2 Village of Suamico Cemetery Committee Initial Introductory Meeting Wednesday, June 27, : 30 p.m. Members Present: Robert Vickery, Ron Schiltz, Eileen Olson, and Michelle Eckert Absent: Josh Schiltz Staff: Steve Kubacki, Administrator; Jessica Legois, Director of Administrative Services; and Melissa Hongisto, Deputy Cleric 1. Call to Order Meeting was called to order at 3: 34 p.m. by Administrator Kubacki 2. Selection/ Election of Officers a. Chairman Motion by Eckert to nominate Bob Vickery as chairman, second by Olson, motion carried ( Abstain Vickery) b. Sexton Motion by Vickery to nominate Ron Schiltz as sexton, second by Olson, motion carried Abstain Schiltz) 3. Review & Approval of 2018 Cemetery Budget Motion by Eckert to approve the 2018 Cemetery Budget, second by Schiltz, motion carried Review & Approval of Summary of Cemetery Funds Discussion on what account is operating or perpetual. 5. Review & Approval of Job Titles Necessary Positions and Job Descriptions Discussion of the jobs and descriptions. Village will take care of secretary and treasurer, Sexton and Chairman will be members of committee. Will review descriptions and reconfigure then bring them back. 6. Review & Approval of Village of Suamico Cemetery General Rules Copy of SCA General Rules 6/ 17/ 2015) Discussion regarding General Rules. 7. Review & Approval of Village of Suamico Cemetery Rates/ Fees/ Disbursement Fees Discussion to look over the fees and see what everyone else is charge.
3 Motion by Eckert to approve current rates/ fees/ disbursement fees, second by Vickery, motion carried Review & Approval of Village of Suamico Cemetery Deed Format/ Template Discussion regarding deeds was held. The village would like to implement a way to have copies of all deeds. All records should come to the village from now on, all invoices should be through the village and paid to the village. 9. Set Next Meeting Date July25, 2018 at 3: 30 p.m. 10. Adjournment Motion by Eckert to adjourn at 4:45 p.m., second by Schiltz, motion carried 4-0.

6 CEMETERY PLOT DEED Know all men by these presents: that SUAMICO MUNICIPAL CEMETERY, owned and operated by the Village of Suamico, a Wisconsin Municipality, in consideration of dollars, does hereby sell and convey the cemetery plot identified below, subject to the conditions, limitations, privileges, and restrictions herein after provided, to the following Purchaser: Name: Phone No.: Address: Plot # Row Section of Suamico Municipal Cemetery, situated in the County of Brown and State of Wisconsin as shown by the plat of said cemetery on file in the office of the Village of Suamico. The cemetery plot is to be used as a place of internment forever, and for no other purpose. No sale or transfer of the above described plot, or any part thereof, shall be valid without the written consent of the Village of Suamico. This deed is made by the Village of Suamico and accepted by the Purchaser, subject to all the laws of the State of Wisconsin now in force or that may hereinafter become in force, applicable to the cemetery: also subject to the conditions, limitations, privileges, and restrictions specified in the rules, regulations, and by-laws of the cemetery now in force, or that may hereafter by enacted by it in conformity to law. This deed is subject to all municipal liability limitations including, but not limited to, those specifically set forth at Section Wis. Stats., as amended. 7 SUAMICO CEMETERY ASSOCIATION GENERAL RULES EFFECTIVE AUGUST, 2018 There is NO reserving of lots. All lots have to be paid in full at time of purchase. Opening of a grave: payment must be made up front with the Village of Suamico, before an opening can proceed. The Sexton and/or his or her designee shall be authorized to locate grave sites and/or monument locations. Deeds will be issued, by the Village of Suamico, within thirty (30) days of purchase. Cremation and infant burials, a total of four (4) are allowed on a lot or existing grave. Only one ( 1) monument is allowed on a burial lot. Any other marker must be flat in ground. Decorations will only be allowed by the headstone monument. ALL decorations must be removed by October 31. See rate sheet for current prices and fees. See financial statement for Income/ Expenditures & Investments.
8 JOB DESCRIPTIONS SEXTON 1. Maintain cemetery in a clean and pleasant condition. 2. Remove leaves in spring and in the fall. 3. Cut grass and use weed -eater around monuments and trees. 4. Do layout for burials. Use metal location pipes to layout lots. 5. Layout and mark location for monuments. 6. Open and close grave sites. Hole must be 40" wide x 96" long x 54" deep. Remove top soil and set to the side, remove sand, when closing put top soil back on grave site. 7. October, remove all decorations. 8. Inform Village of Suamico Cemetery Committee of any problems or concerns. 9. Cutting grass on ditch on North side of cemetery is an agreement between sexton and Village of Suamico. The Village of Suamico Cemetery Committee lawnmower may be used. COMMITTEE MEMBERS 1. Attend all meetings of the Village of Suamico Cemetery Committee. 2. Participate in all decision making, such as: policies, fees, rules and operations. 3. To monitor financial and operational efforts to ensure both efficiency and effectiveness. 4. Ensure proper maintenance of the cemetery. CHAIRMAN 1. Act as the Committee Chair at all meetings. 2. Set the agenda and lead discussion for all meetings working in conjunction with the Village Clerk. 3. Working with Village Staff and the Village of Suamico Cemetery Committee to establish policies, fees and rates that govern the operations of the Village of Suamico Cemetery Committee. 4. Sign cemetery deeds.
9 WOODLAWN CEMETERY REGULATIONS FOR LOT DECORATIONS The Association/Cemetery Board reserves the right to regulate the method of decorating the plots/monuments so that a uniform beauty may be maintained. We are reminded by lot owners -and rightfiilly so -that lots were sold with a clear outline as to floral and plant decoration regulations and that disregard of these rules destroys the originally intended effect. The Associationis not responsible or liable for theft,vandalism, or other causes beyond its control. Floral Containers allowed: Concrete urns, clay pots, pedestal baskets or metal pinpoint containers are allowable. No glass, plastic, or wood containers are permitted. One container per lot of 1, 2 or 3 graves is acceptable or, 2 containers on 4 or more graves. Containers must be placed at the side of the monument, on a concrete foundation plate, installed by cemetery personnel. Single grave lots may have a pot on top of the flush marker, a pedestal basket at the side or a metal pinpoint container at the side of the marker.
