6 DSC 2015 COACH S AND SCHOOL S RELEASE AND INDEMNITY AGREEMENT SCHOOL: [School Name] I have submitted a roster with the names of all players participating in the 2015 Dublin Soccer Challenge. I have received a completed Combined Liability/Medical Release Form (DSC WAIVER) for each player on the roster. They include: -ATTACH ROSTER TO BACK- I certify on behalf of my school,, that the above players are the only individuals participating in the 2015 Dublin Soccer Challenge. Players not listed above will not play in any game. I have forwarded an Ohio Department of Health Youth Sports Organization Concussion Form to each player listed above. I assume personal responsibility for any and all injuries to players where I have not secured a DSC Waiver form. Further, I represent I am the Coach and official representative of my school,, and have authority to sign this document in that capacity. As a result, (school) agrees to assume all liability for any and all injuries to players that have not submitted a DSC Waiver form in the 2015 Dublin Soccer Challenge. Emergency Contact Information for each player will be available on the field. (school) agrees to defend and indemnify Dublin Jerome High School for any and all claims or lawsuits brought by players who have not signed the DSC Waiver form. 7 DUBLIN SOCCER CHALLENGE COMBINED LIABILITY/MEDICAL RELEASE FORM The following form must be completed for all participating players and must accompany the coach to the Dublin Soccer Challenge Soccer Tournament. I,, am the parent/legal guardian of (name of student-athlete) Recognizing the possibility of serious physical injury associated with soccer and in consideration of my child/ward's participation in the Dublin Soccer Challenge soccer tournament, I HEREBY ACKNOWLEDGE THAT MY CHILD/WARD ASSUMES ALL THE RISKS ASSOCIATED WITH SUCH PARTICIPATION, I EXPRESSLY CONSENT TO SUCH PARTICIPATION BY MY CHILD/WARD AND I AGREE TO WAIVE ALL CLAIMS OF WHATEVER NATURE, fully and finally, now and forever, for my child/ward, for myself, my estate, my heirs, administrators, executors, assignees, and successors, and TO RELEASE, EXONERATE, DISCHARGE AND HOLD HARMLESS Dublin City Schools, it's affiliated booster organizations, tournament directors, all coaches, all assistant coaches, athletic trainers, as well as any other person, sponsor, employee, board member, agent, or associated personnel, providing services or assistance to the Dublin Soccer Challenge from any liability, claims, causes of action or demands arising out of any injuries to my child/ward or to his or her property or losses of any kind which may result from or in connection with his or her participation in any activity related to the Dublin Soccer Challenge. As the parent/legal guardian of the above named player, I hereby give my consent to have an athletic trainer provide medical assistance to my child/ward. I further authorize emergency medical transport and care prescribed by a duly licensed Doctor of Medicine or Doctor of Dentistry and agree to be financially responsible for the cost of such care. 9 2015 Tournament Information and Rules Dublin Jerome High School Field Location: 8300 Hyland Croy Road, Dublin OH, All rules will follow the OHSAA guidelines, except for the following modifications: Games will be 50 minutes long. There will be a two-minute stoppage of play at an appropriate time midway during the game for team water break and instruction. The clock will be continuous with no stoppage for scores. Exceptions will be for serious injuries only. Teams may dress as many players as they like. Each team should bring a set of pennies in case of a color conflict. Numbered jerseys are not required but would be helpful. You may substitute players on any dead ball Game-worthy balls should be provided by the teams on the field. Teams will warm up on the practice field behind the school and south of the stadium. There is also warm-up space West of field 4. Teams should be ready to take the field IMMEDIATELY at the conclusion of the previous game. Jerome has air-conditioned locker rooms with restroom facilities for the players. If weather is an issue, the locker rooms will be available for all teams. Parking is available in the student lot, which is off of Brand Rd., or the staff lot, which is off of Hyland Croy Rd. There is a commercial grade ice machine at the stadium. Teams are welcome to use it as they need. There will be a coach s hospitality stand and info booth. Both will be under the big white tent as you enter the stadium. Teams need to check in at the info booth (white tent), before their first game. Bob Evans will be on site with a food trailer. They will serve breakfast in the AM and hamburgers, brats, hotdogs, etc in the PM. Kona-Ice will also be on site with flavored shaved ice. There is plenty of bleacher seating in the stadium. Spectators may want to bring chairs for fields 1,2, 3, and 4. There is a filming tower above the press box if anyone would like to do filming in the stadium.
