3 3 Qualified Safety Plan Requirements 1. League Safety Officer: Tom Wilson on file with Little League Headquarters. 2. Distribution of Safety Plan: will distribute a paper copy of this Safety Manual to all: Mangers/Coaches, League Volunteers and District Administrator. 3. 4 4 This list will be posted in the concession and dugout areas. Attached is s Emergency Contact Procedures. 4. Volunteer Application: will use the Official Little League Volunteer Application form to screen all our volunteers. 5. Fundamentals Training: TBD ( At least one manager/coach from each team must attend the training. Every Manager/Coach will attend this training at least once every 3 years. First Aid Training: TBD ( will require at least one manager/coach from each team to attend. Each manager/coach must attend this training every year. Concussion Safety form will be reviewed and signed by each parent/guardian of every player.

5 5 6. Field Hazard Inspection: Coaches are required to walk/inspect the fields prior to every practice and game. Umpires will also be required to walk the fields and inspect for hazards before every game. 7. Little League Facility Survey: has completed and updated our 2016 Facility Survey Online. 8. Concession Stand Safety: Menu shall be posted and approved by League Safety Officer and League President Our Concession Safety Procedures will be posted in numerous locations in concession stand. See Concession Stand Safety Procedures. 9. Equipment Inspection: The League Safety Officer will inspect all equipment in the pre-season Managers/Coaches will inspect equipment prior to each game. Umpires are required to inspect equipment prior to each game.

Implement Prompt Accident Reporting. will use the provided incident tracking form from the LL website and will provide completed Accident forms to the Safety Officer within hours of the incident. Please see copy of Accident Reporting Form. 11. First Aid Kit: Each team will be issued an updated First Aid Kit and is a requirement to have it at every practice and game 12. Enforcement of Little League Rules: Eastern Little League will require ALL TEAMS to enforce ALL Little League Rules including Proper Equipment for catchers No on-deck batters Coaches will not warm up pitchers Bases will disengage on all fields 13. Qualified Safety Plan Registration Form: Is accompanied with the ASAP plan. 14. League Player Registration Data or Player Roster Data, Coach and Manager Data: will be submitted to Little League Data Center (
