The Hope 4 L.Y.F.E foundation ("the applicant") has requested to host its first annual Somi Hope 4 L.Y.F.E 3K Breast Cancer Walk from 5:00 a.m. to 3:00 p.m. on Saturday, October 7,2017. This event will begin and end at the Gibson-Bethel Community Center 5800 SW 66 th Street, South Miami, FL The walk will take place through the South Miami CRA neighborhood, starting at 68 th Street and east of 58 th Place and will begin heading west, making a right turn on 58 th Place, down to 66 th Street, turning right to 5i h Avenue and ending on 68 th Street. After the walk participants will return to the Gibson-Bethel Community Center for music, educational exhibits and live entertainment from local speakers, cancer survivors, doctors and more. To register, participants will be charged: $25 for individuals, $15 for military members or spouse, $5 for Youth (up to 21 years old with Student ID) or $100 for a team of 5 participants. Funds raised will be donated to Florida International University (IfF.I.U") and will be used towards the screening of breast cancer in men and women in the City of South Miami only, and who may need further testing beyond F.I.U's Mammogram Van Services. The applicant has completed the special event application and has submitted the necessary notification/petition approval form signed by homeowners and businesses that will be affected by the requested sidewall< closure. The applicant is requesting a waiver for all Special Event fees in the amount of $5, , as illustrated in the table below:

This Event will be held at Gibson-Bethel Community Center in South Miami, Florida. Attendees will enjoy educational exhibits and exciting entertainment which promotes empowerment and breast cancer awareness, while participating audiences will experience the joy of giving to others in need through a cause they love. We have also planned to host a Kick-Off event for this walk on Saturday, September 2nd, The event, known as 'Bedazzle Your Bra', kick-off event will take place from 3pm until 7pm at the Elks Lodge located at 6304 SW 78 th Avenue, South Miami, FL To ensure the success of the Hope 4 L.Y.F.E, 3K Breast Cancer walk, we are seeking partnerships, financial support and key stakeholder's involvement to support and promote this important and unique cause. We will continue to prove that investing in "Hope 4 L.Y.F.E 3K Breast Cancer walk" is a smart business decision and we will continuously reach new heights to give you the best possible experience with us.
