. Secure all equipment used to load material and go back to location. The same Journey Management may be used if more than one trip is required for new material. 11) Arrive back at location. a) Use a spotter to position the truck close enough to the wellhead that material can be place safely. Stay at a minimum of 2-3 feet away from the wellhead with truck. As a spotter DO NOT position yourself between wellhead and moving equipment. b) Use hand tools to empty clean material from the back of the truck. Use 3 points of contact when climbing on and off of the back of the truck. Page 6 of 8

7 12) Using hand tools, dress the area around the wellhead and trombone with the clean material. 13) Return unit guards by using team lift. Some wellheads may not have had guards in place. 14) Pick up all cones and tools on location and secure the equipment on the truck. 15) Review the Lock-Out/Tag-Out procedure and remove your locks and tags. Your Supervisor may require you to call prior to removing locks. Page 7 of 8

8 16) Completion a) Check that all tools/equipment is picked up and secure. b) Prepare Journey Management to next location. c) Leave location. Turn in your list of spoil piles. Page 8 of 8
