1 Welcome to Cheerstrike Allstars. We are delighted that you have chosen to become a part of the most prestigious all-star programs in the valley. Our mission at Cheerstrike Allstars is to enrich the lives of the children and their families through the sport of competitive cheerleading. Our goal each day is to teach our athletes the value of commitment, hard work, integrity, leadership, self-confidence, positive attitude and a love of the sport. It is our goal to be the very best all-star program in the country. Each and every family is important to us and our program. 4 Cheerstrike Allstars PARTICIPANT AGREEMENT, RELEASE AND ASSUMPTION OF RISK In consideration of the services of Cheerstike Allstars, its owners, agents, officers, employees, and all other persons or entities acting in any capacity on their behalf (hereinafter collectively referred to as CSA ). I hereby agree to release, discharge, and hold harmless CSA, on behalf of myself, my children, my parents, my heirs, assigns, personal representative and estate as follows: 1. I understand and acknowledge that the activities that I or my child engage in while on the premises or under the auspices of CSA pose known and unknown risk which could result in injury, paralysis, death, emotional distress, or damage to me, my child, to property, or to third party. The following describes some, but not all, of those risks: Cheerleading and gymnastics, including performances of stunts and use of trampolines, entail certain risks that simply cannot be eliminated without jeopardizing the essential qualities of the activity. Without a certain degree of risk, cheerleading students would not improve their skills and the enjoyment of the sport would be diminished. Cheerleading and gymnastics expose participants to the usual risk of cuts and bruises, and other more serious risks as well. Participants often fall, sprain or break wrists and ankles, and can suffer more serious injuries. Traveling to and from shows, meets and exhibitions raises the possibilities of any manner of transportation accidents. In any event, if you or your child is injured, medical assistance may be required which you must pay for yourself. 2. I expressly agree and promise to accept and assume all of the risks, known and unknown, connected with CSA related activities, including but not limited to performance of stunts and use of trampolines. My participation and that of my child is purely voluntary. No one has forced or coerced me or my child to participate. I elect for myself and my child to participate in such activities in spite of the risks. 3. I hereby voluntarily release, forever discharge, and agree to hold harmless and indemnify CSA from any and all liability, claims, demands, actions or rights of action, which are related to, arise out of, or are in any way connected with my child s participation in CSA related activities. 4. Should CSA require to incur attorney s fees and costs to enforce this agreement, I agree to indemnify and reimburse CSA for such fees and costs. 5. I certify that my child has health, accident and liability insurance to cover bodily injury or property damage that may be caused or suffered while participating in this event or activity, or else I agree to bear the costs of such injury or damage to my child. I further certify that I am willing to assume and bear the costs of all risks that may arise or be created, directly or indirectly, through or by any such condition. 6. In the event that I file a lawsuit against CSA, I agree to do so solely in the State of Texas and I further agree that the substantive and procedural laws in that state shall apply in any such action without regard to the conflict of laws rules thereof. I agree that if any portion of this agreement is found void or unenforceable, the remaining portions shall remain in full force and effect. 7. By signing this document, I acknowledge that if anyone is hurt or property is damaged during my participation or the participation of any of my children in this activity, I may be found by court of law to have waived my right to maintain a lawsuit against CSA on the basis of any claim from which I have released CSA by signing this Agreement. 7 ABSENCE POLICY The only excused absence is a school function that results in a grade. All other absences are unexcused. Part-time jobs, dances, concerts, banquets, long distance driving, family reunions, wedding, recitals, school/church socials, and any other non-related school activities are unexcused. Absences are NOT allowed the weekend and week prior to a National competition. An unexcused absence may result in the athlete being moved to an alternate position or pulled from an upcoming performance at the discretion of the staff. It is the responsibility of the athlete to learn any changes prior to practices. Alternates must follow all requirements and guidelines. An ABSENCE REQUEST FORM must be submitted 2 weeks prior to the absence. Do not or fax absence forms. These forms are for absences including school cheerleading, gymnastics, choir, band, and all other school sports. Your school activities are planned well in advance. Please submit your request in advance. A completed form does not automatically excuse the absence. Absences must be approved. We will verify with school administration and the teaching staff if we think that someone is being dishonest. Be prepared to show documentation from a teacher or instructor. You will be removed immediately if the request is falsified. Absences are not permitted the week on a National event. SICK POLICY You must attend practice when you are sick but not contagious i.e. Flu/Strep/Mono. Students are often removed from our program for failing to follow this rule. We will not make exceptions. You will not be required to participate; however, you must be present. An athlete will jeopardize their position if this rule is not followed. In addition, you must provide a doctor s note with detailed information regarding the illness, treatment, and length of recovery. This includes dental, medical and any other health related issue that would prohibit a student from participating. COMMUNICATION Most communication is set forth in monthly notes, calendars and s. Each team will have a designated Head coach and is responsible for relaying information to the parents and the Cheerstrike Allstar staff. Any questions or team related problems should first be addressed with the Head Coach if the problem can not be fixed by the head please contact our Director. TEAM PRACTICE RULES All practices are CLOSED to BOYFRIENDS/GIRLFRIENDS and athletes from other gyms. We also reserve the right to close all practices to parents! PRACTICES WILL ALWAYS BE CLOSED IN FEBRUARY. There is to be NO JEWELRY worn at practices, which includes all earrings, belly button rings and necklaces. This is not simply for appearance sake, but for the safety to the one wearing jewelry and for those either flying or basing who might be injured as a result. All CELL PHONES should be turned OFF during practice times. This includes parent cell phones. Also, please do not bring your family PETS to practices at the gym! There is to be NO GUM whatsoever at practices or competitions. There is to be no excessively long fingernails or sculpts. There is to be NO videotaping of practices. Practice clothing must be worn as scheduled on the monthly calendars. Lost, misplaced or damaged clothing will be replaced at your expense. Anyone late or dressed incorrectly will jump after practice. Once again, our program is about building champions and teaching responsibility to our athletes.

8 Code of Conduct Acknowledgment I will not be involved in any matter of disrespect towards any members of the Cheer Strike staff, parents or students. Anyone threatening to quit or pull their child from a squad may be dismissed from the program immediately. I understand that athletes are required to wear the appropriate practice attire to every practice. I understand the coaches reserve the right to suspend any athlete s or parent s participation in activities at Cheer Strike or at Cheer Strike functions as disciplinary action. I will not design, purchase,distribute or sell any clothing, bows or other items with our prior written consent from David Rodriguez. This includes team t-shirts and spirit items, This does not include snacks, cakes, cookies or consumable goodies. I will not participate in negative gossip or communications that adversely affect Cheer Strike. This includes but is not limited to Facebook, Instagram, Snapchat, GroupMe, Chat Rooms or other Websites. I will not post Cheer Strike music, choreography, routines, stunts, etc. on any website or any social media. I will maintain and/or improve the skills I performed at tryouts through hard work and dedication. I will not hold or threaten to hold my child from practice or competition as a form of punishment. I will conduct myself with respect and integrity, displaying positive sportsmanship at all times. I understand all athletes at Cheer Strike are expected to arrive at practices and competitions on time with a positive attitude and try their best effort in every endeavor. I will abide by my team s hair and make-up policy. I will not use inappropriate language. I understand it is sometimes necessary for athletes to be moved from one team to another. I understand that if I leave a competition early or before any awards or athletes gifts are given out, I may or may not received my gifts or awards as coaches will and may be busy assisting other athletes. I understand that some Cheer Strike locations have viewing of practice from outside of the gym and that area should be a positive atmosphere, and I may be prohibited from using that area if I cannot maintain a positive attitude while there. No Parent or non Cheer Strike member is allowed on our practice floor during any practice i.e tumbling, all star practice or dance practice. I understand that if I need to speak to the coaching staff I will arrange a meet time with them in advanced and not distract or interrupt class time. I understand monthly billing will be processed at the 1st through 3rd of every month, failure to take care of my fees will result in an automatic $10.00 late fee for each transaction.

9 I understand that is a 12 month commitment I am making. I will honor my commitment. I understand that if I quit or I am removed from the program, I will not be entitled to a refund of any kind. This includes all monies paid to Cheer Strike All star including prepaid expenses. Cheerleaders, parents, relatives and friends are never allowed to contact competition companies or their officials for any reason. 12 Dear Sir/Madam: Today cheerleading is recognized as a sport in almost every state in the nation, including Texas. Athletes in cheerleading compete nationally and train for many hours each week. Today s competitive cheerleaders must be skilled gymnasts and perform acrobatic feats that require extreme flexibility, strength and agility. Many all-star and collegiate team competitions are broadcast on national television. Top athletes receive college scholarships. Cheer Strike Allstars of South Texas comprise a group of five competitive teams, a total of more than 100 athletes, between the ages of five and nineteen. These athletes compete year round at local, regional and national competitions. In ten years, Cheer Strike Allstars of South Texas have captured numerous regional wins and over 250 National Championship Titles! Cheer Strike Allstars of South Texas are not affiliated with any school and receive no funding to assist in their competitive endeavors. Therefore, to assist in offsetting the extremely high costs of training, travel, and competition fees Allstars seek support in the form of corporate sponsorships and fund raisers. 16 TEAM PLACEMENT INFORMATION Each athlete will remain on their original team as long as they: 1. Maintain or improve the skills they completed at tryouts with same technique 2. Are able to successfully fill a stunt position 3. Abide by the rules and regulations of our program All-Star cheerleading is scored based upon the execution of tumbling, jumping, dancing, motion technique, tosses, stunting, performance and choreography. All are important in putting together a successful team. Athletes will be placed on teams based upon their abilities as well as their mental mindset. Additional Information: Property Rights Agreement: No Cheer Strike All-Stars, Pro Elite LLC program related items may not be sold without prior written consent from Cheer Strike All Star Company. The company name and its entity are solely owned by Cheer Strike AllStars and Pro-Elite LLC. Any violation of this agreement may result in legal action. Outside Merchandise Sales Policy : No spirit wear of any kind may be designed, purchased or distributed without prior written consent from Danielle Valdez or David Rodriguez. All team t-shirts and spirit items will only be purchased through per-approved vendors by Cheer Strike. This does not include snacks, cakes, cookies or consumable good bag items. In addition, no solicitation of any items while Cheer Strike property will be allowed unless you have written consent for Danielle Valdez or David Rodriguez. Valuables in Gym: It is not possible to monitor valuables brought into the Cheer Strike All Stars facility. Please leave valuables at home or in the car. Cheer Strike will not be responsible for any lost or stolen items. All practices are mandatory: Any extra practice that is scheduled at least one week in advance must be attended by all team members. (Remember summer you are allowed some missed practice please attend parent team meeting for more information) Absences from a Competition: Every team member is expected to attend every competition. However if there is an emergency issue wedding funeral or school event that results in loss of grade you must prepay your competition fees and find someone to replace you for the event at least 1 month in advance. Choreography: Mandatory choreography will begin Aug 1st. Throughout summer we will be focusing on all tumbling stunts motions and conditioning needed so that choreography will be easily executed. Maintaining your spot on a team: Our hope is that your season is great and goes according to plan.
