2 CALL TO MEET Host Site: Thunder Bay Gymnastics Association Claydon Building, Canadian Lakehead Exhibition Grounds 425 Northern Avenue, Thunder Bay, Ontario P7E 2V Meet Director: Debra Barrett (Club number) Gymnasts Eligibility: Women: Levels 2 9, Masters Regulations: As per the Gymnastics Ontario Technical Rules and Regulations Manual Warm Up Format: Option D Music: Refunds: All CD s must be clearly labelled with the athlete name, level, age category and club. All music must be on the announcer s table by the commencement of the general warm up for each flight. No cassette tapes accepted. If an athlete who is registered in the competition cannot participate due to medical reason, their club must submit a written request to the Meet Director. Refunds will be granted until March 1st, 2013 after which time no refunds will be honoured for any reason. Coaches Certification: Coaches must be fully certified NCCP Level 2 Women s Artistic and have up to date Risk Management certification (Part A: Risk Management & Part B: Respect in Sport) to be eligible for the competition floor. Please note that as of September 1st, 2011 all coaches attending competition must present their valid Coach Photo ID to gain access to competitive and warm up area. No access will be given to coaches without an updated Coach Photo ID. In case of a lost, stolen or forgotten ID, coaches will be required to sign form number CP001 stating a minimum level 2 certification for verification. The host club registration desk will have this form available. For replacement IDs contact Education Coordinator to discuss options of acquiring a replacement ID or an exemption.

3 Medical: Coaches are responsible for ensuring that they have the GO Participant Consent and Medical Data Record forms for all participating gymnasts in their possession at the competition. Gymnastics Ontario sanctioned events are NOT guaranteed to be Peanut/Tree Nut Free events. Attendees are asked NOT to bring peanuts, peanut products, or products containing traces of peanuts or tree nuts into the gymnasiums/venue and/or change rooms where participants are present. Host clubs have been asked to refrain from serving/selling items with peanuts, peanut products, or products containing traces of peanuts or tree nuts at this event. Gymnastics Ontario asks for your assistance in supporting an inclusive environment for all participants. PLEASE DO NOT BRING RAW CARROTS we have a Coach with a fatal allergy. Accommodations: A block of rooms (max 4 people per room) has been arranged at the Days Inn and Suites at a cost per night of $ Please be sure to quote TBGA Invitational Meet.
