2 Lifetime Achievement Bowlers who have performed tireless service for the general welfare and progress of the sport for the Great Salt Lake Bowling Association and who have accomplished a high standard of bowling ability as well as a high level of integrity and sportsmanship in both local leagues and tournaments. The following criteria will apply to ALL applicants for the years under consideration: Must be a present or former member of the Great Salt Lake BA (formerly SLCBA, SLWBA and Salt Lake YABA/AJBC) for a minimum of 15 years. It is not necessary that the years be consecutive. A minimum of 40 years of age Must have bowled in at least FIFTEEN (15) GSLBA certified tournaments. Outstanding service in the promotion and betterment of the Sport of Bowling through service or outstanding leadership to the bowlers in the GSLBA by holding a position of line officer, director, committee chair, or other service for at least TEN (10) years. *********************************************************************************** Please complete (additional pages are welcome if needed) A. Bowling nominee must be of good repute and have displayed a high level of integrity and sportsmanship in all areas of competition. Candidate must have at least one of the following: o 200 Average for 20 years in GSLBA (male candidates) o 190 Average for 20 years in GSLBA (female candidates) o Candidate shall have at least three (3) top three finishes in any of the tournaments sponsored by the GSLBA. These include the Open, Women s, Fall Classic (formerly the scratch tournament), and Senior Tournaments. There will be a male and female division with separate balloting for each performance category
